Objective To research the clinical value of mechanical ventilation in the treatment of acute left ventricular failure. Methods Of 74 case with acute left ventricular failure,26 cases recevived routine treatment including nasal-catheter inhale of oxygen or veil inhale of oxygen and inotropic agent, diuretic, and dilation as well as sedative while the mechanical ventilation group were treated only with mechanical ventilation depending on the condition. The clinical indications, parameters of blood gas analysis and hemodynamies were compared before and after treatment. Results 18 cases from routine treatment group died(70.9%)and 5 died in mechanical ventilation group (10.4%).There was significant difference in heart rates (HR),central venous pressure (CVP),the mean arterial blood pressure (MAP) and some parameters of blood gas analysis before and after the treatment (P<0.05 or P
2.EFFECTS OF INTERMITTENT OVERPRESSURE IN EAR CANAL ON ENDOLYMPHATIC HYDROPS IN GUINEA PIGS
Medical Journal of Chinese People's Liberation Army 2001;0(11):-
Intermittent overpressure of 20cmH 2 O was delivered to the external ear canal of guinea pigs. Histopathological changes in the middle and inner ear were examined under light microscope. Auditory evoked potential was recorded. The expression of atrial natriuretic peptide (ANP) was investigated by immunohistochemical SP method. We found that intermittent overpressure could minimize, but could not interrupt the development of endolymphatic hydrops, and could not improve hearing function. Expression of ANP decreased in the cochlear lateral wall after 2 week intermittent overpressure. The results suggested that intermittent overpressure of 20cmH 2 O was suitable in this experiment, and that ANP positive cells had an autocrine/paracrine function.

Objective To study the effects of ulinastatin on proinflammatory cytokine and oxygen free radicals during acute lung injury in rabbits. Methods Eighteen health New Zealand rabbits were randomly divided into 2 groups: oleic acid group(n=9) and ulinastatin group(n=9). The rabbits of oleic acid group were intravenously injected analytical pure oleic acid(0.12ml/kg) to set up a model of acute lung injury. The rabbits of ulinastatin group were intravenously injected ulinastatin 50000u/kg before injecting oleic acid and received intravenous infusion of ulinastatin 5000u?kg -1 ?h -1 after injecting oleic acid. Before injecting oleic acid and 1h, 2h, 3h and 4h after injecting oleic acid, venous blood samples were taken to determine the concentration of plasma IL-8, MDA and active oxygen, and arterial blood samples were also taken for gas analysis. Results The concentration of IL-8, MDA and active oxygen and PaCO 2 gradually increased, and PaO 2 gradually decreased after injecting oleic acid in the both groups of rabbits. The changes of the above indices in the oleic acid group were larger than those in the ulinastatin group, and the difference between the two groups was significant. Conclusion Ulinastatin inhibited the release of proinflammatory cytokine and reduced the production of oxygen free radicals during acute lung injury.

