The trophoblast is fetal-derived and in direct contact with maternal tissues. The expression of the various types of MHC antigen in trophoblast cells is very important for successful pregnancy. Among nonclassical MHC class Ⅰ antigens, HLA-G is expressed specifically on cytotrophoblast cells and its expression protects the fetal from maternal immune attack. The expression of the distinct subtypes of classical MHC class Ⅰ antigen is different in different subtypes of trophoblast cells. The expression of MHC class Ⅱ antigen is inhibited in trophoblast during pregnancy. It is suggested that C Ⅱ TA plays an important role in regulation of MHC class Ⅱ gene expression.

Objective To review our experiences of diagnostic methods and surgical treatment of the left superior vena cava (LSVC) draining into the left atrium. Methods Nineteen patients with LSVC draining into the left atrium were diagnosed and treated surgically from February 1998 to January 2012.All the cases were combined with other congenital heart diseases including patent ductus arteriosus, ventricular septal defect, atrial septal defect, single atrium, triatriatum, partial endocardial cushion defect, anomalous pulmonary venous drainage, right ventricle outflow stenosis, pulmonary valve stenosis, tetralogy of Fallot, double outlet right ventricle, complete endocardial cushion defect, tricuspid atresia. The patients were diagnosed through different methods including echocartiographic examination, cardiac catheterization, computer tomography, and explored during the operation, even postoperatively. All were treated surgically with four techniques including simple ligation to the LSVC,including ligation during a redo procedure; intra atrial rerouting to drainage the flow from the LSVC to the right atrium, atrial septum reconstruction to make the outlet of the LSVC lying in the right side of the patched atrial septal, and bidirectional Glenn shunt to get a physiological result. Results No mortality postoperatively. All the cases were uneventful postoperatively. And the main postoperative course was related only to the main diagnosis of congenital heart disease, not to the left superior vena cava draining into the left atrium. The echo examination result was satisfied before the discharge. The early and long term follow-up(1-11 years) results are excellent,no arrhythmia,no cardiac deficits after echo examination, including stenosis obstruction and residual shunt. No death. Conclusion The diagnostic methods of the LSVC draining into the left atrium are difficult, the only way to make the diagnosis clearly enough before the operation is depending on improving of more and more comprehending to this rare cardiac anomaly,by the pediatric cardiologists, the sinologist, the intensive care unit, and the pediatric cardiae surgeons. The choice of different surgical treatment is depending on the diagnosis and the findings during the operations, and the results are excellent after a suitable choice made.

OBJECTIVETo investigate the treatment of the displaced neck and subcapital fractures of the fifth metacarpal.
METHODSThirty-one patients with the neck and subcapital fractures of the fifth metacarpal had been operated on with antegrade intramedullary fixation between January and August 2005. There was 11 subcapital fractures (group A), and 20 neck fractures (group B). Following closed reduction of the fracture, a blunt 2.0 mm diameter K-wire, which was pre-bent into 20 degrees at the distal end, was inserted into the medullary canal of the fifth metacarpal and fixed the fractures. Postoperatively, patients in group A were immobilized in a short arm plaster splint for 4 weeks, and the ones in group B were treated with unrestricted mobilization.
RESULTSOperative time was 18 min averagely (range 5 to 30 min). Twenty-nine of 31 patients obtained anatomic reduction, and 2 patients had 2/3 apposition of bone end and no rotational deformity. Follow-up was available for all patients. The average follow-up interval was 4 months, with a range of 3 - 6 months. The head/shaft angle of the fifth metacarpal in group A was 63.4 degrees +/- 14.5 degrees preoperatively, and 15.0 degrees +/- 2.5 degrees postoperatively, and 15.4 degrees +/- 2.6 degrees in 3 months postoperatively. The difference between preoperative and postoperative angles was highly significant. The range of motion of the metacarpal joint was 89.5 degrees +/- 4.3 degrees postoperatively, which was not significantly different compared with that of uninjured side. The head/shaft angle in group B was 59.1 degrees +/- 10.0 degrees preoperatively, and 15.9 degrees +/- 2.5 degrees postoperatively, and 15.5 degrees +/- 2.8 degrees in 3 months postoperatively. The difference between preoperative and postoperative angles was highly significant. The range of motion of the metacarpal joint was 88.6 degrees +/- 3.6 degrees postoperatively, which was not significantly different compared with that of uninjured side.
CONCLUSIONSThe technique is technically easy to perform, minimally invasive, low-cost, allowing early hand mobilization, with good functional results and low morbidity, and it has been proved to be an ideal method.

