1.Evaluation of left and right ventricular volume and systolic function in tetralogy of Fallot children by real-time ;three-dimensional echocardiography

Objective To investigate the characteristics of left and right ventricular volume and systolic function of tetralogy of Fallot(TOF)children by real-time three-dimensional echocardiography (RT-3DE).Methods Forty-five TOF children and 46 normal age-matched children were recruited in this study. Full volume imaging of left and right ventricle were obtained by RT-3DE and analyzed off-line by TomTec LV-Analysis and RV-Function.The measurements including:end-diastolic volume (LVEDV/RVEDV),end-systolic volume (LVESV/RVESV),ej ection fraction (LVEF/RVEF),peak systolic volume rate (LVPSVR/RVPSVR),stoke volume (LVSV/RVSV),cardiac output (LVCO/RVCO).And the volumetric parameters were indexed by BSA(indexed EDV,indexed SV,indexed CO).The measurements were compared between TOF group and normal group.The left and right ventricular measurements were also compared with each other within group.Results Compared with control group indexed LVEDV reduced in TOF group(P =0.000),while indexed RVEDV increased (P =0.002 ).For systolic function indexes:LVEF,RVEF, LVPSVR and RVPSVR in TOF group were both lower than that in normal group (P <0.05 ),indexed LVSV and indexed LVCO were also reduced (P <0.05 ),while indexed RVSV had no difference with normal(P >0.05)and indexed RVCO was increased (P =0.016).The volume and function comparision between left and right ventricle demonstrated:in normal group no difference were found in biventricular EDV,PSVR,SV,CO(P >0.05),except that LVEF was a litter higher than RVEF(P =0.000).In TOF group RVEDV,RVSV,RVCO were all greater than that of left ventricle(P <0.05),but RVEF was lower than LVEF(P =0.039).Conclusions In TOF group biventricular systolic function were both impaired and lost banlance between them.Under the volume and pressure overload the right ventricle improved pumping function by increasing heart rate.
2.Cumulative live birth rates after frozen embryo transfer and fresh embryo transfer with oocyte numbers more than 15.

To compare the cumulative live birth rates (CLBR) and the incidence of ovarian hyperstimulation syndrome (OHSS) between fresh embryo transfer (ET) and frozen ET (the freeze-all policy), when oocyte numbers are more than 15 in the first treatment of in vitro fertilization or intracytoplasmic sperm injection, and to evaluate the benefits of the freeze-all policy.
Methods: We retrospectively analyzed clinical data of 2 842 patients whose oocytes numbers were more than 15, including 1 095 frozen ET patients and 1 747 fresh ET patients. The patients general data, a baseline features, CLBR, and the incidence of OHSS were compared between the 2 groups.
Results: There were 598 patients in the 2 groups after they experienced the propensity score matching. No significant differences were found in age, infertility causes, body mass index, basal follicle stimulating hormone level, the total days and total dose of using gonadotrophin (Gn) between the 2 groups (all P>0.05). The CLBR of the freeze-all cycles increased along with the number of oocytes (P>0.05), and the oocyte numbers were greater in freeze-all group than those of the fresh ET group (P<0.001). There was no significant difference in CLBR after one complete cycle between the 2 groups (P>0.05), but after the first embryo transfer cycle, the CLBR in freeze-all group was higher than that in the fresh ET cycle group (P<0.05). The incidence of OHSS in patients with freeze-all was significantly lower than that in the patiants with fresh ET (P<0.05).
Conclusion: Patients with oocytes over 15 and OHSS tendency who accepted the freeze-all strategy can help them to prevent OHSS and they have a higher CLBR than fresh ET cycles.
