2.The effect of recombinant human endostatin combined with 5-Fluorouracil on the growth of xenografted gastric cancer in nude mouse

Objective This experiment was to identify the influence of recombinant human endostatin (rhES) combined with 5-FU on the growth of a xenografted gastric cancer in nude mouse. Methods After the establishment of the model, mice were divided into 4 groups (six in each) receiving respectively, for consecutive 10 days: NS, 5-FU (10 mg/kg) intraperitoneally; rhES (2 mg/kg); and 5-FU + rhES. On 15th day tumor volume was measured. The expression of VEGF, bFGF, VEGF-C, PCNA, bcl-2 and apoptosis index (AI) were examined. The tumor inhibition rate(TIR) and tumor shrinking rate(TSR) were calculated. 3.Long-term effect of combined portoazygous devuscularization and shunt for treatment of portal hypertension.

ObjectiveTo evaluate the therapeutic effect of combined portoazygous devascularization and shunt on portal hypertension. MethodsThe data of 56 cases of portal hypertension treated with portoazygous devascularization and shunt from April 1987 to April 1999 was summarized. ResultsThere was no death in 54 patients receiving selective operation;of 2 cases receiving emergency operation, one died. Among 49 followed-up cases, there was one rebleeding,2 hepatic encephalopathy, and one died of liver failure 4 years after opeation. The mean portal pressure before and after operation was(3.42±0.46) kPa and (2.50±0.35) kPa, respectively. ConclusionCombined portoazygons devascularization and shunt not only decreases portal pressure but preserve hepatic blood flow to some extent.

Objective To study the expression of nuclear factor-?Bp65 (NF-?Bp65) in gastric carcinoma and its relationship with vascular endothelial growth factor (VEGF). Methods The expression of NF-?Bp65 and VEGF in 56 gastric carcinomas was detected with immunohistochemistry and compared with benign tissues. Results The positive rates of NF-?Bp65 and VEGF in 56 gastric carcinomas were 62.5% and 76.8% respectively,and which were higher than those of gastric mucosal atypical hyperplasia (33.3% and 44.4%) and the normal gastric mucosa(0 and 8.3%) (P0.05). Objective To investigate the pathogenesis of portal hypertensive gastropathy(PHG). Methods Two rat models with portal hypertension(PHT) and a sham operation group were established to detect the pathological changes in histology and ultrastructure of the gastric mucosa as well as quantitative changes in histological morphology by graphic analysis computer system. Results Prominent edema, scattered red dots/ecchymosis were found in gastric mucosa in rats with PHT. Light and scan electron microscopy showed swelling or exfoliating of the epithelium cell, reduction of gastric gland number, thin of gastric mucosa, while infiltration of inflammatory cells, epithelium metaplasia were not found. The most characteristic findings were the changes of the mucosal capillaries and venules in both mucosal basal lamina and submucosa layer light microscopically, as well as the transmutation, stenosis and loose intercellular joining of the capillaries on electron microscopy. Ultrastructure observation revealed numerous pinocytes in epithelial cells as well as proliferation and hyperplasia of smooth muscle, collagenous fiber and extracellular matrix in venules. Quantitative analysis showed that the ratios of the cross sectional area and the vascular wall area between the gastric wall area(CSA/GWA & VWA/GWA) was higher in liver cirrhosis(LC) and portal vein stenosis(PVS) groups than that in sham operation(SO) group. There was a positive correlation between portal vein pressure and the ratio of the length of damaged mucosa and the length of mucosa(LDM/LM that was higher in LC group than in PVS group). Conclusions In rats with cirrhotic portal hypertension, distinct gastric microvascular morphological changes are the major etiological factor of PHG as a part of pathological changes in portal hypertension.

Objective To investigate the relationship of greyscale ultrasonographic signs and clinical pathological characteristics with axillary lymph node metastasis in invasive breast cancer.Methods Three hundred and thirty-five patients with pathologically confirmed breast cancer were retrospectively analyzed,which categorized into 2 groups,including lymph node metastasis group and lymph node non-metastasis group.The clinical and pathological characteristics included age,primary tumor size,histological grade,and molecular subtypes.The ultrasonographic features were examined including orientation,shape,margin,echo pattern,posterior acoustic feature,and calcification.Chi-Square test and Logistic regression analysis were performed to analyze the relationship of clinical pathological characteristics and greyscale ultrasonographic signs with axillary lymph node metastasis.Results Age (<45 years),primary tumor size (≥2 cm),histological grade(Ⅲ grade),molecular subtype (Luminal B),and number of margin angulation (> 10) contributed to axillary lymph node metastasis in 335 cases,according to univariate analysis (P <0.05).Multivariate Logistic regression analysis showed age (OR =0.573,P =0.019),the primary tumor size (OR=2.359,P =0.001),histological grade (OR=0.529,P =0.008),and number of marginangulation (OR =1.889,P =0.031) were risk factors of axillary lymph node metastasis in breast cancer.Conclusions Univariate and multivariate analyses show the correlation between ultrasonographic features and axillary lymph node involvement.Combining with clinical pathological data,it can provide a predictor of axillary lymph node metastasis.

To establish a new extrahepatic growing hepatocellular carclnoma cell line. Methods: A speciment that was pathologically identified extrahepatic growing hepatocellular carcinoma were cultured in vitro. The morphology,karyotype analysis, DNA analysis, the tumor formation of heterotransplantation were observed. Results: Morphological observation and functional analysis showed that it had the comnnon features of HOC.It was found to be able to secret AFP in vitro. Conclusion: EGHC-9901 has characteristic of the extrahepatic growing hepatocellular carcinoma cell line.

Objective To study the anatomy of pelvic autonomic nerves, to avoid autonomic nerves damage during rectal cancer surgery. MethodsAnatomical dissections were carried out on 7 adult cadavers, and clinically autonomic nerve-preserving rectal cancer resection was performed. Results The superior hypogastric plexus lies just posterior to the inferior mesenteric vessels. There were no obvious autonomic nerve trunks in the loose areolar tissue plane between the parietal fascia and the visceral fascia right posterior to the rectum. There were no obvious autonomic nerve trunks between the rectum and the seminal vesicles or the prostate. The inferior hypogastric plexus was a rhomboid shaped plaque of nervous tissue. The main components of the lateral ligament were autonomic nerves passing from the pelvic plexus to the rectum within a condensation of connective tissue. WT5”HZConclusionsThe inferior mesenteric vessels can be used a landmark intraoperatively to identify the superior hypogastric plexus. In order to preserve the inferior hypogastric plexus while dissecting the lateral of rectum, we should dissect along the medial surface of the inferior hypogastric plexus, and along its curvature. 10.Treatment of inflammatory breast cancer

Objective To evaluate the effect of combined therapy for inflammatory cancer of breast (ICB). Methods A retrospective analysis was made on the clinical manifestation, pathological type and treatment modalities of 25 patients with ICB from 1985 to 2000.Results All of the 25 patients were treated with mastectomy, including typical Halsted mastectomy in 12 cases, typical Halsted mastectomy with reconstruction of latissimus dorsi flap in 4, and palliative mastectomy in 9. Of the 21 patients 20 received preoperative chemotherapy; and all the 21 patients received postoperative chemotherpyl; while hormonal therapy was used in 11 cases. A mean survival was 23 months (0.5~5years) in 14 followed up patients; and now 5 patieats remain alive (2months~5 years). Conclusions A combined therapy consisting of chemotherapy, surgery and hormonal therapy may improve the prognosis of ICB.
