1.Relationship between discomfort experience and factual recollection in patients of intensive care unit

Objective To analyze the correlation of discomfort experience and factual recollection (FR) in patients of intensive care unit (ICU). Methods Fifty-five ICU patients and 63 ward patients par-ticipated in the study. Standardized questionnaires were used to evaluate all patients" FR and the results underwent analysis. Results The prevalence of recollection of any type of discomfort in the ICU patients was 64%. The presence of an endotracheal tube, pain and hallucinations were identified as sources of dis-comfort.The median score for FR in the ICU patients was 7.25. The median score for FR in the ward pa-tients was 13.50. The difference between them was highly significant (t =3.34, P < 0.01 ). Logistic analysis revealed that discomfort, especially discomfort caused by the presence of an endotracheal tube, pain and hallucinations, was positively related to FR, 0R95%CI (1.2~2.1), P < 0.01. Conclusions The presence of an endotracheal tube, pain and hallucinations were most frequently reported as sources of discomfort in ICU patients and their memory was often impaired. FR and recollection of discomfort appear to be related significantly in ICU patients.
2.Study on prediction of high risk group of Type 2 diabetes based on Apriori algorithm

Objective: To discover and prevent diabetes early in high-risk group. Methods:Chose 323 records of type 2 diabetes or IFG,IGT patients with first course which come from a large hospital in Lanzhou between 2009 January to 2014 March; Use Apriori algorithm and the SPSS Clementine 12 software design;Set up prediction model of type 2 diabetes. Results:A total of 10 strong association rules are formed. The strong association rules contain some associations between factors and type 2 diabetes. Conclusion: Through the association rules, the initial judgment on the high-risk group of diabetes is simple and accurate.
3.Treatment of perinail refracture following surgery of proximal femoral fracture

Objective To investigate the treatment of perinail refracture after surgery of proximal femoral fracture.Methods From January 2010 to January 2015,we treated perinail fractures in 31 patients who had undergone surgery for proximal femoral fracture.They were 11 men and 20 women,with an average age of 75.6 years (range,from 24 to 87 years).On average,their refracture occurred 9.4 months after primary fixation (range,from 3 to 60 months).With reference to the Vancouver classification of peri-prosthestic refractures in the proximal femur and the position and bone quality of perinail refractures,we tried to classify the perinail fractures and chose different treatment protocols accordingly.In our cohort,6 were type A,5 type B,15 type C,and 5 type D.Type A cases were treated conservatively,and types B and C cases with locking compression plate or less invasive stabilization system.In one case of type D,dynamic hip screws were implanted to fixate the femoral neck fracture after removal of the original intramedullary nail,and hip replacement was conducted in the other 4 after removal of the original intramedullary nail.Results The operation time averaged 2.1 hours (range,from 1.6 to 3.0 hours) and intraoperative bleeding 600 mL (range,from 150 to 800 mL) in this cohort.Of them,27 were followed up for an average of 15 months (range,from 12 to 24 months),giving a follow-up rate of 87.1% (27/31).Six type A fractures obtained bone union after protected weight-bearing walking for 12 weeks.All the 16 fractures of types B and C healed after an average period of 4.2 months (range,from 3 to 6 months).Of the 5 type D fractures,one obtained bone union 12 weeks after change into dynamic hip screwing and 4 had fine functional recovery after hip replacement.No infection,nonunion,or implants failure occurred.Conclusions We have set an exploratory classification system for the perinail refractures at the proximal femur with reference to the Vancouver classification of peri-prosthestic refractures.Our classification can provide effective guidance for the treatment of perinail refracture after surgery of proximal femoral fracture.

6.Research progress of the relationship between hydrogen peroxide and glioma cell apoptosis

Glioma is the most common intracranial malignant tumor,with high incidence and recurrence rate,high fatality rate and the characteristics of low cure rate.Current treatment is given priority to with surgical treatment,auxiliary comprehensive therapy such as radiotherapy and chemotherapy.Hydrogen peroxide (H2O2) is a intermediate product of the cell oxygen metabolism which is a universal phenomenon in aerobic organisms.H2O2 is indispensable at various stages of tumor cell proliferation,infiltration and metastasis.Studies of the production of H2O2 and its function,the mechanism of apoptosis and the relationship between H2O2 and glioma cells can provide corresponding guidances for looking for a target gene for the treatment of gliomas.
7.Effect of Low-Level Laser Irradiation Preconditioning on Milieu of Infarcted Myocardium in Experimental Rats

