1.Therapeutic Observation of Electroacupuncture and Prucalopride Succinate Tablets for Functional Constipation

ObjectiveTo observe the clinical efficacies of electroacupuncture and Prucalopride succinate tablets in treating functional constipation.MethodForty patients were randomized into two groups, 20 cases in each group. The treatment group was intervened by electroacupuncture, and the control group was byoral administration of Prucalopride succinate tablets. The major symptoms scores, stool shape score, serum substance P (SP), and somotostatin (SS) were observed.ResultAfter treatment, there were no significant inter-group differences in comparing the changes of defecation frequency and difficulty scores (P>0.05), while there were significant inter-group differences in comparing the changes of incomplete bowel evacuation sensation, abdominal pain, and stool shape scores (P<0.05); the serum SP and SS levels were significantly changed after treatment in both groups (P<0.05); the total effective rate was 90.0% in the treatment group versus 85.0% in the control group, and the difference was statistically insignificant (P>0.05).ConclusionElectroacupuncture and Prucalopride succinate tablets can produce equivalent efficacies in treating functional constipation, and have their own advantages in improving the relevant symptoms. Therefore, treatment should be chosen according to the disease condition.

3.Research and Analysis of 180 Package Inserts of Oral Chemical Prescription Drug

OBJECTIVE:To probe into the quality of the package inserts of oral chemical drugs to lay a foundation for the further standardization of drug package inserts.METHODS:180 package inserts of oral chemical drugs current on market were selected and subjected to inspection in accordance with the Regulatory Guidelines for the Package Inserts of Drugs announced by SFDA as well as other related references to find out the problems existing in the package inserts,and a statistical analysis was conducted on the typical problems.RESULTS:Among the 180 package inserts,109(60.56%)had problems(totaled 167 defects)predominantly in drug name,storage,usage and dosage.CONCLUSION:It is urgent to improve the quality of the package inserts.
4.Research progress in the diagnosis and treatment of adenoid cystic carcinoma of the salivary gland

Adenoid cystic carcinoma(ACC) of the salivary gland constitutes approximately 21.9%-24.0% of all salivary gland malignant tumors,and is described as a tumor with indolent,but persistent and recurrent,growth and late onset of metastases,leading eventually to death.This review focuses on the advances in the studies of the diagnosis,treatment,and prognosis of ACC in the recent years.
5. Preparation and purification of siRNA targeting a proliferation-inducing ligand of pancreatic cancer cell line

Objective: To prepare and purify siRNA targeting a proliferation-inducing ligand targeted (APRIL-siRNA), so as to provide a basis for studying the role of APRIL in human pancreatic cancer. Methods: pET-22b-APRIL was constructed to express APRIL dsRNA of human pancreatic cancer cell line CFPAC-1 in E. coli and the product was purified by chromatography using CF-11 column. APRIL dsRNA was digested by RNase III to prepare APRIL siRNA, then the reaction mixture was loaded onto a DEAE ion exchange chromatography to remove RNase III from oligonucleotides, and size exclusion chromatography was used to purify 21 bp siRNA. The purified APRIL siRNA was used to transfect Chinese hamster ovary (CHO) cells and the expression of APRIL in CHO cells was observed under fluorescence microscope. Results: APRIL dsRNA was successfully expressed in E. coli after IPTG induction and was purified by CF-11 column. dsRNA was hydrolyzed with RNase III and was purified by DEAE ion exchange chromatography and size exclusion chromatography. 15% nondenaturing PAGE and 12% SDS-PAGE confirmed that RNase III was removed from oligonucleotides and 21 bp siRNA was purified with size exclusion chromatography. It was also found that APRIL siRNA obviously depressed APRIL expression in CHO cells. Conclusion: We have successfully constructed APRIL siRNA targeting APRIL gene of CFPAC-1 cells with in vitro transcription, which provides a basis for knock-down of APRIL gene in CFPAC-1 cells.
6.Percutaneous fixation for the treatment of un-displaced scaphoid fractures with herbert cannulated screws through volar approach.

