Objective To study the significance of circadian gene Period2 expression in epithelial ovarian cancer tissues and the effect of gene overexpression on the growth of ovarian cancer xenografts in nude mice. Methods Twenty-two cases of ovarian cancer paraffin specimens in the First Hospital of Shanxi Medical University (ovarian cancer group) were chosed during Jau. 2010 to Dec. 2013, including 8 cases of stageⅠ, 8 cases of stageⅡ, and 6 cases of stageⅢ, while 6 cases of benign ovarian epithelial tumor paraffin specimens were selected as control (benign tumor group). Period2 gene were detected by real-time quantitative PCR and western blot methods in different stages of ovarian cancer tumor tissues. Established theovarian cancer xenografts in nude mice with ovarian cancer cell line SKOV3, and they weredivided into 3 groups (n=8), including the recombinant plasmid group, empty plasmid group and control group. Using gene transfection technique to transfer Period2 gene into tumor tissues, tested the expression of Period2 mRNA in tumor tissues by real-time quantitative PCR after transfection into all nude mice, monitoredthetransplant tumor growth and calculating the tumor inhibition rate,detected the antiapoptotic gene BRE, apoptosis related tumor necrosis factor receptor (TNFR1) andtumor suppressor gene NIX in tumor tissues by real-time PCR and western blot in different groups. Results (1)The expression level of Period2 mRNA in tumor tissues amongovarian cancer group stageⅠ,ⅡandⅢwere respectively 2.59±0.50, 0.47± 0.08 and 0.42 ± 0.08, but benign tumor group was 6.59 ± 1.05. The expression level of Period2 protein in ovarian cancer group stage Ⅰ,Ⅱ and Ⅲ were respectively 0.835 ± 0.087, 0.412 ± 0.035 and 0.199 ± 0.031, while benign tumor group was 0.874 ± 0.094. The expression level of Period2 mRNA and protein in benign tumor group was higher than those in ovarian cancer group stageⅠ,ⅡorⅢ(P<0.01). With ovarian cancer stage increased, the expression of Period2 mRNA and protein were decreased or absent (P<0.05).(2)Two weeks after transfection, the expression level of Period2 mRNA in recombinant plasmid group tumor tissue was significantly higher than those in the empty plasmid group or the control group (6.11±0.56 vs 0.50±0.09 vs 0.44 ± 0.08, respectively;P<0.01), the transplanted tumor volume of recombinant plasmid group was significantly less than those in empty plasmid group or the control group[ (486±70) mm3 vs (835±106) mm3 vs (846 ± 110) mm3, respectively;P<0.01], the tumor inhibition rate of the recombinantin plasmid group was as high as 42.9%, that was significantly higher than those in the empty plasmid group and the control group (3.8% and 0, respectively;P<0.05).(3)The expression level of BRE mRNA and protein in transplanted tumor tissues in the recombinant plasmid group were significantly lower than those in empty plasmid group and the control group;the expression level of TNFR1 and NIX were significantly higher than those in the empty plasmid group and the control group (all P<0.05). Conclusions Period2 mRNA and protein expression are absent in ovarian cancer of advanced stage. Transfection and stable expression of Period2 gene could slow down the growth of ovarian cancer, and the tumor inhibition rate could be significantly increased. Period2 gene may promote ovarian cancer cells apoptosis through inhibition of BRE gene expression and promoting TNFR1, NIX gene expressionto exert anti-tumor effect.

