2.Diagnosis and surgical management of lumbar degenerative juxtafacet joint cysts

[Objective]To discuss the diagnosis and surgical management of lumbar degenerative juxtafacet joint cysts. [Methods]From June 2004 to June 2007,seven patients were diagnosed as degenerative juxtafacet joint cysts and were treated by surgery. The authors made a retrospective analysis on the clinical data of these 7 patients. Among these 7 patients,there were one as ligamentum flavum cyst and six as facet joint cysts. Five lesions were located at the L4、5 level and two were located at the L5S1 level. According to the preoperative lateral films and CT scan,segment instability were found in two patients. Fenestration decompression,resection of cyst and 1/3 medial articular process were performed on five patient without preoperative segment instability,while additional posterior fixation and posterolateral fusion were done to two patients with preoperative segment instability.The mean length of follow up were 14 months (range: 9~24 months ). The VAS score and ODI score were used for clinical outcome assessment.[Results]The pathological results confirmed the preoperative radiological diagnosis in all patients. Improvement were found in all patients,including completely recovery in five patients and significant relief of pain in other two patients.[Conclusion]Lumbar degenerative juxtafacet joint cysts has been proved to be a cause of low back pain. Preoperative CT scan and MRI are helpful in the diagnosis and the clinical outcome of surgery is satisfactory.
3.Clinical research for thulium laser vaporization enucleation of the prostate combined with traditional transurethral resection of prostate for therapy benign prostate hyperplasia larger than 80 millilitres

Objective To evaluate the feasibility and the safety of thulium laser vaporization enucleation of prostate(TVEP) combined with traditional transurethral resection of prostate (TURP) for therapy benign prostate hyperplasia (BPH) with its volume larger than 80 millilitres.Methods Twenty-five BPH patients (volume larger than 80 millilitres) underwent TVEP combined with TURP.The operation time,intraoperative blood loss,enucleation time,cutting time,bladder irrigating time,catheterization time,perioperative and 6 months' follow-up data such as the international prostate symptom score,quality of life score,the maximum urinary flow rate,the residual urine volume and so on were observed.Results The operation time was (66 ± 26) min.The enucleation time was (25 ± 9) min.The cutting time was (32 ± 8) min.The intraoperative blood loss was (140 ± 25) ml.The bladder irrigation time was (3.0 ± 1.0) d.The catheterization time was (5.7 ± 1.0) d.After 6 months,the maximum urinary flow rate,residual urine volume,international prostate symptom score and quality of life score were improved:(18.7 ± 1.7) ml/s vs.(6.8 ± 1.7) ml/s,(18.9 ± 1.8) ml vs.(65.7 ±8.1) m1,(8.7 ± 1.6) scores vs.(25.7 ±4.3) scores,(1.7 ± 1.2) scores vs.(4.7 ± 1.1) scores,and there were significant differences (P < 0.05).Urethral stricture developed in 2 patients and epididymitis happened in 3 patients.No blood transfusion events and transurethral electric cutting syndrome occurred.Conclusion TVEP combined with TURP for therapy BPH larger than 80 millilitres is safe,and the incidence of complications is low.
4.Application analysis of MSCT multiphase enhancement scanning in the atypical hemangiomas

Objective To analyze multislice CT (MSCT) multiphase multiphase enhancement scanning in the atypical hemangiomas. Methods During February 2013 from January 2014, chosing125 cases of patients with atypical hemangiomas in our hospital were retrospectively analyzed. Results There was a total of 240 lesions in 125 patients, multiple (2 or higher) in a total of 70 cases, 55 cases of single. A total of 170 were located in the right lobe, 70 were located in the left lobe, lesions was 0.8 to 4.5 cm in diameter, the average was (2.53±0.21) cm. A total of 205 lesions were circular, a total of 35 as lobulated or irregular. CT scan, a total of 155 were low density, 85 in equal density. A total of 120 state clearly, realm for part of the fuzzy and parts out of a total of 35, 85 was fuzzy. A total of 45 lesions density uneven, a total of 195 was lesions density uniformity. In 240 intrahepatic lesions, a total of 145 lesions present performance1, 80 lesions belong to 2 kinds of forms, 15 lesions characterized by 3 kinds of forms. Conclusion Although a few hemangiomas reinforcement is not typical, by the mutiperiodic enhanced MSCT scanning remain their respective characteristics, in identification of its pathological changes has important diagnostic value.
5.Effects of purified product from trapa manshurica fler on apoptosis and cell cycle progression of tumor cells in vitro

Objective To observe the effects of 3,4,5-trihydroxybenzoic acid dimmer (TAD) in different doses on apoptosis and cell cycle progression of liver cancer SMMC-7721 cells in vitro and explore its possible mechanism of inhibiting tumor growth.Methods The experiment was divided into 0,3.125,6.250,12.500 and 25.000 ?g TAD groups. Apoptosis was observed with JEM-1200EX transmission electron microscope (TEM).The changes of apoptosis and cell cycle progression were measured with flow cytometry.Results The typical features of apoptosis were found in tumor cells,the nuclei were broken to pieces,mitochondrion cristae were disrupted and vacuoles were showed in nucleus and cytoplasm in 25.000 ?g TAD group observed under TEM.Meantime,the apoptotic percentages of the cells treated with 3.125—25.000 ?g TAD were increased significantly as compared with control group (P

