1.Establishment and characterization of a multidrug resistant human mucoepidermoid carcinoma xenograft model

Objective:To establish a multidrug resistant model of human mucoepidermoid carcinoma using nude mice. Methods:Multidrug resistant MEC/5-FU cells were inoculated intradermally into nude mice. Solid tumors were locally measurable after 10 days and 5-FU was repeated intraperitoneal injected into tumor-bearing mice. The tumor cells in nude mice (MEC/5-FU/NU) were isolated, cultured and examined. Results:The xenografts were similar to the original mucoepidermoid carcinoma from which the cell line was derived. The resistance index (RI) of the MEC/5-FU/NU cells to 5-FU was 27.82. Compared to the MEC, the expressions of ABCB1, ABCB11 and GSTA1 genes and MDR-1 protein increased in the MEC/5-FU/NU cells(P<0.05). Conclusion:The xenograft model is a good model of human multidrug resistant mucoepidermoid carcinoma, and may be useful in studying drug resistance mechanism in vivo.

5.Dynamic expression profile of HBsAg according to hepatic parenchyma cells' volume at different liver fibrosis stages in the immune clearance phase.

The aim of this study was to determine the dynamic expression profile of hepatitis B surface antigen (HBsAg) according to hepatic parenchyma cells' volume at different stages of liver fibrosis during the immune clearance phase. Eighty-nine patients with HBeAg-positive chronic hepatitis B (CHB) in the immune clearance stage were recruited for study. Each patient's serum HBsAg levels were detected by electrochemiluminescence. The serum HBsAg levels were apportioned according to hepatic parenchyma cells' volume at liver fibrosis stages 1, 2, 3, and 4 and compared by ANOVA. The unapportioned serum HBsAg levels (IU/mL) at liver fibrosis stages 1 (227.2+/-237.7), 2 (211.0+/-131.4), 3(300.1+/-144.6), and 4 (278.7+/-148.8) were not significantly different (all comparisons, P range: 0.061 to 0.759). However, when the serum HBsAg levels were apportioned by the same hepatic parenchyma cells' volume at liver fibrosis stages 1 (343.9+/-359.8), 2 (336.4+/-209.5), 3 (508.7+/-245.1), and 4 (525.2+/-274.8), the levels were significantly different (all comparisons, F = 3.045 and P = 0.033; stage 1 vs. 3, P = 0.041; stage 1 vs. 4, P = 0.046; stage 2 vs. 3, P = 0.028; stage 2 vs. 4, P = 0.034). During the immune clearance phase of chronic hepatitis B, increased HBsAg expression is associated with increased hepatic parenchyma cells' volume and progressive liver fibrosis stage.

6.Effects of cleaners on the color stability of prosthesis silicone rubbers

Objective To evaluate the effect of different cleaners on the color stability of two silicone rubbers used for maxillofacial prosthesis,and to provide recommendations for clinical use.Methods Thirty skin-color columniform specimens(12 mm diameter,10 mm height)of two silicone rubber(A:A-2000;Z:ZY-1)were prepared,randomly divided into 6 groups according to the table of random number,and cleaned with the following solutions:isopropyl alcohol(I),three kinds of denture cleaners(P:Polident,S:Steradent,C:Cleansofi)and distilled water(D),simulating the total immersion time of 1 year (1,15,10,3 and 10 min each time respectively).Control group was kept in dark place without treatment.The L*,a*,b* value were tested before and after immersion.Then color difference value was calculated.Results Color differences were different among groups.Color difference in group I(A:2.15,Z:2.00)were significantly greater than that in any other group.There were no significant differences between groups using denture cleaner P(A:0.36,Z:0.36),C(A:0.42,Z:0.37)and S(A:0.33,Z:0.38),and group D(A:0.22,Z:0.23).Conclusions Isopropyl alcohol causes the most severe fading,and denture cleaners and distilled water cause obscure fading.
7.Prognostic value of metastatic lymph node ratio in gastric cancer patients undergoing radical gastrectomy.

