1.Acute renal failure in systemic lupus erythematosus

Objective To understand the clinical features of acute renal failure(ARF)as the initial presentation of systemic lupus erythematosus(SLE).Methods Eight cases of ARF in SLE from Jan 1995 to Apt 2006 were investigated,descriptive analysis and literature review were performed.Results①The symp- tom of ARF in SLE was mainly oliguria,with severe accompany symptoms and complications.②The level of leucocyte and hemoglobin was low in laboratory tests,also the complement level decreased significantly.The most frequent renal pathology was typeⅣ,Ⅳ+ⅤLN.③Large dose steroid and CTX were the mainstay of treatment.In addition,SCUF,CVVHDF and hemodialysis could be used for lethal conditions.Conclusion ARF can be the first manifestation of SLE and it usually represents more severe disease and more complica- tions.Large dose steroid and CTX can improve prognois.In cases refractory to steroid and if the effect is obso- lete,CTX treatment SCUF,CVVHDF and hemodialysis can be use.
2.Effect of hemodialysis with plasma-based dialysate plus high volume hemofiltration on plasma cytokines in patients with liver failure

Objective To propose a new blood purification modality-hemodialysis with plasma- based dialysate (HD-PBD) plus high volume hemofiltration (HVHF) for patients with liver failure, and to evaluate the effect of this treatment on plasma cytokines.Methods Twelve patients with liver failure were included in this study.All patients received HD-PBD therapy in the first 6 hours,and then were treated with HVHF for 24 hours with the same filter (AV600).The levels of TNF-?,IL-1?, IL-6 and IL-8 in plasma before and after HD-PBD plus HVHF for 6 and 24 hours were examined respectively by ELISA,and changes of clinical parameters were observed at the same time point. Serum bilirubin,total bile acids (TBA),serum ammonia,blood urea nitrogen (BUN) and serum creatinine (Scr) were detected before and after treatment.Arterial blood gas analysis and the concentration of electrolytes were monitored before and after treatment.Results (1)HD-PBD for 6 hours was more effective than HVHF for 24 hours in removal of serum bilirubin and TBA(P<0.05). (2)Serum ammonia,BUN,Ser,arterial blood HCO_3~-,PCO_2,PO_2 and electrolytes did not show significant difference before and after HD-PBD (P>0.05),but these parameters significantly changed before and after HVHF (P<0.05).(3)The average level of serum bilirubin was sharply decreased after HVHF for 24 h following HD-PBD(P<0.05).(4)After HD-PBD plus HVHF,there was a marked reduction of the plasma levels of TNF-?,IL-6 and IL-8.Conclusions HD-PBD plus HVHF,a newly proposed modality for patients with liver failure,can effectively decrease serum bilirubin,TBA,BUN,Scr,ammonia and cytokines,and adjust water-electrolyte as well as acid- alkali balance.It is a low-cost,safe,simple and convenient therapy.
3.Expression of NADPH oxidase subunit p22phox in myocardial infarction rats

AIM: To determine the relevance of NADPH oxidase subunit p22hox and the expression of superoxide anion on ventricular remodeling in myocardial infarction (MI) rats. METHODS: MI of Sprague-Dawley rats were established by left anterior descenting coronary artery ligation. 8 weeks after MI, Doppler echocardiography, hemodynamic study and histomorphometry were performed to analyze the ventricular remodeling. The level of thiobarbituric acid reactive substance in plasma and myocardium were measured, and the distribution of superoxide anion was observed with laser scanning confocal microscope. The expression of p22phox mRNA and protein level was detected by RT-PCR and immunohistochemistry. RESULTS: The left ventricular remodeling was significant in MI rats, also the level of thiobarbituric acid reactive substance increased in the plasma and non-infarcted myocardium. The expressions of p22-phox mRNA and protein levels, and superoxide anion increased in infarcted and non-infarcted myocardium in MI rats. CONCLUSION: Our results suggest that the expression of NADPH oxidase and its derived superoxide anion may take part in left ventricular remodeling through oxidative stresss after MI.
4.Advances in high-throughput transcriptome research of traditional Chinese medicines.

