OBJECTIVETo investigate the contents of poison in fishes in the Zhoushan Fishery and to analysis its relationship with the degree of ocean pollution.

METHODNine kinds of major fishes in the Zhoushan Fishery were sampled and tested. Canonical correlation was conducted to evaluate the correlation between contents of poison in fishes and the degree of ocean pollution.

RESULTSThe average contents of nickel among 9 kinds of fishes exceeded normal value by 1.177 times. No significantly statistical differences were found between the levels of poison contents in nine kinds of major fishes during 1997 to 2003 in the Zhoushan Fishery (the P values are all less than 0.05), except that the levels of cadmium and hydrargyrum in some fishes (the P values are all more than 0.05). The coefficient of fishes accumulated from different poisons ranged from 101.80 to 71 210.97. The canonical correlation coefficients of the variables of the contents of plumbum, arsenic and benzene hexachloride, chlorophennothane between fishes and ocean were 0.784 and 0.808 respectively. Positive correlation between them was shown (chi(2) = 20.994, P < 0.01; chi(2) = 38.017, P < 0.01).

CONCLUSIONThe sanitary quality indexes of all poisons which had been tested fell in the normal ranges except for nickel. The degrees of poisons accumulated among distinct fishes varied, showing the relationship of medium positive correlation between the poison contents of fishes and the degree of the ocean pollution.

Objective To analyze the clinical characteristics of urinary tract infection after kidney transplantation and to analyze the distribution of pathogenic virulence factors.Methods From January 2014 to June 2016,a total of 255 cases were subjected to DCD renal transplantation in our center,and 45 cases of urinary tract infection occurred after operation.Among them,urinary tract E.coli infection occurred in 32 cases.The virulence factors gene aer,hly,irp2 and iucD in the 32 strains of E.coli isolated from urine were tested using PCR and multiple PCR.Results Thirty-two cases (71.1％) of urinary tract infection after renal transplantation was caused by E.coli,and divided into 9 cases of nosocomial infection group,and 23 cases of community-acquired group.The positive rate for aer,hly,irp2 and iucD in Nosocomial infection group was 66.7％,33.3％,88.9％ and 77.8％,and that in community-acquired group was 30.4％,8.70％,39.1％ and 34.7％,respectively.There were statistically significant differences among four kinds of virulence factors between two groups (P＜0.05).The positive rate of virulence factors was significantly higher in nosocomial infection group than in community-acquired groups.The total resistance rate was 84.38％,and there was significarit difference in multi-drug resistance rate between two groups (P＜0.01).Conclusion E.coli is the main pathogenic bacteria of urinary tract infection after renal transplantation,especially nosocomial infections.The detection rate of virulence factor is high,and the multiple drug resistance is outstanding,so the attention should be paid clinically.

The experience of various countries proves that the hierarchical diagnosis and treatment model can lead to reasonable resources allocation and utilization, however, the waste of China''s disordered treatment model has not been estimated.Nowadays, it is very important to establish a hierarchical diagnosis and treatment system during China''s healthcare system reform.Therefore, it is necessary to estimate the benefit the reform may bring.We take Zhejiang Province as an example, and adopted the system dynamics model to build the health delivery system, to emulate the hierarchical diagnosis and treatment model and the disordered model, and to evaluate the benefit.Results showed that if first option at the primary health institutions increased 10%, it could save 0.251 billion yuan, and if increased 15%, it could save 0.39 billion yuan.If the first option at the primary health institution reached 72.35% and the treatment in county reached 90%, the outpatient in tertiary hospital could reduce by 13 million, and those in primary healthcare institutions could increase by 74 million yuan.The direct healthcare cost could reduce 3.016 billion Yuan.