OBJECTIVETo assess the prognostic value of metastatic lymph node ratio in gastric cancer patients undergoing radical gastrectomy (D2).
METHODSPrognostic analysis of 1042 gastric cancer patients undergoing radical gastrectomy (D2) was performed based on metastatic lymph node ratio (MLR), the N staging in the 6th and 7th edition of UICC staging system respectively. Homogeneity, discriminatory ability, and gradient monotonicity of these three staging methods were compared using linear trend χ(2), likelihood ratio χ(2) statistics and Akaike information criterion (AIC) calculations, respectively. The area under the ROC curve (AUC) was calculated to compare the prognostic value of these three staging methods.
RESULTSThe 5-year survival rate of 1042 patients was 47.5%. The metastatic lymph node ratio (P<0.01) and N staging of the 7th edition UICC (P<0.05) were independent prognostic factors according to univariate and multivariate analyses. The AUC was 0.754 in MLR staging group, higher than that in N staging of the 6th (0.692) and 7th (0.705) edition of UICC group. Compared to the 6th and 7th edition of UICC N staging group, homogeneity and linear curve were better and AIC value was lower in MLR staging group (7240.017 vs. 7364.073 and 7325.731).
CONCLUSIONPrognostic value of MLR staging is better than that of UICC N staging for patients undergoing radical gastric cancer resection. The MLR staging can be a new method of lymph node staging for gastric cancer patients.

OBJECTIVETo discuss the experiences of diagnosis and treatment for vasoactive intestinal peptide-secreting-tumors (VIPoma) by summarizing clinical informations of 15 patients with VIPoma.
METHODSTo analyze Clinical manifestations, laboratory examinations, imaging features, operation, pathological findings and follow up survey of 15 patients, among them 1 case from our hospital and the other 14 cases were searched in chinese biological and medical literature database from Jan 1987 to Dec 2002.
RESULTSThe main clinical manifestation include periodical secretory watery diarrhea, hypokalemia, achlorhydria, in addition, periodical backache, skin rash, and polyps of colon were presented in the case in our hospital. The immunohistochemical expression of many kinds of digestive hormone including VIP presented positive; All clinical symptoms of which except polyps disappeared after operation, elevated VIP data in serum also markedly decreased. Part resection of superior mesenteric vein was performed in the same patient.
CONCLUSIONSVIPoma is rare. Typical symptoms and the serum value of VIP were keys to diagnosis, the operation is the most effective means for treatment. Resection of tumor, Radiofrequency tissue ablation, liver transplantation can be selected for metastatic VIPoma in the liver.

9.Integrative therapy for postoperative intractable enterococcus faecium infections.

OBJECTIVETo assess the treatment of patients with infection of enterococcus faecium after surgery who failed to respond to antibiotics.
METHODSFive patients after surgery were proved to have Enterococcus faecium infection by bacterial culture. They were treated by sensitive antibiotics but failed. Comprehensive treatment prescribed included immunoenhancements, enteral nutrition, and traditional Chinese medicines.
RESULTSFour patients were discharged from the hospital after recovery, and was cared else where after 1 month treatment.
CONCLUSIONComprehensive treatment is a better way to treat patients with refractory enterococcus faecium infection after surgery.

10.Prognostic significance of minimal residual disease detected by multiparameter flow cytometry in acute myeloid leukemia.

This study was aimed to explore prognostic significance of minimal residual disease (MRD) detection in patients with acute myeloid leukemia (AML) by multiparameter flow cytometry (MCF). Leukemia-associated immunophenotype (LAIP) of newly diagnosed AML patients were determined by 4-color 5 antibody panels and patients with sensitive LAIP were chosen for MRD detection. 601 bone marrow samples from 95 patients were acquired after treatment and MRD were considered positive by the critical normal value plus twice standard deviation in normal bone marrow specimen. The patients were divided into three groups and the clinical significance was analyzed every 2 months within initial 6 months after induction treatment. The results showed that the relapse rate and relapse-free survival (RFS) rate were all significantly different between MRD positive and MRD negative patients in the three groups (p < 0.05). Patients with MRD positive had a median relapse-free survival time of 11 months, 11.5 months and 11 months at 1 - 2, 3 - 4 and 5 - 6 months respectively, while all patients with MRD negative were not observed to reach median release-free survival time (p < 0.05). Furthermore, the clinical significance was analyzed after induction and one course of consolidate treatment, the relapse rate of MRD positive and MRD negative patients were 57.14% versus 0% and 91.67% versus 2.27% respectively (p = 0.000 and p = 0.000). It is concluded that MRD detection by multi-parameter flow cytometry can predict outcome of AML patients, which should be continuously monitored after treatment.
