1.Role of lamotrigine in repair of hemisection of spinal cord in mice models

Objective To investigate the effect oflamotrigine (LTG) on repair ofhemisection of spinal cord in mice models.Methods A total of 80 female C57BL/6 mice were employed to establish the models of spinal cord hemisection,and randomly divided into 3 groups:spinal cord injury (SCI) group (n=27),SCI+LTG group (n=26) and SCI+0.9% saline group (n=26);mice in SCI+LTG group were given intraperitoneal injection oflamotrigine (25 mg/kg) for a consecutive 7 d,and mice in the SCI+0.9% saline group were given the same volume of 0.9% saline.Basso,Beatti,Bresnahan (BBB) scale was performed 1,7 and 14 d after SCI;6 mice in each group were sacrificed 1,7 and 14 d after SCI,and glial fiber acidic protein (GFAP) and ionized calcium-binding adaptor molecule 1 (Iba1)-positive cells were observed by immunofluorescence and the expressions of inflammatory factors interleukin (IL)-1,IL-10 and tumor necrosis factor (TNF-α) were observed by ELISA.Results On the 7th and 14th d of injury,the BBB scale scores in the SCI+LTG group were significantly higher than those in the SCI group and SCI+0.9% saline group (7 d:5.1667±0.40825,4.0000±0.63246 and3.8333±0.40825;14 d:6.5000± 0.5477,5.5000±0.5477 and 5.3333±0.5164,P<0.05).On the 7th and 14th d of injury,less percentage of GFAP positive ftuorenscent area and fewer number of Iba1 positive cells in the SCI+LTG group were noted than those in the SCI group and SCI+0.9% saline group (P<0.05).On the 7th d of SCI,the IL-1 and IL-10 expressions in the SCI+LTG group were obviously lower than those in the SCI group and SCI+ 0.9% saline group (P<0.05).Conclusion Lamotrigine improves the motor function after SCI by decreasing the secretion of inflammatory factors and activation of glial cells.
2.Study of the biological characteristics of p75 neurotrophin receptor positive tongue squamous cell carcinoma cells.

OBJECTIVETo study the biological characteristics of p75 neurotrophin receptor positive (p75(NTR+)) tongue squamous cell carcinoma cells which were separated by flow cytometry cell sorting.
METHODSTo determine the biological characteristics of p75(NTR+) cells which were separated from Tca-8113 and Cal-27 tongue squamous cell carcinoma cells by flow cytometry cell sorting, including study the capacity of cloning, 3-(4,5)-demethylthiazo(z-y1)-3,5-diphenytetrazoliumromide (MTT) assay, wound healing assay. p75(NTR+) cells with non-sorted cells were as control group.
RESULTSIn Tca-8113 and Cal-27 tongue squamous cell carcinoma cell lines, the percentage of p75(NTR+) cells were 3.1% and 1.9%. Compared with p75(NTR+) cells with non-sorted cells, p75(NTR+) cells possess higher capacity of cloning (Tca-8113, P=0.024; Cal-27, P=0.009). The percentage of p75(NTR+) cells of the progeny cells generated from monoclonal p75(NTR+) cells decreased to 14.5% (Tca-8113) and 5.8% (Cal-27) after cultured two weeks. p75(NTR+) cells possessed higher proliferation ability and higher metastasis ability than non-sorted cells.
CONCLUSIONp75(NTR+) cells isolated from tongue squamous cell carcinoma have the characteristics of cancer stem cells.

