2.The Comparative Study on CT and MRI of Vertebral Burst Fracture

Objective To evaluate the value of CT and MRI in diagnosis and directing clinical management of vertebral burst fracture.Methods The imaging features of vertebral burst fracture in 66 cases were analyzed,including cervical vertebrae 6,thoracic vertebrae 3,thoracolumbar area 48,and lumbar vertebrae 9 cases.There were 50 male and 16 female,the average age was 35 years.CT and MRI were taken in all patients.Results CT clearly demonstrated the vertebrae and accompanying appendix fracture,spinal canal stenosis and retropulsed fragment.While MRI in showing injury and tear of ligament and intervertebral disc,abnormal signals due to spinal cord compression were superior to CT.Conclusion CT in combination with MRI in diagnosis and directing surgical management is of important value.
3.Clinical Study on Yiqi Gubiao Pill Combined with Pingchuan External Application Ointment for Treatment of Patients with Chronic Obstructive Pulmonary Disease at Stable Stage

Objective To observe efficacy and safety of Yiqi Gubiao Pill combined with Pingchuan External Application Ointment for the treatment of COPD (lung-spleen qi deficiency type) at a stable stage. Methods A randomized, double-blind, and placebo-controlled method was used. Totally 266 cases of patients diagnosed with COPD lung-spleen qi deficiency syndrome were randomly divided into four groups. Experimental Ⅰ group (72 cases) was given Yiqi Gubiao Pill, 10 pills each time, three times a day, orally; at the same time, experimental Ⅰ group was given simulated Pingchuan External Application Ointment (blank medical bag), for Feishu (BL13), danzhong (RN17), Pishu (BL20), Fengmen (BL12), and Dingchuan, external application at 11am each day, external heating stickers, 4-6 h each time, twice a week. Experimental Ⅱ group (64 cases) was given simulated Yiqi Gubiao Pill combined with Pingchuan External Application Ointment. Experimental (64 cases) was givenⅢ Yiqi Gubiao Pill combined with Pingchuan External Application Ointment. Control group (66 cases) was given simulated Yiqi Gubiao Pill combined with simulated Pingchuan External Application Ointment. The treatment lasted for three months, and the follow-up was three months. TCM symptom scores, FEV1%, FVC%, CAT, BODE index, mMRC, and 6MWT were observed. Results After three-month treatment and three-month follow-up, the improvement of TCM symptom scores of the 3 experimental groups was better than the control group (P<0.05). The improvement of CAT, mMRC, BODE index, 6MWT, mMRC in the three treatment groups were better than the control group (P<0.05). The improvement of mMRC and BODE in experimental Ⅰ and Ⅱ groups were better than those of experimental group (Ⅲ P<0.05). After three-month treatment, the improvement of FVC% in the three experimental groups was better than the control group (P<0.05). The improvement of FEV1% in the experimental Ⅱ and groups was better than the control groupⅢ(P<0.05). Conclusion Yiqi Gubiao Pill combined with Pingchuan External Application Ointment has obvious efficacy for the treatment of COPD with lung-spleen qi deficiency type.
4.Effects of Modified Zhisou Power on Epidermal Growth Factor Receptor and Neutrophil Elastase in Rats with Cold-Dryness Chronic Obstructive Pulmonary Disease

Objective To reveal the effects of cold-dryness and modified Zhisou Power on epidermal growth factor receptor (EGFR) and neutrophil elastase (NE) in rats with COPD. Methods COPD model was established with an elastase dose into the trachea combined with exposure to smoking for 90 d; cold-dryness COPD group was further developed by exposure to a cold, dry environment for 90 d. After 90 days, cold-dryness COPD rats was divided into cold-dryness group and Zhisou Power intervention group (treated with modified Zhisou Power for 7 days). On the 97th day, all rats were killed. Pathological changes in lungs were observed. mRNA and protein expression of EGFR were measured by RT-qPCR and Western blot, and the amount of NE in serum and BALF were examined by ELISA. Results EGFR mRNA and protein expression were significantly higher in COPD group, cold-dryness group and Zhisou Power intervention group than in control group. EGFR was significantly lower in Zhisou Power intervention group than in COPD group and cold-dryness group. NE was significantly higher in serum and BALF in COPD group, cold-dryness group and Zhisou Power intervention group than in control group. NE in BALF was significantly higher in cold-dryness group than in COPD group. NE in BALF was significantly lower in Zhisou Power intervention group than in cold-dryness group. Conclusion Modified Zhisou Power can down-regulate the expression of EGFR and the mount of NE in cold-dryness COPD rats to treat COPD.
5.Effect of Modified Zhisou Powder on the lung function of chronic obstructive pulmonary disease model rats of northwest China cold dryness syndrome.

