1.Clinical and EEG Characteristics of Benign Epilepsy of Childhood with Centro-temporal Spikes

Objective To summarize the clinical and EEG characteristics of benign epilepsy of childhood with centro-temporal spikes(BECCT). Methods The clinical and EEG data of 78 children with BECCT were retrospectively analyzed. Results The onset age of this disease was 3 to 14 years, and 62 patients aged 5 to 10 years(62/78,80%). The seizures of 72 cases(72/78,92.3%)were associated with sleep. 63 cases(73/78,80.8%)were partial seizures, and 15 cases(15/78,19.2%) were generalized seizures. EEG demonstrated that there were sharp or spike waves of interior discharge in the centro-temporal area of one or two sides, 55.4% cases were positive in awake EEG, and 92% cases were positive in sleep EEG. 93.2% cases were responsive to antiepileptic drugs. Conclusion The characteristics of BECCT were as follows: partial epilepsy related to sleep, partial seizure types and sharp or spike waves of EEG in centro-temporal area. EEG sleep induction test could increase positive diagnostic rate of this disease, and the prognosis of this disease was fine. The early diagnosis is helpful for the patients' prognosis, and can alleviate psychological suffering of the patients and their families.
2.Effects of infrasound on sperm quality and tissue antioxidant system in testes of rats

AIM:To observe the effects of infrasound on sperm quality and explore some underlying mechanisms. METHODS: Adult male SD rats were exposed to infrasound of 8 Hz at 90 dB or 130 dB for 1,7,14 or 21 d(2 h/d), whose epididymises were obtained for sperm count, abnormity and activity ratio inspection and whose testes for GSH-Px and MDA inspection. RESULTS: With prolonged exposure, sperm quality and GSH-Px in 90 dB and 130 dB groups decreased, while MDA increased, and at the same time point, the indexes of 130 dB groups changed more profoundly than those in 90 dB groups. CONCLUSION: Biological effect of infrasound on testes depends on the exposure parameters. The mechanisms of which could be unbalanced antioxidant system induced by infrasonic exposure.
3.The effect and safety of new chemotherapy regimen for advanced gallbladder carcinoma: an open multicenter randomized controled trial

Comprehensive therapy is the leading method to improve the overall survival for patients with advanced gallbladder cancer, especially the study of new chemotherapy regimen are on demand.From 2010 to 2011, a phase Ⅲ study of new chemotherapy regimen in the treatment of advanced gallbladder carcinoma would be undertaken nationwide to assess the clinical outcome and safety, in 31 clinical centers led by the department of General Surgery of Xinhua Hospital Affiliated to Shanghai Jiaotong University School of Medicine, and a total of 260 patients would be accrued for this study.Chemotherapy consists of 2 regimes, conventional regime(cisplatin, epirubicin, and 5-fluorouracil, leucovorin)and new regime(conventional regime plus somatostatin).At present, the study has been approved by Clinical Trial Registration Platform of National Institutes of Health in U.S.A., and more than 30 patients are rolling in the study.The clinical trial could provide the high-quality evidence of EBM(evdence based medicine)for the chemotherapy of advanced gallbladder caner, and it will be one of the important components in the large-scale clinical study of chemotherapy for the advanced gallbladder cancer in China and worldwide.
4.Radical surgery of lung cancer with chest wall invasion and reliable methods of chest wall reconstruction

Objective To study the efficacy of radical surgery for the invasion of chest wall by lung cancers and the different methods of chest wall reconstruction. Methods A retrospective analysis of clinical data was conducted in 27 cases of lung cancer with chest wall invasion in this hospital. 24 cases received lobectomy, 3 cases received pneumonectomy, and all cases received wall reconstruction. The size of chest wall defect after resection ranged from 6.5 cm × 5.4 cm × 5.0 cm to 15.5 cm × 12.5 cm ×10.0 cm. The number of rib resection for each patient ranged from 1 to 4. The reconstructive methods were as follows: using polyester fabric to reconstruct costal pleura, using plexiglass, stainless steel wire and grid-like titanium plate to reconstruct bone defect, and using nearby muscle flap and pedunculated muscle flap to reconstruct muscle defect. Results Pathologic diagnosis was non-small cell lung cancer,including 22 cases of squamous cell carcinomas and 5 adenocarcinomas. There were pulmonary infections in 6 cases, chest hemorrhage in 1 case and chest purulence in 1 case. One-year survival was 77. 9 %,3-yearsurvival was 58.3 %, and 5-year survival was 29. 1 %. Conclusion The lung cancer with chest wall invasion is advisable to make active use of radical surgery. The different methods and repair materials are chosen to reconstruct the chest wall defect, supplemented by comprehensive treatments.
5.Effects of infrasound on rat myocardial cell and its underlying mechanism

