1.The status and analysis of pulmonary function assessment of the elderly in communities of Beijing

Objectives To explore the status and characteristics of changes in lung function in the elderly in communities of Beijing,and to provide reference for improving the prevention and control of chronic lung disease in the elderly.Methods A stratified,segmented and cluster sampling method was used to investigate 1 144 subjects aged 60 years and over in communities of Beijing (Huairou District,Daxing District rural area and Xicheng District).Health status and pulmonary function were assessed,and characteristics of lung functions and their changes were analyzed in the elderly.Results The average vital capacity(VC)was 2.53L,and forced vital capacity(FVC) was 2.29 L,forced expiratory volume in one second (FEV1)was 1.81 L,1 second rate (FEV1/FVC) was 83.42 %.Static vital capacity was decreased in 1/3 of the elderly,and dynamic vital capacity,including FVC and FEV1,was reduced in about half of the elderly.Ratio of decline in lung function in the elderly was higher in male and in urban area than in women and in rural areas.Ratio of decline in each index of pulmonary ventilation function showed a significantly increased tendency along with the ageing.Lung dysfunction types in the elderly included a restricted (23.5%)and obstructive ventilation disorder (19.7%)and mixed ventilation disorder(12.8%).Conclusions The ratio of decline in lung function in the elderly is increased along with aging,and more significantly higher in female and in urban than in male and in countryside.Restrictive ventilation disorder in the elderly is more often in urban areas,while obstructive ventilation disorder occurs mostly in rural areas.Corresponding measures should be adopted for different groups of people.
2.Laparoscopic combined with multi-video debridement in treatment of complicated infectious pancreatic necrosis

Objective:To study the clinical application of laparoscopic combined with multi-video debridement in treatment of complicated infectious pancreatic necrosis (CIPN).Methods:The clinical data of 34 patients with CIPN who were treated at the Department of General Surgery, Xuanwu Hospital, Capital Medical University from August 2017 to December 2019 were retrospectively studied. Based on the different video methods used, these patients were divided into 3 groups: the laparoscopic combined with intraoperative ultrasound group, the laparoscopic combined with choledochoscopy group and the laparoscopic group. The number of operations, operation time, blood loss, postoperative complication rates, mortality rates and total length of hospital stay were compared.Results:There were 13 patients in the laparoscopic combined with intraoperative ultrasound group, with age of (56.4±13.4) years. There were 7 patients in the laparoscopic combined with choledochoscopy group, with age of (48.0±8.4) years. There were 14 patients in the laparoscopic group with age of (51.4±15.6) years. The number of operations of the laparoscopic combined with intraoperative ultrasound group, the laparoscopic combined with choledochoscopy group and the laparoscopic group were (2.2±1.1), (1.6±0.8), (2.9±1.4), respectively. The number of operations of the laparoscopic combined with choledochoscopy group were significantly less than that of the laparoscopic group ( P<0.05), but there were no significant differences among the other groups ( P>0.05). The operation time of the laparoscopic combined with intraoperative ultrasound group, the laparoscopic combined with choledochoscopy group and the laparoscopic group were (70.5±22.9) min, (65.7±19.9) min, (51.5±15.4) min, respectively. The operation time of the laparoscopic combined with intraoperative ultrasound group was significantly longer than that of the laparoscopic group ( P<0.05), but there were no significant differences among the other groups ( P>0.05). There were no differences in blood loss, postoperative complication rate, mortality rates and total lengths of hospital stay among the three groups ( P>0.05). Conclusion:Laparoscopic combined with multi-video debridement after making full use of the advantages of each of the video methods, can be used to improve treatment outcomes of patients with CIPN.
3.Sebaceous carcinoma of the eyelid:a clinicopathological study

Objective To review the clinical and histopathological features,management and outcomes in 67 patients with seba- ceous carcinoma of the eyelid.Design Retrospective case series.Participants Sixty-seven cases of the sebaceous gland carcinoma were retrieved from the ophthalmology archives of Tongren Hospital between 1996 and 2005.Methods Clinical data of all patients were re- viewed from the charts.Pathology specimens were reviewed in all cases.Main Outcome Measure Presenting features,sites of origin, location,differentiation,management,histopathologic findings,and incidence of recurrence in 67 patients.Results A total of 36 female and 31 male patients were identified.The mean age at diagnosis was 59 years (range from 35 to 84).Initial clinical diagnoses elsewhere were sebaceous carcinoma (43.28%),chalazion (25.37%),basal cell carcinoma(7.46%).Initial histopathologic diagnoses were sebaceous carcinoma(79.10%),basal cell carcinoma(5.97%),squamous cell carcinoma(1.49%) and other or not available(4.48%).The initial treat- ment involved excisional techniques with reconstruction in 42 cases,anti-inflammatory in 12 cases,without any treatment in 10 cases. 61.19% cases presented the pagetoid invasion.Local recurrence developed in 58.21% and two died from metastatic sebaceous carcino- ma (8.70%).Regional lymph node excision was performed in 13.04% because of local metastases.Five year survival rate was 94.87%. Conclusions About 1/3 cases had not receive the right treatment at initial diagnosis eyelid.The recurrence rate of patients was still high.
4.Evaluation of Urodynamic Study and Somatosensory Evoked Potentials before and after Operation in Children with Lipomyelomeningocele

