1.Cholesterol crystal embolism after coronary intervention

Objective To analyze the clinical characteristics of patients with cholesterol crystal embolism (CCE) after percutaneous coronary intervention. Methods Six patients with atherosclerosis presenting with simultaneous occurrence of acute renal failure and peripheral ischemic changes were diagnosed as cholesterol crystal embolism and their clinical data were analyzed. Results The patients, 5 men and 1 woman, had an average age of 72 years. Most of them had risk factors of atherosclerosis such as hypertension, diabetes and smoking. The levels of serum creatinine increased progressively after coronary angiography. All patients had concomitant skin lesions, including blue toes. Cutaneous biopsy of 1 patient found cholesterol emboli in arterioles. All patients received statins, and 2 received dialysis. Three patients died, and 3 patients remained chronic renal failure. Conclusion Since the morbidity of CCE is growing and the disease is iatrogenic in origin, special attention should be paid to this disease.
2.3 years follow-up of refractory systemic lupus erythematosus patients with transplantation treating of umbilical cord mesenchymal stem cells

Objective:To observe immunological indexes,the quantity of cytokine expression and clinical curative effect of umbilical cord mesenchymal stem cells between before and after the treatment of systemic lupus erythematosus patients.Methods:Selected 10 cases of SLE,on the basis of glucocorticoid and immune inhibitor treatment,intravenous injection UC-MSC of cultivating proliferation within 6 generations.Before and after treatment of UC-MSC testing the relative quantity of cytokine of CTLA-4,IL-15,IL-2,CD86,IL-17c,Foxp3,TGF-β2 which were related of immunopathogenesis of SLE.Before and after treatment to determined SLE disease activity index (SLEDAI) score and detection of blood in the urine routine,liver and kidney function,24 hours urinary protein quantitative,immunoglobulin and complement levels.Results:After treatment the relative expression value of IL-15 and IL-2 was decreased,CTLA-4 was risen.There had no significant difference with the relative expression value of CD86,IL-17c,Foxp3,TGF-β2 in before and after treatment of UC-MSC.After treatment serum complement C3 and C4 level,serum albumin,were risen.24 hour proteinuria and SLEDAI were decreased.There was no serious adverse reaction occurred,no complications related to transplantation in 10 cases.Conclusion:UC-MSC can regulate the expression of cytokines of participate in the immune response in the patients with SLE.Treatment of SLE by UC-MSC can elevate serum albumin and C3 and C4 level,reduce the 24 hours urinary protein quantity,relife kidney damage,improve clinical symptoms;UC-MSC transplantation in patients with SLE have good security;UC-MSC transplantation may be a feasible method for the treatment of SLE.
3.The multiple mediating effect of sleep quality and internet addiction between negative life events and mental health among college students

Objective To explore the multiple mediating effect of sleep quality and internet addiction between negative life events and mental health among college students.Methods A cross-sectional study was used to investigate 733 college students with adolescent self-rating life events checklist,Pittsburgh sleep quality index,the general health questionnaire and internet addiction scale for Chinese revision.SPSS and Bootstrap method were used to test the hypothetical model.Results The scores of Negative life events,sleep quality,interaet addiction and mental health were 46.0 (33.0,66.5),5.0 (3.0,7.0),44.0 (37.0,50.0) and 5.0(2.0,9.0) respectively,and they were positively correlated with each other (r=0.39-0.55,P<0.01).Negative life events had a direct effect on mental health.The effect value was 0.013,and the effect size was 17.97%.The simple mediating effects of sleep quality and internet addiction between negative life events and mental health were significant.The effect values were 0.047 and 0.009,and the effect sizes were 64.85% and 11.94%,respectively.The chain mediating effect of sleep quality-internet addiction was also significant,and the effect value was 0.004 and the effect size was 5.24%.Conclusion Negative life events not only directly affect mental health in college students,but also indirectly affect their mental health through the multiple mediating effects of sleep quality and internet addiction.
4.Quality Improvement Pathway of Cultivated Chinese Medicinal Materials Based on Physiological Ecology of Plants Under Adversity