BACKGROUND: Considerable studies demonstrate that nitric oxide synthase (NOS)/nitric oxide (NO)plays an important role in maintaining normal function of Sertoli cells, and influences spermatic generation and activation as well as fertilizability.OBJECTIVE: To observe the effect of goat testis extract on NOS activity in Sertoli cells of mice with testis injury caused by heavy mental Pb.DESIGN: Randomized controlled animal trial.SETTING: Department of Histology and Embryology, Jilin Medical College.MATERIALS: This trial was carried out in the laboratory of Histology and Embryology, Jilin Medical College (Key laboratory of the general logistics department of P.L.A) during March 2004 to August 2005. Thirty healthy Kunming male mice were involved and randomized into 3 groups, with 10 in each group: control group, testis injury model group (model group) and goat testis extract-treated group (treatment group).METHODS: The mice in the model group and experimental group were daily administrated with 100 g/L lead acetate, 0.2 mL/(mouse·d), 5 times/wk within 2 weeks, then withdrawal for 1 week. Simultaneously, the mice in the treatment group were subcutaneously injected with goat testis extract at 0.5 mL/(mouse ·d). The mice in the control group were given redistilled water of the same dose as that in the treatment group. After being poisoned fully, the mice were fasted for 12 hours and weighted, finally sacrificed by decapitation. Bilateral testis were dissected, immediately weighted, fixed with formalin and sliced. The NOS changes in Sertoli cells of mice in each group were observed with reduced nicotinamide-adenine dinucleotide phophate-diaphorase(NADPH-d) histochemical method combined with microscope image.MAIN OUTCOME MEASURES: Body mass, bilateral testis mass and NOS absorbance (A) in Sertoli cells of mice in each group after contamination expiration.RESULTS: All the 30 mice were involved in the result analysis, without deletion. ①After contamination expiration, the body mass, bilateral testis mass and NOS A value in treatment group were significantly than those in the model group [(22.47±3.49) g vs. (19.13±3.46) g;(0.113±0.021 ) g vs.(0.089±0.017) g; 0.236±0.020 vs. 0.146±0.023, t=2.151-3.314,P < 0.05-0.01]; ② In the model group, NOS positive Sertoli cells swelled and degenerated; The morphology of NOS positive Sertoli cells in the treatment group was close to that in the control group.CONCLUSION: Goat testis extract can boost the NOS activity in Sertoli cells of mice with testis injury caused by heavy mental pliumbum and has some repairing and protective effect on testis injury, which can provide new thinking for treatment of male sterility.

Objective To investigate the advantages, indications, operative techniques and pre-liminary curative effect of proximal femoral nail antirotation(PFNA) in surgical treatment of intertrochan-teric fractures. Methods From March 2006 to June 2007, PFNA fixation was done on 38 patients with intertrochanteric fractures including 17 males and 21 females, at an average age of 65. 4 years(35-93 years). According to AO/ASIF classification, there were 10 patients with type 31-A1 fractures, 21 with type 31-A2 and 7 with type 31-A3. All patients were reduced through longitudinal traction on the traction table, with implantation of PFNA via a mini-incision. Results The mean operation duration was 50 minutes(35-105 minutes)and mean blood loss 100 ml(50-300 ml). All patients were successful except for 1 patient with split lateral cortical bone of distal shaft of the femur during operation. All pa-fients were followed up for average 13 months(4-19 months), which showed long-team bed stay due to severe complieations in 1 patient. Function recovery in 32 and deterioration of the injured limbs in 5. Cer-ebral infarction was found 6 months after operation in 1 patient, who was able to walk under support. Pri-mary bone union was achieved in all patients, with average healing time of 9. 5 weeks. There were no complications such as infection of incisional wound, pulmonary infection, phlebothrombosis, over incision or coxa adducta, except that 2 patients had perioperative respiratory system disease. According to Harris hip rating scale system. The result was excellent in 20 patients, good in 13, common in 4 and poor in 1, with total excellence rate of 89%. Conclusions As a new technique, PFNA has advantages of litth trauma. Early get-out-of-bed exercise, few complications, rigid fixation and high bone union rate and is suitable for all kinds of intertrochanteric fractures especially for old patients with osteoporosis.

Data quality management system is essential to ensure accurate, complete, consistent, and reliable data collection in clinical research. This paper is devoted to various choices of data quality metrics. They are categorized by study status, e.g. study start up, conduct, and close-out. In each category, metrics for different purposes are listed according to ALCOA+ principles such us completeness, accuracy, timeliness, traceability, etc. Some general quality metrics frequently used are also introduced. This paper contains detail information as much as possible to each metric by providing definition, purpose, evaluation, referenced benchmark, and recommended targets in favor of real practice. It is important that sponsors and data management service providers establish a robust integrated clinical trial data quality management system to ensure sustainable high quality of clinical trial deliverables. It will also support enterprise level of data evaluation and bench marking the quality of data across projects, sponsors, data management service providers by using objective metrics from the real clinical trials. We hope this will be a significant input to accelerate the improvement of clinical trial data quality in the industry.
