1.Nursing intervention in intense insulin therapy of critical patients

Objective To investigate the clinical effect of improved nursing intervention in blood glucose control of critical patients. Methods By means of historical control, 258 critical patients undergone intense insulin therapy were divided into the control group (n = 126) and the test group (n = 132). The control group was given conventional intensive insulin therapy. The test group was given improved guidelines on the use of insulin and improved nursing intervention. The following indexes of the two groups were documented and compared: the time required to achieve target blood glucose level [(11.95 ± 1.97) h vs (18.85 ± 3.59) h, t =19. 237 3 ,P <0. 01]; frequency of blood glucose measuring in four days, the range of everyday blood glucose and the occurrence of hypoglycemia during therapy. Results The test group had shorter time required to achieve target blood glucose level, lower frequency of blood glucose measuring, less fluctuation range of blood glucose and reduced occurrence of hypoglycemia during therapy than the control group (P < 0. 01). Conclusions The use of improved guidelines on the use of insulin and improved nursing intervention in intensive insulin therapy of critical patients can effectively control blood glucose, reduce frequency of hypoglycemia and improve prognosis.
2.Analysis of relationship between P27, P53 and PCNA expression and its clinical significance

Objective: To investigate the relationship between P27,P53 and PCNA expression in human gastric carcinoma tissues and clinicopathological parameters. Methods: The expression of P27,P53 and PCNA in 62 human gastric carcinoma tissues was examined with immunohistochemistry SP method. Results: Positive rates of P27,P53 and PCNA expression were 37.1%, 40.4%,83.9%. P27 expression was related with Bormann type, infiltrative depth, lymph node and distant metastasis and clinical stage. P53 expression was related with sex of patients, distant metastasis and clinical stage. PCNA expression was related with age of patients and infiltrative depth of tumor. P27 positive expression group was higher than negative group as to 5-year survival. P27 expression was in reverse relation with PCNA expression. Conclusion: The expression of P27, P53 and PCNA may be regarded as an important marker in judging malignant degree of gastric carcinoma,distant metastasis and prognosis.
3.The citation analysis of the publications in Chinese Journal of Preventive Medicine, 2000-2005.

OBJECTIVETo carry out the citation analysis of publications in Chinese Journal of Preventive Medicine (CJPM) among the preventive medicine authors and analyze the impact of this magazine in preventive medicine domain.
METHODSUsing Chinese scientific periodical literature evaluation and statistical analysis system (V1.0), the citation status of all CJPM publications in 2000-2005 was analyzed, the analysis covered 21 columns, including the review, editorial and original article, the data were collected up to November, 2007.
RESULTSFrom 2000 to 2005, CJPM had more than 30 columns and carried 1196 articles and 92. 89% (1111/1196) articles were from 21 main columns. During 2003 to 2005, the impact factors of CJPM were 0. 897, 1.011 and 0. 891 respectively. Among 21 main columns, the citation frequency of six columns including original article, editorial, review, courses, discussion and case report were higher than 80%. In five columns (original article, editorial, report, review and academic trends), the average citation frequency of individual articles was more than 4 times. The citation frequency of 20 authors was higher than 20 times and these authors were from medical schools, teaching hospitals, centers of diseases control and the research institutes. The individual citation frequency of 17 articles was more than 20 times and the individual citation frequency of three articles was more than 50 times. 34.9% of the citations of the 2000-2005 CJPM articles were from the top 20 journals, and the self-citation rate was 4. 85%.
CONCLUSIONThe publications in Chinese Journal of Preventive Medicine are most frequently cited, which indicated that those publications have high quality, this journal has a great effect in preventive medicine field of China.

4.Cultivation, identification and ultrastructural observation of the proliferative cells from the newborn rat cochlea.

