1.Imolantation of Quadrate Muscle Thigh with Bone Flap Combined with internal Fixation for Infracaput Fracture of Collum Femoris in the Youth

Infracaput fracture of collum femoris in the youth is usually caused by tremendous violenceand characterized by obvious translocation and high ratio of ischemic necmeis of femoral head. Artificialfemoral head replacement or hip replacement is not suitable for the youth. Twenty-eight cases of suchfracture were treated with implantation of quadrate muscle of thigh with bone flap combined with internalfixation by screw,and a follow-up Period of 8 ~ 36 months was carried on. The results showed that allcases resulted in a bony union and ischemic necrosis of femoral head did not occur.It is suggested thatthe opelation should be Performed as early as possible to restore the blood circulation of femoral head,and proper reduction and firm internal fixation guarantee the union of fracture,Quadrate muscle of thighwith bone flap has a rich blood supply and operation can be accomplished in the same incision of internalfixation,so that the injury is iess and the successful rate is high.

Hypertension is a common and chronic disease,and is also the mainly risk factor of cardio-cerebrovascular disease.Essential hypertension is a complex disease caused by the common function of multiple genes and environmental factors,but the exact pathogenesis is still unclear.With the study continues,the role of DNA methylation and other epigenetic factors in pathological process with essential hypertension (EH) gradually be taken seriously.This review summarizes the research progress of some essential hypertension related methylation genes' function in the course of hypertension,to explain the relationship between DNA methylation markers which can be influenced by environment factor and essential hypertension and help us have a further understanding about the hypertension pathogenesis.
3.The prevalence and clinical features of fulminant type 1 diabetes

Objective To characterize the clinic features of fulminant type 1 diabetes mellitus (IT1DM).Methods It was a retrospective study analyzing the clinical and laboratory data of patients diagnosed as FT1DM and type 1 diabetes in the Department of Endocrinology in China-Japan Friendship Hospital from January 2011 to November 2015.Results A total of 125 newly diagnosed type 1 diabetic patients were included in the study,14 of them (11.2%) met with the criteria of FT1DM.The age at onset was (35.8 ± 8.3) years.The time from onset to ketosis was 2 (0-7) days.The plasma glucose levels were extremely high[(33.79 ± 14.13)mmol/L],while glycosylated hemoglobin A1c[HbA1c,(6.9 ±0.7) %] and serum glycosylated albumin [(21.8 ± 4.5)%] levels were only slightly above the normal range.Moreover,the C-peptide levels were extremely low and the situation kept after one month to two years' follow-up.Four (28%) patients were glutamic acid decarboxylase antibody (GADA) positive at the onset with two turned negative.One patient was GADA negative at the onset and turned positive after one month.The levels of serum transaminases increased with enlargement of liver in one of the patients after insulin therapy,who was diagnosed as glycogenic hepatopathy.Conclusions FT1 DM is an extremely rapidly progressing type of diabetes and life threatening disease.Causations have to be taken in medical practice.

5. Effects of different exercise intensities on visfatin level and glucose metabolism in type 2 diabetic rats

BACKGROUND: Visfatin has an insulin-like hypoglycemic effect with positive significance in the treatment of type 2 diabetes. Different exercise intensities have different effects on the level of visfatin. OBJECTIVE: To observe the effects of 6-week aerobic exercise of different intensities on glucose metabolism and the level of visfatin in type 2 diabetic rats and to explore the relationship between them. METHODS: The male Sprague-Dawley rats were fed with high-fat diet and injected with streptozotocin to establish a type 2 diabetes model. The 43 model rats were randomly divided into quiet control group (n=10), exercise group I (n=11), exercise group II (n=11) and exercise group III (n=11). The exercise intensities of the three exercise groups were 10, 15, and 20 m/min, respectively. Exercises of different intensities were performed 1 h/d, 5 days per week, for 6 continuous weeks. Blood samples were collected at the end of 6 weeks for determination of glucose metabolism-related indicators and serum and visceral levels of visfatin. RESULTS AND CONCLUSION: The indexes related to glucose metabolism (fasting blood glucose, glycosylated serum protein, insulin, insulin resistance index, etc.) in the three exercise groups were improved. The serum level of visfatin in the exercise groups I and II were significantly decreased (P < 0.01), while the serum level of visfatin in the exercise group III was decreased to some extent, but not significantly (P > 0.05). There was a significant decrease in visfatin level in the three exercise groups (P < 0.05), especially in the exercise group II (P < 0.01). The results indicate that low-intensity aerobic exercise can effectively improve insulin resistance and reduce blood glucose concentration in type 2 diabetic rats, and it is related to the loss of visceral fat and the reduction of visfatin secretion, where visfatin may play a positive role.
6.Predictive model for early neurological deterioration in minor ischemic stroke without reperfusion therapy

