Objective To explore surgical strategy for patients with hilar cholangiocarcinoma (HCC) and study prognostic factors after curative treatment. Methods We retrospectively reviewed medical records of 41 patients with HCC surgically treated in our department during the 9-year period from January 1999 to February 2007. Clinicopathological factors were evaluated for their association with post-operational survival by univariate and multivariate analysis using Cox proportional hazard model. Results All the 41 patients underwent laparotomy following preoperative assessment of extent of disease and 21 patients (resectability rate 51.2%) ultimately underwent resection with curative in-tent. In the resection group, R0 radical resection was possible in 11 patients, while R1 resection in 6and R2 in 4. Different types of hepatectomy were combined to accomplish resection. Meanwhile, por-tal vein wedge resection or reconstruction was needed in two patients. The 1-, 3-and 5-year survival rates were 41.5%, 14.6% and 4.9% in the overall group and 71.3%, 28.6%, 9.5% in the resection group, respectively. In R0-resection, Rl-resection and R2 resection group, the 1-,3-and 5-year sur-vival rates were 81.8% ,45.5% ,18.2% ;66.7% ,16.7% ,0 and 50% ,0,0, respectively. Survival rates after resection were significantly higher than those after palliative drainage and exploratory laparotomy (P<0. 001). Higher survival rates were seen in R0-resected patients when compared with Rl-or R2-resected patients (P<0.001). Multivariate analysis revealed that tumor-free margins, pTNM stage and combined hepatectomy were independent prognostic factors affecting survival. Conclusion Only surgery can provide chance to achieve the possibility of cure and long-term survival. Tumor-free margins, pTNM stage and combined hepatectomy are the most important prognostic factors affecting the survival.

Objective:To evaluate the effects of different enamel conditioners in combination with an adhesive on the shear bond strength and the adhesive remnant index in vitro. Methods:One hundred and eight permanent healthy premolars from orthodontic patients and fresh saliva from a healthy young volunteer were collected. The teeth were cleaned, dried and etched by phosphoric acid, polyacrylic acid or Transbond plus SEP respectively(36 teeth for each agent). Then the teeth were bonded to brachet by RMGICs with the presence of water, saliva on the tooth surface or without any of them respectively(12 teeth in each group). After bonding, all samples were tested on a testing machine of Instron.Shear bond strength and adhesive remnant index were recorded. Results:Highest mean shear bond strength was observed in the group of teeth etched by Traspond plus SEP and bonded by RMGICs with the presence of saliva.Higher mean shear bond strength was obtained with the presence of saliva or water on the tooth surface than without any of them. Conclusion:Transbond plus SEP conditioning followed by RMGICs bonding may produce high shear bond strength between moistured tooth surface and brachet.

Objective:To compare the regional load deflection rate(LDR) of multiloop edgewise arch wire(MEAW) in the individual interbracket span,to understand the mechanical properties of MEAW.Methods:The stainless steel wire of 0.41 mm?0.64 mm was bent with single L-loop of 5 different sizes as described by Kim.The LDR of each L-loop at the individual interbracket span when loading and unloading was measured by using the Instron at a crosshead speed of 1 mm/min and maximum deflection of 1.0 mm at room temperature.Five specimens were tested under each experimental condition to eliminate the possibility of changes in the physical properties of the wires as a result of the stress from repeated measurements. The date were analysed by SPSS 11.0.Results:L-loops of MEAW had different LDRs regionally, except the regional LDRs between the canine and the first premolar and that between the first and second premolar were identical and moderate,a high value for the anterior segments and a low value for the posterior segments, the difference was statistically significant(P

Journal of Practical Stomatology 2016;32(5):712-717
Objective:To assess whether familial occurrence has an influence on the morphological characteristics of patients with nonsyndromic cleft lip and/or palate (NsCL/P).Methods:A case-control analysis was performed on the morphological characteristics of familial group and sporadic group, using medical records of 1967 patients with NsCL/P treated in the Affiliated Stomatological Hospital of Nanchang University from 2002 to 2014.Results:164 (8.34%) cases presented a positive history of cleft in their families. The cleft types, the positive familial rate of cleft lip only (CLO),cleft lip and alveolar ridge(CLA),cleft lip and palate (CLP) and cleft palate only (CPO) were 8.11%,8.54%,6.19% and 9.65% respectively.A positive family history of NsCL/P was associated with 0.66 times risk of CPO (P =0.036,OR =0.66,95% CI 0.44-0.98) compared to those of CLO,CLA and CLP.In familial group of CLP,the lateral incidence of male patients was different from that of female patients (P < 0.001).There was no significant difference between familial group and sporadic group on birth weights, parental child-bearing age and clinical manifestations of patients. Conclusion:Familial occurrence might have an influence on cleft type, laterality and gender of the patients with NSCL/P.

Objective To evaluate the diagnostic value of CT in the spondylolysis of lumbar spine and improve the technique of CT scan. Methods The CT appearances of the spondylolysis of lumbar spine were analyzed in 20 cases. Results CT could demonstrate the spondylolysis and its abnormal features that led to compress nerve root. Conclusion CT scan plays an important role in the diagnosis of the spondylolysis of lumbar spine and in selecting treat methods.Technique of CT scan improved can depict the specific feature of spondylolysis truely.

The purpose of this study is to demonstrate if Gadolinium-enhanced MRI can detect early reversible ischemia of the femoral head epiphysis caused by hip hyper-abduction in piglets. Between 3 and 6 h consistent hyper-abduction, gadolinium-enhanced MRI was performed in 20 femoral heads of 10 piglets. After completion of MRI scan, the piglets were allowed to ambulate freely for 1 or 7 days and re-imaged. The enhanced-MRI results of epiphyseal and physeal cartilage and the secondary center of ossification were observed. MRI appearances and histological findings were compared. On Gadolinium-enhanced MRI, decreased or absent enhancement was seen in 14 cartilaginous epiphyses of all 20 femoral heads. Reperfusion was completed in 10 of 14 femoral heads after one day of ambulation and in the rest 4 after 7 days of ambulation. Gadolinium-enhanced MRI can identify early ischemia and its reversal of the capital femoral epiphysis induced by hip hyper-abduction.