3.Molecular mechanism of P75 NTR gene-induced apoptosis in tongue squamous cell carcinoma cell

Objective To investigate the molecular mechanism of P75NTR gene-induced apoptosis in tongue squamous cell carcinoma Tca8113 cell lineage.Methods P75NTR specific siRNA was transferred into P75NTR positive tongue squamous cell carcinoma Tca8113 cells.P75NTR positive Tca8113 cells were divided into 4 groups:blank group (without transfection),negative control group (transfected with negative control siRNA ), experiment group-776 (transfected with siRNA-P75NTR-776 ) and experiment group-1234 (transfected with siRNA-P75NTR-1234).Transfection efficiency and cell apoptosis were detected by flow cytometry.The interference effect of P75NTR mRNA expression was detected by fluorescence quantitative PCR. 3-(4,5-dimethyl-2-thiazoly)-2,5-diphenyl-2H-tetrazolium bromide assay was applied in measuring cell prolife-ration.The protein changes of P75NTR were detected by Western blotting.The distributions of nuclear factor-κB(NF-κB)of cells were observed by cell immunofluorescence labeling method.Results The transfection efficiency was 30%.The apoptosis rate of experiment group-776,experiment group-1234 and negative control group was (20.35 ±0.18)%,(12.32 ±1.51)% and (2.63 ±0.10)% respectively.Compared with the negative control group,the differences of the former two group had statistical significance (t =177.20,P <0.005;t =37.12,P <0.005).The P75NTR gene interference was successful.The inhibition rate of P75NTR protein reached 31% in experiment group-776.The cell viability of Tca8113 cells after P75NTR-siRNA inter-
ference was 70.02%,78.01% and 95.81% in experiment group-776,experiment group-1234 and negative control group.And there were significant differences between experiment group-776 and negative control group (χ2 =235.3,P <0.010),and between experiment group-1234 and negative control group (χ2 =117.5,P <0.005 ).NF-κB distribution was increased in cell cytoplasm in the interference group than that in control group.Conclusion P75NTR may promote the proliferation or inhibit the apoptosis of tongue squamous cell carcino-ma,and the molecular mechanism may be correlated with hindering the transportion of NF-κB into cell nuclear.
4.Protective effects and its mechanism of rebamipide on aspirin-induced injury in human gastric mucosal epithelium cells

Objective To investigate the protective effects and its mechanism of rebamipide on aspirin-induced injury in human gastric mucosal epithelium cells (GES-1).Methods GES-1 cells monolayer culture model was established in vitro.Then the cells were divided into negative control group,aspirin injured group and combination of rebamipide at different concentration (0.2,0.5,1.0 mrnol/L) and aspirin groups.The cell proliferation,the content of malondialdehyde (MDA) and the activity of superoxide dismutase (SOD) of each group were detected.The ultrastructural changes of each group were observed by transmission electron microscopy (TEM).The expressions of nuclear factor erythroid 2-related factor 2 (Nrf2) and heme oxygenase-1 (HO-1) at protein level in the cells of each group were detected by Western blot.Nrf2 interfering suppression test was performed and then the influence of Nrf2 small interfering RNA (siRNA) on the expression of HO-1 protein was observed.One-way analysis of variance was performed for comparison among multi-groups and t-test was used for comparison between the two groups.Results The cell viability of aspirin injured group and combination of rebamipide at different concentration (0.2,0.5,1.0 mmol/L) and aspirin groups were (49.56±3.88)%,(59.34±4.36) %,(70.79 ± 5.96) % and (86.07 ± 5.20) %,respectively,and the difference was statistically significant (F=30.634,P< 0.01).Compared with aspirin injured group,the content of MDA significantly lowered in combination of rebamipide at different concentration (0.2,0.5,1.0 mmol/L) and aspirin groups ((2.26±0.25) nrnol/rng vs (1.85±0.13) nmol/mg vs (1.62±0.11) nmol/mg vs (1.13±0.15) nmol/mg),and the difference was statistically significant (F=23.821,P<0.05).Compared with aspirin injured group,the activity of SOD significantly increased in combination of rebamipide at 0.5 and 1.0 mmol/L and aspirin groups ((8.49±0.89) U/rng vs (11.50±1.03) U/mg vs (13.74±0.76) U/mg),the difference was statistically significant (F=25.666,P<0.05).Under TEM,the cell ultrastrucmral was obviously inured in aspirin treated,while rebamipide could relieve the injury.The differences of relative expression quantity of Nrf2 and HO-1 at protein level among combination of rebamipide at 0.2,0.5 and 1.0 mmol/L and aspirin groups and aspirin injured group were statistically significant (0.35±0.04 vs 0.46± 0.05 vs 0.84±0.08 vs 0.15±0.02,0.72±0.09 vs 0.93±0.11 vs 1.29±0.14 vs 0.39±0.07,F=92.550and 38.235,both P<0.05).After transfected with Nrf2 siRNA,the expression of HO-1 was 0.38±0.04 in aspirin injured group and 0.62±0.08 in combination of rebamipide and aspirin group,which was lower than that before transfection (0.61 ± 0.05,1.33± 0.09),respectively.The differences were statistically significant (t =6.276 and 10.444,both P<0.05).Conclusion Rebamipide may activate Nrf2/HO-1 pathway and relieve aspiriwinduced oxidative stress in GF1 ceils.
5.Weighted index of comorbidities evaluates the impact of underlying diseases on hospital mortality of pneumonia patients

