Objective To understand the homology of clinical isolates from patients with carbapenem-resistant Klebsiella pneumoniae(CRKP)infection and isolates from environment in a medical institution.Methods One CRKP strain isolated from a patient in this hospital and 4 strains of Klebsiella pneumoniae(K.pneumoniae)isolated from patient's surroundings were collected,susceptibility of 5 strains to commonly used antimicrobial agents was detected,production of carbapenems in 5 strains were detected by modified Hodge testing and carbapenem inactivation method(CIM),homology analysis was performed by pulsed-field gel electrophoresis(PFGE).Results Antimicrobial susceptibility testing results showed that 5 strains of K.pneumoniae(1 from patient,4 from the patient's ward surroundings,including hands of nursing aides,solution bottle opening,handle for lifting and dropping bed,and bedrail)were all resistant to other antimicrobial agents except to cephamycin and aminoglycosides.The modified Hodge testing and CIM confirmed that 5 strains all produced carbapenemases;PFGE results showed that electrophoretogram of CRKP isolated from solution bottle opening of ward,bedrail,and handle for lifting and dropping bed were the same as CRKP isolated from patient,while electrophoretogram of CRKP isolated from hands of nursing aides had 2 different bands,there was a close relationship between the strains.Conclusion The same type of CRKP were isolated from patient and his surroundings,it is necessary to implement healthcare-associated infection(HAI)control system,isolate infected patient,and strengthen environmental cleaning and disinfection,so as to avoid the outbreak of HAI.

BACKGROUND: Donor complications have been detected following autologous tendon transplantation for posterior cruciate ligament reconstruction. Although artificial tendon development and tissue-engineered tendon have achieved great progresses, there are some issues in clinical application. Since 1980's, allogenic tendon transplantation has aroused increasing attention. OBJECTIVE: To explore the selection of allogenic tendon materials and the effect of their application on reconstructing posterior cruciate ligament. METHODS: A total of 17 patients with posterior cruciate ligament injury of knee joint were treated with cryopreserved allogenic tendon by Tibial-inlay technique. During the operation, two tracts of tendons soaked in gentamicin saline for 15 minutes were conduplicated, and one end of the tendon was cancellous bone screw and fixed to the tibia attachment point of posterior cruciate ligament, and the other end was introduced into the joint through retention suture. The posterior joint capsule was repaired. The patient was placed at supine position, and the knee was flexed for 90°. The other end of the graft was introduced to femoral tunnel, and anterior drawer was tensed, and fixed by screw. RESULTS AND CONCLUSlN: The preoperative posterior drawer test of patients was >2+, including 7 cases of 3+ and 6 of 4+. The postoperative posterior drawer test was 0 in 4 cases, 1+ in 8 cases, 2+ in 4 cases and 3+ in 1 case, suggesting the posterior movement of the knee joint was significantly improved. Lysholm scores of patients were (48.5±4.3) points before operation and (88.3±5.4) points after operation. Results show that cryopreserved allogenic tendon by Tibial-inlay technique could restored function of posterior cruciate ligament with a favorable effect.

Objective:To explore the improvement effect of tirofiban on the myocardial reperfusion of non-ST segmant elevated myocardial infarction (NSTEMI) patients underwent percutaneous coronary intervention (PCI) treatment.Methods:78 NSTEMI patients underwent PCI in our hospital from April 2012 to April 2015 were selected and divided into the observation group (n=38) and the control group (n=40) according to different drugs.Patients in the control group were given asprin,clopidogrel and heparin,while patients in the observation group were additionally given tirofiban.Then the myocardial contrast echocardiography (MCE) was taken to evluate the myocardial reperfusion.Results:No statistical difference was found in the levels of A,β,A β,CK-MB and cTnⅠ before PCI between 2 groups.The levels of β,A β of observation group were obviously higher,CK-MB and cTnⅠ were obviously lower than those of the control group(P＜0.05).The MACE rate of observation group was 2.63％,which was 5.00％ in the control group,no significant difference was between two groups (P＞0.05).Conclusion:Tirofiban could obviously improve the myocardial reperfusion of NSTEMI patients underwent PCI with high safety.