Objective To evaluate the effectiveness of suberselective uterine arterial embolization for uterine fibroids.Methods Uterine arterial embolization with golyvimylalcohol(PVA) particles or Iodized oil and Gelfoam or Pingyangmycin lipiodol and Gelfoam was performed in 182 patients with uterine fibroids.Results Bilateral and unilateral superselective uterine arterial embolization were performed in 173 cases and 9 cases respectively. 6～28 months (mean 11 months) after the procedure, complete disappearance of tumor(16 cases), an average shinkage of 67% in tumor volume(152 cases) and a mean 42% reduction of uterine volume were obtained in 168 followed-up cases. The clinical symptoms were relieved significantly.The main side effets were hypogastic pain(135/182).Conclusion Superselection uterine arterial embolization is an effective and microinvasive method in treating uterine fibroids.

Objective To analyze the effect of endoscopic ultrasonography guided interstitial iodine 125 seed implantation in treatment of unresectable pancreatic cancer and migration of implanted seeds. Methods A total of 17 patients with unresectale pancreatic cancer underwent endoscopic ultrasono- graphy guided interstitial iodine seeds implantation.The therapeutic plan system was used to calculate the number of implanted seeds before procedure.The patients were followed-up monthly and tumor alteration and migration number or loss of implanted seeds were investigated by abdominal plain film.Results Io- dine 125 seeds were successfully implanted in all patients under endoscopic ultrasonography. Before treatment,the median size of tumor was 5.4cm(ranged from 3.7 to 9.0cm)and the number of implan- ted seeds was 27,with median 14(ranged from 7 to 24)once for 2 times.The mean radioactivity per seed was(0.6894?0.016)mCi.The patients were follow-up of 2 to 14 months(mean 4.8 months). Three months after implantation,partial relief was observed in 5 patients(29.4%),no change in 7 patients(41.2%)and progressing in 5 patients(29.4%).The implanted seeds migrated into spleen in 1 case, into liver in 1 case and into bowel in 3 cases.The seeds loss was found in 4 patients.Conclusions Endoscopic uhrasonography guided interstitial implantation of iodine 125 seeds in treatment of advanced pancreatic cancer may significantly inhibit tumor growth,and attention must be paied on the complications of seeds migration or loss,which will impair the nearby organs because of its radioactivity.

Objective To investigate the value and efficacy of continuous renal replacement therapy(CRRT) for treatment of heat stroke patients complicated by multiple organ dysfunction syndrome(MODS). Methods The clinical data of 19 heat stroke patients complicated by MODS admitted into the hospital in a period from July 15,2010 to August 30,2010 and treated by CRRT were analyzed retrospectively. Continuous venovenous hemofiltation(CVVH) mode was used in all patients and the initial temperature of replacement fluid range was 28℃to 32℃persisting in 2.0 to 2.5 hours and afterward it maintained at 36℃. Prognosis and adverse effect were observed,the patients' body temperature,heart rate(HR),mean arterial pressure(MAP),acute physiology and chronic health evaluationⅡ(APACHEⅡ)scores,oxygenation index(PaO2/FiO2),the levels of serum urea nitrogen(BUN),serum creatinine(SCr), myoglobin(Mb),creatine kinase(CK),alanine aminotransferase(ALT),aspartate aminotransferase(AST)and arterial lactate(Lac)were monitored before and after CRRT treatment. Results Fifteen patients were cured or improved,and 4 died. Compared with those before CRRT treatment,body temperature(℃),HR(bmp),MAP(mm Hg,1 mm Hg=0.133 kPa),APACHEⅡevaluation(score),PaO2/FiO2(mm Hg)were significantly improved(body temperature:36.8±0.2 vs. 41.6±0.3,HR:93.6±10.3 vs. 132.5±11.4,MAP:69.8±9.9 vs. 45.2±7.7,APACHEⅡ:12.3±3.9 vs. 29.6±4.6,PaO2/FiO2:213.6±95.4 vs. 126.5±87.4,all P<0.05);the levels of BUN(mmol/L),SCr(μmol/L), Mb(μg/L),CK(U/L),ALT(U/L),AST(U/L),Lac(mmol/L)were significantly reduced after the treatment(BUN:23.9±5.3 vs. 42.6±5.4,SCr:123±47 vs. 356±51,Mb:201±45 vs. 468±39,CK:217±32 vs. 843±41,ALT:79±36 vs. 894±88,AST:57±28 vs. 867±92,Lac:3.5±2.4 vs. 16.6±3.9,all P<0.05). In the process of the treatment,hemodynamics was stable,and no obvious side effects occurred. Conclusion CRRT treatment can exactly and safely reduce the core body temperature of patients with heat stroke,and it can also effectively eliminate metabolites of BUN,Cr,Mb,etc,ameliorate the inflammatory reaction and supporting the functions of liver,kidneys and other vital organs,thus the treatment is also safe and effective for such patients complicated by MODS.