Objective To estimate the validity of Charlson' s weighted index of comorbidities (WIC) used to predicting 28-day mortality among ICU pneumonia patients with underlying diseases.Methods Aretrospective analysis of 160 adult patients with pneumonia admitted to a multi-discipline ICU of Shanghai Changzheng hospital between October 2010 and February 2012 was carried out.Clinical data were collected including age,gender,community acquired pneumonia (CAP) or hospital acquired pneumonia (HAP),underlying diseases,severity-of-sepsis,and 28-day mortality.WIC scores,acute physiology and chronic health evaluation (APACHE) Ⅱ,and sepsis related organ failure assessment (SOFA) were assessed within the first 24 hours after admission.Logistic regression analyses were used to evaluate the predictors for outcome.The receiver operating characteristic curve (ROC) was used to compare the performance of these scores between different methods.Results Of 160 enrolled patients,76 (48.8%) were CAP,82 (51.2%) HAP,and 106 (66.3%) male,54 (33.7%) female,and 99 (61.9%) patients survived and 61 (38.1%) died.The average age was (62.4 ± 17.3) years old.Compared with survivors,WIC,APACHE Ⅱ and SOFA scores were significantly higher in death group (P < 0.05).The multivariate logistic regression revealed that risk of death depends predominantly on age (OR =1.049,95% CI:1.011-1.088,P =0.011),WIC (OR =1.725,95% CI:1.194-2.492,P =0.004),APACHE Ⅱ score (OR =1.175,95%CI:1.058-1.305,P =0.003),SOFA score (OR =1.277,95% CI:1.048-1.556,P =0.015),presence of ARDS (OR =0.081,95% CI:0.008-0.829,P =0.034),and complicated with severe sepsis (OR =0.149,95% CI:0.232-0.622,P =0.004).The area under the receiver operating characteristics curve in predicting mortality was 0.639 (0.547-0.730) for the WIC,0.782 (0.709-0.856) for APACHE Ⅱ score,0.79 (0.714-0.866) for SOFA score and 0.842 (0.777-0.907) for the merger of three.Conclusions In pneumonia patients of ICU,WIC is a useful approach to predicting 28-day mortality,and the risk of death significantly depends on co-morbidities.
6.Laparoscopic adjustable gastric banding for morbid obesity

