1.Evaluation on laboratory diagnosis of catheter-related bloodstream infec-tion due to Rhizobium radiobacter in pediatric patients

Objective To evaluate diagnostic method and infection factors for catheter-related bloodstream infec-tion(CRBSI)due to Rhizobium radiobacter (R.radiobacter)in pediatric patients.Methods Clinical data of 1 014 pediatric patients with central venous catheter(CVC)-related treatment in a hospital from February 2012 to February 2014 were collected,semi-quantitative culturing of catheter,colony forming unit (CFU)ratio of catheter to venous blood,difference in culture time between venous blood and catheter were detected among patients with R.radiobact-er infection;factors for R.radiobacter infection were analyzed.Results Of 1 014 pediatric patients who used CVC, 32 were detected R.radiobacter from catheter blood,28 were detected from venous blood,27 were detected from both catheter and venous blood.Catheter semi-quantitative culture of 27 patients were ≥ 15 CFU,5 were< 15 CFU;CFU ratio of catheter to venous blood of 22 patients were ≥5:1,6 were<5:1;difference in culture time be-tween venous blood and catheter of 26 patients were ≥2 hours,2 were<2 hours.Risk factor analysis revealed that child’s age,catheter indwelling time of each group were significantly different (both P <0.05).Conclusion Cathe-ter semi-quantitative culture,CFU ratio of catheter to venous blood,and difference in culture time between venous and catheter blood are important diagnostic basis for confirming R.radiobacter bloodstream infection;patients aged <2 years,catheter indwelling time >5 days are risk factors for R.radiobacter bloodstream infection.
2.Analysis of catheter-related blood infections due to Rhizobium radiation in pediatric patients

Objective To analyze catheter-related bloodstream infections due to Rhizobium radiation (R.radiobacter) in pediatric patients.Methods Clinical data of 1 014 pediatric patients with intravascular catheterization were collected from February 2012 to February 2014,including age,length of time a catheter remained in place,laboratory findings and outcome of R.radiobacter bloodstream infection in order to explore the factors for R.radiobacter infection.Results There were 26 children contracting R.radiobacter bloodstream infection children,and of them,21 were under 2 years old (80.77%),and the length of time for catheter dwelt in vessel longer than 5 days in 20 children (76.92%).There were significant differences in age under 2 years old,length of time longer than 5 days for catheter remained in place,CD4 and CD4/ CD8 between 26 children with R.radiobacter infection and uninfected children (P < 0.01).The re-infection with different varieties of bacterial strains was found in 12 of all R.radiobacter infection children (46.15%) after treatment,and of them,R.radiobacter bloodstream infection was detected twice in 2 children,and 2 died (7.69%).Conclusions Age under 2 years old,the length of time for intra-vascular catheter remained in place longer than 5 days,and weakened immunity are the important risk factors of R.radiobacter bloodstream infection,and the improper medical care may be the cause of re-infections in pediatric patients with prolonged intra-vascular catheter-dwelling.
3.Effective research for systemic intervention to occupation burnout of medical staffs

Objective To study effectiveness of System intervention to occupation burnout of medical staff.Methods Specimens were classified as interventional group and control group by 1:1 partnership rule.Systemic intervention,which came from organization,family and individual,was only applied to interventional group.The objective was to establish requirement and resource mechanism,continually increase response resource,and improve matching state of individual & job.It allowed full play to the first grade prevention(development intervention),the second grade prevention(symptom intervention)and the third grade prevention(burnout intervention).It determined effectiveness of intervention via indexes of occupation burnout,mental health and satisfaction within group or internal groups.Results Before systemic intervention,there was no significant difference between intervention group and control group(P>0.05).After systemic intervention,the scores of burnout(emotion exhaustion(17.54±9.97)vs(14.30±11.87),P<0.01;depersonalization(5.29±5.01)vs(4.63±5.65),P<0.05;low personal accomplishment(6.47±4.89),(5.61±5.10),P<0.01),check list-90(SCL-90),personal satisfaction in intervention group showed apparent difference compared with that in control group.But there was no significant difference in control group before and after intervention.Conclusion On the whole systemic intervention to health care staff obtains obvious achievement,effectively reduces emotional exhaustion,obsessive symptom and terror emotion,and enhances individual satisfaction.