Objective To investigate the effects of different cardiopulmonary resuscitation (CPR) qualifies under Q-CPR control on hemodynamics and aerobic metabolism in pigs with cardiac arrest. Method After eighteen pigs (mass [ (30+ 1) kg] were anesthetized,a Swan-Ganz catheter was positioned through the right femoral vein and linked with a continuous cardiac output monitor. An electrode cable was positioned into right ventricle through left internal jugular vein. Then different catheters were positioned in the arteriae aorta, carotid artery and fight atrium successively. Ventricular fibrillation (VF) was induced by a programmed electrical stimulation equipment. All animals underwent 4 minutes of untreated VF and then were randomly assigned to either standard CPR or nonstandard CPR. A Philips HeartStart MRx Monitor/Defibrillator was used to ensure qualifies of chest compression (e.g., depth, rate and chest recovery) by Q-CPR technology. Pigs in standard CPR group were treated with standard compression with a compression rate of 100 beat/min, a compression-ventilation ratio of 30:2, and a depth of with 38～51 mm with 100% decompression. Pigs in nonstmdard CPR group were treated with nonstandard compression with a depth of 60%～70% of standard depth with unsatisfactory decompression. Defibrillations were administered after 9 minutes of compressions and vetilations. Cardiac output (CO), mean aortic pressure (MAP) and coronary perfusion pressure (CPP) were recorded at different periods. Oxygen delivery (DO2) and oxygen consumption (VO2) were calculated according to blood gas analysis. At last,number of the survivals was recorded.Data were calculated by SPSS 11.5 statistics software. Statistical analysis was performed using Chi-Square Test and two independent samples t testing. Results The return of spontaneous circulation (ROSC) achievement ratio with nonstandard CPR was 28.6% and with standard CPR was significantly greater (90.9%, P=0.013).Hemodynamics (e. g., CPP, CO and MAP) and systemic oxygenation with standard CPR were much better. Conclusions In this animal model of cardiac arresr in pigs, standard CPR could improve hemodynarmics by increasing CPP and CO. And standard CPR could beneficial aerobic metabolism by increasing DO2 and VO2. As a result, ROSC achievement ratio of pigs with standard CPR was much greater.

Objective To explore the application effect of clinical nursing pathway on maintenance hemodialysis patients.Methods Totally 120 patients with maintenance hemodialysis between January 1 and December 31 2009 were randomly assigned to the control group and the observation group with 60 patients in each group.The clinical pathway was established and applied to the observation group.The treatment effect was compared between two groups.Results The incidence of complications and nursing errors declined with the application of clinical nursing pathway,and the doctor's orders compliance and the satisfaction and quality of care were improved significantly.Conelusions The application of clinical nursing pathway in maintenance hemodialysis patients is an effective method for improving overall care quality and the satisfaction degree of the patients with nursing,so as to improve their survival rate.

Objective To investigate effect of probiotics in the treatment of patients with severe hepatitis and its influence on inflammatory cytokines.Methods A retrospective analysis of the data of patients with severe hepati-tis were maded.There were 100 patients,which including 50 cases of the observation group and 50 cases of control group,the control group was given conventional treatment,the observation group was given Bacillus viable capsule (Zhengchangsheng) for 4 weeks,2 granule a time and three times a day.The clinical symptoms were observed before and after treatment,the blood was exsanguinated for detecting bowel flora,total bilirubin,liver function and inflamma-tory cytokines.Results The total effective rate of the observation group was 86%,which was significantly higher than 56% of the control group(χ2 =11.409,P <0.01).The enterobacter of the observation group after treatment was (6.71 ±1.30)lgn/g,which was lower than before treatment(t =3.602,P <0.05),the bifidobacterium,lactobacillus, bacteroides were (7.88 ±1.18)lgn/g,(8.08 ±1.65) lgn/g,(8.30 ±1.20)lgn/g,respectively,which were higher than before treatment(t =3.772,3.707,3.523,all P <0.05).The intestinal flora in the control group had no differ-ence before and after treatment.After treatment,the total bilirubin,ALT,plasma endotoxin of the observation group and the control group were (195.0 ±25.4)μmol/L and (307.6 ±40.5)μmol/L,(58.2 ±11.5)U/L and (90.2 ± 10.5)U/L,(0.18 ±0.06)EU/mL and (0.29 ±0.08)EU/mL,respectively,which were lower than before treatment (t =16.307,9.408,10.157 and 4.032,8.075,5.076,all P <0.01).The serum TNF -α,IL -6 levels of the obser-vation group were (106.2 ±12.5)pg/mL,(82.3 ±18.9)pg/mL,which were lower than before treatment(t =3.732, 4.017,all P <0.05),which of the control group were (130.5 ±20.6)pg/mL,(110.2 ±17.8)pg/mL,there was no difference before and after treatment.The IL -2,IL -10 levels of the observation group were (60.7 ±10.0)pg/mL, (31.7 ±6.6)pg/mL,which were significantly higher than before treatment(t =3.757,3.877,all P <0.05),which of the control group were (46.2 ±5.8)pg/mL,(23.4 ±5.1)pg/mL,which had no difference before and after treat-ment.The total bilirubin,ALT,plasma endotoxin,serum TNF -α,IL -6 levels of the observation group were lower than the control group(t =7.653,8.104,3.309,3.511,3.787,all P <0.05).The IL -2,IL -10 levels of the obser-vation group were higher than the control group(t =3.487,3.428,all P <0.05).Conclusion Probiotics can correct intestinal flora,reduce plasma endotoxin,inhibit inflammatory mediators,reduce inflammation of the liver damage, improve liver function and clinical symptoms and should be introduced in the treatment of severe hepatitis.