To investigate the effect of cetirizine hydrochloride on the expression of neurokinin 1 receptor (NK-1R) and cytokines production induced by substance P (SP) in HaCaT cells (a human epidermal keratinocyte cell line) and dermal fibroblasts. The effect of cetirizine on the expression of NK-1R protein was detected by flow cytometry and Western blotting analysis. The modulation of cetirizine on the production of interferon (IFN)-gamma, interleukin (IL)-1beta, IL-6 and IL-8 in HaCaT cells and fibroblasts was measured by ELISA. The results showed that cetirizine significantly inhibited the expression of NK-1R in HaCaT cells and fibroblasts. SP induced the production of IFN-gamma, IL-1beta and IL-8 in both cell types. Cetirizine 1-100 micromol x L(-1) inhibited SP-induced IL-1beta and IL-8 production in HaCaT cells and fibroblasts, while had no effect on the production of IFN-gamma in both cells. Both SP and cetirizine had no effect on the secretion of IL-6 in HaCaT cells and fibroblasts. These findings suggest that cetirizine may be involved in the treatment of SP-induced skin inflammation by inhibiting the expression of substance P receptor and regulation the production of IL-1beta and IL-8 in epidermal keratinocyte and dermal fibroblasts.

Apogossypolone (ApoG2), a novel derivative of gossypol, has been shown to be a potent inhibitor of antiapoptotic Bcl-2 family proteins and to have antitumor activity in multiple types of cancer cells. Recent reports suggest that gossypol stimulates the generation of cellular reactive oxygen species (ROS) in leukemia and colorectal carcinoma cells; however, gossypol-mediated cell death in leukemia cells was reported to be ROS-independent. This study was conducted to clarify the effect of ApoG2-induced ROS on mitochondria and cell viability, and to further evaluate its utility as a treatment for nasopharyngeal carcinoma (NPC). We tested the photocytotoxicity of ApoG2 to the poorly differentiated NPC cell line CNE-2 using the ROS-generating TL/10 illumination system. The rapid ApoG2-induced cell death was partially reversed by the antioxidant N-acetyl-L-cysteine (NAC), but the ApoG2-induced reduction of mitochondrial membrane potential (MMP) was not reversed by NAC. In the presence of TL/10 illumination, ApoG2 generated massive amounts of singlet oxygen and was more effective in inhibiting cell growth than in the absence of illumination. We also determined the influence of light on the anti-proliferative activity of ApoG2 using a CNE-2-xenograft mouse model. ApoG2 under TL/10 illumination healed tumor wounds and suppressed tumor growth more effectively than ApoG2 treatment alone. These results indicate that the ApoG2-induced CNE-2 cell death is partly ROS-dependent. ApoG2 may be used with photodynamic therapy (PDT) to treat NPC.

BACKGROUND: Preliminary studies have found that poly(lactic-co-glycolic acid) (PLGA) can improve the compressive strength and degradability of calcium phosphate cement. OBJECTIVE: To prepare a self-setting calcium phosphate cement which has better mechanical properties, biocompatibility and degradability on the basis of the previous findings. METHODS: Beta-tricalcium phosphate (β-TCP), pure calcium phosphate cement (CPC) and PLGA powder were mixed at different mixing ratios for preparation of PLGA/β-TCP/CPC. Setting time, compressive strength, elastic modulus and degradation properties of the composite bone cement were evaluated to screen the optimal level of β-TCP. MC3T3-E1 cells were cultured in CPC extract (control), PLGA/β-TCP/CPC extract (experimental), α-MEM containing 10% fetal bovine serum and 1% penicillin-streptomycin double antibody (negative control), and 6.4% phenol liquid (positive control). MTT method was used to detect cell proliferation at 1, 3, 5 days after culture, and lactate dehydrogenase activity in culture media was detected at 1 and 3 days after culture. MC3T3-E1 cells were seeded on the surface of PLGA/β-TCP/CPC and pure CPC respectively, and were then observed by scanning electron microscopy after 3 days. RESULTS AND CONCLUSION: Initial setting time and final setting time among of the composite bone cement were increased with the increasing of β-TCP content, but had no significant difference from those of the CPC (P > 0.05). The compressive strength and elastic modulus of the composite bone cement were higher than those of the CPC, and moreover, the composite bone cement with 20% β-TCP exhibited the highest compressive strength and higher elastic modulus as compared with the other groups. Therefore, PLGA/20% β-TCP/CPC was selected in the cell test. Moreover, the degradation properties of the composite bone cement were also better than those of the CPC. (3) With the growth of culture time, cell absorbance value and lactate dehydrogenase activity were gradually increased in the experimental group, and no difference existed between the experimental group and the negative control group. The cells in the experimental group also grew well. (4) MC3T3-E1 cells grew well and fully extended on the surface of PLGA/β-TCP/CPC, and cell pseudopodia on the material surface were tightly adhered to the material. To conclude, PLGA/20% β-TCP/CPC has better compressive strength, elastic modulus, degradation properties and cytocompatibility relative to the CPC, and moreover, the composite bone cement has no obvious cytotoxicity.
