1.Assessment of efifcacy of GnRH analogs by LHRH stimulation test in children with central precocious puberty

Objective To evaluate and monitor the efifcacy of GnRH analogs (GnRHa) therapy. Methods Thirty girls with central precocious puberty diagnosied by LHRH stimulation test were treated with GnRHa for 6-24 months. The LHRH stimula-tion test were performed again at 3 months after initiation of therapy and then every 6 months during treatment. The relationship of peark LH and clinical suppressing pubertal (including Turner stage, bone age, grwoth speed) were compared. The monitor effect of peak LH to efifcacy of GnRHa were eveluated. Results Ninety LHRH stimulation tests were performed, only 7 cases were found to have clinical pubertal development. After 6 months treatment, the base LH level of thirty girls (0.48±0.20) IU/L was signiifcantly lower than that before the treatment (0.75±0.35 IU/L) (P=0.000). The correlation coefifcient between base LH and peak LH was 0.62. The best correlation between clinical suppressing pubertal and LHRH stimulation test was achieved when peak LH was less than 2 IU/L (85.7%sensitivity, 100%speciifcity). Conclusions Base LH value can be used in preliminary as-sessment of the efifcacy of GnRHa therapy for girls with central precocious puberty. The peak LH less than 2 IU/L can be as the indicator of treatment efifcacy.

3.The application of scene simulation teaching on cardiopulmonary resuscitation training for the practice students in emergency department

Objective To compare scene simulation teaching method with traditional methods on training in cardiopulmonary resuscitation (CPR) for the practice students. Methods A convenience sample of 62 nursing students in the emergency department of our hospital in 2014 were recruited as the observation group,and 75 practice students in 2013 were recruited as the control group. The observation group used the scene simulation teaching method for students and the control group used the traditional methods. The students′theoretical knowledge, operation skill of CPR and total score of core capability were compared between two groups. Results The theoretical knowledge, operation skill of CPR and total score of core capability in the observation group were (85.23±6.36), (86.90±4.85), (217.98±6.06), significantly higher than those of the control group, which were (75.36±7.77), (82.38±8.84), (209.33±8.91), t= 8.02, 3.60 and 6.50, P<0.01. Conclusions The scene simulation teaching method is an effective form of emergency department training in CPR, help to improve students practice ability and the ability of emergency cardiopulmonary resuscitation.
4.Expression of GABAR1 and NMDAR2B in aged rat cerebral temporal lopes after isoflurane inhalation

Objective To investigate the effects of different concentration and inhalation duration time of isoflurane on cognitive performance and the expression of GABAR1 and NMDAR2B in aged SD rat cerebral temporal lopes.Methods Aged male SD rats (9 months) were randomly divided into control group (n=10) and test group (n=80).The control group received air at room tempreture.Test groups were divided into four groups: group S1 (1.5%-2 h),group S2 (2.5%-2 h),group S3 (1.5%-4 h),group S4 (2.5%-4 h)according to isoflurane concentration and inhalation duration time.Every group was equally divided into two groups and Morris water maze test was performed day 1 and day 7 after isoflurane inhalation.Then the right temporal lobe was gathered and the mRNA transcription and protein expression of GABAR1 and NMDAR2B were detected by RT-PCR and Immunofluorescence technique.Results One day after isoflurane inhalation, accompanied with increased isoflurane concentration and inhalation duration, the spatial memory ability of every test group decreased continually, and the mRNA transcription and protein expression of GABAR1 increased and the mRNA transcription and protein expression of NMDAR2B decreased compared with control group (P<0.01).Seven days after isoflurane inhalation, the spatial memory ability of group S4 decreased, the mRNA transcription and protein expression of both GABAR1 increased, the mRNA transcription and protein expression of NMDAR2B decreased compared with control group and the other test groups (P<0.01).There was no significant difference between the control group and groups S1, S2, S3.Conclusion Continuous inhalation of isoflurane has great effects on spatial memory ability.And impaired spatial memory by isoflurane inhalation of high concentration with long duration is present in a long time.Thoses are related with the mRNA transcription and protein expressions of GABAR1 and NMDAR2B in cerebral temporal lope.
5.Study on the Role of p53 Gene Transfer on Human Glandular Lung Cancer Cell Growth in Nude Mice

