1.Clinical differences between artificial pneumothorax and double lumen intubation in thoracoscopic esophagectomy

Objective To explore the advantages and disadvantages of the double lumen endotracheal intubation and single -lumen endo-tracheal intubation and continuous carbon dioxide insufflation in thoracoscopic esophagectomy .Methods The clinical data of 90 patients in our department of thoracic surgery after thoracoscopic esophagectomy from January 2014 to April 2015 were analyzed .All patients were divid-ed into single-lumen endotracheal intubation (group A)and double lumen endotracheal intubation group (group B).The endotracheal intuba-tion time,operation time,incidence of pulmonary infection,intraoperative and postoperative PaO2,PaCO2,incidence of anastomotic fistula, hospitalization expenses ,length of hospital stay and the incidence of postoperative chylothorax between two groups were compared .Results The difference in intraoperative PaO2,PaCO2,incidence of pulmonary infection,endotracheal intubation time,operation time,hospitalization days and the hospitalization cost between two groups were statistical significance .The difference of the rest index between two groups were no statistical significance.Conclusion Group A has certain advantages in perioperative management ,hospitalization cost and so on,but has disadvantages in perioperative hypoxemia and carbon dioxide retention and acid -base balance disorders .
2.Clinical analysis of the relationship between chemotherapy after pneumonectomy and bronchopleural fistula

Objective To evaluate the relationship between chemotherapy after pulmonary resection and the incidence rate of bron-chopleural fistula. Methods 246 patients who received pulmonary resection in our hospital from January 2009 to June 2014 were chosen, and they were divided into the chemotherapy group and the non-chemotherapy group. The 138 patients in the chemotherapy group received chemotherapy one month after resection while the other 108 in the non-chemotherapy group did not. Bronchopleural fistula of the two groups were diagnosed and analyzed in order to evaluate the relationship between chemotherapy after pulmonary resection and incidence rate of bron-chopleural fistula. Results There were 12 cases of bronchopleural fistula in the chemotherapy group with an incidence rate of 8. 70%, while there were 2 cases of bronchopleural fistula in the non-chemotherapy group with an incidence rate of 1. 85%. The difference between the two groups is statistically significant (P<0. 05). Conclusion Chemotherapy after pulmonary resection will increase the incidence rate of bron-chopleural fistula.
3.A randomized controlled clinical study on the combination of preservative-free sodium hyaluronate and fluorometholone eyedrops for cataract with dry-eye

Background Cataract with preexisted dry eye is a common eye disease.It is speculated that preservative-free artificial tears can inhibit inflammatory procedure,prevent postoperative eye infections and reduce dry eye symptoms.However,relevant literature is rare up to now.Objective This study was to evaluate and compare the therapeutic effects between preservative-free or preservative sodium hyaluronate combined with fluorometholone eyedrops for cataract with dry-eye.Methods A randomized controlled single-blind clinical study was performed.Sixty patients with dry-eye syndrome who was going to receive surgery for cataract removal were enrolled in Yuhuangding Hospital from January to December 2015 under the informed consent.The patients were randomly divided into the test group and control group.Preservative-free or preservative 0.1% sodium hyaluronate combined with 0.1% fluorometholone eye drops were topically administered in the eyes of the test group and control group,respectively.Ocular Surface Disease Index (OSDI) score,breakup time of tearfilm (BUT),Schirmer Ⅰ test (S I t),corneal fluorescein staining,impression cytology,goblet cell density and the concentrations of interleukin (IL)-1β,tumor necrosis factor (TNF)-α,catalase (CAT),superoxide dismutase 2 (SOD2) in tears were evaluated and compared.Results There were significant differences in gender,ages,OSDI scores,BUT,S I t value,corneal fluorescein staining scores,impression cytology findings,and goblet cell density between the two groups (all at P>0.05).OSDI,corneal fluorescein staining scores and imprint cellular level were evidently reduced,and BUT,S I t values and goblet cell density were significantly increased 1 month and 2 months after operation in comparison with the baseline values in the test group (F =13.058,8.027,3.755,21.652,70.962,92.354,all at P < 0.05).The concentrations of IL-1β and TNF-α in tears of the test group were significantly lower,and CAT and SOD2 in the tears of the test group were significantly higher than those in the control group 1 month and 2 months after operation (F=18.731,9.070,15.357,351.359,all at P>0.05).Conclusions 0.1% preservative-free sodium hyaluronate combined with 0.1% fluorometholone eyedrops can relieve the symptoms and signs of dry-eyes following cataract surgery by playing antiinflammatory and antioxidant effects.
4.Urodynamic study on evaluation of diabetic patients’ bladder function