AIM:To evaluate the recovery about the visual cortex function of stereopsis in anisometropic amblyopia after regular amblyopia treatment 6, 12 and 18mo with blood oxygenation level dependent - function magnetic resonance imaging techniques ( BOLD-fMRI) .
METHODS: In this study, self-controlled study before and after treatment was used, and blocks-designed fMRI was performed on 11 children which was the first phase of research for amblyopic treatment. Functional MRI data were processed by using SPM8 which based on the Matlab 7. 12. 0. 635. Through the hypothesis drive method, the differences range of activated area in each group were compared by before and after amblyopia treatment matched t-test.
RESULTS: The functional area that was left occipital lobe (BA18), middle occipital gyrus (BA19), limbic lobe (BA19), lingualis gyrus of the right occipital lobe (BA17) and the bilateral parietal lobe ( BA7 ) expanded after amblyopia treatment 6, 12mo, compared those treatment phase, mean t value was 1. 5762, 1. 6856 respectively (P<0. 001). However, the difference of activated intensity was lower after 18mo, mean t value was 1. 1473 (0. 001 CONCLUSION: In children anisometropic amblyopia, the speed of function reconstruction about visual cortical functional mediating stereopsis increase slowly after amblyopia treatment 1a.

BACKGROUNDRecently, the new generation of metal-on-metal total hip resurfacing arthroplasty is well known for preserving the proximal femoral bone stock, minimizing the risk of postoperative dislocation using large femoral heads, and expecting low wear of metal-on-metal articulation for longer prosthesis survival. It also has the advantage in biomechanical loading in the proximal femur. The osteoarthritis secondary to developmental dysplasia of the hip (DDH) has been the most common reason for total hip arthroplasty. Most of the patients are young and active, who require improved range of motion of the hip besides relief of the pain, even expect to resume the ability to run and jump after the joint arthroplasty, thus to be allowed an active lifestyle. The objective of the current study was to evaluate the early outcome of resurfacing arthroplasty for the mild DDH cases (Crowe type I and II).

METHODSBetween September 2005 and May 2007, twenty-one consecutive patients (twenty-six hips) with the diagnosis of osteoarthritis secondary to DDH underwent metal-on-metal resurfacing arthroplasty. The average age at the time of surgery was 46.5 years (range, 37-59 years). Six patients (28.6%) were male and fifteen (71.4%) were female. Clinical and radiographic results were observed. The follow-up was performed at 6 weeks, 3, 6, 9 months and then yearly.

RESULTSAll patients were followed for a mean of 18 months (9-29 months). During the follow-up period no complications, such as dislocation of hip joints, infection or femoral neck fracture occurred. The clinical outcomes, as rated with the Harris hip score, improved significantly compared with the preoperative ratings. The mean postoperative Harris hip score was 90.7, compared to 35.5 preoperatively. The radiographic analysis showed that all prostheses were fixed with no radiolucencies. All of the patients who had equal limb lengths preoperatively had equal lengths postoperatively. Of the nine patients with preoperative limb-length discrepancy of 0.8 to 1.2 cm, all regained equal limb length postoperatively. In addition the pain was nearly completely relieved, the range of motion was remarkably improved and no restriction was needed after operation regarding early exercise.

CONCLUSIONSThe new generation of metal-on-metal resurfacing arthroplasty may be a reasonable option for DDH of the Crowe types I and II.