Objective To observe the effects of infrasound on rat myocardial cells and to study its underlying mechanism. Methods One hundred Sprague-Dawley rats were divided into a normal control group and a infrasound exposure group, composed of various subgroups subject to exposure to infrasound for 2 hours daily for 1 d,7 d,14 d,21 d,and 28 d, respectively. The ultra-microstructure and apoptosis of the rat myocardial cells were observed, and SOD and MDA were measured. Objective To observe the effect of low level infrasound on neuropathic pain and explore its underlying mechanism. Methods Rats were divided into experimental and control groups after their L5 spinal nerves had been ligated to create a neuropathic pain model. The experimental group was subjected to 40 to 80 dB infra sound. The control group received no infrasound treatment. Paw withdrawal latency in response to heat radiation was measured and the average gray scale of the microglia in a slice of the L5 spinal cord was compared. Results The experimental group expressed significantly lengthened paw withdrawal latency on the 12th and 14th day. The average gray scale showed significantly weakened activation of spinal microglia in the 2nd week of infrasound treatment compared with the control group. Conclusion Low level infrasound can ameliorate neuropathic pain to a certain extent,which might be related with inhibition of spinal microglia.
7.Surgical management of mediastinal lymph node metastasis of thyroid carcinoma

Objective To explore the surgical technique for mediastinal lymph node metastasis of thyroid cancer.Methods We retrospectively reviewed clinical records of 46 patients with metastatic thyroid cancer in mediastinal lymph nodes and having received surgical treatment in Department of Head Neck Surgery and Thoracic Surgery of Sichuan Cancer Hospital from Feb.2004 to Apr.2015.We analyzed the diagnosis,surgical treatment methods,operative approach,and postoperative complications of these patients.Results The main metastatic region was superior mediastinum especially level 2 (2R/2L,lower parathymic lymph nodes) according to AJCC-UICC standard in 31 patients (67.39%);16 patients (34.78%) had level 3 and level 4 (4R/4L lower parathymic) lymph node netastasis and 4 patients (8.696%) had level 5 (subaortic lymph node) and level 6 (para-aortic lymph node) metastasis.39 patients were pathologically diagnosed with papillary carcinoma,6 patients were diagnosed with medullary carcinoma,and 1 patient was diagnosed with follicular carcinoma.There were 14 patients with stage Ⅰ disease,5 patients with stage Ⅱ disease,3 patients with stage Ⅲ disease,22 patients with stage Ⅳa disease,and 2 patients with stage Ⅳc disease.The most common complications were hoarseness,chylous fistula,hypocalcaemia,and airway obstruction.Patients were followed-up from 1 to 8 years.During the follow-up period,4 cases were lost to follow-up and 2 patients died.Conclusions The main treatment approach for mediastinal lymph node metastasis of thyroid cancer is surgical operation.Pre-operative CT or MRI is needed to evaluate the metastatic region of the lymph nodes and to choose appropriate operative approach.
8.Evaluation application of death indicators and unplanned return indicators based on provincial diagnosis-related groups platform

Objective:To explore the relationship between death indicators and unplanned return indicators on healthcare quality evaluation.Methods:A total of 836 976 medical record data were collected from 31 tertiary public general hospitals in a diagnosis-related groups(DRG) data platform in 2017. Multiple death indices(low and low-risk risk group mortality, high-risk group mortality, crude mortality, and risk adjusted mortality) and unplanned return indices(31-day unplanned readmission rate and 31-day unplanned return to surgery rate) were calculated. Pearson′s correlation coefficient was used to examine the relationships among those indices.Results:Death indicators were correlated with each other, but the unplanned readmission rate was not correlated with the unplanned reoperation rate( r=0.305). There was no correlation between unplanned re-entry rate and death rate. The correlation coefficients were as follows: unplanned readmission rate versus low and low-risk group mortality( r=-0.227), versus high-risk group mortality( r=-0.098), versus actual mortality( r=-0.130), versus risk adjusted mortality( r=0.010); unplanned reoperation rate versus low and low-risk group mortality( r=0.105), versus high-risk group mortality( r=0.030), versus actual mortality( r=-0.004), versus risk adjusted mortality( r=-0.141). Conclusions:The indicators of death and the indicators of unplanned return are not the same in terms of actual management technology and evaluation effect. They are complementary to each other and can form an ideal combination of quality evaluation indicators.

