1.Correlation analysis of Sirt1 gene variant and susceptibility to chronic obstructive pulmonary disease

Objective To explore the relationship between Sirt1 single nucleotide polymorphisms ( rs2236319 ) and chronic obstructive pulmonary disease ( COPD ) susceptibility. Methods The subjects included 149 patients with stable COPD in the First People′s Hospital of Jining outpatient or ward from March 2014 to March 2015, and 160 cases of the same period health check. Sirt1?rs 2236319 genotype distribution frequency was detected by real?time PCR,and the levels of inflammatory factors were compared,including IL?6 and CRP. Results The AA,AG,GG genotype frequency of COPD group were 59. 1%( 88/149) ,35. 6%( 53/149),5. 4%(8/149) respectively,of control group were 45. 0%(72/160),48. 8%(78/160),6. 3%(10/160) respectively,the allele A,G frequency of COPD group were 76. 8%(229/298),23. 2%(69/298) respectively,of control group were 69. 4%( 222/320 ) , 30. 6%( 98/320 ) respectively, there were statistically significant differences between the two groups(χ2=6. 23,6. 06,P<0. 05) . IL?6 and CRP levels were significantly increased in COPD group carrying AA genotype as compared to COPD group carrying AG+GG genotype((10. 30±0. 40) mmol/L,(10. 16±0. 36) mmol/L;(2. 56±0. 20) mg/L,(2. 46±0. 22) mg/L;F=4. 52,8. 04,P<0. 05);There were no significant differences in terms of IL?6 and CRP levels in control group carrying AA genotype as compared to control group carrying AG+GG genotype ( P>0. 05 ) . Both the level of IL?6 and CRP were significantly increased in COPD group carrying AA genotype and AG+GG carriers compared to controls((10. 30 ± 0. 40 ) , ( 9. 88 ± 0. 56 ) mmol/L, ( 10. 16 ± 0. 36 ) , ( 9. 86 ± 0. 57 ) mmol/L;( 2. 56 ± 0. 20 ) , ( 2. 28±0. 25) mg/L,(2. 46±0. 22),(2. 26±0. 26) mg/L;F=34. 52,11. 09,73. 06,19. 38;P<0. 01). Conclusion The polymorphisms of Sirt1?rs2236319 may be an additional risk factor for COPD and associate with inflammatory factors. Furthermore the involvement of rs2236319 in the initiation and progression of COPD may relate to inflammatory processes.
2.The protective effects of OLA-PENa on experimental liver injury in rats

To investigate the protective effects of OLA-PENa on experimental hepatic injury in rats and compare it with OLA in the dose-response relationships. METHODS: Biochemical indexes and histopathological examination of hepatic injury in rats caused by toxicant chemicals [D-galactosamine (D-Galn) and CCl4] were determined. RESULTS: OLA-PENa obviously inhibited the rising of serum alanine transaminase (ALT) caused by D-Galn and CCl4 and dramatically decreased liver fat storage as well in a dose-dependent manner. Histopathological examination showed that OLA-PENa can evidently alleviate the condition of the degeneration of hepatic cells and that of necrosis. CONCLUSION: The protective effects of OLA-PENa on experimental liver injury in rats, all the mentioned effects, are more powerful than those of OLA with the same dosage.
3.Influence of atorvastatin to high sensitive C-reactive protein in the new onset type 2 diabetes of normal blood lipid with macrovascular disease

Objective To observe influence of atorvastatin to high sensitive C-reactlve protein in the new on-set type 2 diabetes of normal blood lipid with macrovascular disease. Methods 96 cases with the new onset type 2 diabetes of normal blood lipid with macrovascu]ar disease were randomly divided into two groups,the treatment group contains 49 cases who were given atorvastatin 10rag for 12 weeks,the control group contains 47 cases who were given placebo for 12 weeks. Two groups were underwent the following test,fasting plasma glucose(FBG) ,post-OGTT 2 hour plasma glucose(PPG) ,glycosy-lated hemoglobin(HbAic) ,total triglyceride(TG) ,total cholesterol(TC) ,low density lipoprotein cholesterol(LDL-C) ,high sensitive C-reactive protein(hs-CRP). Results High sensitive C-reactive pro-tein(hs-CRP) in treatment group is significantly lower than control group[(2.13±1.38) mg/L and (4.04±3.14) mg/L,P < 0.05]. Conclusion Atorvastatin can significantly decease the level of high sensitive C-reactive protein (hs-CRP) to the new onset type 2 diabetes of normal blood lipid with macrovascular disease.
4.Resistant Genes of Beta-lactam Antibiotics from Clinical Isolates of Pseudomonas aeruginosa

