1.Application of Defibrillation Techniques in Cardiopulmonary Resuscitation

Objective To investigate the techniques of electrical defibrillation and their application to cardiopulmonary re-suscitation(CPR).Methods On the basis of reviewing the definition of electrical defibrillation,the main defibrillation tech-niques and their developments were introduced,as well as the selection of shock energy.The role that defibrillation played during CPR was defined according to the consensus.Results As an unfashionable technique,monophasic waveforms was inclined to be replaced by present biphasic waveform including biphasic truncated exponential(BTE) waveform and recti-linear biphasic waveform(RBW).The selected shock energy varied with the waveform that a defibrillator made use of,i.e.a monophasic defibrillator required a higher energy level as usual,whereas a biphasic one did not.It was deemed that biphasic technique presented an excellence in less damage to heart in addition to higher rate of successful defibrillation.In the critical care for sudden cardiac arrest with a shockable rhythm,the protocol of "1 shock + 5 cycles of CPR" should be recommended.Conclusion Electrical defibrillation techniques consist mainly of monophasic waveform and biphasic wave-form,between which the latter gains an advantage over the former.On the grounds of modern emergency medicine,defib-rillation has to be performed in combination with CPR.
2.Studies on the Resistance of seed of Ural Licorice(Glycyrrhiza uralensis)

Several resistant characteristics of Glycyrrhiza uralensis seeds were studied. The results showed that hard seed can resist attack from fungi or bacteria. It can endure high temperature for 4h at 80℃ and 10 min at 100℃, but when at 100℃ for 1 h it loses its germinating power. The seeds stored for 4 years do not reduce viability. After long period storage for 13 years,about 60% of the seed still retains its germinating power.
3.Prospective Application of Automated External Defibrillator in Inferior Troops

4.Effect and Mechanism of Cortex Lycii Radicis Extracts on the Proliferation and Apoptosis of Pancreatic Beta Cell

Objective] Observing Cortex Lycii Radicis' effect(Cortex Lycii) on rat insulinoma cells(INS-1) proliferation and apoptosis. To explore the mechanism of Cortex Lycii on Pancreatic Beta Cell Proliferation and apoptosis.[ Methods] After primary culture, cells were randomly divided into blank control group:control,11.1mmol·L-1 glucose,HG,30mmol·L-1 glucose, HG+Cortex Lycii(1g·L-1), HG+Cortex Lycii(2g·L-1), HG+Cortex Lycii(4g·L-1), the survival rate of cells was observed by cell counting kit-8(CCK-8);the apoptosis rate was observed by Annexin V stammg. [Results] ①Compared with HG the better effect of cell proliferation groups of Cortex Lycii(P<0.01), the best is group of Cortex Lycii(2g·L-1)(P<0.05) ②Compared with HG group the higher survival rate is group of Cortex Lycii(P<0.01), the lower apoptosis rate of Cortex Lycii(2g·L-1) compared with Cortex Lycii(1g·L-1). [Conclusion] Cortex Lycii can promote the proliferation of pancreatic beta cell, inhibit the apoptosis to protect the pancreatic beta cell. The optimal concentration is 2g·L-1.
5.Combination of arterial infusion chemotherapy and embolization for the treatment of giant carcinoma of kidney in 32 cases

Objective To explore the efficacy of arterial infusion chemotherapy combined with embolization for the treatment of giant carcinoma of the kidney.Methods Arterial infusion chemotherapy combined with embolization was performed through the renal artery in 32 cases of giant carcinoma of the kidney from April 1990 to April 2003.The chemotherapeutic agents were epirubicin(30~40 mg),mitomycin(20~30 mg),and 5-FU(500 mg).The embolization was conducted with anhydrous alcohol,sodium morrhuate,and lipiodol plus gelatin sponge.Results The arterial infusion and embolization was successfully administered in all the 32 cases.Surgical resection was accomplished in 8 cases 1~3 months after the embolization,and little or no blood supply to the tumor was seen during the surgery.Pathological findings revealed marked necrosis of tumor cells with inflammatory cell infiltration around and fibrous proliferation as well as capillary embolization.The total response rate(CR+PR) was 87.5%(28/32) and the tumor decreased in size in 90.6% of the cases(29/32).The survival rates at 6 months,1-,2-,and 3-year were 78.1%(25/32),68.8%(22/32),46.9%(15/32),and 31.2%(10/32),respectively.The mean survival time was 41.5 months,with the longest time of 84 months.Conclusions Combination of infusion chemotherapy and embolization through the renal artery for the treatment of giant carcinoma of the kidney offers good clinical effects.
6.Application of "Three-step training and Two-part Examination" in Utilization and Training of Defibrillator