Objective: This study was designed to explore the significance and the role of wild-type p53 (wt-p53) gene and mutant p53 gene(mt-p53) transfer on human glandular lung cancer cell growth in nude mice. Methods: wt-p53 gene and mt-p53 gene were transfected and lipofectin-mediated into the human glandular lung cancer cell line GLC-82. And the growth of gene-transfected cell lines were observed in vitro and in vivo. Results: The colony number in the colong-forming experiment and the volume and weight in nude mice were greater in the mf-p53 tranfecting cells group than in the control group. The tumor resulting from the cells transfected with the wt-p53 gene grew more slowly and was smaller than that from control GLC-82 cells. In contrast, the tumor from the cells transfected with the mt-p53 gene grew faster than that produced by cells transfeted with the wt-p53 gene and that produced by control GLC-82 cells. Conclusion: The wild-type p53 gene could inhibit the glandular lung cancer cell growth in nude mice and the mutant p53 gene could enhance the glandular lung cancer cell growth in nude mice.
6.A comparison of calcitonin gene-related peptide effects on coronary flow and cardiac conduction system in the guinea pig

The purpose of this study was to examine the effects of calcitonin gene-related peptide (CGRP) on coronary flow and various parts of cardiac conduction system (CCS) in the isolated guinea pig heart by recording the cardiac electrogram and His bundle activity simultaneously. The coronary flow (CF) was measured and the right atrium was paced at incremental frequencies periodically. Perfusing heart with CGRP at the concentrations of 3~30 nmol/L increased CF strikingly and dose-dependently. CGRP (30 nmol/L) caused a moderate and significant increase in the spontaneous heart rate, but failed to influence the atrioventricular nodal and His bundle-Purkinje fiber conduction time and the longest atrial pacing cycle length inducing 3∶2 Wenckebach and 2∶1 AV nodal conduction. The results indicate that CGRP in the guinea pig heart might act mainly on coronary vessels and then the sinus node, but did not affect the conductivity and refractoriness of the AV conduction system.
7.Effects of MOOC on standardized training of nurses

Objective To evaluate the effects of Massive Open Online Course (MOOC) on standardized training of nurses.Methods The 148 new nurses from August 2015 to July 2016 were selected as the control group, and trained in traditional teaching mode. The 150 new nurses from August 2016 to July 2017 were recruited as the observation group, and trained with MOOC on the basis of traditional teaching mode. The scores of core ability, including the theoretical and operational assessment, critical thinking and clinical nursing were compared between the two groups.Results After one year of training, the scores of theoretical and operational assessment of the observation group were (97.16±1.92) and (96.56±1.87) respectively, which were better than those of the control group, and the difference were statistically significant (t=12.091, 7.895;P<0.001). The scores critical thinking / academic research ability and clinical nursing ability of the observation group were (25.66±5.12) and (25.12±4.42) respectively, which were better than the control group, and the differences were statistically significant (t=2.191, 2.030;P<0.05).Conclusions The application of MOCC with traditional teaching mode on the new nurse standardized training is helpful to improve new nurses' theoretical and operational knowledge, enhance the satisfaction of clinical departments, and to cultivate new nurses critical thinking, ethical practice and other core competencies.
8.Investigation of TCM Syndromes on Maternal Separation in C5 7/BL6 Mice

