1.A novel diterpene from Caesalpinia minax

Objective To study the chemical constituents of Caesalpinia minax.Methods The chemical constituents were isolated and purified by various chromatographic methods and their structures were elucidated by the analysis of spectral data and physicochemical properties.Results One diterpene compound was isolated from the 95% alcohol extract of C.minax,which was named minaxin A,and the inhibitoryactivitiesofthecompoundonHepG2cellwerestudiedbyuseofMTTmethod.ConclusionMinaxin A is a new compound and shows the significant inhibitory activities on HepG2 cell.
2.Establishment and application of networked information management system for clinical laboratories in community of Suzhou City

Objective To establish a networked information management system for clinical laboratories in community of Suzhou and evaluate its clinical application.Methods Based on dedicated network of public health information in Suzhou City,laboratory information management system (LIS) and hospital information management system (HIS),the management for centralized detections of clinical samples from the health institutions of intra-regional community were performed as the key mission,and a regional,networked and informationized management system was designed for the clinical laboratories of community.Results The informationized management of whole process including test application,specimen collection,delivery and receiving,report releasing and payment,etc.was achieved on line.The information of clinical laboratories among the communities was transferred and shared in whole Suzhou City.Conclusion The established system should be safe,convenient and efficient online network-based information management system between the Suzhou clinical laboratory center and the laboratories in each local community,by which the centralized detections in the clinical laboratories of community could be performed readily.
3.Optimizing Extraction Technics for Paishi Granule by Orthogonal Test

OBJECTIVE:To optimize the extraction process of paishi granule.METHODS:The extraction process of paishi granule was optimized by L 9 (3 4 )orthogonal test with total flavonoids as index,and with duration of decoction,times of de?coction,the amount of water added and the amount of alcohol used for extraction as factors of investigation.RESULTS:The optimum extraction condition was the following,the amount of water added to which was12times that of the medical material,and it was decocted120min for3times,which then was dried and crushed,then60%alcohol,which was40times that of the amount of medicinal material,was extracted by refluxing for3times with30min each time,filtered while hot,and the alcohol was recovered.The yield of extractum and the content of the total flavonoids were fairly high,and there was a good repro?ducibility.CONCLUSION:The optimized process is stable and feasible.
4.Effects of carvedilol on cardiomyocyte apoptosis and sarcoplasmic reticulum Ca~(2+) AT-Pase activity in rats with congestive heart failure

Objective To study the mechanism of carvedilol therapy of congestive heart failure. Methods After chronic heart failure model was established by ligation of the left coronary artery in rats, rats were treated with carvedilol and terazosin. Then hemodynamic parameters, activity of sarcoplasmic reticulum(SR) Ca 2+ pump(SERCA 2a ) and the rate of cardiomyocyte apoptosis were determined. Results Compared with those of the control group( group C), the activity of SR Ca 2+ pump in the heart failure group(group F) decreased( P 0.05). Carvedilol intervention reduced the rate of cardiomyocyte apoptosis, but enhanced the activity of SR Ca 2+ pump significantly in dose dependent manner. 6.Beta-lactamase Gene and intⅠ1 Gene in ESBLs-producing Klebsiella pneumoniae

OBJECTIVE To investigate the beta-lactamase gene and intⅠ1 gene in ESBLs-producing Klebsiella pneumoniae.METHODS Beta-lactamase gene and intⅠ1 gene were analyzed by PCR.RESULTS The positive rates of TEM,SHV,OKP,CTX-M-1group,CTX-M-2group,CTX-M-9group,GES,PER,VEB,OXA-10,ACT-1,LEN,DHA,and intⅠ1 were 51.4%,0%,0%,17.1%,0%,11.4%,0%,0%,0%,0%,0%,5.7%,17.1%,and 60%,respectively.Beta-lactamase gene was found in 62.9%,intⅠ1 gene was found in 60%.CONCLUSIONS There are very high positive percentages of beta-lactamase gene and intⅠ1 gene in ESBLs-producing K.pneumoniae.
7.MRI diagnosis of epithelioid angiomyolipoma of liver

Objective To retrospectively analyze the MRI findings of epithelioid angiomyolipoma (EAML) of the liver.Methods The MR findings in a series of 8 patients with pathologically proven liver EAML were retrospectively analyzed. Distribution of lesions, signal intensity characteristics, tumor vascularity,feeding arteries and draining veins, fat, hemorrhage, cystic necrosis and pseudocapsule formation were evaluated and correlated with pathology.Results A single lesion was found in each of the 8 patients.All eight lesions showed long T1,long T2 inhomogeneous signal,and high DWI signal. There was only 1 lesion presented with fat,3 lesions presented with hemorrhage. On the dynamic contrastenhanced MRI scan,all 8 lesions manifested as hypervascular with tortuous vasculature in 7 patients.Dilated feeding arteries and draining veins were seen in 5 patients. Conclusion Liver EAML has unique MR imaging features,exhibiting hyperwascular lesions with dialated,tortuous feeding arteries,and draining veins on DCE MRI,which could be helpful in the differential diagnosis of liver EAML.
8.The Expression of Muti-Genes Via a Recombinant Adenovirus and Their Effects of Inducing Apoptosis on Lung Cancer Cells