Objective To evaluate the effect of laparoscopic adjustable gastric banding(LAGB) in patients with obesity and obesity-related comorbidities.Methods From Oct.2006 to Dec.2007,50 morbidly obese patients including 11 cases with type 2 diabetes,3 with hypertension,15 with hyperlipidemia,28 with fatty liver,1 with obstructive sleep-apnea syndrome and 2 cases with gallstones underwent LAGB.The mean follow-up period for these patients was 11.2 months.ranging from 6 to 18 months.The weight loss,obesity-related comorbidities,outcomes and complications were evaluated.Results Mean BMI decreased significantly from preoperative(39±6)kg/m2 to postoperative(31±4)kg/m2,(28±7)kg/m2 and(27±7)kg/m2 respectively at 9,12 and 18 months(P<0.05).The mean excess weight loss at 9,12 and 18 months postoperatively was 30%±11%、42%±13%and 45%±13% respectively.At 12 and 18 months,respectively,20%and 44%of patients had>50%excess weight loss.The obesity-related comorbidities resolved or improved in 66%~100%of the patients at 12 and 18 months postoperatively.Complications occurred in 4 cases,among them 3 cases were cured conservatively and in 1 case reoperation was performed. Conclusions Based on short-term follow-up results,LAGB is a safe,effective and feasible technique in the treatment of patients with morbid obesity and obesity-related comorbidities.
7.Molecular mechanism of paeoniflorin regulating transforming growth factor-β in inhibiting the epithelial-mesenchymal transition of glioma U87 cells

Objective To investigate the effect of paeoniflorin on epithelial-mesenchymal transition (EMT),migration and invasion of human glioma U87 cells and its underlying molecular mechanism.Methods Glioma U87 cells were treated with different dose of paeoniflorin.CCK-8 was used to measure the survival rate of glioma cells.Glioma U87 cells were infected with carrying transforming growth factor-β(TGF-β)lentivirus to establish the stable overexpression TGF-β cell line.Wound healing assay and transwell assay were performed to measure cell migration and invasion ability,respectively.Western blot was applied to examined the expression of related proteins.Results Treatment with proliferation of 5 mmol/L and 10 mmol/L had no significant effect on cell survival of U87 cells ((96.00± 3.61) %,(92.33± 4.04) %;P5 =0.593,P10 =0.284).But when the dose was up to 15 mmol/L,the survival rate of U87 cells decreased to(75.67±4.58)%,P=0.008.In addition,paeoniflorin inhibited the migration ability of U87 cells and the wound healing rate was(68.20±4.39) %,(37.70±7.01) % when incubated with 5 mmol/L and 10 mmol/L.Similarly,paeoniflorin suppressed invasion ability of U87 cells and the average number of infiltrated cells was (237.67±24.00),(130.67± 32.65),(59.67± 18.15) respectively (P5 =0.002,P10 < 0.01).What's more,paeoniflorin down-regulated the expression of MMP2,MMP9 and epithelial-mesenchymal transition-related proteins (P<0.05).Overexpression of TGF-β reversed the effect of paeoniflorin on migration,invasion and epithelial-mesenchymal transition of glioma U87 cells.Conclusion Paeoniflorin suppress TGF-β-induced migration,invasion and epithelial-mesenchymal transition in glioma U87 cells.
8.Lgr5 and CD44 expressions in different types of intestinal polyps and colorectal cancer.

OBJECTIVETo study the expression of tumorigenesis-related stem cell markers Lgr5 and CD44 in different pathological types of intestinal polyps and their clinical significance in predicting tumorigenesis.
METHODSA total of 145 cases of colorectal polyps, adenomas and cancer tissues were obtained by colonoscopy biopsy. Immunohistochemistry was employed to detect the expression of Lgr5 and CD44 to analyze their relationship with the occurrence and prognosis of colon and rectal cancer.
RESULTSThe expression of CD44 in colon cancer tissue was 95.65%, significantly higher than that in normal mucosa (5%), inflammatory hyperplastic polyps (22.58%), tubular adenomatous polyps (55.26%) and villous polyps (75.76%) (P<0.05). The expression of Lgr5 in colorectal cancer was up to 95.65% while negative in normal colorectal tissue and was 16.12% in inflammatory hyperplastic tissues (P<0.05). The expression rate of Lgr5 was 86.84% in tubular adenoma and 93.94% in villous polyps, both comparable with that in colon cancer (P>0.05). Correlation analysis indicated that the expression of CD44 and Lgr5 were positively correlated with the progression of intestinal polyp tumorigenesis (rs=0.69377, P<0.0001; rs=0.81637, P<0.0001).
CONCLUSIONLgr5 and CD44 are highly expressed in colorectal cancer tissues in close correlation with the clinical and pathological features. The expression profiles of Lgr5 and CD44 represent a distinct feature to differentiate colorectal cancer from normal intestinal mucosa. Lgr5 is more closely correlated with tumor progression of polyps than CD44. This means detecting of the expression of Lgr 5 together with CD44 is important and necessary in clinical diagnosis of patients with early stage colorectal diseases such as polyps and their canceration.

