3.The 2 nd phase of research for the visual cortical functional mediating stereopsis in children anisometropic amblyopia:evidence from fMRI

AIM:To evaluate the recovery about the visual cortex function of stereopsis in anisometropic amblyopia after regular amblyopia treatment 6, 12 and 18mo with blood oxygenation level dependent - function magnetic resonance imaging techniques ( BOLD-fMRI) .
METHODS: In this study, self-controlled study before and after treatment was used, and blocks-designed fMRI was performed on 11 children which was the first phase of research for amblyopic treatment. Functional MRI data were processed by using SPM8 which based on the Matlab 7. 12. 0. 635. Through the hypothesis drive method, the differences range of activated area in each group were compared by before and after amblyopia treatment matched t-test.
RESULTS: The functional area that was left occipital lobe (BA18), middle occipital gyrus (BA19), limbic lobe (BA19), lingualis gyrus of the right occipital lobe (BA17) and the bilateral parietal lobe ( BA7 ) expanded after amblyopia treatment 6, 12mo, compared those treatment phase, mean t value was 1. 5762, 1. 6856 respectively (P<0. 001). However, the difference of activated intensity was lower after 18mo, mean t value was 1. 1473 (0. 001
CONCLUSION: In children anisometropic amblyopia, the speed of function reconstruction about visual cortical functional mediating stereopsis increase slowly after amblyopia treatment 1a.

5.Staging bilateral CEA perioperative management of blood pressure

Objective To evaluate the perioperative management of blood pressure in patients with bilateral carotid artery severe stenosis underwent staging carotid endarterectomy (CEA).Methods This retrospective study included 31 patients with bilateral carotid stenosis who underwent bilateral revascularizations in our department from April 2012 to November 2015.Patients were recorded with general information,and the changes of blood pressure in preoperative,intraoperative and postoperative were observed,respectively.Regulation and control of blood pressure were performed according to individual patient's condition.Results Thirty one patients underwent a total of 62 consecutive procedures successfully.Postoperative symptoms disappeared or significantly reduced.Twenty three cases (74.2%) had high perfusion symptoms in the ipsilateral CEA,and 5 cases (16.1%) with high perfusion syndrome in the contralateral surgery.Patients were improved after strict control of blood pressure and dehydration reduced intracranial pressure.The high peffusion symptoms in patients were significantly improved or disappeared after 1 week.Doppler ultrasound was used to evaluate the carotid artery before discharge.Conclusions For patients with bilateral carotid stenosis,staging purposes CEA is safe and effective.However,perioperative blood pressure management is very important.Individual control of blood pressure can significantly reduce the risk of postoperative high perfusion and cerebral infarction.Blood pressure control is recommended after 1 week in about 85% of preoperative blood pressure.
6.CDNA cloning of human leptin and its expression.

To clone cDNA of human leptin gene and obtain leptin protein for future study on leptin binding proteins. The cDNA of human leptin with 6 x his-tag was cloned by over-hang extension PCR protocol using human genomic DNA as template, and subcloned into in vitro expression vector pIVEX2.3MCS, and the fusion protein was expressed in vitro by Rapid Translation System (RTS) (RTS500 cycle primer Kit and RTS500 ProteoMaster of Roche company). The apparent molecular weight(19.46 kD) and the immuno-specificity of the fusion protein were confirmed by SDS-PAGE and Western blot, and the expressed fusion protein stayed mainly in the supernatant of the reaction mixture in soluble form. This work provides us solid basis for further study on new leptin-associated proteins.

7.The Clinical Study of Anterior Cruciate Ligament Reconstruction Using Anterior Tibial Muscle Tendons Allograft and Hamstring Tendon Autograft

Objective To compare the clinical results of anterior cruciate ligament(ACL)reconstruction using anterior tibial muscle tendons allograft and hamstring tendons autograft.Methods According to the graft used for ACL reconstruction,60 patients with chronic ruptures of ACL were divided into group A(30 patients,using anterior tibial muscle tendons allograft)and group B(30 patients,using semitendinosus and gracilis tendons autograft).The average follow-up periods after surgery were 29.4 and 31.6 months respectively.Lysholm,Tegner,IKDC clinical rating scales and KT2000 were used for evaluation pre-and post-operation.The muscle strength of the extension,flexion,internal rotation and external rotation of knees was evaluated by the isokinetic tester.Results There were statistically significant differences in Lysholm,Tegner,IKDC clinical rating scales and KT2000 results between pre-and postoperation(P<0.01).However,there were no statistically significant differences between group A and group B after operation(P>0.05).The Isokinetic muscle strength tester showed that the knee muscle strength of the internal rotation and flexion were decreased post-operative in group B,rather than group A,which showed no significant differences between pre-and post.operation(P<0.05).Conclusion For ACL reconstruction under arthroscopy,anterior tibial muscle tendons allograft was a favorable substitute for autograft materials for its convenience,satisfactory clinical effect and less complications.
8.Evaluation of visual quality in patients with meibomian gland dysfunction