Objective To explore independent risk factors for early neurological deterioration in first-ever minor ischemic stroke without reperfusion therapy,and to achieve individualized prediction by constructing a nomogram model.Methods We enrolled first-ever minor ischemic stroke patients of non-reperfusion therapy from January 1,2017 to December 31,2019 in the Zhaoqing First People’s Hospital.The clinical and imaging data of the Neurological deterioration group(END group) and the non-END group were analyzed.The independent related factors were screened and the nomogram model was constructed.Results A total of 384 cases were enrolled,including 66 cases (17.19%) in the END group and 318 cases (82.81%) in the non-END group.Logistic regression analysis showed that diabetes mellitus,TOAST typing,albumin levels,and cerebral atherosclerosis score (CAS) were independent influencing factors (P<0.05).The nomogram model is constructed and the ROC curve is generated,and the area under the curve is 0.859[95%CI(0.758,0.864)].Model consistency can be hinted at by the calibration curve and the Hoster-Lemeshow test.Finally,the clinical decision curve indicates that the model has clinical application value.Conclusion This study explores the nomogram model for END in first-ever minor ischemic stroke without reperfusion therapy,which has some clinical application value,is composed of four major factors:diabetes mellitus,TOAST typing,albumin level and CAS.
7.Efficacy of probiotics in preventing late-onset sepsis in very low birth weight infants: a Meta analysis.

OBJECTIVE:
To study the efficacy of probiotics in preventing late-onset sepsis (LOS) in very low birth weight (VLBW) infants.
METHODS:
Databases including PubMed, Web of Science, Cochrane Library, Wanfang Data, China National Knowledge Infrastructure, and Chinese Biomedical Literature Database were searched for randomized controlled trials (RCTs) of probiotics in preventing LOS in VLBW infants. LOS was classified as clinical LOS and confirmed LOS. RevMan 5.4 was used to perform the Meta analysis.
RESULTS:
A total of 31 RCTs were included, with 3 490 VLBW infants in the probiotics group and 3 376 VLBW infants in the control group. The Meta analysis showed that compared with the control group, the probiotics group had significantly lower risks of clinical LOS (
CONCLUSIONS
Current evidence indicates that probiotics may reduce the risk of clinical LOS and clinical/confirmed LOS in VLBW infants, and the risk of confirmed LOS in VLBW infants who are exclusively breastfed.

8.Effects of Different Magnitudes of Mechanical Stretch on Human Intervertebral Disc Cells

Objective To investigate the effects of different magnitudes of mechanical stress on human interverte-bral disc degeneration. Methods The human intervertebral disc cells were subjected to different magnitudes of mechanical stress (0, 6%, 12%, or 18%elongation) for 24 h using a Flexercell Strain Unit. The mRNA expressions of anabolic genes (col-lagen-1A1, collagen-2A1, aggrecan and versican) and catabolic genes (MMP-3, MMP-13, ADAMTS-4 and ADAMTS-5) were examined by real-time PCR and Western blot methods. Results The expression levels of collagen-1A1 and collagen-2A1 were increased at 12%of mechanical stress, and collagen-2A1 was decreased at 18%of mechanical stress compared with those of control. The mRNA expressions of catabolic genes, MMP-13 and ADAMTS-5, were increased at 12%and 18%of mechanical stress than those of control. The mechanical stretch induced a magnitude-dependent increase in ADAMTS-4 synthesis, which was finely tuned by stretching-triggered activation of distinct mitogen-activated protein kinase cascades. Specifically, an ERK1/2 specific inhibitor, U0126, significantly inhibited the stretching-induced ADAMTS-4 expression, whereas the inhibitors of p38 and JNK, SP6000125 and SB203580, showed only slightly effect on the stretching-induced ADAMTS-4 expression. Conclusion The different magnitudes of mechanical stretch exhibited different effects on the bio-logical behavior of intervertebral disc cells, which profoundly affects the intervertebral disc degeneration.

Zhaoqing city is located in the midwest of Guangdong province,the middle and lower reaches of the mainstream of the Xijiang river.Many species of southern medicines grow very well in the Xijiang river basin of Zhaoqing because of existing of enormous mountainous and hilly areas,loose and fertile soil,warm climate,as well as abundant sunshine and rain.Zhaoqing has a long history of cultivating some species of southern medicines.For example,Cinnamomi Cortex was planted more than 300 years ago in Zhaoqing.Under the advocacy of national policy,the planting scale of some species of southern medicines expanded rapidly since 1980s.Up to now,plantation of the 4 species of southern medicines,namely,Morindae Officinalis Radix,Citri Sarcodactylis Fructus,Polygoni Multiflori Radix and Cinnamomi Cortex,has developed into a important representative industry in Zhaoqing.Zhaoqing also develops into the genuine producing area of this 4 species of southern medicines currently,and these products are sold well at home and abroad.Although great progress has been made in southern medicine industry in Zhaoqing,there still exist lots of problems for its future development,such as scattering planting instead of overall program,impure germplasm resources,plant diseases and insect pests,pesticide residue and pollution of heavy metal,lack of quality control in trading process,as well as absence of deep-processing and additional value of products.This paper analyzes the development status,advantages and problems of southern medicine industry in Zhaoqing,and puts forward some corresponding measures and suggestions.These proposals may have important guiding significance for promoting the development of local southern medicine industry.
