Objective To investigate the learning curve of laparoscopy-assisted anorectoplasty in the treatment of high imperforate anus.Methods Sixty patients with high congenital anal atresia who were treated with laparoscopy-assisted anorectoplasty in the Capital Institute of Pediatrics from December 2008 to October 2015 by one experience surgeon were selected as observation group.Based on the criteria,the patients were divided into 4 groups(15 cases in each group) according to the sequence of the operation and every 15 cases served as a learning phase.The operating time,frequency of operation,complication rate,conversion rate to open surgery,blood loss,the rates of blood transfusion and hospital stay were compared among the 4 phases.Results The mean operation duration was (176.3 ±41.6) min,(128.8 ± 36.1) min,(127.3 ± 35.2) min,(124.0 ± 30.1) min in group A,B,C and D,respectively.The operation duration of 4 groups showed a declining trend.Group A has longer operation time than group B,C and D,the difference were statistically significant (P ＜ 0.05).In detail,the comparison of operation time between group A and group B was statistically significant (P ＜ 0.05).Furthermore,group A had significantly more median blood loss than group B,C and D,the differences were statistically significant [the 4 groups of data,respectively,group A:(17.5 ±6.5) mL,group B:(15.0±5.0) mL,group C:(14.5 ±5.5) mL,group D:(8.5 ±6.5) mL,P＜0.05].The mean capacity of blood loss was 12.7 mL.No significant difference was found in surgical complications,the rates of blood transfusion,total hospital stay,and postoperative hospital stay among the 4 groups(P ＞ 0.05).But the postoperative hospital stay decreased from 9.8 d (group A) to 7.4 d (group D).Conclusions For a well-trained surgeon in imperforate anus surgery.The learning curve of laparoscopy-assisted anorectoplasty in the treatment of high imperforate anus on about 15 cases helps to make a skillful surgeon.

Objective To investigate the changes and clinical significance of serum lactate dehydrogenase (LDH),creatine kinase (CK),glutamate pyruvate transaminase (AST),and cerebrospinal fluid lactate dehydrogenase (CSF LDH) in adult patients with acute central nervous system infection.Methods The levels of myocardial enzymes (AST,LDH,and CK) in serum of 96 adult patients with acute intracranial infection in 7days and 39 healthy people were measured by Beckman automatic biochemical analyzer and enzyme rate assay,and CSF LDH level in 96 patients were measured simultaneously.Results (1) The serum myocardial enzymes (LDH,CK,and AST) of intracranial infection group (47 cases with viral encephalitis,30 cases with tuberculous meningitis,and 19 cases with purulent encephalitis) were significantly higher than those of normal control group (P ＜0.01).(2)The myocardial enzymes (LDH,and AST ) of patients with cerebral functional disorder were significantly higher than those of patients with normal cerebral function (P ＜0.05).(3)The levels of serum AST,LDH,and CK in the virus encephalitis group,serum AST and LDH in the purulent encephalitis group,and serum LDH in the tuberculous meningitis group were significantly higher than those in the control group (P ＜ 0.01).The CSF LDH level in the viral meningitis group was prominently lower than that in the tuberculous encephalitis group and purulent encephalitis group,respectively (P ＜0.01).(4) No correlations were found between CSF LDH and serum myocardial enzymes (P ＞0.05).Conclusions (1)There is significant change in the levels of serum LDH,CK,AST,and CSF LDH of adult patients with acute intracranial infection,especially in infected patients with cerebral functional disorder,and the change of LDH is the most obvious.(2)The levels of serum myocardial enzymes and CSF LDH are helpful to the differential diagnosis of intracranial infection in early stage,and judging the severity of the illness.