OBJECTIVE To investigate ?-lactamase coding genes and oprD2 gene in multidrug-resistant Pseudomonas aeruginosa isolated.METHODS Polymerase chain reaction(PCR) was used to detect various ?-lactamase coding genes including TEM,SHV,OXA,PER,GES,IMP,VIM,plasmid type AmpC ?-lactamase DHA,MIR and oprD2 in 20 strains of P.aeruginosa.RESULTS The detection rate of ?-lactamase coding genes plasmid type AmpC,VIM,TEM,SHV,OXA and IMP were 15%,5%,15%,10%,10% and 5%,respectively,the loss rate of oprD2 was 25%,but PER and GES genes were negative.CONCLUSIONS The study indicated that the P.aeruginosa is carrying genes of DHA,TEM,SHV,OXA,VIM,IMP and being lost oprD2 gene,the latter is the essential resistance mechanism of P.aeruginosa to beta-lactam antibiotics in local area.
5.Median effective dose of ropivacaine for spinal anesthesia when combined with sufentanil in patients undergoing caesarean section

Objective To determine the median effective dose (ED50 ) of ropivacaine for spinal anesthesia when combined with sufentanil in patients undergoing caesarean section. Methods Twenty-eight ASA Ⅰ or Ⅱ parturients, aged 18-40 yr, weighing 50-110 kg, undergoing cesarean section under combined spinal-epidural anesthesia, were enrolled in this study. Combined spinal-epidural anesthesia was performed at L2,3 interspace. The mixture of ropivacaine and 5 fig sufentanil was injected into the subarachnoid space over 30 s. The initial dose of ropivacaine was 11 mg. The dose was increased/decreased by 1 mg in the next patient. The ED50 and 95% confidence interval were calculated by up-and-down method. Results The ED50 of ropivacaine was 7.780 mg (95% confidence interval 6.850-8.836 mg). Conclusion When combined with sufentanil 5 μg, the ED50 of ropivacaine for spinal anesthesia is 7.780 mg in patients undergoing caesarean section.
6.Comparative analysis of domestic and foreign medical MOOC

Objective To provide the effective and convenient medical MOOC and remote medical education by comparatively analyzing domestic and foreign medical MOOC.Methods The following items were comparatively an-alyzed, including the number of medical MOOC, the universities offering medical MOOC, the languages used in teaching medical MOOC, the identification of MOOC, and the development of domestic and foreign medical MOOC on platforms of Coursera, edX, China university MOOC and people's health MOOC.Results The domestic medical MOOC still had a longer way to go than foreign medical MOOC in their number, scale and identification.Conclu-sion Domestic medical workers should grasp the opportunity to provide more effective and convenient medical MOOC platform.
7.Inhibition Effect of Non Custodial Terpenes-3β-Alcohol to Autoimmune Encephalomyelitis

Objective To study the inhibition effect of non custodial terpenes-3β-alcohol to experimentally in-duced autoimmune encephalomyelitis in guinea pigs. Methods Different doses (25 mg/kg, 50 mg/kg and 100 mg/kg) of non custodial terpenes-3β-alcohol were given to the experimentally induced autoimmune encephalomyelitis model of guinea pigs by gavage for 8 weeks. Plasma levels of CD4+/CD8+, IL-1, IL-2, IL-6, IL-10, neuropeptide Y (NPY), beta endorphin (β-EP) , transforming growth factor-β(TGF-β), matrix metalloproteinase (MMP-2), nitric oxide synthase (NOS) and leuko-cyte differentiation antigen CD3 were assessed. The brain neuron morphology changes was observed under light microscopy while its ultrastructure changes was observed under electron microscope. NOS expression in neurons was observed through immunofluoresce technology. Results Non custodialterpenes-3β-alcohol inhibited the increase of plasma CD4+/CD8+, IL-1, IL-2, IL-6, IL-10, MMP-2, CD3 and NPY while decrease of plasmaβ-EP, brain TGF-β. It also increase NOS expres-sion in neuronal cytoplasm and maintained neuron morphology. Conclusion Non custodial terpenes-3β-alcohol inhibit-ed the experimental autoimmune encephalomyelitis in guinea pig.
8.Current situation and countermeasures of smoking cessation service system construction in China