Objective To evaluate the optical quality in patients with meibomian gland dysfunction (MGD) by using double-pass optical quality analysis system (OQAS-Ⅱ) for offering the reference data for clinical practices.Methods A total of 50 patients with a diagnosis of MGD,who were divided into MGD with dry eye disease (DED) group (n =22) and MGD without DED group (n =28) according to the results of tear film break-up time (BUT) and Schirmer I test (SIT),and another 25 healthy subjects were enrolled in this study.Then several parameters,including modulation transfer function cutoff (MTF cutoff),Strehl ratio (SR) and objective scattering index (OSI),were measured to evaluate the optical quality by using OQAS-Ⅱ.Additional clinical examination,including meibomian gland and tear film assessment were per formed and compared between the three groups.In addition,a correlation analysis was conducted among OSI,MTF cutoff,SR,BUT,SIT and meibomian gland assessment.Results Statistical differences were approached in MTF cutoff,SR,OSI of the three groups (all P < 0.05).The MTF cutoff and SR in MGD without DED group were significantly lower than those in the control group [(31.36 + 1.83) c · deg-1 vs.(35.87 ± 1.59)c·deg-1,(0.21 +0.02) vs.(0.23 ±0.03)],but the OSI got higher [(0.57+ 0.06) vs.(0.45 ±± 0.06)] (all P < 0.05).The SR and MTF cutoff in MGD patients with DED was significantly lower than those in MGD patients without DED [(27.87 ±± 3.08),c·deg-1 vs.(31.36±±1.83)c· deg-1,(0.16 ±±0.02) vs.(0.21 ± 0.02)],but OSIgot significantly higher [(0.72 ± 0.10) vs.(O.57 ± 0.06)] (all P < 0.05).Among the three groups,OSI,MTF cutoff and SR showed no significant correlation with BUT,SIT and meibomian gland assessment (all P > 0.05).Conclusion There is changes in visual quality parameters in MGD patients by OQAS-Ⅱ,and MGD with DED patients has significant changes than MGD patients without DED.
9.Specific small interfering RNAs inhibit replication of hepatitis B virus.

OBJECTIVETo obtain specific small interfering RNAs (siRNA) for hepatitis B virus (HBV) and evaluate their interfering effect.
METHODSThree siRNAs were transfected into HepG2.2.15 cells, and the amount of HBV mRNA in the cell culture medium was quantified with real-time fluorescence quantitative RT-PCR. HBsAg in the culture media was assayed with Western blotting at different time points after transfection.
RESULTSThe cells transfected with specific siRNAs showed decreased levels of HBV mRNA and HBsAg (P<0.05), but those with nonspecific siRNA transfection as the negative control did not show such changes (P>0.05).
CONCLUSIONSpecific siRNA can significantly inhibit protein expression and mRNA synthesis of HBV in HepG2.2.15 cells in vitro.

10.Abnormalities of meiotic recombination in Han Chinese azoospermic patients.

OBJECTIVETo analyze defective homologous chromosomal recombination in Han Chinese azoospermic patients.
METHODSTesticular biopsy samples from 7 healthy controls and 7 Han Chinese azoospermic patients including 2 obstructive azoospermia (OA group) and 5 non-obstructive azoospermia (NOA group) were analyzed. Immunofluorescence staining was performed to categorize early stage cells at meiosis prophase and to analyze chromosome pairing and recombination of pachytene spermatocyte. Newly developed meiotic proteins antibodies (anti-SCP3, anti-synaptonemal complex proteins 3, anti-MLH1, anti-Mut-L Homolog 1, anti-CREST, chromosome centromere antibody) were used to identify synaptonemal complex (anti-SCP3), recombination sites (anti-MLH1) and centromere (anti-CREST), respectively. Staging of spermatocyte was determined according to SCP3 formation progression. Qualitative data were compared by a Chi-square test, and ANOVA was used to analyze quantitative data.
RESULTSRespectively, 2346 and 2932 spermatocytes were categorized in the controls and azoospermic patients. The proportions of zygotene cells in both OA group and NOA group were significantly higher than that of the control group. Investigation of 1967 pachytene cells from the controls and 354 pachytene cells from azoospermic patients indicated that the mean MLH1 foci per pachytene cell of NOA group was statistically lower than that of the controls. Compared with the controls, incomplete synaptonemal complexes cells (containing gap and/or split) were significantly increased in the NOA group.
CONCLUSIONDelayed meiosis prophase is relatively common in azoospermic patients, and changes in quantity and distribution of recombination foci may be the cause for spermatogenesis arrest in Han Chinese population.