Objective The purpose of the present study is to illustrate the effect of olfactory ensheathing cells(OECs) on the survival and neurite outgrowth of GABAergic neurons in vitro. Methods OECs were dissociated from olfactory bulb and neurons from spinal cord of E12 mouse. Objective To evaluate the feasibility of 80 kV tube kilovoltage in aortic artery CT angiography with 256-slice CT. Methods A total of 62 patients undergoing aortic artery CTA were enrolled into this study and divided into conventional tube kilovoltage (120 kV, n = 31 ) and low tube kilovoltage (80 kV, n = 31 ). The signal-to-noise ratio (SNR) , contrast-to-noise ratio (CNR), volume CT dose index (CTDIvol) and effective dose (E) were evaluated, respectively. Results The mean image SNR was ( 35.92 ± 5.04) and ( 33.95 ± 8. 30) for conventional tube kilovoltage and low tube kilovoltage,respectively, with no significant difference (t = 1. 131, P =0. 263). The mean image CNR was (30. 32 ±4.78) and (28.71 ± 7.96 ) for conventional tube kilovoltage and low tube kilovoltage, respectively, with no significant difference ( t = 0. 964, P = 0. 339 ). The average effective dose ( E ) was ( 14. 28 ± 0.96 )mSv and (9. 72± 0. 81 )mSv for conventional tube kilovoltage and low tube kilovoltage, respectively, with significant difference ( t = 20. 12, P < 0. 001 ). Conclusions 80 kV tube kilovoltage aortic artery CTA can reduce radiation dose by 31.9% , and contrast dose 50% ,and maintain image quality compared with 120 kV tube kilovoltage.
8.Long term results of open reduction and Salter osteotomy for developmental dislocation of the hip in children

Objective To retrospectively review long?term results of developmental dislocation of the hip patients treated with open reduction and Salter pelvic osteotomy, and to analyze the potential risk factors. Methods Sixty?eight developmental dislocation of the hip cases (81 hips) were treated with open reduction and Salter pelvic osteotomy from September 1992 to Octo?ber 2002, 45 cases (52 hips) were followed up to skeletal maturity. None had undergone other treatment before the key surgery. Some cases received skeleton traction before surgery. The radiographic and functional results were assessed at last follow?up. The possible relevant factors for unsatisfactory outcome were analyzed. The correlation between type ⅡAVN and the coverage rate of the femoral head were evaluated. Results The mean age at surgery was 2.5 years (ranged from 1.4 to 5.3 years). At last follow?up, 44 hips (84.6%) had excellent or good functional results. 23 hips (44.2%) had avascular necrosis of varying severity, of which 16 hips were typeⅡ. There was significant relevance between the AVN severity and the Severin classification result (P=0.018). Theover correctionhips, that were overcorrected during surgery, were more likely to get severe type AVN ( P=0.037). There was no correlation between TypeⅡAVN with uncoverage according to CE angle and Reimers index (P=0.457, 0.308 respectively). Con?clusion 1) Satisfactory functional and radiographic results could be expected in developmental dislocation of the hip cases with?out AVN;2) High risk of AVN existed in the late diagnosised hips that underwent open reduction and Salter pelvic osteotomy, typeⅡwas the most common type;3) TypeⅡAVN was not a predictor for progressive femoral head coverage deficiency;4)Over cor?rectionin Salter pelvic osteotomy was related with severe type AVN which should be avoided.

10.Application of Da Vinci Robot System in Pediatric Hydronephrosis

[Objective] To explore the application of Da Vinci robot in laparoscopic pyeloplasty treatment for pediatric hydronephrosis and to improve the effect of operation and shorten the operation time effectively.[Methods] A summary and analysis was performed on the intraoperative cooperation about the clinical data of 14 pediatric hydronephrosis cases (16 sides) whom were performed RALP from November 2015 to July 2016.The intraoperative cooperation includes preoperative education,adequate preparation of surgical instruments,ensuring the normal operation of robot system before surgery and also,the proper body positions,the protection for patients,the establishment of aseptic barrier during operation as well as the maintenance of robot after operation.[Results] All procedures were performed via transperitoneal approach.Docking time was shortened from 40 to 20 min,operation time lasted (115± 38) min.One severe case presented with anastomasis stenosis even after PCN dilatation,and was redid dismembered pyeloplasty half a year later and the ultimate result was good.The other cases recovered well and the overall success rate was 93.75%.There were no robot failure caused by improper cooperation and no complications caused by improper nursing among the 14 pediatric hydronephrosis cases (16 sides) in pediatric hydronephrosis operations.[Conclusion]Da Vinci robot performed well in assisting laparoscopic pyeloplasty treatment for pediatric hydronephrosis,for it leaves light surgical trauma and short duration of operation.Adequate preoperative preparation,medical staff's formal professional technical training and professional cooperation team can not only improve the operation efficiency,but also be the key to the success of the operation.
