Objectives To examine clinical characteristics,predisposing factors,treatment and care of depression and/or anxiety symptoms in hospitalized patients with cardiovascular diseases.Methods A retrospective analysis of clinical data of 1 005 hospitalized patients with cardiovascular diseases at our hospital from January 2015 to December 2015 was conducted and psychological assessment for all patients was carried out using the Hamilton Anxiety Rating Scale and the Hamilton Depression Rating Scale.The incidence of depression and anxiety and predisposing factors were investigated.The effectiveness of psychological intervention in addition to cardiovascular medications was evaluated.Results The overall incidence of depression and anxiety was 23.4％ (235 cases) among the 1 005 hospitalized patients with cardiovascular diseases.Of the 235 cases,73 cases(31.1％) had anxiety,33 cases had depression (14.0％),and 129 cases had both anxiety and depression (54.9％).The occurrence of anxiety was significantly higher than that of depression(P＜0.05).Risk factors for anxiety and depression in patients with cardiovascular diseases included work-related fatigue,working conditions and health status.The rate of effectiveness of psychological intervention was 95.4％,and no serious adverse events were observed.Conclusions Depression and anxiety are common in hospitalized cardiovascular patients and can be attributed to factors such as work-related fatigue,working conditions and health status.Favorable clinical outcomes can be achieved through psychological interventions,in addition to cardiovascular drug therapy.

Objective To study the effect of Simiaoyongantang on human umbilical vein endothelial cells(HUVECs) injury induced by hypoxia.Methods The HUVECs were divided into normal group,model group and Simiaoyongantang group,and the hypoxia model was established at the latter two groups.The livability of HUVECs was measured by MTT assay.The transudation of LDH in the medium was measured by biochemistry and the positive expression of VEGF was detected by immunofuorescence chemistry.Results The level of LDH was reduced significantly and the cell activity indicated by MTT was increased.The number of the cells stained by VEGF increased in the treatment group of Simiaoyongantang.Conclution Simiaoyongantang has protective effect on hypoxia injured HUVECs,which may be related to its effect of increasing the cells livability,reducing the injury induced by hypoxia,up-regulating expressions of VEGF and promoting angiogenesis.

Objective To explore the feasibility and safety of laparoscopic total colectomy for familial adenomatous polyposis. Methods All 9 patients with familial adenomatous polyposis received laparoscopic total colectomy were analyzed. Results Laparoscopic total colecto-my was successfully performed in all the 9 patients. The operative time,blood loss,flatus and hospitalization were respectively 230~310 min, 80~210 mL,2~3 days and 12~14 days. All patients were followed up for 6~36 months. The postoperative recovery was good. Conclusion Total colectomy can be successfully perfomed with laparoscopic assisted in treating familial adenomatous poliposis. It is a feasible and safe surgical procedure and can significantly improve the quality life.

BACKGROUND:Osteofascial compartment syndrome is usual y due to application of external fixation for fracture treatment. Vaseline gauze is used to cover and compress the wound. Frequent dressing change induces infection and is harmful to wound drainage and tissue granulation, final y causing prolonged time of hospitalization and bad results. ＜br> OBJECTIVE:To investigate the clinical efficacy of Ilizarov external fixation combined with vacuum sealing drainage treatment for limb fractures with osteofascial compartment syndrome. ＜br> METHODS:A total of 32 patients with limb fractures with osteofascial compartment syndrome were randomly divided into two groups. Al patients adopted the same fasciotomy and drug treatment. In treatment group, the fracture was fixed with Ilizarov external fixation and the wound was covered with the vacuum sealing drainage dressing. In control group, the fractures were treated by unilateral external fixation and the wound was covered with the vaseline gauze. The fracture healing, clinical effects, hospitalization duration and costs between two groups were compared. ＜br> RESULTS AND CONCLUSION:In the treatment group, there were 10 cases of healing wel , 4 cases of muscle contracture and 2 cases of amputation. In the control group, there were 7 cases of healing wel , 6 cases of muscle contracture and 3 cases of amputation. No significant difference in clinical outcome measures was detected between the two groups. Fracture healing rate and costs were higher in the treatment group than in the control group (P＜0.05), but hospitalization duration, skin grafts and the incidence of infection were lower in the treatment group than in the control group (P＜0.05). Results suggested that on the basis of timely fasciotomy and decompression, Ilizarov external fixation combined with vacuum sealing drainage treatment for limb fractures with osteofascial compartment syndrome has a high limb salvage rate, which can effectively treat fractures, elevate the healing rate of fractures, reduce infection rate and skin grafting rate, shorten hospital stays. However, the hospitalization expense is higher.