OBJECTIVETo observe Modified Zhisou Powder (MZP) on the lung function of chronic obstructive pulmonary disease (COPD) model rats of northwest China cold dryness syndrome (NCCDS).
METHODSTotally 90 male Wistar rats were randomly divided into three groups, i.e., the normal control group (n =20), the COPD model group (n =35), and the COPD of NCCDS group (n =35). The COPD model was established by tracheal dripping porcine pancreatic elastase (PEE) in combination with fumigation for 90 days. The COPD of NCCDS model was set up by tracheal dripping PEE +fumigation + cold and dry environmental stress for 90 days. Then rats in the COPD of NCCDS were randomly divided into the MZP intervention group (n =11 )and the normal saline intervention group (n =10).All intervention lasted for 15 successive days. The lung function was detected using Small Animal Lung Function Device at day 90 and day 105. And the lung pathology was also observed.
RESULTSLittle amount of sputum sound could be heard in the airway of the COPD model group and the COPD of NCCDS group. Pathological section showed alveolar ectasia, narrowed and broken alveolar septa, forming larger capsular space with infiltration of inflammatory cells. Rats in the COPD of NCCDS group showed chills, increased amount of drinking water, and loose stool. MZP could improve their symptoms. As for lung function test, compared with the normal control group, Te increased in the COPD model group (P <0.01), and EF50 decreased (P<0.05). PEF and EF50 decreased (P <0.01), Ti and Te increased (P <0.01, P <0.05) in the COPD of NCCDS group. Compared with the normal saline intervention group, PEF and EF50 increased (P < 0.01), Ti and Te decreased (P <0.01) in the MZP intervention group.
CONCLUSIONMZP could improve the symptoms of COPD rats of NCCDS, and delay the velocity of decreased lung function.

7.Effects of Propafenone on Action Potential of Rabbit Ventricular Myocytes and the Use-dependent Block of Transient Sodium Current

Objective: To study the effects of propafenone on action potential (AP) of rabbit ventricular myocytes with the tonic block and use-dependent block of transient sodium current (INa-T). Methods: A total of 10 adult New Zealand white rabbits were sacriifced and 10 individual ventricular myocytes were isolated by enzyme digestion method. Microelectrode technologies were used to record AP-related parameters: maximum diastolic potential (MDP), maximum rate of rise of the action potential upstroke (Vmax), action potential amplitude (APA) and action potential duration at 20%, 50% and 90% (APD20, APD50 and APD90).INa-T was measured, I-V curves and peak currents at different frequencies were detected by whole cell patch clamp before and after propafenone perfusion at 10 μmol/L. Results: There was no statistical difference in MDP at before and after propafenone perfusion as (-80 ± 6) mV vs (-82 ± 5) mV,P>0.05. After perfusion, APA was signiifcantly decreased as (95 ± 12) mV vs ( 125 ± 10) mV,P<0.05, the Vmax slowed down as (330 ± 43) V/s vs (420 ± 54) V/s,P<0.05, while APD20, APD50 and APD90 were unchanged as (8 ± 2) ms vs (6 ± 2) ms,P>0.05, (16 ± 3) ms vs (12 ± 3) ms,P>0.05 and (86 ± 14) ms vs (85 ± 12) ms,P>0.05. After propafenone perfusion, I-V curve ofINa-T was shifted upward and the peak current was decreased as (3001 ± 383) pA vs (4193 ± 378) pA, P<0.05. Before perfusion, when stimulated at 0.06 Hz, 1 Hz, 2 Hz, 5 Hz and 10 Hz, there were no signiifcant use-dependent block inINa-T , and no real difference inINa-T between the 10th and 1st pulse,P>0.05. After perfusion, no significant use-dependent block was observed when stimulated at 0.06 Hz and 1 Hz,P>0.05, while at 2 Hz, 5 Hz and 10 Hz, propafenone perfusion demonstrated signiifcant use-dependent block uponINa-T with the inhibition fractions of (22 ± 11)%, (38 ± 14)% and (52 ± 17)% respectively, those were signiifcantly different from the inhibition fractions at either 0.06 Hz or 1Hz,P<0.05. When the inhibition fractions were compared by each 2 conditions, allP<0.05. Conclusion: Propafenone may slow down the Vmax of AP, reduce APA and without the impact on APD; the effects onINa-T is not only in tonic block, but also more obviously in use-dependent block in isolated ventricular myocytes of New Zealand rabbit. Such inlfuences minimized the impact on QT interval and meanwhile, decreased the incidence of brad arrhythmia.
8.The combinative biological reconstruction of bony defect following limb bone tumor resections

