BACKGROUND: In recent years, there are many studies designed to explain the protective effect of lidocaine on brain, but few about the therapeutic effect on traumatic cerebral edema. The content of aquaporin-4(AQP-4) in brain tissue is the highest and it has been proved that AQP-4participants in the formation of cerebral edema induced by cerebral trauma, cerebral infarction, eerebrai tumor and other reasons.OBJECTIVE: To observe the effect of lidocaine on the expression of AQP-4 of experimental rats following brain injury and analyze the therapeutic effect of lidocaine on brain edema.DESIGN: A randomized and control animal experiment.SETTING: Department of Anesthesiology, Affiliated Hospital of Medical College of Qingdao University.MATERIALS: The experiment was carried out in the Institute of Brain Disease, the Medical School Hospital of Qingdao University between January and July 2004. Totally 65 three-month-old healthy male Wistar rats,were enrolled in the experiment and randomly divided into 3 groups: normal control group (n=5), model group (n=30) and treatment group (n=30). Thirty rats in the model group and treatment group were respectively assigned into 6 subgroups according to 6 different time points: 1,4,6,12,24 and 48 hours following brain injury, with 5 rats at each time point.METHODS: Animal models of brain injury at right parietal lobe were created according to the method from Feeney et al. As for the rats of normal control group, they only underwent operation to be injured at the corresponding part. Rats recovered the access to food and water 2 to 8 hours after operation. For the rats in the treatment group, they were intraperitoneally injected of lidocaine at the 1st, 4th,6th, 12th, 24th, and 48th hours following injury, 5 rats at each time point. The initial dosage was 30 mg/kg, then 15 mg/kg was maintained . Administration was conducted every 6 hours in 3 days; For the rats in the model group, they were intraperitoneally injected of 30 mg/kg normal saline and rats in the normal control group were given no special treatments. Water content of brain was calculated 5 days following brain injury with dry and wet weight method:water content of brain=[brain mass (wet)-brain mass (dry)]/brain mass(wet)×100%. Expression of AQP-4 of brain tissue of rats was detected with immunohistochemical method and cytomorphological change of brain tissue was observed under optical microscope.MAIN OUTCOME MEASURES: ① Water content of brain of rats in each group. ② Expression of AQP-4 of brain tissue of rats in each group.③ Pathological results of brain tissue of rats in each group.RESULTS: No rats died accidentally or for other factors in the process of experiment, finally, all the 65 rats entered the stage of result analysis. ①Compared with model group, administration of lidocaine within 6 hours following brain injury could significantly decrease the water content of brain tissue [1 hour after brain injury: (81.09±0.29)%, (83.04±0.25)% ,P < 0.05];[4 hours after brain injury: (81.34±0.35)%, (83.31±0.48)%,P < 0.05] ;[6 hours after brain injury: (82.01±0.21)%, (83.25±0.37)% ,P < 0.05]. Compared with model group, administration of lidocaine could significantly decrease the expression of AQP-4 [1 hour after brain injury:(0.19±0.02), (0.24±0.03),P < 0.05]; [4 hours after brain injury: (0.21±0.05 ), (0.25±0.05) ,P < 0.05]; [6 hours after brain injury: (0.21±0.03 ),(0.24±0.02) ,P < 0.05]. There were no significant differences of AQP-4expression and water content of brain tissue when administration was conducted at the 12th, 24th and 48th hours following brain injury (P > 0.05). ②Under the microscope, AQP-4 positive cells presented vacuolus, they mainly lay in the edema area of peripheral part of trauma, cortex of traumatic side and around the blood vessel as well as astrocyte of white substance, choroid plexus and ependymal layer. ③ Necrosis was found in most cells in the central area of trauma and apoptosis in most cells in the peripheral area. Compared with model group, necrotic and apoptotic cells were significantly less within 6 hours following trauma, but not at the 12th,24th and 48th hours following trauma in the treatment group.CONCLUSION: High dosage of lidocaine can decrease the expression of AQP-4 and lighten cerebral edema following brain injury, but administration should be given as early as possible.
3.Surgical revascularization of patients with chronic total coronary occlusion combined with diffuse distal atherosclerosis

Objective To evaluate the effect of off-pump coronary endarterectomy (CE) plus off-pump coronary artery bypass grafting (off-pump CABG) on patients with chronic total occlusion (CTO) combined with diffuse distal atherosclerosis. Methods From October 2006 to August 2009,65 CTO patients with 176 angiographically confirmed vascular stenosis or occlusive lesions, 70 of which were complete occlusion, underwent off-pump CABG. During the operation, diffuse intimal thickening distal to occlusive lesion was found, and blood flow of the bridges was unfavorable.Results Therefore endarterectomy was performed, followed by CABG. The blood flow in the bridges were 2-10 ml/min versus 14-37 ml/min before versus after endarterectomy. Pulsatility index (PI) was 5.1-15.6 versus less than 5 before versus after endarterectomy. Left ventricular ejection fraction was also improved significantly [before operation: (0.47±0.12)%, after operation: (0. 52±0.15)%, t=2.17, P<0.05]. Peri-operative myocardial infarction occurred in 2 cases, but without significant cardiac homodynamic changes. And 23 patients underwent coronary angiography to evaluate graft patency 3-18 months after operation, all of them had favorable blood flow. Conclusions It is feasible to perform off-pump CABG plus coronary endarterectomy for patients of chronic coronary total occlusion combined with diffuse distal atherosclerosis. This treatment is safe and effective.

Bacterial N-Acyl-L-homoserine lactones (AHLs)-mediated quorum sensing is involved in the regulation of diverse biological functions. As the bacterial pathogen population density increases, AHLs concentration secreted by pathogen reaches a threshold and then interacts with their intercellular receptor and triggers expression of virulence genes. It is a promising approach to biologically control bacterial disease in plants and animals by manipulating bacterial AHL-quorum sensing with AHLs-degrading enzyme and AHL analogue.

