1.A clinical study on osseous regeneration in the jaw defects using a composite of coral and bone marrow

Objectives:To evaluate the efficiency of packing jaw defects with a composite of coral and bone marrow. Methods:Twelve patients (9 with jaw cyst and 3 with ameloblastoma) underwent enucleation of jaw lesion and packing with a composite of coral and bone marrow.Repair of the bone defects was evaluated at 1 week,1,6 and 12 months postoperatively by clinical examination and X-ray films. Results:Wound healing after the operations on 10 patients was uneventful,and definite ossification around the implanted material could be detected at 1 month postoperatively.A lot of bone formation and partial resorption of coral were observed at 6 months postoperatively.Complete resorption of coral and complete bone repair were obtained at 12 months postoperatively.Wound breakdown was observed on two other patients,and the composite had to be removed completely. Conclusions:A composite of coral and bone marrow may enhances bone healing in jaw defects after cyst or ameloblastoma removal.
2.Three-dimensionally printed porous beta-tricalcium phosphate scaffold loading poly(lactic-co-glycolic acid)/anti-tuberculosis drug control-release microspheres:a cytotoxic evaluation

BACKGROUND: So far there is a lack of reliable biomedical evidence about the effects of three-dimensional y (3D) printed porous β-tricalcium phosphate (β-TCP) scaffold loading poly(lactic-co-glycolic acid) (PLGA)/anti-tuberculosis drug control-release microspheres on the growth and proliferation of cel s, especial y osteoblasts. OBJECTIVE: To construct porous β-TCP scaffold loading PLGA/anti-tuberculosis drug control-release microspheres by 3D printing technology and to detect its cytotoxicity. METHODS: Twenty porous β-TCP scaffolds whose aperture was 400 μm were prepared by 3D printing technology. Ten of these scaffolds were randomly selected for loading PLGA/anti-tuberculosis drug control-release microspheres, and the others were without any drugs. Then the extracts from two groups were cultured with osteoblasts for 72 hours. Afterwards, cel morphology was observed by inverted phase contrast microscope, and the absorbance value was detected using cel counting kit-8 assay. Besides, the relative growth rate of osteoblasts was calculated to evaluate the cytotoxicity of the scaffold. RESULTS AND CONCLUSION: The drug-loaded scaffold exhibited with moderate size, regular structure and uniform pores. Within 72 hours of culture in the extracts from the drug-loaded scaffold, elongated or fusiform osteoblasts appeared, with less karyopycnosis. Moreover, the drug-loaded scaffold showed slight cytotoxicity, which was classified as grade 1. In conclusion, the 3D-pinted porous β-TCP scaffold loading PLGA/anti-tuberculosis drug control-release microspheres exhibits no obvious cytotoxicity.

Objective: To study stress distribution and displacement by different distract directioos on mandibular corpus with finite element method. Methods:A three-dimensional finite element model of mandibular distraction osteogenesis was established.Von Mises stress as well as the displac ement of gnathion and gonion under different loads were measured. Result s:The stress and displacement were positively related with distract forc e. Von Mises stress primarily accumulated in distracted areas and were higher un der bilateral distraction parallel to the occlusial plane. Gnathion and gonion w ere positively displaced on X and Z axes and negatively displaced on Y axis. Distraction parallel to the mandibular body induced positive displaceme nt of mandible on X and Y axes and negative on Z axis. Concl usion:Transversal displacement to the opposite side is greater during un ilateral distraction, while displacement on the saggittal plane is greater durin g bilateral distraction.
4.Cholesterol granuloma of the maxillary sinus

Objective:To investigate the etiololgy and pathogenesis of cholesterol granuloma of the marillary sinus. Method:Five patients with cholesterol granuloma of the maxillary sinus were analysed retrospectively. Result: The cholesterol granuloma was a particular form of tissue reactions of granulation. The clinical symptoms of the disease was short of specificity. Conclusion; The pathogenesis of cholesterol granuloma includes hyperlipemia, obstruction of airfilled cavity ventilation and impaired drainage. The final diagnosis was based on pathologic examination. The treatment should begin with the intervention and treatment of hyperilpemia. Radical cure can be a-chieved by operation.
5.A clinical analysis of 43 patients with mucoepidermoid carcinoma of the salivary gland