5.Surgical treatment of giant cell tumor of bone around the knee: a multicenter retrospective study

Objective To retrospectively analyze treatment status quo of giant cell tumor of bone around the knee in several institutes,and to investigate risk factors affecting selection of surgical manners,tumor recurrence,and functional outcomes.Methods A total of 222 patients with giant cell tumor of bone around the knee confirmed by pathology,who had undergone surgical treatment in 5 institutes from March 2000 to May 2012,were enrolled in this study.There were 120 males and 102 females,with an average age of 35.5 years.The epidemiology,clinical and radiographic features and risk factors affecting selection of surgical manners,tumor recurrence,and functional outcomes were analyzed.Results Intralesional curettage was performed in 128 patients(57.6%),intralesional curettage combined with partial resection in 13 patients (5.8%),and marginal excision in 79 patients (35.5%).A total of 159 patients were followed up.The local recurrence rate was 19.0% for patients treated with intralesional curettage,8.9% for marginal excision and 0% for intralesional curettage combined with partial resection,and the total local recurrence rate was 14.5%(23/159).The mean duration from primary operation to recurrence was 23.9 months.Univariate analysis indicated that surgical manner was the only factor affecting local recurrence.Pathological fracture,Campanacci grades,Enneking system as well as soft tissue mass had a significant influence on the selection of surgical manners.The mean MSTS score after intralesional curettage was obviously higher than that after marginal excision.Conclusion The surgical manner is the only factor affecting local recurrence and limb function.
6.Transient blocking of both pulmonary artery and veins for surgical treatment of stage Ⅲ central lung cancer

Objective To investigate the feasibility of transient blocking of both pulmonary artery and veins for surgical treat- ment of central lung cancer with stage Ⅲ to preserve the normal pulmonary.Methods Firstly,the relation of the pulmonary artery, the lung neoplasm and the enlarged mediastinal lymph nodes was investigated.If the hilum of lung remained frozen,the pericardium was opened and the pulmonary artery,the upper and lower lobe pulmonary veins were dissected.Then those three vessels were blocked.When the pulmonary cireulation was stopped,bloodless lobectomy and pulmonary artery angioplasty and/or anastomosis were performed.Then the blockers were released,and pulmonary circulation was restored.The time of blocking was(35?15)minutes (16~66 minutes).Results All 20 patients suffer from stage Ⅲ central lung cancer,which' s the hila of lung remained frozen,re- ceived complete resection of the tumor.The normal functioning pulmonary tissue in the 20 patients was preserved instead of pneumone- ctomy.The average amount of bleeding was 256 ml(180~420 ml)during operation.All 20 patients recovered well.Conclusion Transient blocking of both pttlmonary artery and veins for surgical treatment of stage Ⅲ central lung cancer is and innovation in surgical technique,which makes the operation safe and easy.This technique may provide a chance to patients,with poor cardio-pulmonary function.In addition,this technique widens the surgical indications for patients suffering from lung cancer.

9.Pharmacokinetics of genistein in beagle dogs.

AIMTo study the pharmacokinetics of genistein in Beagle dogs.
METHODSGenistein, suspended in 0.5% CMC-Na solution, was orally administered to Beagle dogs at the dose of 5.34 mg.kg-1. At various time intervals, 1.5 mL of blood was drawn from the vein of dogs in their front legs. At the same time, urine and feces were collected. After the collection, the feces were homogenized with physiological saline (to 1 g feces, 10 mL physiological saline were added). The genistein in plasma, urine and homogenized feces was extracted twice by vortexing with 2.0 mL mixture of methyl tert-butyl ether and pentane (8:2). The organic phase was transferred into tubes and evaporated in ventilation cabinet. The residue was dissolved in 50 microL of methanol and 20 microL of the solution was drawn and detected by high-performance liquid chromatography. The pharmacokinetic parameter was calculated by 3P97 software.
RESULTSThe plasma concentration-time curve was fitted to a one-open-compartment model. The peak time was 0.29 h, and the elimination half-life was 0.52 h. After genistein was administered, 10.79% of genistein were excreted from urine and 21.55% from feces within 24 h. It was also found that 13.00% genistein were excreted from urine and 52.46% from feces within 60 h.
CONCLUSIONIt showed that the speed of absorption and elimination of genistein was high in Beagle dog, and genistein was mainly excreted in the form of parent compound in urine and feces.