To observe the expression of human wild-type p53, B7-1 and GM-CSF genes mediated by recom-binant adenovirus and their effects of inducing apoptosis on lung cancer cells. Methods; Human wild-type p53, B7-1 and GM-CSF genes were transfected into lung cancer cells mediated by recombinant adenovirus. The expression products of these genes were detected by immunohistochemistry assay, flow cytometric analysis and ELISA. Cell growth assay was carried out by counting alive cells after trypan blue exclusion. Cell apoptosis was detected by TdT assay of DNA fragmentation . Results: The multi-genes could be efficiently expressed in lung cancer cells mediated by recombinant adenovirus, which could suppress lung cancer cell growth and induce their apoptosis. Conclusion: These results suggest the feasibility of muti-gene therapy for lung cancer.
9.Therapeutic evaluation between coventional fraction radiotherapy and intensity modulated radiation therapy combined with concurrent chemotherapy for esophageal carcinoma

Objective To evaluate the curative effect between conventional fraction radiotherapy (CFRT) and intensity modulated radiation therapy (IMRT) combined with concurrent chemotherapy for unresectable esophageal carcinoma.Methods 144 patients were enrolled into this study.They were divided into CFRT group and IMRT group.CFRT group (60 cases):irradiation field encompassed primary lesion and mediastinal lymph drainage regin.Neck and thoracic upper segment cases were made front thorax T field irradiation first,DT 36 Gy/18 f,followed by two front oblique field made Iso-central irradiation,field width was 4.5-5.0 cm,gantry angle 50°-60°,wedge filter 30°,DT added to 66 Gy.For thorax middle or lower segment patients,field width was 6.0-6.5 cm,field length was that the tumor upper and lower each extended 3.0-5.0 cm,vertically irradiated in thoracic front and at the back,after DT 36 Gy/18 f,changed to two back ablique field,made Iso-central irradiation,DT added to 66 Gy.IMRT group (84 cases):GTV was esophageal primary lesion and metastasis lymph node (GTVnd);CTV included GTV,GTVnd and mediastinal lymph drainage region,at GTV around all was widened 0.8-1.0 cm,its upper and lower each extended 3.0-5.0 cm;PTV were in the basis of CTV,all were widened 0.5 cm again.With ≥95 % isodose curve covering the PTV and the target dose was prescribed to PTV 60-64 Gy/30-32 f.Organ at risk (OAR):both lungs V20≤30 %,spinal cord ≤45 Gy/6 weeks,heart V30≤40 %.Chemotherapy of two groups was same,DDP 25-30 mg/m2 × 3-5 days,5-Fu 450-500 mg/m2 × 5 days civ,28 days were one period,total 2 period,after 1-3 months,strengthen chemotherapy 3-4 period.Results Leucopenia and radiation-induced esophagitis as shown by the WHO staging system were the most common acute toxicities,but were mainly grade 1 or 2.The incidence rate of leucopenia and radiation-induced esophagitis in CFRT group were 78.3 % (47/60) and 83.3 % (50/60),IMRT group were 82.1% (69/84) and 79.8 % (67/84).After symptomatic treatment,all patients completed the planner treatment.During radiotherapy and after treatment,the patients showed grade 1-2 radiation-induced pneumonitis,CFRT group were 36.7 % (22/60),but IMRT group were only 9.5 % (8/84) (x2 =9.99,P < 0.01).In CFRT group and IMRT group,the 1-,3-,5-year survival rates were 75.0 % (45/60),36.7 % (22/60),20.0 % (17/84) and 79.8 % (67/84),35.7 % (30/84),21.4 % (18/84),respectively.Conclusion The overall response and 5-year survival rates of CFRT and IMRT combined with concurrent chemotherapy are similar,however,the incidence of grade 1-2 radiation-induced pneumonitis of IMRT is low,which indicates that IMRT can reduce the lungs irradiational volume and dose.
10.Investigation on clinical efficacy of the fourth-generation ultrasonic lithotripsy system in the treatment of urinary tract stones

ObjectiveTo evaluate the fourth-generation ultrasonic lithotripsy system for treating the urinary tract stones.Methods243 cases of urinary tract stones who received treatment of fourth-generation EMS ultrasonic lithotripsy system were analyzed retrospectively.ResultsImmediate phase I lithotrotrisy was performed successfully in 227 cases and the successful rate of phase I was 93.3% (227/243).Delayed phase Ⅱ lithotripsy was performed for 16 patients.Complications happened in 2 cases,one case occurred hydropneumothorax during operation,required indwelling thoracic drainage tube processing,the other had severe bleeding which conservative treatment was ineffective,cured by the intervention of super-selective renal artery embolization.Conclusion Fourth generation of ultrasonic lithotripsy for treating stones on different anatomical locations of urinary system was safe,practical and efficient,worthy of clinical application.