Objectives: To study the histopathological grading and the clinical-staging system on the surgical prognosis in patients with mucoepidermoid carcinoma(MEC) of the salivary glands. Methods: Analysis were made to the primary sites, clinical findings and the correlation between the histopathological-grading/clinical-staging system and the surgical prognosis in 43 patients with pathological evidence of MEC. Results: Recurrence or regional lymph node metastasis were diagnosed in 6 of the 36 MEC patients, among which 4 were well differentiated and 2 were poorly differentiated, 1 was in stage Ⅰ to Ⅱ, and 5 were in the stage of Ⅲ or Ⅳ, there were 23 survivors. Thirteen of the 36 patients died, 4 of whom from MEC and 9 from reasons not related to MEC. All the died were in stage Ⅲ or Ⅳ. Conclusions: Recurrence or metastasis of MEC correlates with clinical-staging, but not related to the histopathological grading, this suggested that ablative surgery of the primary tumor and dissection of the regional lymphatic system are required in cases of Ⅲ and Ⅳstage MEC. Postoperative irradiation is proposed in the event of inadequate resection.
6.Experimental study on induction of allo-hyporesponsiveness by ICOS-Ig in vitro and in vivo ZHANG

Objective To express human inducible eostimulator (ICOS) extracellular region and IgG Fc fusion protein, and analyze their function in allogenie lymphocyte proliferation in vitro and in vivo. Methods Human ICOS extraeellular region and IgG Fc fragment were cloned into a soluble expression vector. ICOS-Ig fusion protein was expressed and purified in CHO cells. To monitor primary MLR, Balb/c spleen T cells were isolated as responder cells, and irradiated C57BL/6 spleen cells as stimulator cells. 50 μg/ml ICOS-Ig or IgG was added to primary MLR cultures. The cells responsive rates were detected by 3 H-TdR methods. ELISA tested supernatants for eytokines (IL-2,IL-4, IL-10 and IFN-γ). T cells of each group in primary MLR were cultured as responder cells for secondary MLR, and irradiated C57BL/6 (donor) or C3H (third party) spleen cells as stimulator cells. Similar indexes were detected in secondary MLR. Then vital dye CFSE was used to study alloreactive T cell proliferation in vivo. CFSE-labeled C57BL/6 spleen cells were transferred to irradiated Balb/c mice. Mice were then intraperitoneally injected with 0. 2 mg IgG, ICOS-Ig or CsA each day.At the 3rd day after transfection, the spleen cells of the mice were harvested to detect CD4+ CFSE+ and CD8+ CFSE+ by FACS. Results In primary MLR, ICOS-Ig inhibited allogenic T-cell proliferation with inhibition rate being (58 ± 8)% in 50 μg/ml, and increased IFN-γ secretion. In secondary MLR, ICOS-Ig specifically inhibited the proliferation of donor spleen cells with inhibition rate being (42±8)%, and in ICOS-Ig group the levels of IL-4 and IL-10 were lower and the level of IFN-γ higher than in IgG group. However, ICOS-Ig didn't inhibit the proliferation of third-party spleen cells. In the CFSE dye assay, CFSE intensity of CD4+ and CD8+ T cells in ICOS-Ig and CsA groups was stronger than that in control group (P < 0. 05), while CFSE intensity in combined treatment group were even stronger than that in ICOS-Ig and CsA groups (P<0. 05). Conclusion ICOS-Ig could inhibit allo-reactive T cell proliferation in vitro and in vivo, and induce donor-specific T cell hyporesponsiveness specifically.
7.The induction and elimination of bacteria's resistance

AIM Objective By comparing E.coli's resistance to antibiotics and Chinese medical herbs before and after incubated in LB containing Neomycin or Chinese medical herbs, we try to understand the difference in their mechanism of resistance. METHODS We incubated E.coli cells under the culturing media with low concentration of Neomycin and Chinese medicine herbs respectively; lately, the culturing process were continued under the media with no Neomycin or Chinese medicine herbs. We tested the minimal inhibitory concentrations(MIC)of Neomycin(Kanamycin, Gentamicin, Tetracyclin etc) and Chinese medical herbs to culture cells. Result The experimental data indicated that increased resistance of population cells in the broth including Neomycin happened easily, as well as multi resistance simultaneously. As for the Chinese medical herbs, the case is different. After growing in the LB containing one of Huang Lian (rhizoma coptidis), Berberine or San-Huang-Tang of low concentrations, population cells did not increase their resistance either to antibiotics or to Chinese medical herbs. Removing the pressure of Neomycin, E.coli population cells resistance came back to the initial level. CONCLUSION The increased antibiotic resistance of population cells is unstable, and alternative using of different antibiotics maybe contribute to the alleviation of antibiotics resistance.
8.High-energy extracorporeal shock waves in treatment of nonunion and delayed union:preliminary results

