3.Continuous renal replacement therapy in treatment of patients with severe multiple injury
Chinese Journal of Primary Medicine and Pharmacy 2009;16(1):35-37
Objective To evaluate the value of continuous vent*venous hemofiltration(CVVH) in hindering systemic inflammatory response syndrome(SIRS) of severe multiple injury to proceed to multiple organ dysfunction syndrome(MODS). Methods 58 patients were randomly divided into routine group(n =31) and CVVH group(n = 27). Both groups were given routine treatment, while the patients in the CVVH group were given CVVH in addition. Additionally ,20 healthy persons were enrolled as normal group. The condition of SIRS were observed, and the serum C reactive protein(CRP)., tumor necrosis factor-α(TNF-α) , interleuldn-6 (IL-6), interleuldn- Ⅰβ(IL- Ⅰβ) levels were measured before and after treatment in CVVH group, routine group and in normal group at the time of health examina-tion. Results Temperature(T), respiration rate (RR) , heart rate (HR) and white blood cell (WBC) were reduced significantly in CVVH group(P < 0.05 or P < 0.01). The serum CRP, TNF-α, IL- Ⅰβ and IL-6 levels of CVVH group were reduced significantly(P <0.01). The number of patients with duration of SIRS were fewer and the incidence of MODS was lower in CVVH group than those in routine group(P <0.05) ,and the fatality rate was reduced significant-ly in CVVH group(P < 0.05). Conclusion CVVH would significantly improve the inflammatory symptom and signs of SIRs,inhibit the production of inflammatory cytokines in the treatment of SIRS. It can effectively prevent SIRS of severe multiple injury to proceed to MODS.

Objective To evaluate the clinical value of laparoscopic nephrectomy. Methods Laparoscopic nephrectomy was used in 26 cases (transperitoneal approach in 19 cases and retroperitoneal approach in 6 cases), including 8 cases of radical resection of renal cancer, 7 cases of total resection of kidney and ureter (2 cases of carcinoma of renal pelvis and 5 cases of carcinoma of ureter ), 6 cases of non-functioning or atrophic kidney and 5 cases of living donor nephrectomy. Results All the operations were accomplished successfully. The operative time was 120 min ~ 250 min (mean, 190 min) and the intraoperative blood loss was 50 ml ~ 200 ml (mean, 130 ml). No blood transfusion was needed and no operative complications occurred. Follow-up for a mean of 16 months in 8 cases of renal cancer and 20 months in 7 cases of total resection of kidney and ureter revealed no recurrence or metastasis except for 1 case of local recurrence of ureteral carcinoma. Conclusions The laparoscopic nephrectomy has the advantages of minimal invasion, less blood loss and rapid recovery.

Medical Journal of Chinese People's Liberation Army 2001;0(10):-
To explore the hemodynamic characteristics during burn shock stage in dogs immigrated onto high altitude, dogs scalded with 40% TBSA of Ⅲ degree burn (n=8) were randomly divided into a high altitude treatment group (H group) and a flatlands treatment group (F group) . All subjects were infused with lactated Ringefs solution at 1 postburn hour according to the Parkland formula. The changes in hemodynamic parameters, organ water content, pulmonary microvascular permeability, serum activity of PLA2 and CK-MB were determined. The results showed that under simulated high altitude condition sustained hypoxemia and acidosis as well as hypoxic damage of myocardial function were present before scalding, and sustained increase of PAP, PCWP and RVWI, even pulmonary hypertension and lung edema were found at 24 postburn hour in H group. At 48 postburn hour, the organ water content of heart, lung, etc increased more remarkably but urine output decreased more obviously in H group than those in F group. Serum activity of PLA2 and CK-MB increased gradually after burn. It is suggested that tissue edema may easily supervene with infusion according to Parkland formula under plateau condition. Hypoxic damage of organs should be corrected by increase of colloid volume resuscitation and dehydration treatment. Administration of hypertonic solution is able to correct burn shock.

Chinese Journal of Hospital Administration 1996;0(10):-
Objective The objective of digitalized patient records control is to meet some new demands set on patient records control under the new circumstances. Methods A computer system for digitalized patient records control and supporting processing, management and applications setups were established. Results The system put the contents of the patient records into various categories with difierent standards and designed relatively strict means of authorization. It rendered patient records easier to use and their control more strict, making it possible to make general use of the records without access to the original documents. As a result, data loss was avoided, authority over the data was put under control, data sharing was provided, and full and convenient use of data was realized. Conclusion Digitalized patient records control, a form of shaping computer-based patient records before the latter fully comes into being, enhances the efficiency of patient records application and reduces the pressure of paper records storage. It is a practical and effective way of controlling patient records.