OBJECTIVETo explore whether there could be proliferative cells in the cochlea of the newborn rat or not and what kinds of cells should be differentiated from the proliferative cells while to study the effect of the growth factors on the proliferative cells and the ultrastructure of the proliferative cells.
METHODSThe Corti's organ were dissected from the cochlea of newborn SD rats and cultured. The proliferative condition of cells was tested by infusing the 5-bromo-2-deoxyuridine (BrdU) into the culture medium. And the variety of the spheres and differentiated cells were identified by immunohistochemistry. Corti's organ from forty-eight surface preparations was randomly divided into 4 groups: control group; epidermal growth factor (EGF) group; basic fibroblast growth factor (bFGF) group and EGF + bFGF group, with each group including 12 Corti's organ, and then the number of cell spheres of each Corti's organ was counted. The data was statistically analysed with ANOVE. Finally, the proliferative cells were observed under scanning electron microscope and transmission electron microscope.
RESULTS(1) The cell spheres can be observed in the cell culture of the Corti's organ. In present experiment, 90.1% of cells in spheres were labeled by BrdU, while nestin of spheres, the marker of hair cells--myosin 7A, espin, and phalloidin of the differentiated cells were positive. The marker of neuron-microfilament-M was also positive, and some differentiated cells were labeled by myosin 7A and BrdU, espin and BrdU, NF-M and BrdU at the same time. (2) The average number (x +/- s) of spheres from single Corti's organ was: 45.3 +/- 23.00 in control group, 86.2 +/- 34.1 in EGF group, 96.5 +/- 33.6 in bFGF group and 131.2 +/- 47.00 in EGF + bFGF group. There were significant differences between other groups respectively (P < 0.05) but there was no significant differences between EGF group and bFGF group (P > 0.05). (3) Scanning electron microscopy and transmission electron microscopy showed that cells of the spheres were round and had the same size and many short and thin microvilli on the surface of these cells. The cytoplasm were rich of organellae such as endoplasmic reticulum, mitochondrion, and cytoskeleton such as microfilament, microtube, et al. Tight junction, desmosomes and gap junctions between two adjacent cells were seen.
CONCLUSIONSThe proliferative cells are observed in the cochlea of the newborn rats and proliferative cells could differentiated into hair cells with bundles-like structure and neuron. Both EGF and bFGF possess the promoting effects for proliferation on the proliferative cells while the proliferative cells have characters of earlier immature cells.

5.Different levels of nitric oxide in seminal plasma of fertile and abnormospermic men.

OBJECTIVESTo understand the relation between nitric oxide (NO) level in seminal plasma and male fertility.
METHODSThe levels of NO in seminal plasma of 174 fertile males and 217 abnormospermia patients were measured by using the reductase of nitric acid, Greiss reagent and spectrophotometry.
RESULTS1. NO was found in all 174 samples (20-49 years) of fertile males which was (27.78 +/- 5.81) mumol/L. The NO level in seminal plasma in fertile males was became higher after age 40, and there was no significant difference between 20-29-year-old [(26.25 +/- 5.52) mumol/L] and 30-39-year-old[(28.11 +/- 5.87) mumol/L]. But the group of 40-49-year-old[(30.17 +/- 6.14) mumol/L] had a higher level of NO in seminal plasma than 20-29-year-old (P < 0.05). 2. The seminal plasma samples from nine types of abnormospermia were measured, which all had a higher level of NO than fertile males. In abnormospermia, the level of NO in seminal plasma of the patients with single abnormality increased slightly, two abnormality obviously increased, and the highest level of NO in seminal plasma appeared in three abnormality.
CONCLUSIONSThis results confirmed that proper level of NO in seminal plasma may regulate the spermatogenesis.

6.Risk prediction model of perinatal congenital heart disease

Through analyzing the influencing factors of congenital heart disease (CHD), it is aimed to establish CHD risk prediction model in fetus, and simultaneously provide theoretical foundation for CHD prevention. One-factor logistic regression method was used to screen the significant factors regarding CHD, and to separately adopt multiple-factor non-conditional logistic regression method and decision tree to set up model prediction fetus CHD risk and to analyze the advantages and shortcomings. Correct classification rates turned to be 80.93% and 82.79% respectively among 215 'training samples' by the two methods and the rates were 85.45 % and 89.09% respectively among 55 'testing samples'. The alliance of logistic regression and decision tree can overcome influence by co-linearity to guarantee the accuracy and perfection, as well as promoting the predictive accuracy.