Traditional Chinese medicine is a treasure of Chinese culture, absorbing the wisdom of the Chinese people. Continuous application of new technologies makes traditional Chinese medicine research advance with the times. After several years of development, high-throughput transcriptome study has become a mature research tool in biology. This paper reviewed the advances in medicine transcriptome study, and compared two sequencing platforms, Roche's GS FLX platform and Illumina's HiSeq 2000 platform. Moreover, this paper introduced medicine transcriptome analysis process, with Panax quinquefolius and Lonicera japonica for examples, showing the characteristics of traditional Chinese medicine transcriptome studies. High-throughput transcriptome studies facilitate traditional Chinese medicine research with overall understand of functional genes, give clear elucidation of metabolic pathways, lay molecular foundation for the traditional Chinese medicine research and offer modern interpretation for traditional Chinese medicine theory. However, the current study faces several difficulties, including weak molecular basis, high sequencing cost and staff shortages in data anaysis. In the future, with the development in sequencing technology, the combination of transcriptome and other genomics, such as proteome and metabolome, will lay a solid foundation for the new high-throughput screening and developing model for the traditional Chinese medicine industry.

5.Biocompatibility of rat olfactory ensheathing cells and fibrin glue

BACKGROUND: Fibrin glue has been demonstrated to function as a kind of biomaterial with high quality. It has been used in nerve tissue engineering and proved to be a kind of scaffold for some cells.OBJECTIVE: To explore the biocompatibility of fibrin glue and olfactory ensheathing cells (OECs).DESIGN, TIME AND SETTING: An in vitro control trial based on cytology was performed at the Institute of Neurobiology,Nantong University from August 2007 to February 2008.MATERIALS: Fibrin glue was made of fibrin and catalyst, and OECs derived from rats' olfactory bulb were normally primary-cultured.METHODS: OECs were divided into control (OECs clone spheres were cultured alone) and in fibrin glue (OECs clone spheres were cultured and combined with fibrin glue) groups. After 1 week of culture, the proliferation of OECs were observed by convert microscope and detected by S-100 immunofluorescence histochemical staining.MAIN OUTCOME M EASURES: OECs morphology, cell count, the area of the cell bodies and the perimeter of the cell were determined.RESULTS: OECs could survive, migrate in fibrin glue, and float in the fibrin glue in the lower layer. After 7 days of incubation, cell body exhibited fusiform or triangle, predominantly bipolar or bipolar. The number of the S-100 positive cells was more, and cell bodies were larger in fibrin glue group than control group (P < 0.05). However, there was no obvious difference between two groups in cell perimeter (P > 0.05).CONCLUSION: Fibrin glue has good biocompatibility with OECs, and OECs can survive and migrate in fibrin glue.
6.Investigation of major work stresses and coping styles for psychiatric male nurses in grade Ⅲ class A hospital

Objective To explore the major work stresses and coping styles of psychiatric male nurses.Methods 95 psychiatric male nurses from a grade Ⅲ class A hospital in Beijing were surveyed by Nurse Job Stressors Questionnaires(NJSQ) and Simplified Coping Style Questionnaire (SCSQ).Results There was no significant difference in the total scores of JSQ among psychiatric male nurses with different position titles ( P>0.05).However,statistical differences in the scores of work load and work time allocation between nurses-in-charge group,nurses group and nurse practitioners group[ (12.5±2.5) vs (8.7 ±3.7) vs (9.8 ±3.9) ;F=4.311,P<0.05 ].The scores of married male nurses group in the source of work stress and work time allocation ( 10.6 ± 3.8) were higher than the score of single nurses group (7.8 ± 3.2 ) and showed significant difference (t =-3.198,P<0.05 ).The scores of experienced male nurses group and unexperienced nurses group in the source of work stress were ( 14.9 ± 4.3 ) and ( 13.0 ± 4.3 ) respectively; the scores in work time allocation were ( 10.8 ± 3.6) and ( 8.5 ± 3.8 ) ; the scores in patients care were ( 19.3 ± 7.0) and ( 16.1± 6.8 ) ; the scores in management and relationships were ( 13.3 ± 5.5 ) and ( 10.7 ± 5.5 ).All the scores of experienced group were significantly higher than the unexperienced group ( t =- 2.230,- 3.021,- 2.229,- 2.268,respectively;P < 0.05 ).There was no significant differences found between the score of coping mode and position title,years of work,marital status,education or age.Conclusions Psychiatric male nurses with different position title,work experience and marital status have more work stress.They should learn self-control,release pressure and cope with it actively.
7.Tracing to the source of staphylococcus aureus isolates from ice cream.