Objective To evaluate the Chinese decoction prescriptions combined with incision drainage operation in treatment of perianal abscess after leukemia chemotherapy.Methods One hundred cases of perianal abscess after leukemia chemotherapy were randomly divided into two groups,treatment group (Chinese decoction prescriptions combined with incision drainage operation) and control group (potassium permanganate combined with incision drainage operation).Observe and record the efficacy and ache alleviated after treatments,recurrences were followed up in and 3 years.Results After the treatments,the total effective rate was 90.0％ in the treatment group,significantly more than control group(72.0％),the difference was significant(x2 =10.52,P ＜ 0.05).And treatment after a week the VAS score in the treatment group and control group were(2.2 ± 0.4) and(4.7 ± 1.2),(t =14.62,P ＜0.05).The healing time were(13.4 ±9.3) d and(19.8 ±11.6) d,(t =4.95,P ＜ 0.05).After 3 years follow-up the recurrence rate and fistula formation rate in the treatment group and control group were 2.0％ and 2.0％,22.0％ and 26.0％,(x2 =18.93 and 23.92,P ＜ 0.01).Conclusion Chinese decoction prescriptions could collaborative incision drainage operation treatment of perianal abscess after leukemia chemotherapy,effective and safe,suitable for extensive application and promotion.

In order to solve the problems of erratic drug absorption and low bioavailability after oral administration for poorly-water soluble drugs due to low solubility, a series of novel pharmaceutical dosage forms as solid dispersion, liposome, microemulsion, vesicle, cyclodextrin inclusion complexes and drug nanocrystal have been developed in recent years. Among which drug nanocrystal attracts more attentions for its simpler preparation method, higher drug loading and easier manufacturing technology in the design of dosage forms suitable for different administration routes. In this paper, the nanocrystals of the poorly-water soluble drugs prepared based on bottom-up and top-down technologies were introduced. The characteristics and applications of the nanocrystal-based dosage forms as suspension, tablet and capsule were also introduced and carefully evaluated with the focus on their pharmacokinetics, pharmacodynamics and tissue targeted drug distribution after delivery by oral administration, intravenous injection and pulmonary inhalation. The advantages of drug nanocrystals in their therapeutics effects over the bulk drugs were discussed together with the inherent mechanism. Finally, the problems existing in basic research and scaled-up manufacture of drug nanocrystal as well as the possible ways of solution were listed out so as to make the nanocrystal-based preparations exert their maximum therapeutic effect after clinical application.

Objective To observe the application and the curative effects of vacuum sealing drainage (VSD) in the treatment of skin injury of trauma surgery .Methods The 50 cases with severe traumatic skin injury treated with VSD were compared with tra-ditional dressing group(control group 50 cases) to observe the curative effect of the two groups .Results Compared with control group ,VSD group improved the success rate of follow-up skin graft .Good skin graft in VSD group were obvious more than the con-trol group(P<0 .01) .Conclusion The VSD can improve the success rate of skin graft ,accelerate wound healing .At the same time , VSD can avoid the pain due to reducing the times of changing dressings .It is a good method of treating severe traumatic skin injury .

ObjectiveTo investigate the effects of chitosan/PVA nerve conduits which used for repairing sciatics nerve defect in rats.MethodsTwenty-seven rats were divided into 3 groups randomly,with 9 rats in each group. Firstly, the 15mm defects in the left sciatic nerves were made in the rats and were respectively repaired with chitosan/PVA conduits graft (group A), the silicon conduits graft (group B),and autografts (group C). At 12 weeks after the operations, the left sciatic nerves were taken out, and the comparative evaluation was made on the repairing effects by wet weight of gastrocnemius and soleus muscles, histological examination,computerized imaging analysis and True Blue retrograde tracing. ResultsThe wet weight of gastrocnemius and soleus muscles showed no significant difference between the chitosan/PVA graft and autograft groups (P ＞ 0.05). The wet weight of gastrocnemius and soleus muscles in significant difference between the chitosan/PVA graft and the silicon group at 12 weeks after the operation(P ＜ 0.05). The nerve fiber density showed no statistically significant differences between the chitosan/PVA and autograft groups(P＞ 0.05).The regenerative nerve fiber in group B had normal morphological and structural characters under transmission electron microscope.True Blue-labeled neuron cell bodies were found within both anterior horn of gray matter in the spinal cord and dorsal root ganglions (DRGs) ipsilateral to the operated side of the tested rats on illumination with ultra-violet light 1 week after the injection of True Blue.Conclusion Chitosan/PVA nerve conduit can effectively promote the nerve regeneration and myelinization of rat sciatic nerve, which is expected to substitute for autograft to repair nerve defects succesfully.
