1.Protection of L-arginine cardioplegia on myocardium during cardiopulmonary bypass

Objective:To study if L-arginine cardioplegia can protect myocardium from ischemia/reperfusion injury. Method:Fourteen patients undergoing valve replacement were randomly divided into two groups:control group (crystal cardioplegia,C group),and test group(crystal cardioplegia+L-arginine,E group). Before aortic-clamping and after aortic unclamping,blood samples were taken to measure the concentrations of NO_2~-/NO_3~-, lactic acid (LA), malonylaldehyde (MDA), superoxide dismutase (SOD), xanthine oxidase (XOD). Immediately after aortic clamping and before aortic-unclamping, tissues were taken from the left atrial and examed with electron microscopy. Objective To explore the clinical effects of aminophylline on bronchiolitis in children.Methods One hundred and twen-ty-eight children with bronchiolitis in our hospital were randomly divided into the experiment and treated control group,in the meantime,and divided into the severe and mild group according to their condition.The children in control group were treated with traditional methods,in experiment group were treated with aminophylline(2 mg/kg,every day one time),as well as used traditional methods.All patients in 2 groups were checked by volume (VT), inhale time and exhale time ratio (Ti/tE),time to peak expiratory flow and expiratory time ratio(tPTEF/Te),time to peak expiratory flow to total expiratory time and volume ratio(vPTEF/vE) respectively. 3.Pregnancy outcome among patients with chronic myelogenous leukemia treated with tyrosine kinase inhibitors.

OBJECTIVETo observe the pregnancy outcome among patients with chronic myelogenous leukemia (CML) treated with tyrosine kinase inhibitors (TKIs).
METHODSData associated with pregnancy, delivery and neonate from the patients or patient's spouse who conceived while receiving TKIs were collected retrospectively.
RESULTSTwo young female patients (who had been on imatinib therapy for 90 and 91 months, respectively) and spouses of 10 male patients (involving 7 patients who had received imatinib for a median of 60 months and 3 who had received dasatinib for 2.5 months to 7 months, respectively) with median age of 33.5 years (range 26 - 46 years) conceived and gave birth to 12 babies. One woman took imatinib throughout her pregnancy except one month. The other one took imatinib throughout her pregnancy and had breast-fed while on imatinib therapy for nearly half a year postpartum. Among the 12 babies, one was born prematurely with low birth weight and hypospadias (surgical repair after birth), the others were all healthy with no congenital defects. The median age of the children at the date of this report is 17.5 months (range 3 to 101 months), and they all have a normal pattern of growth and development.
CONCLUSIONSConception among patients with CML while receiving TKIs may result in normal pregnancies. The possible effects of TKIs on birth abnormalities cannot be ruled out. It is recommended that childbearing female patients should be advised to practice adequate methods of contraception and should not breast-feed while on TKIs therapy. In cases of accidental pregnancy, risk/benefit evaluations must be carried out carefully on an individual basis. No special precautions apply for male patients being treated with imatinib.

5.Epidemiological analysis of brucellosis in Hebei province from 2006 to 2010

ObjectiveBy analyzing the epidemiological characteristics and trends of brucellosis in Hebei province,provide a scientific basis for the formulation of strategies for effective prevention and control of the disease.MethodsUsing the descriptive epidemiological method,data of the “China information system for disease control and prevention” from 2006 to 2010 were statistically analyzed.ResultsThere were 13 632 reported cases from 2006 to 2010 in Hebei province,no death,the annual incidence rates reported were 3.4068/10 million,3.4851/10 million,4.5701/10 million,4.6045/10 million,and 3.5582/10 million,respectively.Eleven counties throughout the province had reported cases.The cases were found intensively in Zhangjiakou,Chengde,Baoding,Handan and Shijiazhuang,which accounting for 90.02% (12 271/13 632) of the total cases.The disease was found each month throughout the year,and showed a seasonal cycle with peak period in spring and summer.The disease was most commonly found in 25 - 65 age people,which accounting for 84.57%(11 529/13 632).The incidence in male was higher than that of female,and male to female ratio was about 3.56:1.00.Vocational high risk population was farmers,accounting for 91.15% of the total cases(12 425/13 632).ConclusionsBrucellosis epidemic in Hebei province is relatively serious,and the epidemic range has expand each year,even highly active in some particular areas.To control the outbreak of brucellosis,departments cooperation between health and animal husbandry should be strengthened; management of source of infection should be strengthened; health education and behavior intervention should be carried out thoroughly and deeply for high-risk groups.
6.Analysis on the factors that influence the treatment outcome of Tibetan nationality new smear-positive pulmonary tuberculosis patients in Qinghai Province

Objective To analyze the treatment outcome and related influencing factors of Tibet- an nationality new smear-positive pulmonary tuberculosis patients in Qinghai Province,so as to provide evidence for tuberculosis control and treatment among Tibetan population. Methods Statistical analysis was conducted on 5 564 Tibetan nationality new smear-positive pulmonary tuberculosis cases in Qinghai province who were reported in the China Tuberculosis Information Management System and approved to receive treatment from 2008 to 2017. The main influencing factors were detected by unconditional Logistic regression model analysis,dependent variable was successful treatment or not,independent variables were other factors related to the treatment outcome. Results The treatment success rate of Tibetan nationality new smear-positive pulmonary tuberculosis cases was 87. 1% ( 4 848 /5 564) ,and the adverse outcome rate was 12. 9% ( 716 /5 564) . Unconditional Logistic regression model analysis indicated that non-full- course supervision management,living in agricultural and pastoral area,having severe disease,floating population,and age older than 60 years were risk factors of adverse outcome. The odds ratio( OR) 95% confidence interval( CI) of the above risk factors were 13. 044( 10. 671-15. 944) ,2. 305( 1. 703-3. 119) ,
2. 090( 1. 346-3. 243) ,1. 967( 1. 443-2. 682) ,and 1. 909( 1. 410-2. 586) . Clinical consultation,farmers and herdsmen were protective factors. The OR( 95% CI) were 0. 451( 0. 375-0. 543) ,and 0. 786( 0. 627- 0. 985) . Conclusions Treatment success rate of Tibetan nationality new smear positive pulmonary tuberculosis cases was low. Therefore,the directly observed treatment short-course ( DOTS) strategy should be strictly implemented and the full-course supervision management should be strengthened to improve the treatment success rate. More attention should be paid to the elderly,severe,floating,agricultural and pastoral populations among the Tibetan population.
7.Decreased expression of calcium-sensing receptor involved in the progression of diabetic cardiomyopathy.