Objective To research the applicable ways on training and examination of defibrillator utilization in grassroots units. Methods The use of the "three-step training and two-part examination" mode, that is, three-step training(theoretical instruction, grouped practice and animal experiment), a total of 83 medical officers from inferior troops of PLA were trained for the use of defibrillators, and through passing two-part test(written test and skill test), the trainees were subsequently examined so that the efficacy of the training could be evaluated. Results The trainees can be acquired satisfying achievement in the two-part test after having passed the three-step training. The respectively average scores were 86.7 ?15.9 (written test) and 90.1?14.8 (skill test). Conclusion The "three-step training and two-part test" mode is a successful defib- rillators' training mode for inferior troops.
7.Application of patients with specific behaviors in the facial trauma clinical teaching

Objective To detect the feasibility of patients with specific behaviors in the facial trauma clinical teaching. Method 47 medical students were randomly divided into experimental group ( n = 22 ) and control group(n=25). Students from experimental group at least dealed with 5 ~ 10 facial trauma patients with specific behaviors, while specific behaviors patients in control group were not more than 2. Teaching results were compared. Results There was no significant difference in operation test scores between the two groups ( (84.68 ±3.41 ) vs ( 83.96 ± 3.46 ), P = 0. 476 ). Composite score of teachers in the experimental group was significantly higher than that in control group( (90.09 ± 5.69 ) vs (83.48 ± 5.19), P < 0.01 ). The results of questionnaire survey showed that two in five self-assessment results were higher in the experimental group(P < 0. 01 ). Conclusion Medical students are more appropriately involved in treating special behaviors patients, which can improve the overall quality of students. Teaching with special behaviors patients has good effect, and is feasible.

9.The effect and influence factor analysis ofⅠandⅡ stage DLBCL patients who receive radiotherapy after chemotherapy

Objective:To study the effect and influence factor analysis of DLBCL patient with early stage who receive radiotherapy after chemotherapy.Methods: 374 cases of patients with DLBCL was selected from January 2010 to December 2015 in our hospital. By random number table method, the patients were divided into CHOP group (n=104), R-CHOP+RT group (n=93), R-CHOP group (n=80), CHOP+RT group (n=97). CHOP chemotherapy was given to all patients, 180 patients received radiotherapy after chemotherapy, and 169 patients received rituximab. Survival rates were compared between the 4 groups.Results: The survival rate of R-CHOP group in 12 months, 24 months, 50 months and 100 months were lower than R-CHOP+RT group, but the difference was not statistically significant in twelfth months(x2=2.02,P>0.05). The differences of 24 months, 50 months and 100 months were statistically significant (x2=4.08,x2=4.03,x2=8.79;P<0.05); The survival rate of CHOP group was 12 months and 24 months was higher than CHOP+RT group which the difference was not statistically significant (x2=1.05,x2=0.22;P>0.05); The survival rate of CHOP group in 50 months and 100th months was lower than CHOP+RT, but the difference was no significant difference (x2=1.62,x2=0.03;P>0.05). Smoking index, whether the use of rituximab, the age associated with the survival of patients, the difference was statistically significant.Conclusion: Early DLBCL patients with R-CHOP and radiotherapy combined treatment can be effective in patients with survival, while the use of rituximab chemotherapy, in addition to smoking on the prognosis of patients with serious adverse effects.
10.An exploration of contributing factors to the oocyte utility rate of high responders in an in vitro fertilization cycle

Objective:To figure out the clinical factors contributing to the oocytes utility rate (OUR) of high responders in in vitro fertilization (IVF)-embryo transfer treatment.Methods:OUR was defined by the number of usable embryos for transfer and (or) cryopreservation divided by the number of oocytes retrieved in a freeze-all cycle. The cycles with ≥15 eggs were included from January 2013 to December 2019. Those with OUR at the top 10% (Group A) and the bottom 10% (Group B) were picked and compared for patients′ characteristics, parameters relating to ovary stimulation and pregnant outcomes. Multifactorial logistic regression was applied to reveal the risk factors affecting OUR in them.Results:A total of 43 patients were included in Group A (OUR: 77.4%, 601/776) and 47 for Group B (OUR: 11.9%, 104/874). Previous IVF/intracytoplasmic sperm injection (ICSI; OR=0.10, 95% CI: 0.01-0.81) and endometriosis ( OR=0.16, 95% CI: 0.03-0.84) were negative factors for OUR ( P<0.05); dual suppression protocol ( OR=3.74, 95% CI: 1.06-26.86) and longer days of stimulation ( OR=3.24, 95% CI: 1.25-8.42) were protective factors in terms of ovarian stimulation ( P<0.05), on contrary to that, any decline of estradiol during the stimulation led to poorer OUR ( OR=0.16, 95% CI: 0.04-0.64). Although two groups had similar quantities of eggs and metaphase of meiosis Ⅱ (MⅡ) oocytes, distinguished cumulative clinical pregnancy rate and cumulative live birth rate were seen in Group A and group B respectively [95.3% (41/43) vs 40.4% (19/47) and 90.7% (39/43) vs 31.9% (15/47), all P<0.01]. Conclusions:High responders with attempted IVF/ICSI and endometriosis should be considered as risk factors for OUR. Over ovarian stimulation, dual suppression and a slightly longer stimulating duration could be tried; besides, estradiol decline should be prevented for a better OUR.