Objective To analysis the effect of different age factor on the prognosis and efficacy of acute cerebral infraction in patients treated by intravenous thrombolytic therapy with alteplase.Methods Ninety-eight patients with cerebral infarction were divided into older group (n=52) and younger group(n=46).Patients in two groups were trea-ted by intravenous thrombolysis with 0.9 mg · kg-1 alteplase.After 1 and 21 d, the efficacy was observed and scored using national institute of health stroke scale( NIHSS).At the same time,the incidence of adverse drug reactions were observed, and prognosis evaluated using Modified Rankin scale ( mRS ) in 3 weeks.Results NIHSS scores were signifi-cantly lower in the both groups treated by alteplase than those before thera-py( P<0.05).In 1 and 21 d, NIHSS scores were lower in the younger groups than those in elderly group ( P<0.05 ).The incident of adverse drug reaction was lower in the younger groups than the elderly group ( P<0.05).It has shown that the prognosis was better in the younger group than the elderly group ( P<0.05 ) by comparing mRS score in 3 weeks.Conclusion All acute cerebral infraction patients received sig-nificant benefits from the intravenous thrombolytic therapy with alteplase.The younger group had more significant efficacy, less complications and better prognosis than those of elderly group.
9.Relationship between mannose-binding protein gene polymorphisms and disease progression and HBV DNA in patients with chronic HBV infection

Objective To determine the influences of Mannose binding protein (MBP) gene polymorphisms on HBV DNA loads and on the progression of liver disease in patients with chronic HBV infeclion.Method The Codons on 54 MBP gene polymorphisms and HBV DNA loads in a cohort of 395 patients with chronic HBV infection,including 244 with chronic hepatitis B (CHB),151 with liver cirrhosis(LC) and 88 normal controls were examined by polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) and fluorescent quantitative PCR (FQ-PCR).Result The MBP genotype frequencies of GGC/GAC and alleles genetic frequencies of GAC in CHB group showed no significant differences comparing to the normal control group ( P > 0.05 ).The MBP genotype frequencies of GGC/GAC and alleles genetic frequencies of GAC on CHB group (severe),compensation phase of LC group and decompensation phase of LC group were higher than those in the normal control group (P < 0.05 ),the genetic polymorphism of decompensation of LC was 36.5 %,highest of all.The MBP genotype frequencies of GGC/GAC and alleles genetic frequencies of GAC of patients with chronic HBV infection were not changed with the differences of HBV-DNA loads.Conclusion The codes on 54 MBP gene polymorphisms is not closely related to HBV DNA loads,but was associated with the progression of hepatitis B infection.

Treatment of large bone defects caused by trauma, osteomyelitis, and tumors has been a major challenge in clinical. In the past, there have been many ways to repair and reconstruct the large bone defects. However, there is a long period of treatment, high technical requirement and complications such as ununion. After Masquelet reported the induced membrane technology in 2000, the technique was widely used in treatment of trauma, osteomyelitis, and large bone defects caused by tumors. It has been obtained good results. It has the advantages of short course, high healing rate, easy operation and easy to master. The induced membrane has unique structural characteristics and biological characteristics. There are many kinds of osteogenic factors that are included in the membrane, such as vascular endothelial growth factor, and morphogenetic protein-2, transforming growth factor-β1, etc. These osteogenic factors contribute to accelerate bone healing. With the development of induced membrane technology. The technology of Reamer Irrigator Aspirator technology, engineering tissue technology and internal fixation is used in clinic.It can provide bone source, promote bone defect reconstruction, improve long-term limb function and reduce complications.This paper retrospectively summarizes the experimental research and clinical progress of Masquelet technique in the treatment of large bone defects.