Objective To evaluate diagnostic method and infection factors for catheter-related bloodstream infec-tion(CRBSI)due to Rhizobium radiobacter (R.radiobacter)in pediatric patients.Methods Clinical data of 1 014 pediatric patients with central venous catheter(CVC)-related treatment in a hospital from February 2012 to February 2014 were collected,semi-quantitative culturing of catheter,colony forming unit (CFU)ratio of catheter to venous blood,difference in culture time between venous blood and catheter were detected among patients with R.radiobact-er infection;factors for R.radiobacter infection were analyzed.Results Of 1 014 pediatric patients who used CVC, 32 were detected R.radiobacter from catheter blood,28 were detected from venous blood,27 were detected from both catheter and venous blood.Catheter semi-quantitative culture of 27 patients were ≥ 15 CFU,5 were< 15 CFU;CFU ratio of catheter to venous blood of 22 patients were ≥5:1,6 were<5:1;difference in culture time be-tween venous blood and catheter of 26 patients were ≥2 hours,2 were<2 hours.Risk factor analysis revealed that child’s age,catheter indwelling time of each group were significantly different (both P <0.05).Conclusion Cathe-ter semi-quantitative culture,CFU ratio of catheter to venous blood,and difference in culture time between venous and catheter blood are important diagnostic basis for confirming R.radiobacter bloodstream infection;patients aged <2 years,catheter indwelling time >5 days are risk factors for R.radiobacter bloodstream infection.

Objective To analyze catheter-related bloodstream infections due to Rhizobium radiation (R.radiobacter) in pediatric patients.Methods Clinical data of 1 014 pediatric patients with intravascular catheterization were collected from February 2012 to February 2014,including age,length of time a catheter remained in place,laboratory findings and outcome of R.radiobacter bloodstream infection in order to explore the factors for R.radiobacter infection.Results There were 26 children contracting R.radiobacter bloodstream infection children,and of them,21 were under 2 years old (80.77％),and the length of time for catheter dwelt in vessel longer than 5 days in 20 children (76.92％).There were significant differences in age under 2 years old,length of time longer than 5 days for catheter remained in place,CD4 and CD4/ CD8 between 26 children with R.radiobacter infection and uninfected children (P ＜ 0.01).The re-infection with different varieties of bacterial strains was found in 12 of all R.radiobacter infection children (46.15％) after treatment,and of them,R.radiobacter bloodstream infection was detected twice in 2 children,and 2 died (7.69％).Conclusions Age under 2 years old,the length of time for intra-vascular catheter remained in place longer than 5 days,and weakened immunity are the important risk factors of R.radiobacter bloodstream infection,and the improper medical care may be the cause of re-infections in pediatric patients with prolonged intra-vascular catheter-dwelling.

Objective To explorer the therapeutic effect of percutaneous vertebroplasty in the treatment of osteoporotic vertebral fracture.Methods Thirty patients with vertebral diseases were treated with percutaneous vertebroplasty;the therapeutic effects were analyzed statistically and retrospectively.Results The vertebral heights,Cobb angles and VAS scores were improved after operation,and 13 cases had marginal cement leakage; 30cases were followed-up for 2 to 24 months with an average of 8 months; patients with thoracolumbar lesions had obvious pain relief during follow-up.Conclu sions Percutaneous vertebroplasty has exact clinical curative effect in the treatment of elderly osteoporotic vertebral fracture.

Objective To observe the effect of three different methods for the treatment of osteoporotic vertebral compressive fractures on bone density.Methods Fifty-three patients with osteoporotic vertebral compressive fractures were retrospectively analyzed.These patients were divided into conservative treatment group,vertebroplasty(PVP)group and balloon kyphoplasty(PKP)group.Quantitative computed tomography measurement of bone density(L2-4)was performed before treatment,after treatment for 3 months,half a year,1 year,1 and half a year.Results Before and after treatment,no significant changes was found in bone density during follow-up in PVP group and PKP group.Bone density was decreased after treatment for 3 months compared with that before treatment,from(86.12 ± 8.21)mg/cm3 to (85.23 ± 8.31)mg/cm3 in PVP group,from(86.32 ± 8.38)mg/cm3 to(84.98 ± 8.26)mg/cm3 in PKP group,but there was no significant difference(P ＞ 0.05),bone density returned to pre-injury level after treatment for half a year.Bone loss was found significantly after treatment for 3 months and half a year follow-up in conservative treatment group,bone density decreased significantly[(74.42 ± 8.36),(76.10 ± 8.31)mg/cm3 vs.(86.87 ±8.27)mg/cm3],and there was significant difference(P ＜0.05),and there was no significant difference between after treatment for 1 year and before treatment(P＞ 0.05).Bone density after treatment for 3 months and half a year in PVP group and PKP group was higher than that in conservative treatment group [(85.23 ±8.31),(84.98 ± 8.26)mg/cm3 vs.(74.42 ± 8.36)mg/cm3 and(86.23 ± 8.05),(86.41 ± 8.17)mg/cm3 vs.(76.10 ± 8.31)mg/cm3],and there was significant difference(P＜ 0.05).There was no significant difference among three groups after treatment for 1 year and 1 and half a year(P ＞ 0.05).Conclusions PVP and PKP are positive treatment of osteoporotic vertebral compression fractures,which could reduce the loss of bone mass and do function exercise early.It could prevent brittle fracture and vertebral compression fracture further aggravated,which are a better clinical treatment methods.