Objective To investigate the clinical efficacy and the influence on T cell subsets of telbivudine in the treatment of HBeAg -positive chronic hepatitis B , guide clinical treatment .Methods 70 patients HBeAg -positive chronic hepatitis B in our hospital were randomly divided into the telbivudine group which were received telbi -vudine 600mg/d and lamivudine group which were received lamivudine 100mg/d,35 cases of each group ,both group had been treated for 48 weeks,the liver function (ALT,AST) were detected for making a statistics of ALT normaliza-tion,the serum were extracting for detecting HBV -DNA and making a statistics of HBV -DNA negative rate by PCR,the HbeAg were detected for making a statistics of HBeAg seroconversionby ECLIA ,T cell subsets ( CD3+,CD4+, CD8+,CD4+/CD8+) were detected by flow cytometry ,observed adverse events .Results The ALT,AST,HBV-DNA of the telbivudine group and lamivudine group after treatment were (36.04 ±7.62)U/L and (63.53 ±14.13)U/L, (34.26 ±8.51)U/L and (55.31 ±15.48)U/L,(2.22 ±0.73)copies/mL and (3.28 ±0.95)copies/mL,all indi-cators significantly lower than before treatment (t =24.826,18.564,7.495 and 17.413,13.331,5.442,all P<0.05),and the ALT,AST of the telbivudine group were significantly lower than the lamivudine group ( t=3.867, 3.774,3.498,all P<0.05).The ALT normalization,HBV-DNA negative rate,HBeAg seroconversion rate of the telbivudine group and lamivudine group were 97.14%and 74.29%,94.29%and 68.57%,62.86%and 37.14%, telbivudine group,the telbivudine group were significantly higher than that of lamivudine group (χ2 =7.467,7.652, 4.629,all P<0.05).The CD3+,CD4+,CD4+/CD8+of telbivudine group and lamivudine group after treatment were (66.3 ±3.8)%and (60.6 ±3.5)%,(38.8 ±5.4)% and (35.2 ±4.4)%,(1.33 ±0.16)% and (0.95 ±0.11)%,all indicaitons significantly higher than before treatment ( t =5.442,6.173,3.753 and 3.404,3.635, 3.222,all P <0.05),the telbivudine group were significantly higher than lamivudine group (t =3.473,3.207, 3.422,all P<0.05).Conclusion The telbivudine has better efficacy than the lamivudine in the treatment of HBeAg-positive chronic hepatitis B because of enhancing immune function ,can improves ALT normalization ,HBV-DNA negative rate and HBeAg seroconversion rate ,inhibit HBV replication .

Objective To study the regulation effect of salvianolic acid A and C molecular compatibility on inflammatory chemokine monocyte chemotaxis protein 1(CCL2)and CXC chemokine ligand 10(CXCL10)in human renal tubular epithelial(HK-2)cells intervened by human serum albumin(HSA),and to explore the anti-inflammatory and antioxidation effects of salvianolic acid A,C molecular medicine-pair compatibility.Methods The cultured human renal tubular epithelial cells were randomly divided into 5 groups:control group,model group,salvianolic acid A group(20 μmol/L),salvianolic acid C group(20 μmol/L),salvianolic acid A+C group(10 μmol/L salvianolic acid A+10 μmol/L salvianolic acid C).Except for the control group,the other groups were added with HSA intervention for 24,48,72 h,then the each treatment group was simultaneously added with the drug treatment.The enzyme linked immunosorbent assay(ELISA)method was used to detect the expression of CCL2,CC chemokine 7(CCL7)and CXCL10,and SOD,GSH and MDA were detected by WST-1,DTNB,TBA.Results Compared with the control group,the levels of CCL2,CCL7,CXCL10 and MDA at 3 time points of 24,48 72 h in other groups were significantly increased,while the GSH and SOD levels were significantly decreased(P＜0.05).Compared with the model group,the levels of CCL2,CCL7,CXCL10 and MDA in each treatment group were relatively decreased,while the GSH and SOD levels were relatively increased(P＜0.05).In the comparison of various treatment groups,the levels of CCL2,CCL7,CXCL10 and MDA in the salvianolic acid A+C group were lower than those in the salvianolic acid A group and salvianolic acid C group(P＜0.05),while the GSH and SOD levels were higher than those in the salvianolic acid A group and salvianolic acid C group(P＜0.05).Conclusion The salvianolic acid A and B molecular medicine-pair compatibility may improve renal fibrosis by decreasing the expression of inflammatory chemokines and antioxidation.