This paper summarized the recent 30 years research progress of the chemical constituents from Notopterygii Rhizoma et Radix. The chemical constituents from Notopterygii Rhizoma et Radix mainly consist of coumarins, polyene-polyacetylenes, sesquiterpenes, phenolic acids, while steroids and flavonoids were less reported. All constituents were confirmed and corrected through SciFinder. We also checked the Chinese name and English name and listed the CAS number of each compound. It can provide some guidelines for the research, development and utilization of Notopterygii Rhizoma et Radix in the future. Whether there is columbianin in the Notopterygii Rhizoma et Radix need to be further researched.

Objective: To observe the clinical efficacy of mind-refreshing and balance-restoring needling method combined with Frenkel exercises in treating ataxia after cerebral stroke. Methods: The recruited 120 patients were randomized into an observation group and a control group, with 60 cases in each group. The control group was intervened by mind-refreshing and balance-restoring needling method, while the observation group was given additional lower-limb Frenkel exercises. Before and after treatment and at the follow-up, the ataxic lower-limb function was scored using Berg balance scale (BBS) and international cooperative ataxia rating scale (ICARS), and Barthel index (BI) was adopted to score the activities of daily living (ADL). Results: After treatment, the markedly effective rate was 70.2％ and the total effective rate was 96.5％ in the observation group, versus 39.7％ and 87.9％ in the control group, and the differences in the markedly effective rate and the total effective rate were statistically significant (P<0.01, P<0.05). The intra-group comparisons showed that the BBS, ICARS and BI scores after treatment and at the follow-up were significantly different from those before treatment in both groups (all P<0.01).There were significant differences in the BBS score between the two groups after treatment and at the follow-up (P<0.05, P<0.01); the between-group differences in the ICARS and BI scores were statistically insignificant after treatment (both P>0.05), while the between-group differences in the ICARS and BI scores were statistically significant at the follow-up (both P<0.05). The interaction effects between the scoring time of BBS and BI and the group factor were statistically significant (P<0.01, P<0.05). Conclusion: Mind-refreshing and balance-restoring needling can effectively improve the lower-limb ataxic symptoms and ADL after stroke; when combined with Fenkel exercises, this needling method can produce more significant efficacy.

Objective To observe the therapeutic effect of Tiaowei Heying Xuanbi Prescription combined with conventional western medicine on the recurrence of angina pectoris after percutaneous coronary intervention(PCI)for coronary heart disease(CHD).Methods Ninety patients with recurrent angina pectoris of congealing cold in the heart vessel type after PCI for CHD were randomly divided into a control group and a treatment group,with 45 patients in each group.The control group was treated with conventional western medicine,and the treatment group was additionally treated with Tiaowei Heying Xuanbi Prescription on the basis treatment for the control group.The course of treatment for the two groups covered 12 weeks.The changes of traditional Chinese medicine(TCM)syndrome score,angina efficacy score,Seattle Angina Questionnaire(SAQ)score,left ventricular ejection fraction(LVEF)and interventricular septal thickness(IVS)in the two groups were observed before and after the treatment.And after treatment,the clinical efficacy and safety of the two groups were evaluated.Results(1)Both of the control group and treatment group had one case dropped out during the trial,and eventually 44 patients in each of the two groups were included in the efficacy statistics.(2)After 12 weeks of treatment,the total effective rate of the treatment group was 93.18%(41/44),and that of the control group was 81.82%(36/44).The intergroup comparison(tested by chi-square test)showed that the curative effect of the treatment group was significantly superior to that of the control group(P＜0.05).(3)After treatment,the scores of angina attack score,duration of angina,pain level,and nitroglycerin dosage as well as the overall angina efficacy scores in the two groups were significantly lower than those before treatment(P＜0.05),and the scores of each item as well as overall angina efficacy scores of the treatment group were significantly lower than those of the control group after treatment(P＜0.05).(4)After treatment,the SAQ item scores of physical limitation(PL),angina stability(AS),angina frequency(AF),treatment satisfaction(TS),and disease perception(DS)in the two groups were significantly increased than those before treatment(P＜0.05),and the scores of each SAQ item in the treatment group were significantly higher than those in the control group after treatment(P＜0.05).(5)After treatment,the symptom scores of chest pain,chest tightness,shortness of breath,fatigue,and dark purple lips in both groups were significantly lower than those before treatment(P＜0.05),and the TCM syndrome scores of the treatment group were significantly lower than those of the control group after treatment(P＜0.05).(6)The results of echocardiography showed that after treatment,the LVEF of the two groups of patients was improved compared with that before treatment(P＜0.05),while no obvious change of IVS in the two groups was presented before and after treatment(P＞0.05).The intergroup comparison showed that the improvement of LVEF in the treatment group was significantly superior to that of the control group(P＜0.05).(7)During the treatment,the incidence rate of adverse events in the treatment group was 2.27%(1/44)and that in the control group was 18.18%(8/44),and the difference was not statistically significant when compared between the two groups(P＞0.05).There were no significant changes in the safety indexes associated with the blood,urine,and stool routine test,liver function,renal function,and electrocardiogram in the two groups.Conclusion Tiaowei Heying Xuanbi Prescription combined with conventional western medicine can effectively alleviate the clinical symptoms of patients with recurrent angina pectoris of congealing cold in the heart vessel type after PCI for CHD,and improve the patients'TCM syndromes and left ventricular ejection fraction,with remarkable clinical efficacy.