OBJECTIVETo summarize the experience of percutaneous fixation of scaphoid fractures with Herbert screws through volar approach, and provide a reliable and efficient treatment method for scaphoid fractures.
METHODSFrom April 2008 to September 2012,15 patients with scaphoid fractures were treated by percutaneous fixation with Herbert cannulated screws through volar approach including 14 males and 1 female with an average age of 35 years ranging from 25 to 45 years old. Among them, 10 cases were on left side and 5 cases were on right side. Durations from injury to operation ranged from 3 to 10 days with an average of 5 days. All these cases were identified as B2 type according to the Herbert classification based on X-ray and CT scan. In order to assess the function of wrist, all patients were calculated according to Krimmer's score postoperatively.
RESULTSThe follow-up period were from 5 to 18 months with an average of 10 months. At 3 months after operation,X-ray was re-checked every month. All the patients achieved bone union with a mean time of 10 weeks (ranged from 7 to 14 weeks) postoperatively. According to Krimmer's score, 14 cases gained 100 scores as perfect and one case got 90 scores as good. The small volar incision of all patients were union at the first period. No operation complications such as infection and nonunion occurred.
CONCLUSIONPercutaneous fixation with Herbert cannulated screws through volar approach is a reliable and efficient treatment method for scaphoid fractures with small invasion, high bone union rate, and fewer complications.

7.Study on the factors associated with clinical pregnancy rate of in-vitro fertilization in endometriosis related infertility

Objective To evaluate the factors associated with clinical pregnancy rate of in-vitro fertilization (IVF) in endometriosis related infertility.Methods Total of 326 patients with endometriosis related infertility undergoing IVF between January 2007 and December 2011 were studied in Department of Reproductive Medicine,First Affiliated Hospital,Nanjing Medical University,retrospectively,which were divided into 141 cases in clinical pregnancy group and 185 cases in non-pregnancy group.Those factors including age,body mass index (BMI),basic FSH,antral follicle count (AFC),CA125 and CA199,endometriotic stage and history of surgery,stimulation scheme were analyzed by bivariate analysis and multivariable logistic regression.Results (1) Pregnancy rate:total of 141 pregnant cases and 185 nonpregnant cases treated by IVF were observed,pregnancy rate was 43.2% (141/326).(2) Basic parameters:there was no statistical difference in age,BMI,basic FSH,AFC,CA125 and CA199 between clinical pregnancy group and non-pregnancy group (P > 0.05).(3) Bivariate analysis:clinical pregnancy rate of 50.0% (87/174) among patients with infertility year less than five years was significantly higher than 35.5% (54/152) in patients with more than five years.Pregnancy rate of 56.1% (46/82) in stage Ⅰ-Ⅱ was significantly higher than 42.5% (79/186) in stage Ⅲ-Ⅳ.Pregnancy rate of 46.6%(125/268) with history of surgery was significantly higher than 27.6% (16/58) with no history of surgery (P < 0.05).Pregnancy rate of 48.2% (79/164) in long-term scheme was higher than 38.3% (62/162) in short-term scheme,but there was no significant difference (P =0.075).(4) Multivariable logistic regression:clinical pregnancy rate of infertility year with less than 5 years,stage Ⅰ-Ⅱ,history of surgery proved stage Ⅰ-Ⅱ and stage Ⅲ-Ⅳ was significantly higher compared with infertility year more than 5 years,stage Ⅲ-Ⅳ and no history of surgery respectively (adjusted OR and 95% CI:2.003,1.263-3.175; 1.899,1.110-3.248; 3.769,1.802-7.887,P<0.05).Conclusion Factors affecting clinical pregnancy rate of IVF in endometriosis related infertility were infertility year,stage and surgery.
8.Management of bile duct injury in gallbladder bed

Objective To explore the causes for the bile duct injury in gallbladder bed and investigate its diagnosis and management. Methods The data of 37 cases of bile duct injury in the gallbladder bed after cholecystectomy between January 1988 and March 1998 were retrospectively analyzed. Results According to the operative records, 17 among the 37 cases had leakage from Luschka bile duct, 4 from the cholecystohepatic duct and 3 from the right lobular hepatic duct and its branches. The sources of the leakage were not identified in other 17 cases. The injured sites were sutured and drained or drained alone in 30 cases. The other 4 cases without drainage were percutaneously under the ultrasonographic guidance because of an intra abdominal bile collection. The remaining 3 cases were surgically treated again for aggravation of the illness in them. Conclusions During performance of cholecystectomy, surgeons should pay close attention to the bile duct in the gallbladder bed for its vulnerable position. Careful inspection of the gallbladder bed should be a routine procedure after the operation. Once the bile leakage was recognized in the gallbladder bed, intraoperative drainage must be performed.
