2.The Effect of Atorvastatin on Seumr Hypersensitive C-reactive Protein Level in Patients with Cardiac Syndrome X

Objective To explore the level changes of serum hypersensitive C-reactive protein (hs-CRP) in patients with cardiac syndrome X (CSX) and the effect of atorvastatin on hs-CRP level. Methods 25 patients with CSX and 10 matched healthy subjects were enrolled in this study, and the serum hs-CRP levels were compared between the patients and healthy subjects. The patients were randomly devidid into routine therapy group (n=12), which received diltiazem 30mg t.i.d and isorbide dinitrate 10mg t.i.d, and atovastatin thrapy group (n=13), which received the same dosage of diltiazem and isobide dinitrate, and atovastatin 20mg/d additionally. The course of treatment was 12 weeks. At the beginning and the end of study, standardized angina questionnaires and exercise stress test were performed in all patients, and their serum hs-CRP levels were measured. AIM and METHOD:To study the changes of contents of phosphatidyl inositol (PtdIns),phosphatidylserine (Ptdser),phosphatidylethanolamine (PtdEtn) and phosphatidylcholine(PtdCho) in hepatic mitochondria membrane of goats in vivo at 5 h after administration of E. coli endotoxin(1800 U/kg body weight) with HPLC. The membrane fluidity of the erythrocyte and liver mitochondria of E. coli endotoxin treated group was examined with the fluorescence polarization technique, in which 1,6-diphenyl-1,3,5,-hexatriene was used as a fluorescence probe. This article introduces the structure, principle and characteristics of a new anorectal therapeutic apparatus. The adoption of new technologies, new components and new materials lays the foundation for the compatibility for electric energy, magnetic energy and thermal energy as well as the safety and comfortability of the intracavitary electrode. The intracavitary electrode possesses the functions of electrotherapy, thermotherapy, magnetotherapy and massage, and thus can be applied to the physical therapies of colitis, irritable bowel syndrome and prostatitis. The apparatus has such advantages as reliable curative effects, no toxic and side effects, simple manipulations, low cost, etc, and thus should be popularized.
5.Expressions and significance of tumor-associated genes in tissues adjacent to human epithelial ovarian cancer of orthotopic implantation in nude mice

Objective To investigate the feasibility in screening of normal ovarian tissues by evaluating the expressions of tumor-associated genes in tissues adjacent to human epithelial ovarian cancer of orthotopic implantation in nude mice.Methods Human epithelial ovarian cancer cell lines OVCAR3 were grown in subcutaneous tissues,and the tumor tissues were orthotopic implanted.The expressions of CK-7,CA125,P53,survivin,MMP-2,and TIMP-2 were detected by immunohistochemical staining in proximal tissues,middle tissues,distal tissues adjacent to tumor,tumor tissues,and normal ovarian tissues of nude mice.Results 35 samples ovarian tissues with normal biopsy were gained from 40 cases of human epithelial ovarian cancers of orthotopic implantation model in nude mice.The expression rate of CK-7,CA125,P53,survivin,MMP-2,and TIMP-2 were 95.0% (38/40),95.0% (38/40),75.0% (30/40),85.0% (34/40),77.5% (31/40),and 77.5% (31/40) in tumor tissues,respectively; 71.4% (25/35),68.6%(24/35),57.1% (20/35),62.9% (22/35),60.0% (21/35),and 57.1% (20/35) in proximal tissues adjacent to tumor,respectively; 34.3% (12/35),31.4% (11/35),31.4% (11/35),31.4% (11/35),34.3% (12/35),and 31.4% (11/35) in middle tissues adjacent to tumor,respectively; and 25.7% (9/35),22.9% (8/35),25.7% (9/35),25.7% (9/35),28.6%(10/35),and 31.4% (11/35) in distal tissues adjacent to tumor,re respectively.The expressions of CK-7,CA125,P53,survivin,MMP-2,and TIMP-2 were all negative in 23 samples normal ovarian tissues adjacent to tumor.The expressions of CK-7,CA125,P53,survivin,MMP-2 and TIMP-2 in proximal tissues adjacent to tumor were lower than those in tumor tissues(P<0.05),and higher than those in middle tissues and in distal tissues adjacent to tumor(P<0.01).There were no expression difference between in middle tissues and in distal tissues (P>0.05).The strong positive expressions of CK-7,CA125,and survivin were higher than those of P53,MMP-2,and TIMP-2 (P<0.01).No significant difference was found in those expressions in the normal ovarian tissues adjacent to tumor gained from orthotopic implantation model with different severity.The expressions of CK-7,CA125,P53,survivin,MMP-2,and TIMP-2 were all negative in 20 normal ovarian tissues of nude mice.Conclusions The expression of CK-7,CA125,P53,survivin,MMP-2,and TIMP-2 showed decreasing trend to non-cancer direction.Negative expressions of these tumor-associated genes can be used as standard in screening of normal ovarian tissues adjacent to tumor.Relative safe normal ovarian tissues can be obtained from the tissues adjacent to tumor.
6.Volar plates with modified Henry approach with pronator quadratus preservation for treatment of distal radius fractures in elderly patients