OBJECTIVETo evaluate the efficacy and toxicity of Yanshu injection (a compound Chinese traditional medicine from Sophora flauescens Ait) combined with concomitant radiochemotherapy in patients with stage III nasopharyngeal carcinoma.

METHODSSixty patients with stage III nasopharyngeal carcinoma were randomized into Yanshu group and control group (n = 30, each). Patients in the Yanshu group received Yanshu injection in addition to intensity modulated radiation therapy (IMRT) and concomitant chemotherapy, and those in the control group were treated with IMRT and concurrent chemotherapy.

RESULTSThe 1-year, 2-year, 3-year and 4-year overall survival rates were 100%, 93.3%, 86.7%, 80.0% for Yanshu group, and 96.7%, 90.0%, 83.3%, 76.7% for the control group, respectively, with no significant difference between the two groups (P = 0.565). The 1-year, 2-year, 3-year and 4-year progression-free survival rates were 96.7%, 90.0%, 83.3%, 70.0% for Yanshu group, and 90.0%, 86.7%, 76.7%, 66.7% for control group, respectively, with no significant difference (P = 0.554). However, the reaction of mucosa of oral cavity, myelosuppression and thrombocytopenia in the Yanshu group were significantly lower than that in the control group (P < 0.05). The quality of life of the patients in the Yanshu group was significantly higher than that in the control group (P < 0.05).

CONCLUSIONSYanshu injection combined with radiochemotherapy in patients with stage III nasopharyngeal carcinoma show a good efficacy and can reduce the side effects of radiochemotherapy of nasopharygeal carcinoma, and improve the quality of life of the patients.

OBJECTIVETo investigate the expression and the role of secreted frizzled-related protein 2 (SFRP2) in the earlobe keloid and find a valid way to treat the keloid with gene therapy.

METHODSThe expression of SFRP2 mRNA and protein was tested with in situ hybridization and Western Blot Analysis method in the different period of earlobe keloid.

RESULTSThe SFRP2 mRNA and protein expression at the keloid edge was significantly high in 12 month group than in 3 or 6 month groups (P < 0.01), but not than in 24 month group. The SFRP2 expression started to decrease in the keloid center 12 month later (P < 0.01). The SFRP2 expression was always higher in edge than in center during all the period (P < 0.05, P < 0.01).

CONCLUSIONSThe results suggest that SFRP2 may play an important role in the development of keloid, especially at the keloid edge. The high SFRP2 expression in endothelial cells and surrounding tissue is also important. It may be a new way for gene therapy of keloid by decreasing the SFRP2 expression.