Objective To evaluate the bladder functions by urodynamic studies in diabetic patients with lower urinary tract symptoms (LUTS). Methods Urodynamic studies were performed in 42 diabetic patients (24 men and 18 women;age range,38-78 years).The diabetic history of the patients was from 1 month to 25 years. Results Of the 41 cases who underwent all items of urodynamic examination,38 cases (93%) had abnormal findings and 3 (7%),normal.The detrusor was underactive in 14 cases (34%) and areflexia in 10 (24%);BOO was found in 13 (32%).One female patient had stress urinary incontinence. Conclusions There is a high prevalence of bladder dysfunction in diabetic patients with LUTS.It is necessary to perform urodynamic studies in diabetic patients before initiation of therapy,especially in patients who are assigned to undergo bladder and urethral surgery.Preoperative urodynamic studies can contribute to the surgical success rate.
5.Experience of Professor LIU Zhe for Treating Essential Tremor with Acupuncture

[Objective]To study the clinical experience of Professor LIU Zhe treatment of essential tremor with acupuncture.[Methods]The paper states Professor LIU's understanding of treating essential tremor from the pathogenesis, acupuncture points and acupuncture manipulations, then summarizes the principle, regularity, and the features of acupuncture manipulations for Professor LIU treating essential tremor of different types of syndrome, so as to elaborate the clinical experience of Professor LIU. [Result] Professor LIU Zhe considers the age-dependent deficiency of qi and blood and insufficient liver and kidney syndrome to be a common one of essential tremor, as an experienced doctor, he distinguishes different types of syndrome from sign, tongue, pulse and so on. He places great emphasis on nourishing the bone marrow and tonifying liver and kidney. According to the symptoms select acupuncture points with the effects of nourishing the bone marrow and calming the liver to stop the wind, tonifying liver and kidney and supplementing qi and nourishing blood. Professor Liu sets a high value on selecting points of head and makes use of the point of Fengchi with unique acupuncture manipulations to relax cerebrovascular. In clinically, the curative effects are remarkable. [Conclusion]The unique method for treating essential tremor of Professor LIU gives some inspirations for clinic. It is worth of a deep study and spreading.
6.The diagnostic value of pressure-flow study in elderly male patients with dysuresia

Objective To evaluate pressure-flow study in assessment of dysuresia symptom in elderly male patients. Methods A total of 125 elderly male patients with dysuresia and prostatic enlargement underwent urodynamic examination.Their age ranged from 56 to 83 years with a mean of 68 years.If the patients urinated smoothly with satisfying urinary flow curve,the pressure-flow study would be performed on them.According to the results,the patients were classified as 3 groups:BOO,equivocal BOO and non-BOO groups.Their detrusor contractive function was classified as strong,normal,weak or very weak. Results Of the 125 patients undergoing urodynamic test,87 obtained definite pressure-flow study findings.Of the 87 patients,39 were with BOO,18 with equivocal BOO and 30 without BOO.In the BOO,equivocal BOO and non-BOO groups,detrusor pressure at maximum flow was (99.2?34.3) cmH 2O (1cmH 2O=0.098 kPa),(46.9?9.9)cmH 2O and (30.8?10.0)cmH 2O,respectively;intravesical opening pressure was (99.4?39.6) cmH 2O, (43.7?9.9) cmH 2O and (29.9?9.7) cmH 2O, respectively; minimum voiding detrusor pressure was (61.3?27.5) cmH 2O, (33.9?14.1)cmH 2O and (22.1?12.5)cmH 2O,respectively;and maximum detrusor pressure was (113.0?42.1)cmH 2O,(55.8?14.9)cmH 2O and (38.4?11.3)cmH 2O,respectively.The detrusor function was normal or strong in 74.4% (29/39) of patients with BOO,27.8%(5/18) of patients with equivocal BOO and 26.7% (8/30) of patients without BOO.All these parameters of the BOO group were higher than those of the equivocal BOO group and non-BOO group (both P
7. Protective effects of different solvent fractions of Boschniakia rossica on acute liver injury induced by acetaminophen in mice