Objective:To investigate the effect of volar plate internal fixation with modified Henry approach with pronator quadratus preservation for distal radius fractures in elderly patients.Methods:A retrospective case control analysis was performed on 78 elderly patients with distal radius fractures treated with volar plate internal fixation from February 2016 to December 2018 in Second Affiliated Hospital of Wenzhou Medical University and People's Hospital of Qingtian. There were 36 males and 42 females, aged 60-79 years [(66.5±5.6)years]. According to AO classification, there were 16 patients with type B1 fractures, 15 with type B2, 11 with type B3, 12 with type C1, 14 with type C2 and 10 with type C3. Of all, 40 patients were operated using modified Henry approach with pronator quadratus preservation (improvement group) and 38 using Henry approach with pronator quadratus cutting (traditional group). All patients were treated with volar plate internal fixation. The fracture healing, internal fixation and complications were observed. The wrist pain visual analogue score (VAS), forearm rotation range and grip strength were compared between the two groups at 1 week, 1 month, 3 months and 12 months postoperatively. The wrist joint function score of the two groups was compared by using the upper limb function assessment table (DASH) at 3 months and 12 months postoperatively.Results:All patients were followed up for 12-24 months [(15.2±6.3)months]. All fractures were healed after 3 months. The position of internal fixation was good, and there were no complications of infection, nonunion, plate loosening and tendon injury. During the follow-up period, 2 patients in traditional group had carpal tunnel syndrome, which was relieved after removal of internal fixation. The VAS in improvement group was lower than that in traditional group at 1 week, 1 month and 3 months postoperatively [(6.17±0.40)points vs. (6.80±0.45)points, (5.54±0.40)points vs. (6.08±0.70)points, (3.18±0.30)points vs. (3.75±0.40)points, respectively] ( P<0.05). The forearm rotation range in improvement group was larger than that in traditional group at 1 week, 1 month and 3 months postoperatively [(49.40±17.5)% vs. (40.3±13.2)%, (66.7±14.3)% vs. (54.2±18.7)%, (87.3±7.2)% vs. (80.7±8.1)%, respectively] ( P<0.05). There was no significant difference in VAS and forearm rotation range between the two groups at 12 months postoperatively ( P>0.05). There was no significant difference in grip strength between the two groups at 1 week, 1 month, 3 months and 12 months postoperatively ( P>0.05). There was no significant difference in DASH score between the two groups at 3 months and 12 months postoperatively ( P>0.05). Conclusion:For elderly patients with distal radius fracture, modified Henry approach with pronator quadratus preservation combined with volar plates is relatively better in alleviating postoperative pain, improving early rotation function and promoting early functional recovery.
7.The reasons and treatments of recurrent or residual cholesteatoma in middle ears.