Objective: To investigate the effects of oxLDL and HMG-CoA reductase inhibitor simvastatin on PKC activity, and level of cytosol ic free Ca 2+ in cultured human umbilical vein endothelial cells. Methods: Th e activity of PKC was determined by its ability to transfer phosphate from 32P-ATP to lysine-rich histone and level of cytosolic free calcium［Ca2+ ］i was measured by flow cytometric analysis loading with the Ca2+ dye F luo-3/Am. Results: oxLDL increased PKC total activity in a dose-de pendent manner and peaked after 12 min, then decreased slowly and maintained for at least 30 min, while oxLDL induced biphasic ［Ca2+］i responses includ ing the rapid initial transient phase and the sustained phase. Removal of extrac ellular Ca2+ did not inhibit the rapid transient phase, but abolished the sustained phase. When simvastatin was added, the activity of PKC wasmarkedly dec reased with no impairment to the initial peak response, but significantly reduce d the sustained phase. Conclusion: oxLDL can induced dynamic changes of signal transduction of PKC and level of cytosolic free Ca2+ in HUVEC, these 2 events are closely linked. The change of rapid initial transient phase i s the result of mobilization of Ca2+ from intracellular pool and the chang e of sustained phase is from the influx of extracellular Ca2+. The inhibit ion of PKC activity induced by simvastatin may contribute to the changes of ［Ca 2+］i.

OBJECTIVETo observe the change trend of the temperature asymmetry coefficient at acupoints between healthy side and affected side in patients with facial paralysis, to study the correlation between the temperature asymmetry and Facial Disability Index (FDI), to provide scientific guidance for the application of infrared thermography in the examination of severity of facial paralysis.

METHODSUsing the infrared thermography to observe the temperature asymmetry at acupoints, the temperature asymmetry coefficient at acupoints between healthy side and affected side was calculated; the correlation between the temperature asymmetry and FDI was analyzed.

RESULTSThe correlation between the temperature asymmetry coefficient and FDI was statistically significant at acupoints of Yang-bai (GB 14), Cuanzhu (BL 2), Dicang (ST 4), Yuyao (EX-HN 4), Quanliao (SI 18), Jiache (ST 6) (P < 0.05, P < 0.01).

CONCLUSIONThe temperature asymmetry coefficient at acupoints between healthy side and affected side could be a scientific measure to evaluate the severity of facial paralysis.

Objective To investigate the effect of microecological therapy on rotavirus ( RV)-induced diarrhea and infants’ immunologic function in infants. Methods 150 infants with RV diarrhea were randomly divided into the ex-perimental group and the control group. The control group was given the treatment of montmorillonite powder. The experimental group was given live combined bifidobacterium and lactobacillus tablets in addition to montmorillonite powder. The clinical efficacy and the improvement of clinical symptoms were compared between the two groups. RV antigen and immunologic function were detected with colloidal gold method. Immune function was detected by flow cytometry and ELISA. Results The total effective rate of the experimental group was 90.7%,and it was significant-ly higher than that of the control group which was 74.7%(P＜0.05). Compared with the control group, the antidi-arrheal time, fever clearance time and disappearance time of vomiting symptoms in the experimental group were shorter ( P＜0.05) . The negative conversion rate of RV virus antigen in the experimental group after 1 and 2 weeks treatment was significantly higher than that in the control group ( P＜0.05) . After 3 weeks treatment, RV virus antigen in the experimental group all turned negative, but the control group still had a positive rate of 12.0% ( P＜0.05) . The peripheral blood CD3+, CD4+and the ratio of CD4+/CD8+in the experimental group after treatment were significantly higher than those before treatment and those in the control group after treatment ( P＜0.05) , the same phenomenon was found in serum immunoglobulin A ( IgA) and IgM ( P＜0.05) . No adverse events occurred during the treatment period in the two groups. Conclusions Microecological therapy has an exact curative effect and high safety in the treatment of infantile RV diarrhea, it could effectively improve the immunologic function of the infants.