OBJECTIVE: To investigate the protective effect of different solvent fractions of Boschniakia rossica on acute liver injury induced by acetaminophen (APAP) in mice. METHODS: The mice were randomly assigned to the normal control, model control, bifendate (positive control), as well as groups of different solvent fractions of Boschniakia rossica. Animals were treated once daily for 7d. APAP were given intraperitoneally to the mice of groups, then the alanine aminotransferase (ALT), aspartate aminotransferase (AST), albumin (ALB), reactive oxygen species (ROS), lipid hydroperoxide (LPO), malondialdehyde (MDA), glutathione (GSH), catalase (CAT), glutathione peroxidase (GSH-Px), superoxide dismutase (SOD), Na+-K+-ATPase, Ca2+-Mg2+-ATPase nitric oxide (NO) and inducible nitric oxide synthase (iNOS) were detected by the colorimetric method. RESULTS: The administration with butanol soluble fraction and aqueous fraction oi Boschniakia rossica reduced the serum ALT and AST activities, increased the scrum ALB level, decreased the hepatic ROS, LPO and MDA levels, increased the CAT, GPx, SOD activities and GSH level, increased the Na+-K+-ATPase and Ca2+-Mg2+-ATPase activities of liver mitochondria, and decreased hepatic iNOS activity and NO level of liver in mice with acute liver injury. CONCLUSION: The different solvent fractions of Boschniakia rossica have protective effects on acute liver injury induced by APAP in mice, probably via anti-oxidative and anti-inflammatory activities.
8.Surgical treatment for traumatic cataract

·AIM: To study the therapeutic effect of surgical treatment for traumatic cataract.·METHODS: Seventy-three cases(73 eyes) traumatic cataract were performed multi-operation combined cataract extraction, including insertion of a capsular tension ring(CTR), vitreoretinal surgery, ocular foreign body extraction and intraocular lens(IOL) implantation.·RESULTS: Of 73 patients, twenty-seven cases were blunt trauma, while 46 cases were penetrating injuries. Three months after surgery, the final best spectacle-corrected visual acuity(BSCVA) in 5 patients (7%) were 0.05 or less, seventeen(23%) 0.05 to 0.3, while 49(67%) 0.3 or more. There were 93% cases relieved from blindness. Atrophy of eyeball occurred in two cases(3%). Mean follow-up time was 7.8(range 3-15) months.·CONCLUSION: Useful vision can be restored in a proportion of traumatic cataracts after prompt and rational surgical intervention as well as appropriate treatment of sight-threaten complications.

10.Outcome analysis of the management policy for category-B large medical instruments deployed in Beijing

Objective To learn the policy outcomes of the management of category-B large medical instruments deployed in Beijing.Methods Call into play the data from the surveys made in 2005,2007 and 2010 on the five types of category-B large medical instruments deployed in the city.Summarize and analyze the outcomes of such instruments in terms of their total configuration volume,and the growth,distribution and use.Results From 2005 to 2010,CT scored the fastest growth of these five types of instruments,with 48 units deployed; SPECT was the lowest,with 8 units deployed.Conclusion The volume growth,instruments distribution,and use efficiency of such instruments in Beijing are rationally evolving,which proves that the deployment and management of category-B large medical instruments is compliant with conditions of the city.