Abstract
In order to achieve the goal of reducing the smoking rate among Chinese adults at ages of 15 years and above to below 20% by 2030, smoking cessation services require to be reinforced. The current Chinese smoking cessation service system includes short-term smoking cessation interventions and smoking cessation hotlines at the population level, and smoking cessation clinics at the individual level; however, these smoking cessation services suffer from problems of low awareness, low accessibility and low utilization rate. Based on the publications pertaining to smoking cessation services and released policy documents in China until June 2022, this review analyzes the current status, problems and causes of smoking cessation services, and proposes suggestions for improving the smoking cessation service system construction in China.
9.Effect of low protein diet on nitrogen balance in peritoneal dialysis patients

ObjectiveTo observe the influence of different dietary protein intake (DPI) on nitrogen balance and nutritional indices in peritoneal dialysis (PD) patients, and explore the minimal DPI to maintain nitrogen balance.MethodsThirty-four PD patients were randomly divided into group A, B and C with DPI as 1.2, 0.9 and 0.6 g·kg-1·d-1 respectively. All the patients admitted into our hospital and completed a 10-day assessment for nitrogen balance, as well as nutritional status including serum albumin (Alb), pre-albumin at baseline, the 7th and 10th day. ResultsThe DPI of group A, B and C was (1.18±0.05), (0.87±0.02), (0.66±0.03) g·kg-1·d-1, whose differences were significant (P<0.01). The dietary energy intake (DEI) was 129.29 (117.57-133.89), 111.71 (100.42-133.47), 146.86 (128.03-163.18) kJ·kg-1·d-1 respectively. Nitrogen balance was positive in group A, B, C [2.99 (2.15-4.72) g, 1.20(0.59-1.89) g, 0.24 (-0.87-1.27) g]. The BUN decreased at the 7th and 10th day (P<0.01) in group C. The BUN and phosphorus in group A increased, but without significant difference as compared to baseline. No significant differences of nutritional status were found among three groups throughout the trial. ConclusionMinimal DPI 0.65 g·kg-1·d-1 plus the supplement of protein loss in dialysate can maintain the nitrogen balance in peritoneal dialysis patients.
10.Correlation of muscle-derived microparticles and lactadherin proteins with hypercoagulability in patients with perihip fractures

Objective:To investigate the changes of skeletal muscle-derived microparticles (SKMPs) and their scaving proteins lactadherin in peripheral blood of patients with perihip fractures and their effects on blood coagulation. It provides a theoretical basis for clinical evaluation of hypercoagulability in patients with perioperative hip fractures.Methods:A total of 44 patients (72.87±13.9 years old) with perihip fractures from October 2020 to January 2021 were included in the fracture group, and 40 healthy people were included in the group without fracture. Activated clotting time (ACT) in fracture group and control group was measured by coagulation and platelet function analyzer. The level of SKMPs in peripheral venous blood was determined by flow cytometry. Enzyme-linked immunosorbent assay (ELISA) was used to detect the levels of MPs scaving proteins lactadherin in peripheral blood of fracture group and control group.Results:A total of 44 patients with perihip fractures were included. There were 33 (75%) femoral neck fractures and 11 (25%) intertrochanteric fractures. SKMPs concentration in peripheral blood of perihip fracture group 175.73/μl; postoperative fracture group 124.28/μl; Normal control group 90.69/μl. ACT was (377.5±76.2) s in the perihip fracture group, (498.1±97.1) s in the post-fracture group, and (505.7±44.0) s in the control group. Correlation analysis showed that there was a linear relationship between SKMPs count and ACT value in fracture patients ( r=-0.52, P<0.001). There was no significant correlation between SKMPs concentration and ACT value in peripheral venous blood of the post-fracture group ( P>0.05); Lactadherin protein concentration in perihip fracture group: 2 779.61 pg/ml; Fracture postoperative group 1 985.38 pg/ml; Normal control group 1 651.19 pg/ml. There is a positive correlation between Lactadherin concentration and SKMPs concentration with a correlation coefficient of 0.70. Conclusion:SKMPs plays a role in the hypercoagulability of patients with perihip fractures. Decompensation of lactadherin protein may be one of the causes of perioperative hypercoagulation in patients with hip fractures.