Objective To evaluate prourodynamic study and somatosensory evoked potentials(SEPs)before and after operation of children with lipomyelomeningocele(LMMC)and its clinical significance.Methods Urodynamic study(UDS)and SEPs in 31 cases of LMMC who underwent microsurgical release within 1 week preoperatively and 3 months postoperatively were conducted.The 4 parameters used for UDS evaluation were bladder volume,compliance,detrusor activity and residual urine;parameters used for SEPs evaluation were latent period and amplitude.Results There was a statistics significant difference between operation and control group;the similar result was attented in the comparison of before and after operation,also in the comprison of the improved group and the group without improvement after operation.Conclusions UDS and SEPs investigation can provide guidance for the treatment of tethered cord syndrome(TCS)and evaluate clinical prognosis.These cases with the severity changing in UDS and somatosensory shall be avoided unnecessary action in operation.
5.Analysis on SARS‑CoV‑2 contamination in the environment exposure to the SARS‑CoV‑2 cases in Huangpu District of Shanghai in 2022

Abstract: Objective To analyze the detection of SARS‑CoV‑2 in household environment and public place environment of Huangpu District, and describe the feature of SARS‑CoV‑2 contamination in the environment exposure to the infected cases, so as to support the control strategies such as disinfection and health communication. Methods The results of RT-PCR test for the environmental samples exposure to the cases infected by SARS‑CoV‑2 during February 1 to March 31 2022 in Huangpu District of Shanghai were collected as the research data. Pearson χ2 was used to test the significance of the differences between positive rates of SARS‑CoV‑2 contamination. Results From February 1 to March 31, household environment samples had a higher positive rate (6.47%, 234/3 618) of SARS‑CoV‑2 contamination while the public place samples had a lower one (1.22%, 47/3 582) in Huangpu District of Shanghai (χ2=141.908, P<0.01). Among the household buildings, the lane houses of old style representing poorer living condition had the highest positive rates (8.31%, 96/1 155) of SARS‑CoV‑2 contamination while the apartments representing better living condition had the lowest (3.59%, 22/612) (F=5.25,P<0.05). Among the samples from household environment, samples regarding sewerage had the highest positive rates (13.30%,58/436) of SARS‑CoV‑2 contamination, while samples regarding the tool of cooking and sweeping had the lowest (3.10%,17/548) (F=9.84,P<0.01). Among the samples from public place environment, samples regarding entertainment tools had the highest positive rates (13.33%, 2/15) of SARS‑CoV‑2 contamination, while samples regarding the tool of cooking and sweeping had the lowest (0.62%, 4/646) (F=4.22,P<0.01). Conclusion In the environment exposure to the SARS‑CoV‑2 infected cases, the disinfection, ventilation and cleaning should be intensified strictly. SARS‑CoV‑2's surviving in sewage environment should be evaluation dynamically. More health communication should be pushed to people of poorer living condition.
6.Effects of berberine on lipopolysaccharide induced depressive-like behavior and brain neuroinflammation in mice

Objective To observe the effects of berberine on lipopolysaccharide (LPS) induced depressive-like behavior and changes of interleukin-1β (IL-1β) and tumor necrosis factor-α (TNF-α) levels in hippocampus and prefrontal cortex of mice.Methods Fifty male Kunming adult mice were randomly divided into control group,LPS group,low dosage berberine group,middle dosage berberine group and high dosage berberine group (n=10 per group).The mice in the later three groups were respectively administrated 25,50 and 100mg/kg berberine by gavage for 7 days.Then,the later four groups mice were injected LPS intraperitoneally(0.83 mg/kg).Twenty-four hours after LPS injection,the depressive like behavior were observed and level of IL-1β and TNF-α in hippocampus and prefrontal cortex were measured.Results Compared with control group (sucrose intake:(75.7 ± 9.0) %,immobility time:(101.2 ± 29.2) s),the sucrose intake of LPS group((41.0±14.8) %) reduced(t=5.52,P<0.01) and the immobility time ((158.7±31.1) s) in forced swimming test increased(t=4.34,P<0.01).The 50 mg/kg and 100 mg/kg berberine intragastric administration significantly improved sucrose preference ((67.1 ± 15.4) % and (72.0± 13.1) %) compared with that in LPS group (t=4.15,P<0.01;t=4.93,P<0.01));and decreased immobility time in forced swimming test ((119.4±26.3) s and (109.7±21.9)s) compared with that in LPS group((t=2.44,P< 0.05;t=3.69,P<0.01)).The level of IL-1β and TNF-α in hippocampus and prefrontal cortex significantly increased in LPS group compared with those in control group,while significantly decreased in low,middle and high dosages berberine injected mice compared with those in LPS group.Conclusion Berberine can improve LPS induced depressive-like behavior in mice,and these effects may correlate with decreased level of IL-1β and TNF-α in hippocampus and prefrontal cortex.
7.Cerebral microbleeds - prevalence, distribution and risk factors in northeast population without preceding large-area stroke.