Objective To investigate the difference in complete blood count (CBC) within the defined time among 28 hospitals with different grade in Urumchi City, explore its influencing factors,offer the countermeasures, and further to provide scientific evidence for the mutual recognition among different medical institutions. Methods Totally 28 laboratories with different grade were enrolled in the investigation. Three fresh blood samples were employed to perform complete blood counting, in-cluding normal and pathological specimens. The measuring time and turn around time (TAT) were de-fined. Results According to the literature report on biological variability of testing results, the per-missible deviation of CBC was tentatively prescribed as follows: WBC 15% ;RBC 6% ;HB 7% ;HCT 6 % ;PLT 25 %. Qualification decision was based on the external quality assessment (EQA) criteria of Clinical Laboratory Centre of the Health Ministry. Qualification: the total score of five measured items is higher than or equal to 80%;disqualification: the total score of five measured items is lower than 80%. For the first and second sample, 26 laboratories (92.86%) were qualified;for the third sample (only 21 laboratories enrolled), 14 laboratories (67%) were qualified. Conclusion 28 laboratories par-ticipated in this test. The results of five items are congruent for the first and second sample (normalspecimens) in 28 laboratories, and the measured results are comparable. By contrast, the comparative result is unsatisfactory for the third sample (pathological specimen), mainly due to the narrow linear range of equipment. Meanwhile, the laboratory quality management should be strengthened.

Objective To discuss the relationship among hypoxia-inducible factor-1α (HIF-1ct),platelet derived growth factor (PDGF) and vascular endothelial growth factor (VEGF)/VEGFR signaling pathway in breast cancer occurring and metastasis in the early stage.Methods The clinical and pathological features and the expression of HIF-1 α,PDGF-A,VEGF-A and VEGFR-2 were assayed by histomorphology and immunohistochemical staining on tissue specimens from 61 patients of breast cancer and healthy breast tissues healthy breast tissue from 17 patients of benign breast disease.Results VEGF expression was observed in 39 out of 61 malignant breast tissues,more than distribution of positive staining 20 patients (51.3％) of which.VEGFR expression was found in 42 out of 61 lesions,.more than 10％ distribution of positive staining 22 patients (52.4％) of which.Simultaneous expression of VEGF and VEGFR-2 was found in 12 cases.HIF-1α was expressed in 50 and PDGF in 39 out of 61 breast cancer patients,and in 36(72.0％) HIF-1α positive specimens and 23 (59.0％) PDGF positive specimens the staining area was more than 10％.The expression of VEGFR was correlated with patient's age (x2 =11.080,P =0.001) and lymph node metastasis (x2 =4.699,P =0.046).VEGF,HIF-1α and PDGF were not correlated with patient's age (x2 =0.880,P =0.415; x2 =1.620,P =0.303 ; x2 =0.150,P =0.786) and lymph node metastasis (x2 =0.677,P =0.437 ; x2 =1.394,P=0.323;x2 =1.841,P=0.192).Conclusion HIF-1α,PDGF,VEGF and VEGFR-2 express in breast tumor tissues at a high level,and VEGFR-2 is correlated with patient's age and lymph node metastasis.Abovementioned proteins can be used as markers for diagnosis,prognosis and targeted therapy.

Objective　To study the characteristics of choroidal circulation in RP.　Methods　Using ICGA to observe 37 cases of RP and compare with healthy volunteers.　Results　① The earliest fluores-cein filling time of the choroidal arteries in RP group was (14.38±3.95) seconds,the choroidal veinous in RP group was (17.27±5.94) seconds,and there was no obvious difference between RP and control group.②The fluorescein failing time of choroidal vein in RP group was (475.75±153.70)seconds.③The area of the bright fluorenscence in posterior fundus in RP group was (41.20±19.99)　mm2,and compared with the control group,there was significant difference (P＜0.0001).④In the mid to late phase during ICGA，in RP group the veil-like hypofluorescence was found in 61 eyes (84.7%),plaque hyperfluorescence in posterior fundus in 21 eyes (29.2%),and leakage of heperfluorescence in 4 eyes(5.6%).　Conclusion　①The perfusion pressure of choroidal vessels in RP reveals no change.②The blood volume of choroidal vessels becomes decreased in RP.③The choroidal capillaries become atrophic in RP.④Choroidal neovascularization may occur in patients with RP.
