2.Changes of Ultrastructure of Muscle in Rats with Spastic Palsy after Injected with Botulinum Toxin
Journal of Applied Clinical Pediatrics 2006;0(14):-
Objective To observe the ultrastructural changes of end-plate and gastrocnemius muscle of rats with spastic palsy after injected with botulinum toxin type A in order to provide scientific base of histomorphology for clinical use of botulinum toxin type A.Methods Wistar rats′ pyramidal tracts in experiment group were injured by electric current 2.5 mA for 30 s,twice, but 36 normal control rats were only drilled hole in skull(control group).Seventy-two rats were successful in building spastic palsy model.Botulinum toxin type A (6 U/kg) were injected into right gastrocnemius muscles of 36 spastic palsy rats (group B),and physiological saline were injected into the same muscles in 36 spastic palsy rats as control(group A).Control group were not injected. The rats of group A,group B were sacrificed on 3,7,15,30,60 and 90 days after botulinum toxin therapy, and right gastrocnemius muscles was separated for histological analysis. Results Under the electron microscope, the structure of muscle fiber in group B had changed(Z line rupture, disorder and abolition, myofilament lysis, triad tract diso-rder,vacuolar degeneration) and these changes in group B occurred earlier and were severer than those in group A.There was compensatory hypertrophy of muscle fiber at later stages in group B.During the first 15 days after botulinum toxin type A injection, there were many synaptic vesicle without pre-synaptic membranes near terminal portion of nonmedullated nerve fibers.Following this, between 15 to 30 days, there were many folds similar to postsynaptic membranes in the cellular membranes and there were many grains in it.But there was no synaptic vesicle and pre-synaptic membranes near them.Compensatory hypertrophy and atrophy of muscle fiber coexist at 90 days after injection in group B.Conclusions Injection of botulinum toxin type A maybe induce the sprout of nerve and degeneration of the partial postsynaptic where there are many grains. It also accelerates muscle atrophy, but it induces compensatory hypertrophy of muscle fiber at later stage. It implies that injection of botulinum toxin type A can improve spasm symptom of the gastrocnemius muscles and this is good for muscle fiber repairing.

The Interleukin-2 gene cDNA was cloned into the Pichia pastoris expression vector pPICZB,which is under the control of the alcohol oxidase promoter AOX1. The linearized recombinant plasmid of BoIL2-pPICZB,digested by Sac I ,was transformed into X-33 strains by electroporation. The multi-copy insert transformants were screened by Zeocin-resistance and induced by 1% methanol. The intracellular expression products were tested by SDS-PAGE analysis and Western blotting. Purified recombinant BoIL2 was gained by metal-chelating affinity chromatographic (MCAC). Assay with murine CTLL-2 cells showed that the recombinant BoIL2 exhibited the biological activity.

Objective To investigate the efficacy observation of transcranial direct current stimulation (tDCS)for improving the attention in patients with infarction in basal ganglia area. Methods Sixty consecutive patients with basal ganglia infarction admitted to the Department of Rehabilitation Medicine, the First Affiliated Hospital of Nanchang University from May 2015 to May 2016 were enrolled. They were randomly divided into either a control group or a test group according to the random number table (n = 30 in each group). The patients in both groups received routine rehabilitation training, and those in the test group received tDCS therapy synchronously, and the control group received the sham tDCS stimulation. The evaluations and analyses were conducted with the Montreal cognitive assessment (MoCA),d2 test of attention, and event-related potential-P300 (ERP-P300),respectively in all patients before intervention and 4 weeks after intervention, and they were compared between the groups. Results There was no significant difference before intervention between the two groups (all P > 0. 05). Compared with before intervention, the ERP-P300 latencies were reduced, the amplitudes were increased after intervention in the patients of the test group and the control group (all P < 0. 05). The MoCA total scores (the test group:22. 7 ± 2. 7 vs. 15. 5 ±
2. 4;the control group:17. 2 ±2. 5 vs. 15. 6 ±2. 3),attention dimension scores (the test group:4. 6 ± 1. 2 vs. 2. 4 ± 1. 6;the control group:3. 6 ± 1. 5 vs. 2. 5 ± 1. 5),and the total completion of d2 attention test task, total scores,and concentration degree scores (the test group:295 ± 31 vs. 250 ± 45,279 ± 38 vs. 223 ± 52, 97 ± 22 vs. 75 ± 25;the control group:276 ± 33 vs. 247 ± 45,257 ± 39 vs. 211 ± 56,84 ± 23 vs. 71 ± 24) were all increased (all P < 0. 05),and all the indexes of the test group were better than those of the control group (all P < 0. 05). Conclusion tDCS contributes to the improvement of the attention in patients with infarction in the internal capsule-basal ganglia region.

Chinese Journal of Schistosomiasis Control 2014;(3):303-307
Objective To assess the quality of life(QOL)of advanced schistosomiasis patients, and understand its influenc-ing factors. Methods A questionnaire survey was carried out by using WHOQOL-BREF,and the information of demography, family, illness, health status, and health service was collected. Results Among the 217 advanced patients, the average age was (75.33±6.94)years,the ratio of male to female was 2∶5,89.86%of them were farmers,75.58%were illiterate or semi-illiterate, and 88.94%belonged to the splenomegaly type. Totally 61.29%of the cases had the scores over average for the overall QOL,but the scores for the health and well-being were lower. There were a significant difference among the average scores of different do-mains of QOL(χ2 =23.46,P<0.01). Both the year of being diagnosed and clinical classification was not associated with the scores of QOL. Regularly taking physical activities was significantly associated with each of all the 4 domains. The factors such as onset of acute disease in 2 weeks, taking therapeutic pills daily, marital status, age, etc. impacted the specific domains of QOL. The overall QOL and the 4 domains of the patients were at medium level;meanwhile, the score of physiological domain was lower than the scores of the other 3 domains. Conclusions The QOL(s)of advanced schistosomiasis patients in Qingpu District are rel-atively good. It is important to provide effective community health services and encourage the patients to take part in tempered sports or physical activities in order to keep their normal activities of daily living.