BACKGROUNDCerebral microbleeds (CMBs) occur frequently in patients suspected of cerebrovascular disease and they are the principle radiographic findings in patients with sub-clinical neurological impairment. The objective of this study was to assess the prevalence, distribution, severity and associated clinical features of CMBs in a prospective hospital patient based cohort undergoing brain MRI for suspected cerebrovascular disease, excluding cases with known intracranial hemorrhage or prior large-area stroke.
METHODSThe study population consisted of 447 patients who were evaluated with T2*-gradient echo sequences to detect the CMBs lesion number, location, and their association with white matter hyperintensities and clinical parameters, including blood pressure.
RESULTSCMB lesions were presented in 95 of the 447 patients (21.3%). The distribution of CMBs was 43.95% cortical, 19.77% thalamic, 14.41% in the brainstem, 11.58% cerebellar, 6.21% periventricular white matter, 5.64% involving the basal ganglia regions, and 0.28% involving the hippocampus. There was a statistically significant association between the presence of CMBs and advancing age (adjusted OR 2.082, P < 0.01), the severity of hypertension (adjusted OR 2.208, P < 0.01). Also there was a statistically significant (P < 0.01) correlation between the presence of CMBs and the severity of hypertension and white matter lesions.
CONCLUSIONSCMBs occur frequently in patients with no prior large-area stroke who were referred for brain MRI for suspected cerebrovascular disease. The severity of CMBs correlates with the severity of hypertension and the presence of cerebral white matter changes detected by MRI.

Objective To quantitatively compare the diagnostic capability of 68Ga-NGR and 18F-FDG in well-differentiated hepatocellular carcinoma (HCC) bearing mice by microPET/CT imaging.Methods The in vitro cellular uptake, in vivo microPET/CT imaging and biodistribution studies of 68Ga-NGR and 18F-FDG were quantitatively compared in SMMC-7721-based well-differentiated HCC.The human fibrosarcoma (HT-1080) and human colorectal adenocarcinoma (HT-29) cells/xenografts were respectively used as positive and negative reference groups for CD13.The expression of CD13 was qualitatively verified by immunohistostaining.The levels of CD13 and glucose-6-phosphatase (G6Pase) were semi-quantitatively analyzed by Western blot test for all 3 types of tumors.Two-sample t test was used for data analysis.Results The in vitro cellular uptake showed that the 68Ga-NGR uptake in SMMC-7721 and HT-1080 cells was higher than that in HT-29 cells, and the 68Ga-NGR uptake was higher than 18F-FDG uptake in SMMC-7721 cells.The in vivo microPET/CT imaging results revealed that the uptake of 68Ga-NGR in SMMC-7721 tumor was (2.17±0.21) %ID/g, remarkably higher compared to (0.73±0.26) %ID/g of 18F-FDG uptake (t=8.826, P<0.01).The tumor/liver ratio of 68Ga-NGR was 2.05±0.16, which was 2.03-fold higher than that of 18F-FDG.In the HT-1080 tumors, the uptakes of 68Ga-NGR and 18F-FDG were both high, and the values were (2.46±0.23) %ID/g, (3.47±0.31) %ID/g.The uptake of 68Ga-NGR was significantly lower than that of 18F-FDG in HT-29 tumors: (0.67±0.20) %ID/g vs (3.17±0.29) %ID/g;t=4.221, P<0.01.Western blot and immunohistostaining results were as follows: HT-1080(CD13+, G6Pase-), SMMC-7721(CD13+, G6Pase+), HT-29(CD13-, G6Pase-).Conclusions The uptake of 68Ga-NGR is higher than 18F-FDG uptake in SMMC-7721 tumor bearing mice, therefore it is worthwhile to consider the feasibility of clinical translation for PET/CT in diagnosis of HCC.Furthermore, because of the difference in 68Ga-NGR and 18F-FDG avidities in tumors with different molecular phenotypes of CD13 and G6Pase, there is an underlying potential for molecular imaging in the determination of molecular phenotypes.
9.Video assisted retroperitoneal debridement for infected necrotizing pancreatitis

Objective To evaluate the clinical efficacy of video-assisted retroperitoneal debridement (VARD) in the treatment of infected pancreatic necrosis (IPN).Methods 48 infected necrotizing pancreatitis patients underwent video-assisted retroperitoncal debridement from Jan 2013 to Aug 2017.Results There were 30 males and 18 females.After operation,4 patients suffered from postoperative bleeding and underwent angiography.One patient had gastrointestinal fistula,4 patients had newly devoloped organ dysfunction altogether,7 patients had postoperative complications.The incidence of complications was 14.58% (7/48).3 cases died,the mortality was 6.25% (3/48).Conclusion The video-assisted retroperitoneal debridement for pancreatic necrotic tissue is effective,safe and minimally invasive for the treatment of infected necrosis of the pancreas.