OBJECTIVE To investigate TCM syndromes on maternal separation in C5 7/BL6 mice.METHODS Neonatal mice were intervened through maternal separation.The effects of maternal separation on spontaneous activities of mice were e-valuated by field test and the effects on immobility times of mice were evaluated by forced swimming test and tail suspension test.Four diagnostic quantitative indicators measurement and quantitative dialectical method were employed to explore the state of Qi,Blood,Yin and Yang in mice.Hypothalamus-pituitary-adrenal(HPA) cortical axis function was observed through adre-nocorticotropic hormone(ACTH)and corticosterone(CORT)determination.The level of energy metabolism was investigated through the measurement of cyclic adenosine monophosphate/cyclic guanosine monophosphate (cAMP/cGMP),ATPase and superoxide dismutase(SOD).RESULTS No statistical significance(P>0.05)was observed in the total path of animal activi-ties between each group in field test.Compared with the control group and MS1 5 group,in the forced swimming test and tail suspension test,the immobility time of mice of MS18 group was significantly increased(P<0.01).The levels of Yang heat, cAMP/cGMP,Na+-K+-ATPase and SOD significantly decreased(P<0.01),while ACTH and CORT significantly increased (P<0.01)of MS18 group,compared to the control group and MS15 group.CONCLUSION Maternal separation shows sig-nificant effects on behavior in mice.The mice exhibit depression-like behavior,upset HPA axis balance and reduce body energy metabolism,which characterized as Yang deficiency symptoms in TCM.
9.Unilateral buccinator myomucosal island flap with double opposing Z-plasty for wider palatal cleft repair.

OBJECTIVETo explore a method to repair larger cleft palate and lengthen soft palate without oral palate raw surface and scar formation, reduce the effect on maxilla and dental arch development.
METHODSA modified double opposing Z-plasty was used to lengthen soft palate and the nasal palate was closed by using large turn-over mucoperiosteal flaps on the oral surface of the junction of the hard palate and soft palate, oral raw surface on the palate was closed by a buccal myomucosal island flap.
RESULTSThirty-six palates have been repaired by this procedure, all of which had satisfactory results without flap necrosis, infection, difficulties in opening mouth and facial nerve injury except two post-operative fistulas. Eight patients were followed up and all display complete velopharyngeal closure.
CONCLUSIONSUsing unilateral buccinator myomucosal island flap with double opposing Z-plasty to repair wider palatal cleft can get a satisfactory soft palate lengthening. At the same time it can avoid bone surface exposing and scar formation; it is a safe and reliable procedure.

10.Treatment of irritable bowel syndrome by Chinese medicine and pharmacy: an analysis of data mining on experiences of experts.

OBJECTIVEThe pathogenesis of irritable bowel syndrome (IBS), a functional gastrointestinal disorder, is poorly understood. Treatment by Chinese medicine and pharmacy (CMP) is superior to treatment by Western medicine (WM). Therefore, we aimed to analyze Chinese medical experts' experiences in treating IBS by data mining.
METHODSBy retrieving related literatures from 1994 to 2012 from CNKI, we chose proved cases and screened effective medical records to establish a database using structural electronic medical case collecting system and data mining. The thinking ways of modern physicians in diagnosis and treatment of IBS were analyzed.
RESULTSTotally 116 medical cases from 46 Chinese medical experts were recruited and analyzed. Results showed that Gan depression and Pi deficiency were most basic syndrome elements. In addition, cold dampness, damp heat, yang deficiency, and Shen deficiency were main elements for the development of diarrhea predominant IBS (IBS-D). Qi stagnation, yin deficiency, qi deficiency, and blood stasis were main elements for the development of constipation predominant IBS (IBS-C). Soothing Gan-qi and strengthening Pi was the most important treatment method. White peony root, prepared atractylodes, prepared licorice, tangerine peel, poria, bupleurum, Radix sileris, lanceolata, and Radix aucklandiae were the most common Chinese herbs used in treating IBS.
CONCLUSIONS(1) Gan depression and Pi deficiency was the main pathogenesis for IBS. Diagnosis and treatment of IBS should be performed clinically according to its development. Tongxie Yao-fang should be used as the basic prescription with modification according to syndrome differentiation. (2) Data mining will have a wide application prospect in studying medical cases.