9.Evidence summary of surgical site infection prevention in adult inpatients based on guidelines and clini-cal decision making

Objective To provide evidence-based references for the prevention of surgical site infection(SSI)by sum-marizing the best evidence for the prevention of SSI in adult inpatients.Methods The'6S'evidence resource pyramid model was used to systematically search the related evidence in domestic and foreign databases,guideline websites,and academic socie-ty websites from the inception of the database to September 30,2023.Four researchers evaluated the quality of the included guidelines,and two researchers independently evaluated the quality of other types of literature and rated the level of evidence.Results A total of 12 articles were included,including 6 clinical decision making and 6 clinical guidelines.Thirty best items of the evidence were summarized from 7 aspects:diagnosis,clinical symptoms,influencing factors,patient prevention strategies,preventive strategies for medical staff,intraoperative and postoperative treatment,and consultation and education.Conclusion Clinical staff should develop a standardized management plan for infection prevention based on corresponding evidence to reduce the incidence of SSI instead of taking a single measurement.Moreover,they need to formulate a standardized work process for preventing SSI based on the clinical practice and patients'preference.
10.Practice of Promoting Rational Use of Antibiotics in Outpatient Department by Special Prescription Pre-review

OBJECTIVE:To provide reference for the participation of clinical pharm acists in outpatient antibiotics management so as to promote the rational use of antibiotics in outpatient department. METHODS :The pharmacist reset medication rules , classified irrational medication levels ,formulated warning contents ,and established a diagnosis database of mild to moderate non- complex infections ,according to the problems found in the post review of outpatient antibiotics prescription through sorting out the previous rules of antibiotics use in the Yiyao Rational Drug Use Management Software (called“review software ”for short )in our hospital. The special pre-review was performed by using combination of system review and manual review. Pharmacists used the review saftware to count the data of outpatient antibiotics prescriptions before (the first quarter of 2020)and after (the second , third and fourth quarters of 2020)the implementation of special pre-review. The post review for antibiotics prescriptions in Dec. 2019(before the implementation of special pre-review )and Dec. 2020(after implementation of special pre-review )were performed by pharmacists to evaluate the impact of the implementation of special pre-review on promoting rational use of outpatient antibitics. RESULTS:Comared with before the implementation of special pre-review in the first quarter of 2020,the prompt rate of the review software for antibiotics prescriptions was decreased ,and the interception rate was increased after the implementation of special pre-review in the second quarter of 2020(P<0.05). Compared with before the implementation of special pre-review in Dec. 2019,the proportion of outpatient antibiotics prescriptions in all outpatient prescriptions ,the ratio of bi-antibiotics prescriptions in all antibiotics prescriptions in the same month ,the ratio of prescriptions with unsuitable indications in all antibiotics prescriptions in the same month were all decreased E-mail:phdloveyou@163.com significantly (P<0.01), while the ratio of prescriptions with rational antibiotics use in all antibiotics prescriptions in the same month was increased significantly (P<0.01). CONCLUSIONS:Pharmacists establishing a rule for rational use of antibiotics in outpatient department b ased o n the hospital ’s situation,implementing a special pre-review for antibiotics prescriptions with the help of review software ,can promote the rational use of antibiotics in outpatient department.