OBJECTIVETo observe the dynamic expression of calcium-sensing receptor(CaSR) in myocardium of diabetic rats.
METHODSThirty male Wistar rats were randomly divided into 3 groups including control, diabetic-4 week and diabetic-8 week groups(n = 10). The type 2 diabetes mellitus models were established by intraperitoneal injection of streptozotocin (STZ, 30 mg/kg) after high-fat and high-sugar diet for one month. The cardiac morphology was observed by electron microscope. Western blot analyzed the expression of CaSR, phospholamban (PLN), a calcium handling regulator, and Ca+-ATPase(SERCA) in cardiac tissues.
RESULTSCompared with control group, the expressions of CaSR and SERCA were decreased, while the expression of PLN was significantly increased in a time-dependent manner in diabetic groups. Meanwhile diabetic rats displayed abnormal cardiac structure.
CONCLUSIONThese results indicate that the CaSR expression of myocardium is reduced in the progression of DCM, and its potential mechanism may be related to the imnaired intracellular calcium homeostasis.

8.Application of electronic clinical pathway in the Mammotome biopsy of benign breast tumor

Objective To compare the differences between conventional Mammotome biopsy of benign breast tumor and Mammotome biopsy under the electronic clinical pathway,in order to provide basis for Mammotome biopsy of benign breast tumor under clinical pathway.Methods 160 patients with benign breast tumor in affiliated Hospital of Binzhou Medical College from January 2010 to December 2011 were divided into the control group (80 patients treated with conventional Mammotome biopsy) and the observational group (80 patients treated with Mammotome biopsy under clinical pathway).Patients' waiting days before surgery,days in hospital,total cost,obedience,health knowledge,postoperative complications and satisfaction between two groups were compared.Results Days before surgery in the observational group were (1.25 ± 0.33),and days in hospital were (4.28 ± 1.52),both of which were shorter than those in the control group (2.44 ± 0.65),(6.02 ± 4.12).The differences were statistically significant (t =7.48,9.86,respectively ; P < 0.05).Total cost in the observational group was (4 758.48 ± 686.34) yuan,fewer than that in the control group which was (4 998.76 ± 778.32) yuan,and the differences were statistically significant (t =3.46,P < 0.05).Compared with the control group,the observational group had better obedience rate (97.5% vs 85%),better mastering of health knowledge (97.5% vs 82.5%),and higher satisfaction (97.5% vs 85%).The differences were statistically significant (x2 =7.83,10.0,7.83,respectively ; P < 0.05).Conclusions Mammotome biopsy under clinical pathway can further improve patients' obedience and satisfaction,reduce days in hospital,total cost and postoperative complications,thus should be promoted actively.
9.Effects of mechanic auxiliary expectoration on prevention of pneumonia of patients with artificial airway

Objective To research the effects of mechanic auxiliary expectoration on prevention of pneumonia of patients with artificial airway.Methods According to the admission number,116 patients with establishment of artificial airway were divided into treatment group and control group,and 58 cases in each group.Hama mechanic assisted expectoration was used in the treatment group,while traditional chest percussion in the control group.Then,expectoration effect,pulmonary infection,comfort of patients and operation feeling of nursing staff were observed and statistical analyzed.Results The incidence of pneumonia (5 vs 14),sputum volume [(38.6 ± 4.1 ml/d vs 33.8 ± 3.3 ml/d)],expectoration effective cases (18 vs 12),expectoration markedly effective cases (32 vs 26) between treatment group and control group were significantly significant (t/x2 =5.098,2.021,-2.334,respectively; P <0.05).Significant difference were found in the comfort of the patients and operating experience of nursing staff (x2/Z =-5.097,11.226,-3.250,respectively; P < 0.01).Conclusions Mechanic auxiliary expectoration can improve sputum drainage of patients with artificial airway,reduce the rates of lung infection and nursing workload,improve patient comfort.It' s worth popularizing in the clinical application.
10.Correlation of ornidazole concentration in saliva and serum of healthy volunteers.

OBJECTIVETo investigate the distribution of ornidazole in the salivary and serum of healthy adults and explore the feasibility of monitoring serum drug concentration with salivary.
METHODSSix volunteers received a single dose of 0.6 g ornidazole via intravenous infusion. The concentrations of ornidazole in the saliva and serum were assayed by high-performance liquid chomatography, and the correlation of the drug concentrations in saliva to that in serum was analyzed.
RESULTSThe concentration of ornidazole in the saliva was strongly associated with that in the serum (r = 0.825-0.969), and the ratio of saliva-to-serum concentration (S/P) of ornidazole was 0.99 ± 0.13.
CONCLUSIONDetection of saliva ornidazole concentration is feasible for monitoring the therapeutic concentration of ornidazole.