OBJECTIVE:
To explore the reasons and treatments of recurrent or residual cholesteatoma in middle ears after operations.
METHOD:
The clinical data of 102 cases (105 ears) with recurrent or residual cholesteatoma was retrospectively analyzed.
RESULT:
The main reason of recurrent or residual cholesteatoma is incomplete removal of cholesteatoma in the former operations or obstructive drainage of middle ears after operations. Twenty ears healed through cleaning with otoendoscope. Eighty-five ears underwent the second operations of radical mastoidectomy including 23 tympanoplasty meanwhile. The air-conductive auditory threshold of them all decreased more than 15 dBHL.
CONCLUSION
For the ears with low facial ridges and non-obstructive drainage of mastoid, tympanic antrum and tympanic cavity, its possible to be cured through cleaning with otoendoscope. For those with high facial ridges and obstructive drainage , it's essential to perform the second operations of radical mastoidectomy and some of them are suitable for tympanoplasty meanwhile.

8.Establishment and mechanism of a BALB/c mouse model of total hysterectomy

Objective To establish a BALB/c mouse model of total hysterectomy and to investigate and analyze its characteristics and formation mechanism. Methods The control group was fed conventionally without any treatment. The mice in the model group were anesthetized and performed hysterectomy. The animals in the sham-operated group had abdominal cavity opened and only exposing the uterus. The changes of body weight were observed dynamically,and at the end of the fourth weeks. The concentration of serum estradiol was determined and histological structure of the ovaries was examined. Results The general status of all the mice after operation was good without obvious inflammation, and the incision healed completely. There was no significant difference in the body weight before operation among the mice, whereas the weight of model group mice was higher than the control group and sham-operation group at the end of the second,third and forth weeks(P< 0.05). The estradiol contents of the model group was significantly lower than that in control group and sham operation group(P< 0.01). The histological structure of ovaries in the control group and sham-operation group was normal,however the model group showed differences,such as irregular shape of the ovaries,changed number of mature follicles, loosely distributed granulosa cells, and apoptosis. Conclusions The mouse model of total hysterectomy established in our study can well simulate the clinical manifestations of ovarian dysfunction after operation, and is an ideal model for the study of complications after ovariectomy and screening of drugs.
9.Interrupted time series analysis for impacts of day surgery on average days of stay

Objective To evaluate the effects of day surgery on average days of stay, and to provide scientific basis for shorter average days of stay. Methods Data of average days of stay from November 2011 to December 2016 were extracted from 6 departments, Xiangya Hospital, Central South University. Segmented regression analysis of interrupted time series was used to analyze the trend of average days of stay of pre-and post-day surgery. Results Thanks to the application of day surgery, average days of stay decreased by 0. 071 days per month, and the monthly decline increased by 0. 049 days (P<0. 001) than before. Average days of stay in the department of stomatology, ear-nose-throat, general surgery and hepatobiliary & enteric surgery research center were the most obvious, and the monthly decline increased by 0. 110 days (P<0. 001), 0. 049 days (P=0. 008) , 0. 075 days (P<0. 001), and 0. 057 days (P=0. 003), respectively than before. Conclusions Day surgery could decrease average days of stay, enhancing the utilization of hospital resources.
10.Exploring the high-quality development path of public hospitals based on five core indicators

The five core indicators(the case mix index, the proportion of level-4 surgery, the proportion of technical service income in medical income, the proportion of personnel expenditure in business expenditure, and the proportion of fixed part in personnel salary) and target values determined by the pilot goal of high-quality development of high-level public hospitals jointly built by National Health Commission and Provincial Government had become the core indicators for evaluating the quality and efficiency of public hospital development. The authors proposed the optimization and improvement measures for the five core indicators one by one from the internal reform and innovation in hospitals, as well as the synergetic development and governance of " insurance-medical-medicine linkage", combined with the current situation of a pilot hospital. The authors also proposed the starting points and driving force for promoting the high-quality development of public hospitals through the synergetic development and governance of " insurance-medical-medicine linkage", including implementing government investment responsibilities, streamlining the price evaluation of medical services, deepening the reform of medical insurance payment, accelerating the expansion of drug and consumable procurement, and promoting the application of advanced technologies and products, in order to provide reference for the in-depth promotion of high-quality development of public hospitals.