Objective: To explore the effect of low-level laser irradiation (LLLI) preconditioning on milieu of infarcted myocardium in experimental rats.
Methods: The myocardial infarction (MI) model was established by left anterior descending (LAD) artery ligation in female rats. 3 weeks later, the qualified MI rats were randomly divided for 3 groups: ① LLLI preconditioning group, the rats received thoracotomy for LLLI by a 635nm, 5mW diode laser with the energy density of 0.96 J/cm2 for 150 seconds, n=26. ② Control group, the rats received thoracotomy for daylight irradiation, n=27. ③ Sham operation group, the rats received thoracotomy without LAD ligation, n=24. The Expressions of myocardial vascular endothelial growth factor (VEGF), glucose-regulated protein 78 (GRP78), superoxide dismutase (SOD) and malondialdehyde (MDA) were evaluate by real time-PCR, Western blot analysis and other relevant laboratory test at 1 hour, 1 day and 1 week after treatment. The myocardial cell apoptosis was examined by TUNEL staining, and left ventricular function was detected by echocardiography.
Results: LLLI preconditioning obviously increased the myocardial VEGF, GRP78 expression and SOD activity, decreased MDA production; while it could not really improve the myocardial cell apoptosis at peri-infarcted area and left ventricular function in experimental rats.
Conclusion: LLLI preconditioning may improve the milieu of infarcted myocardium via decreasing the oxidative stress in experimental rats.
8.Application of three diagnostic criteria for metabolic syndrome among the elderly in urban community of Beijing

Objective To compare the application of three diagnostic criteria for metabolic syndrome(MS) among the elderly in community. Methods 773 elderly people in the community were diagnosed for MS by the Chinese Medical Association Diabetes Society (CDS),International Diabetes Federation (IDF) and revised National Cholesterol Education Prongram-Adult Treatment Panel(ATP Ⅲ* ) criteria,respectively.The diagnosis accordance rates were compared among the different diagnostic criteria. Results The MS prevalence rates in the elderly were 32.5% (251/773),53.4% (413/773) and 47.2% (365/773) according to CDS,IDF and ATP Ⅲ* criteria,respectively and there were significant difference among the criteria (Q=173.10,P<0.01 ).The MS prevalence was higher in women than in men especially by IDF [57.3% (284/487)vs.45.1% (129/286),x2=12.64,P<0.01]and ATPⅢ* criteria [53.0%(258/487)vs. 37.4% (107/286),x2=17.52,P<0.01].The coincidences in the diagnosis of MS were 88.6% (Kappa=0.773,P<0.01)using IDF and ATPⅢ * definitions,75.7% using CDS and ATPⅢ *,and 73.7% using CDS and IDF.Conclusions The agreement in the diagnosis of MS using IDF and ATPⅢ* definitions is higher than using CDS and IDF or ATPⅢ * definitions.
9.Expression of VEGF and VEGFR-2 in patients with operable advanced laryngeal cancer treated with induction chemotherapy

Objective To assess the prognostic values of the immunohistochemical expression of vascular endothelial growth factor (VEGF) and vascular endothelial growth factor receptor-2 (VEGFR-2) in a cohort of patients with operable advanced laryngeal cancer who have been treated with induction chemotherapy. Methods VEGF and VEGFR-2 expression in the Forty-nine patients was quantified by an enzyme immunosorbent assay in a retrospective series.Results The moderate to high VEGF expression in 34 patients were found and low expression in 15 patients. VEGFR-2 expression was moderate to high in 31 patients and was low expression in 18. The probability of a complete response to induction chemotherapy was significantly higher in patients with none to low VEGF expression.Conclusion VEGF expression seems to be a significant predictor of complete response to induction chemotherapy.
10.Detecting Six Kinds of Antiphospholipid Antibodies in Serum from 32 Patients with Systematic Lupus Erythematosus

Objective To investigate the distribution of antiphospholipid antibodies(APA) in patients with systematic lupus erythematosus (SLE) and normal controls and its significance. Methods Six kinds of APA were detected by enzyme linked immunosorbent assay(ELISA). The examined sera were collected from 82 cases of normal controls and 32 cases of SLE. Results ①The positivity rate of IgG type aCL, aPI, aPA, aPE, aPS and aPC in the serum from normal controls was 6.10%, 6.10%, 7.32%, 7.32%, 3.66% or 3.66%, respectively. ②The positivity rate of IgM type aCL, aPA, aPC, aPS, aPE aPI in serum from normal controls was 2.44%, 3.66%, 3.66%, 6.10%, 6.10% and 4.88%, respectively. ③Compared with normal controls, the A values and positivity rate of IgG type of aCL, aPA, aPS, aPC and aPI in SLE patients were significantly higher. ④Compared with normal controls, the A values and positivity rate of IgM type of aCL, aPA, aPS, aPE and aPC in SLE patients were significantly higher. ⑤The total positivity rate of IgG type and IgM type APA was 68.75% or 71.88%, respectively. Conclusion ①There is lower titer of APA in normal controls, with some relationships between different types of APA and they appear at the same time.②the measurment of APA in sera of SLE might be benefit to the diagnosis of antiphospholipid syndrome.