The quality of Chinese medicinal materials is related to the environment, with an optimal quality under adversity. The origin of Chinese medicinal materials has converted from wild collection to cultivation, and a better cultivation environment leads to a decline in their quality. At present, there are few effective methods to improve the quality of Chinese medicinal materials. Plants are bound to produce a large amount of reactive oxygen species (ROS) under adversity, and the quality improvement of Chinese medicinal materials under adversity may be achieved through ROS. This paper described the relationship between plant adversity-ROS-secondary metabolism: ROS can alter the structure of proteins (including enzymes) and regulate enzyme activities, thus affecting secondary metabolism to improve the adaptive capacity of plants. Therefore, ROS is the essential cause of adversity changing secondary metabolism. The cells of plants are omnipotent, and the medicinal parts of plants can independently complete the whole process of secondary metabolism, so regulation of secondary metabolism during the processing of fresh Chinese medicinal materials can significantly improve the quality of Chinese medicinal materials. Exogenous ROS can be used as inducible factors to stimulate medicinal parts, inducing a physiological state of fresh medicinal parts similar to that under adversity, thus enhancing secondary metabolism, and improving the contents of active ingredients in Chinese medicinal materials. In addition, the content and ratio of each ingredient in Chinese medicinal materials are closer to those of wild Chinese medicinal materials. The mechanism of plant adaptation to adversity is the mechanism of the quality formation of Chinese medicinal materials, and the application of ROS as inducible factors can provide a new pathway for the production of high-quality Chinese medicinal materials.
5.Pancreatic Exocrine Insufficiency After Gastrectomy:A Meta-analysis

Background:Pancreatic exocrine insufficiency(PEI)after gastrectomy can lead to weight loss,diarrhea,abdominal distension,fat malabsorption and other symptoms,which seriously affects patients'quality of life.Aims:To investigate the incidence of PEI after gastrectomy and the efficacy of pancreatic enzyme replacement therapy(PERT).Methods:Studies on PEI after gastrectomy and/or efficacy of PERT were retrieved from PubMed,Embase,Web of Science,CNKI,Wanfang Data and VIP databases from the date of database establishment to January 2024.Two researchers independently screened the literature,extracted data,and assessed the risk of bias about included studies.Meta-analysis was conducted by Stata 12.0 software.Results:A total of 16 studies involving 974 patients were included in the meta-analysis,of which 14 studies reported the incidence of PEI after gastrectomy,and 2 studies reported the efficacy of PERT.The incidence of PEI after gastrectomy was 43.8%(95%CI:0.295-0.585).Subgroup analysis showed that the incidence of PEI after gastric cancer surgery was 82.1%(95%CI:0.541-0.991),the incidence of PEI in patients underwent total gastrectomy with Roux-en-Y reconstruction and partial gastrectomy with Billroth Ⅱ reconstruction were 60.4%(95%CI:0.489-0.719)and 63.5%(95%CI:0.511-0.759),respectively,and the incidence of PEI after bariatric surgery with billiopancreatic diversion with duodenal switch(BPD/DS)was 67.3%(95%CI:0.542-0.804).Early intervention of PERT after total gastrectomy could reduce fecal fat content at 24 hours(SMD=-1.24,95%CI:-1.56--0.91,P<0.001)and 72 hours(SMD=-1.41,95%CI:-1.74--1.07,P<0.001),and the number of patients with 24-hour fecal fat content>7 g(OR=0.21,95%CI:0.09-0.53,P=0.001).Conclusions:PEI is common after gastrectomy,and clinicians should pay more attention on it,especially in gastric cancer patients undergoing gastrectomy,Roux-en-Y reconstruction after total gastrectomy,Billroth Ⅱ reconstruction after partial gastrectomy and bariatric surgery with BPD/DS.Fat malabsorption caused by PEI in patients after total gastrectomy can be improved by PERT.