Objective To analyze the role of telehealth?based dialysis registration systems in real?time and dynamic reflection of renal anemia in hemodialysis (HD) patients, and discuss the prospect of its application in dialysis registration management. Methods The Red China project was to build up a dialysis registration system based on the WeChat mobile terminal platform. Demographic and baseline laboratory parameters such as age, gender, primary disease, dialysis age, creatinine were recorded in this system. Hemoglobin (Hb) level was monthly recorded. The platform generated Hb statistics report for each HD center monthly, including the detection rate, target rate and the distribution level of Hb, and released it to physicians through the WeChat terminal of mobile phone. After that, physicians could change the treatment of anemia individually on basis of this report. Here the demographic and baseline laboratory parameters, the detection rate, target rate, the average level and the distribution of Hb from June 2015 to October 2017 after the project launched were analyzed. Results From June 2015 to October 2017, 8392 maintenance HD patients from 28 HD centers in Shanghai were enrolled, of whom 5059(60.3%) were male.The average rate age was (60.5 ± 13.7) years old. Baseline average Hb was (108.3±16.0) g/L. Baseline detection rate and target rate were 54.2%and 47.5%, respectively. After 28 months follow?up, the detection rate of Hb increased from 54.2% to 73.6% (P<0.001), the target rate of Hb increased from 47.5% to 56.1% (P<0.001), and the level of average Hb rose from (108.3±16.0) g/L to (110.7±16.0) g/L. The difference between average Hb in two consecutive months was less than 1.3 g/L. Conclusions The telehealth?based dialysis registration system can timely report the anemia situation of HD patients, which may improve the awareness rate of anemia, the degree of attention and the compliance of anemia monitoring, so as to improve the detection rate and target rate of Hb and reduce the fluctuation of Hb, which helps to maintain the HD patients to correct anemia in a timely, stable and long?term way. The telehealth?based dialysis registration system, as an improved mode of dialysis registration is a promising way for long?term management of renal anemia in dialysis patients.
10.Diagnostic value of three evaluation methods of ¹⁸F-FDG PET/CT in detecting bone marrow infiltration in patients with newly diagnosed diffuse large B-cell lymphoma.

OBJECTIVETo evaluate three methods of ¹⁸F-FDG PET/CT in detecting bone marrow infiltration in patients with newly diagnosed diffuse large B-cell lymphoma.
METHODSSeventy-seven patients with newly diagnosed diffuse large B-cell lymphoma from July 2012 to June 2014 were retrospectively analyzed. All patients received both ¹⁸F-FDG PET/CT scan and bone marrow biopsy in the region of the posterior iliac crests. There were three evaluation methods of ¹⁸F-FDG PET/CT to detect bone marrow infiltration, including visual comparison (the FDG uptake in bone marrow of iliac crests was higher than the normal liver tissue), the maximal standardized uptake values (SUV(max)) in bone marrow of iliac crests (more than or equal to 2.5), the ratio of maximal standardized uptake values of iliac crests bone marrow to liver parenchyma intensity (more than 1). All results were compared with the bone marrow biopsy.
RESULTSVisual comparison of ¹⁸F-FDG PET/CT could be used to diagnose bone marrow infiltration, with the sensitivity of 100.00%, specificity of 80.00%, positive predictive value of 48.00%, and negative predictive value of 100.00%. When the SUV(max) of iliac crests was used as the diagnostic threshold, the sensitivity was 75.00%, with 92.31% specificity, 64.29% positive predictive value, and 95.24% negative predictive value. The ratio of SUV(max) had the best diagnostic efficiency, with sensitivity of 100.00%, specificity of 90.77%, positive predictive value of 66.67%, and negative predictive value of 100.00%.
CONCLUSIONThe ratio of SUV(max) is a valuable diagnostic method in detecting diffuse large B-cell lymphoatic bone marrow involvement.