Objective To analyze the image and histological results of the combined use of allograft/extracorporeally frozen tumor-bearing bone and vascularized fibular flap for the reconstruction of bony defects following tumor resection,guiding clinical practice.Methods From March 2007 to June 2013,we enrolled 63 patients who had combinative biological reconstruction after bone tumor resection (11 in humerus,22 in femur,21 in tibia,4 in calcaneus).There were 36 male and 27 female in this series.The average age at time of operation was 20 years,ranging from 9 to 48 years.The follow-up ranged from 16 to 102 months with average of 48 months.We investigated the X-ray and CT images for all patients and histological findings of two patients.Patients were assessed functionally with the Musculoskeletal Tumor Society 93 score.Results Three patients with local soft tissue recurrence and one patient with infection underwent amputation.The survival of construct was 93.6%.Bone union achieved in all cases with the average MSTS score of 92.8%.Bone union ranged from 11 to 28 months in allograft group and 9 to 14 months in devitalized tumor bearing bone group.Significant difference of bone union time was found between two groups (Z=-3.638,P=0.000).Viability of the fibular grafts was verified in 58 of 63 patients (92%).Three types of images were observed in complex.Osteopenia and spongy change in fibula were found in 51 patients (81%) with stable fixation of the complex.Five complexes with failed blood supply of fibula and stable fixation revealed no density change of fibula,small amount of callous formation and relative delayed union.In seven complexes (11%) with unstable complex due to patients' incompliance,fibula reacted with dense hypertrophy and microfracture.Fusion of grafts with amount of callus was obviously observed.Union at allograft-host bone junctions occurred by residual host bone-derived external callus and fibular-derived internal callus that bridged the junction and filled the gap between abutting cortices.Callus from fibular graft was mature than that from periosteum of residual host bone.Internal repair was observed at the internal surface of the allografts.Fibula showed significant spongy changes.Conclusion Recycled tumor-bearing bone in combined with fibular flap is a reliable reconstruction as an alternative to traditional Capanna technique.The survival of the fibula is a cornerstone in success of complex reconstruction.Sponginess of fibula and internal repair of allograft compromise the intensity of complex,necessitating the strong instrumentation during reconstruction.
9.Feeding tube placement and postoperative nutritional support for esophageal carcinoma patients

Objective:This study aims to investigate the method and clinical outcomes of feeding tube placement and periopera-tive nutritional support for esophageal carcinoma patients. Methods:A total of 513 esophageal carcinoma patients who have undergone radical resection and reconstruction by a single operating group between January 2012 and December 2013 participated this study. Feed-ing tubes were inserted via the nasal path of 497 cases and by jejunostomy in 16 cases. Early enteral nutrition (EN) was administered through the feeding tubes 24 h postoperatively with a stepwise increase, whereas supplementation of parenteral nutrition (PN) was ter-minated until total EN. Results:Feeding tubes were successfully inserted in all patients during operation. No death or nutritional and metabolic disorders were documented during the observation period. No differences in anastomotic fistula, pulmonary complication, and incision infection were identified between the nasointestinal and jejunostomy groups (P>0.05). A higher incidence of intestinal ob-struction was observed in the jejunostomy group than in the nasointestinal group (P<0.05). Conclusion:Effective placement of nasoin-testinal tube and early enteral feeding are safe and effective methods for patients who have undergone esophagectomy for esophageal carcinoma.
10.The pathological changes of visceral pleura in patients with chronic obstructive pulmonary disease

Objective To observe the pathological change of visceral pleura in patients with chronic obstructive pulmonary disease (COPD), and to discuss the relationship between the changes and COPD airflow limitation. Methods A total of 70 patients received the pulmonary lobectomy or partial resection because of lung tumor in Tianjin Chest Hospital from May 2014 to August 2015 were selected in this study. According to the results of pulmonary function test, the patients were divided into COPD group [forced expiratory volume in one second (FEV1)/ forced vital capacity (FVC) <70%,n=40] and control group (FEV1/FVC≥70%,n=30). The lung tissues, which was not the lesion areas, were used to make tissue sections. The Elastica Van Gieson (EVG) method was used to stain the sections. The thickness of visceral pleural and the proportion of elastic fibers in visceral pleural were observed and calculated under a microscope in the two groups. Results The specimens were derived from upper and middle lobes in 22 cases of COPD group, and from lower lobe in 18 cases. Specimens were derived from upper and middle lobes in 17 cases of control group, and from lower lobe in 13 cases. There were no statistical differences in sampling sites between two groups (χ2=0.019,P>0.05). The visceral pleural thickness and the proportion of elastic fibers in visceral pleural were significantly thinner in COPD group than those of control group ( P<0.01). In both COPD group and control group, visceral pleural thickness was significantly thinner in upper and middle lobes than that of lower lobe (P<0.05), but the proportion of elastic fibers in visceral pleural of upper, middle lobes showed no statistical difference compared with that of the lower lobe (P>0.05). Conclusion The thinner visceral pleural and the reduction of elastic fibers in visceral pleural are one of the causes of expiratory airflow limitation in COPD patients.