OBJECTIVETo investigate the contamination of Staphylococcus aureus isolates in ice cream by phenotypic typing and molecular typing.
METHODSThe Staphylococcus aureus isolates were separated from ice cream, filler, cutter, salves and material. The separated isolates were characterized by drug-resistance, staphylococcal enterotoxin (SEA-E), SE (A-E, G-J) genes and pulsed-field gel electrophoresis (PFGE) types.
RESULTTwo Staphylococcus aureus isolates were separated, one from ice cream, another from cutter. Their characteristics of drug-resistance, staphylococcal enterotoxin (SEA-E), SE (A-E,G-J) genes and PFGE type were the same.
CONCLUSIONThe two Staphylococcus aureus isolates were the same clone. The contaminated Staphylococcus aureus isolates could be traced to the contaminated cutters.

8.Amides from the stems of Uvaria kweichowensis.

Uvaria kweichowensis is a folk nongovernmental herb used to treat cure inflammation and tumour in the Southwest area of China. During the course of our investigation for antitumour agents from the stems of Uvaria kweichowensis, six amides were obtained by means of solvent extraction, chromatography on silica gel and Sephadex LH-20 repeatedly. And their structures were identified as uvariadiamide (1), cepharanone (2), aristololactam A II (3), enterocarpam II (4), aristololactam A Ia (5), and 4,5-dioxodehydroasimilobine (6) on the basis of chemical methods and spectral analyses (EI-MS, 1H NMR, 13C NMR). Among them, compound 1 is a new compound; the other compounds were obtained from this plant for the first time.

9.Evaluation on the performance of China Infectious Disease Automated-alert and Response System(CIDARS)in Zhejiang province

Objective To evaluate the performance of China Infectious Disease Automatedalert and Response System(CIDARS). Methods A retrospective analysis was conducted on data related to the warning signals, the outcome of signal verification, the field investigation of CIDARS,and the emergent events reported through Public Health Emergency Events Surveillance System from July 1,2008 to June 30, 2010 in Zhejiang province. The performance of CIDARS was qualitatively evaluated by indicators on its sensitivity and rote of false alarm. Results In total, 26 446 signals were generated by the system which involving 17 diseases, with an average of 2.83 signals per country per week. Among all the signals, 99.95% of them were responded. 0.90% of the signals were judged as suspected events via the preliminary verification, and 30 outbreaks were finally confirmed by field investigation. The sensitivity of the system was 69.77% with the false alarm rate as 1.39%. Conclusion The system seemed to have worked on the outbreak early warning of infectious diseases and could directly reflect the anomaly event emerged from the infectious disease reporting system.However, more efforts should be paid to the following areas as how to decrease the false positive signals, select suitable thresholds and increase the quality of data in order to enhance the accuracy of the system.
10.Coexistence of Pancreatic Carcinoma and Pancreatic Tuberculosis: Case Report.

Pancreatic tuberculosis (TB) is extremely rare and mimics pancreatic carcinoma both clinically and radiologically. This paper discusses the occurrence of 2 heterogeneous masses located in the head and tail of the pancreas in an adult male. In this patient, laparotomy was performed because of the high suspicion of pancreatic carcinoma. Intraoperative fine needle aspiration biopsy revealed the coexistence of pancreatic carcinoma with pancreatic TB, and a combined resection of the distal pancreas and spleen was successfully performed. Following surgery, the patient received standard chemotherapy for TB. At 7-month follow-up, computed tomography showed resolution of the mass in the pancreatic head. Clinicians must maintain a high index of suspicion for pancreatic TB in patients with pancreatic masses. The coexistence of malignancy and TB should be considered when patients present with multiple pancreatic masses.