Objective To observe the effects of the Ossatron high-energy shock waves on bone nonunion and delayed union.Methods Between October 2004 and October 2005,18 patients with nonunion and four patients with delayed union were treated with high-energy extracorporeal shock wave therapy(Ossatron,HMT Co., Swiss).Facture lines were located with C-arm X-ray before treatment.Parameters of shock waves were decided according to fracture sites.Patients with unstable fixation received plaster or bracing for 6 to 8 weeks after treatment. After therapy,follow-up radiography was performed at 6 and 12 weeks and monthly afterwards until the fracture united. Results The mean follow-up was 6.7 months(range,3 to 12 months).There were no complications except local petechial hemorrhage and local swelling in 10 patients.Thirteen patients(59.1%)got bony union,including 12 patients(66.7% )with stable internal fixation,and the average healing time was 4.3 months(range,6 weeks to 6 months).Nine fractures(40.9%)still suffered nonunion.Conclusion High-energy shock waves are a good alternative therapy for nonunion and delayed union,with obvious advantages of being non-invasive and safe.
9.Expression of p53, p21, and PCNA and its correlation with recur-rence in the negative surgical margin of early-stage laryngeal can-cer

Objective:To explore the correlation among the expression of p53, p21, and PCNA in the negative surgical margin of early laryngeal cancer and its relationship with local recurrence of the cancer. Methods:Data of 92 patients with early laryngeal cancer admitted to the Tangshan Union Hospital from 2004 January to 2010 December were selected. Immunohistochemical method was used to detect the expression of p53, p21, and PCNA in the negative surgical margin tissues of cancer. All patients were followed up for two years to observe survival status and tumor recurrence. Results:The expression of P53, p21, and PCNA had no differences in the cancer tissues in terms of grade and stage. Two years after the surgery, 16 of the 92 patients with laryngeal cancer presented recurrence with a rate of 17.39%. The recurrence rates of expression of p53, p21 and PCNA protein in the positive cut edge were 50.00%, 34.21%and 33.33%, significantly higher than those of negative ones (8.33%, 5.56%and 9.68%). The positive rates of the three proteins were higher in the recurrence specimen than in the non-recurrence while there was a significant correlation between expression of PCNA and p21 (P<0.01). The expression of both p21 and p53 in laryngeal carcinoma was also significantly correlated with the negative margin. Con-clusion:The expression of p53, p21, and PCNA in the surgical margin of early laryngeal cancer are valuable biological markers for the prognosis of these patients. Thus, combined detection of p53, p21, and PCNA in laryngeal margin tissues can be used as a predictive in-dicator of the recurrence.

Objective To understand laboratory diagnosis and clinical characteristics of human immunodeficiency virus (HIV ) infection and AIDS patients in three comprehensive hospitals .Methods Laboratory diagnosis and clinical characteristics dates of people living with HIV/AIDS patients consulted in Chongqing Emergency Medical Center between 2007 to 2013 were retrospective‐ly analyzed .Results Totally 47 355 cases were carried HIV antibody screening during 7 years ,179 cases of HIV antibody were positive in preliminary screening ,171 cases were confirmed as positive .Among 5 cases of HIV antibodies result unconfirmed ,2 ca‐ses were followed up ,1 case was ruled out HIV infection ,1 case was converted to HIV antibody positive .People living with HIV al‐ways merging double or multiple infection with hepatitis b virus (HBV) ,hepatitis c virus (HCV) and treponema pallidum (TP) and so on .People living with HIV aged from 18 to 86 years old ,9 .36% was over 60 years old .Most patient has two or more clinical manifestations when consulted a doctor .Conclusion There were false‐positive of HIV antibody preliminary screening ,HIV anti‐body positive results must be confirmed by Western blot confirmatory test .Uncertainty of HIV antibody results should be judged by regularly follow‐up or combining with other detection methods ,epidemiological data .Routine HIV antibody screening should be adopt for HBV ,HCV and TP infection .Elder patients should not be ignored .Clinical specificity of HIV/AIDS is not strong ,it is need to be valued and identified from other cause similar symptoms of diseases caused by phase identification ,in order to reduce missed diagnosis and misdiagnosis .