Ralstonia metallidurans CH34 was isolated from the deposit of a znic factory .The degradation of phenol by R .metallidurans CH34 was investigated. The results showed that R . metallidurans CH34 possesses high ability to degrade phenol with the biodegradation rate constant of 0.33 . The optimal pH , temperature and volume of medium for phenol degradation are pH 7.0 , 30℃ , and 20%(v/v), respectively . In addition , this strain retains its ability to degrade phenol in the presence of high concentration of heavy metal ion .The sodium citrate , sodium succinate can enhance the degradation of phenol.
6.Inhibition of corneal neovascularization by liposome mediated plasmid encoding endostatin with RGD sequence

Background It has been demonstrated that αvβ3/αvβ5 and α5β1 integrins are overexpressed in neovascular tissue.Consequently,peptide containing the RGD (Arg-Gly-Asp) sequence,which exists in ligands of integrins,is effective in targeting therapeutic reagents to neovascular endothelium.ObjectivePresent study aims to investigate the antiangiogenetive effects of liposome mediated plasmid encoding endostatin with RGD sequence on alkali burn-induced corneal neovascularization (CNV) in rabbits.MethodsCNV models induced by alkali burn were established in 72 eyes of 36 New Zealand white rabbits by putting the filter paper with 1 mol/L NaOH at the central cornea for 20 seconds.The animal models were divided into four groups randomly.0.2mL of liposome and plasmid encoding RGD-ES complex (liposome mediated pCI-RGD-ES injection group),liposome and plasmid encoding ES complex (pCI-ES injection group),liposome and carrier plasmid (pCI) complex (pCI-ES injection group),and PBS were subconjunctivally injected respectively in the models from four groups twice a week for two weeks.The growth status of CNV was observed on day 1,3,7,14 after alkali burn under the slim lamp microscope.Experimental animals were sacrificed on the 3rd,7th and 14th day and the expression of VEGF in CNV was detected by immunohistochemistry.The number of corneal microvessels was counted based on the number of CNV cross-section under the light microscope.ResultsCNV area was significantly smaller in liposome mediated pCI-RGD-ES injection group and pCI-ES injection group compared with PBS group at different time points (P<0.01),but no significant difference was seen in CNV area between pCI-ES injection group and PBS group at different time points (P>0.01).The changes of number of corneal microvessels was followed a similar fashion as the change of CNV area.Expression of VEGF in cornea was obviously stronger in pCI-ES injection group and PBS group than liposome mediated pCI-RGD-ES injection group and pCI-ES injection group.ConclusionEndostatin with RGD sequence could effectively inhibit corneal neovascularization,and liposome is proved to be a potent carrier in gene transfer.
7.Observation on protective effect of acetylcysteine on radiation pneumonitis

Objective To observe the protective effect of acetylcysteine against radiation pneumonia.MethodsTotal of 80 patients who were inoperable were randomly allocated into treatment group and control group.Using conformal radiation technology and the total dose was 65 ~ 75Gy.The patients in treatment group were given acetylcysteine and radiotherapy;the patients in control group were given radiotherapy only.ResultsAll patients were treated radiotherapy.The effective rate( CR and PR) of treatment group was 90%,and that of control group was 85%(P > 0.05);The incidence of acute radiation pneumonia and pulmonary fibrosis in treatment group were 15% and 20%,respectively;and that of control group were 33% and 45% respectively.There was significant difference between the two groups ( P < 0.05 ).ConclusionUsing acetylcysteine during radiotherapy could prevent radiation pneumonia in the non-small cell lung cancer patients.
8.Pharmacokinetic comparison of two ozagrel polymorph forms in SD rats.

To enhance the quality and efficiency of ozagrel by investigating the differences between the ozagrel polymorphs in bioavailability. Solid ozagrel in different polymorph forms were orally administered to SD rats. An HPLC method was established to determinate plasma level of ozagrel. The bioavailabilities of two polymorph forms were calculated and compared. The pharmacokinetic parameters of ozagrel, were as follows: Cmax was 32.72 ± 17.04 and 34.01 ± 19.13 mg · L(-1), respectively; AUC0-t was 61.14 ± 14.76 and 85.56 ± 18.08 mg · L(-1) · h, respectively; t½ was 1.53 ± 0.51 and 4.73 ± 3.00 h, respectively. There was no significant difference in pharmacokinetic parameters between form I and II polymorphs of ozagrel while the t½ of form II is longer, which indicates that the use of form II polymorph as pharmaceutical product may prolong the effective action time in clinics. This would help the polymorph quality control in drug production.

9.Monitoring sub-nanogram amount of acetylspiramycin in human urine using flow injection analysis with chemiluminescence detection.

AIMTo establish a new and simple flow injection method for the rapid determination of acetylspiramycin (ASPM).
METHODSASPM was determined by chemiluminescence (CL) method combined with flow injection (FI) technology, which was based on the inhibitive effect of ASPM on the chemiluminescence reaction of the luminol-K3Fe (CN)6 system.
RESULTSThe decrease of chemiluminescence intensity was proportional to the logarithm of ASPM concentration (0.1-100) microgram.L-1, the detection limit was 40 ng.L-1 (3 sigma). The whole process, including sampling and washing, could be completed in 0.5 min with a RSD less than 3.0% (n = 5).
CONCLUSIONThe FI-CL method is of both high sensitivity and good selectivity giving a throughput of 120 h-1. The proposed method was applied successfully to the determination of ASPM in pharmaceutical preparations and human urine without any pre-treatment. It was found that the ASPM concentration reached its maximum after being orally administrated for two hours.
