1.Early rehabilitation for children with brain impair syndrome and cerebral palsy in hospital and in family

ObjectiveTo compare the early rehabilitation of child with brain impair syndrome and cerebral palsy executed in hospital and in family.Methods180 children of 0~2 years old with brain impair syndrome and cerebral palsy were divided into group A and group B randomly. The children of 0~6 months old were in group A1 and group B1, while that of 6 months~2 years old were in group A2 and group B2, correspondingly. Children in group A accepted early formal strengthen rehabilitation training in hospital, and that in group B accept family rehabilitation through guidance. Development quotient (DQ) were assess with Japan Yuanchengsi infant analysis growth scale half a year after.ResultsThe DQ of group A showed higher than that of group B(P<0.01), and DQ of group A1 was higher than DQ of group A2(P<0.05).ConclusionEarly rehabilitation training in hospital is more effective on early cerebral palsy.
2.Significance and expression of p-STAT3 and survivin in primary gallbladder carcinoma

Objective To detect the expression of p-STAT3 and survivin in primary gallbladder carcinoma (PGC) and explore its significance for the genesis and development of PGC.Methods The expression of p-STAT3 and survivin were determined with immunohistochemistry in samples from 45 PGC tissues and 20 chronic cholecystitis.The relationship between the expression of these proteins and various clinicopathological factors was evaluated.Results The expression rate of p-STAT3 and survivin in 45 PGC tissues was 55.6% ( 25/45 ) and 64.4% ( 25/45 ),respectively,which was significantly higher than those in 20chronic cholecystitis tissues 5.0% ( 1/20),10% ( 2/20 ) ( P < 0.01 ).pSTAT3 expression was correlated with survivin ( r =0.830,P <0.01 ).p-STAT3 and survivin protein expression were significantly associated with histopathological grading,lymph node metastatis,nevin staging and 3-year survival rate.Conclusions p-STAT3 and survivin might play a vital role in the development of PGC.The expression of p-STAT3 or survivin was an independent prognostic factor in PGC.
3.Analysis on Early Jaundice of 36 Dyskinetic Cerebral Palsy Children

Objective To investigate the relationship between the neonatal period hyperbilirubinemia and dyskinetic cerebral palsy.Methods The data of 36 dyskinetic cerebral palsy children with early neonatal period significant jaundice were analyzed retrospectively. The information of children was obtained through a questionnaire that included the case, severing clinical characteristics and treatment.Results 12 cases (33.33%) were born with hemolytice disease of newborn; 8 cases (22.2%) were born with early onset infection; 5 cases (13.9%) were born with cephalohematome; others were unknown etiologies. 6 cases had serum total bilirubin (TSB) value more than 600 μmol/L (the highest value was 792 μmol/L); 12 cases had TSB value ranged from 513 μmol/L to 600 μmol/L; 14 cases had TSB value ranged from 425 μmol/L to 512 μmol/L; 2 cases had TSB value lower than 425 μmol/L (the lowest value was 396 μmol/L). 2 cases had no TSB data. The time of jaundice significantly turned severer was 156±12.56 hours after birth. The time of jaundice continued was 23±5.62 days after birth. 34 cases had been diagnosed and treated. But the first treatment time of all were later 150 hours after birth.Conclusion As the main risk factor of dyskinetic cerebral palsy, severe hyperbilirubinemia need be valued. How to ease bilirubin quickly in the early stage is much important.
4.Financing research on essential medical services in China

To achieve the goal of universal healthcare coverage,and the objective of the ongoing healthcare reform to establish an essential healthcare system,the study proposed a financial framework for building the essential medical service package,covering medical services offered by primary medical institutions,treatment of major diseases,and essential medical services offered by secondary and tertiary hospitals.With data over the years of the total medical expense and medical service usage as the basis,and in the principles of affordability and cost-effectiveness,the total financing quota of essential medical services is expected to reach 1 940.846-2 1 62.41 7 billion,accounting for 30.66%-34.1 6% of the total healthcare expenditure.75% of the financing load should be carried by the government and society, focusing on financing medical services offered by primary institutions and lowering out-of-pocket burden of residents.
5.Analysis of the use of essential medical services and selection of priority services

Objective To divide the medical services currently offered by various medical institutions into priority,extended and non-essential items.Methods The items were divided according to their actual usage at these hospitals,and such services were screened based on hospital positioning and clinical pathway of diseases.Results The selected priority services at the primary,secondary and tertiary hospitals were 255, 378 and 820 respectively.Their proportions in total medical services of these hospitals were 92.9%,95.9% and 97.4% respectively,and the proportion of their costs in total medical service costs were 57.9%,76.8% and 84.5% respectively.Conclusions The selected priority items had covered most of the services and costs,which deserve promotions at all the hospitals as it embodied the principle of benefiting the majority of the population.
6.Definition of priority/major diseases for essential medical services

Objective To determine the main contents and key points of the essential medical services by means of priority setting of diseases with high incidence and serious damage based on the demand of residential medical services.Methods The priority setting method is applied in this study,and the incidence,prevalence,hospitalization rates and the ratio of different types of inpatient are used as indicators to reflect medical demand and utilization.The integrated balance method is also used,and the priority diseases list is made based on the analysis from the view of disease onset,considering the service delivery,social equity and the health financing.Results Based on the data analysis made,this paper proposed that the priority diseases cover 29,66 and 103 types for primary hospitals,secondary hospitals and tertiary hospitals respectively.The main diseases so determined include hypertension,diabetes, maternal and child health,severe mental illness,infectious diseases,emergency treatment,etc.Conclusions The method and result of setting priority disease and main disease can be the basis of setting for main diseases in essential medical services.
7.Thoughts and framework on how to define essential medical services

Boundaries definition plays a key role in defining the scope of essential medical coverage of the country and the governmental role positioning in medical service offerings.It is also a precondition of furthering the ongoing healthcare reform.This paper analyzed the data of health service demand,supply and financing using the priority setting and the integrated balance methods.It suggested that the definition of the essential medical services should embody Chinese characteristics and be consistent with the Party′s governing philosophy and social core values.It also should be fully considered that the administration system,the governing philosophy,the medical insurance system and the government duty in the healthcare system of China.This paper proposed a multiple-criteria defining of the essential medical services,which should focus on main healthcare issues in China,and be adapted to the current healthcare reform process.Three dimensions need to be considered in the defining,which are the demand,supply and financing of the healthcare services,along with the impact of the housing,equipment,personnel, technology,supplies,drugs and other medical service elements.This paper presented the overall framework of essential medical services in four levels,which is composed of the basic package,the core package,the priority package and the expansion package.
8.Characteristics of the Children Abandoned Treatment after Cerebral Palsy

ObjectiveTo investigate the time, causes and modes of the cerebral palsy children abandoned treatment. MethodsThe information of 50 cerebral palsy children who abandoned treatment were analysed retrospectively. Results and ConclusionCerebral palsy children abandoned their treatment in 3~9 months after treatment mostly. The main causes included their parents had little cognition and confidence to the treatment, or were in financial problem. 10 cases were without any treatment, 18 cases tried unofficial treatment, 19 cases accepted consolatory treatment, and 3 cases fell back on superstition.
9.Effects of Playful Training on Home Environment and Development in Children with Cerebral Palsy

Objective To investigate the effects of playful training on home environment and development in children with cerebral palsy. Methods89 children with cerebral palsy were divided into observation group (45 cases) and control group (44 cases). All children were treated with normal strengthen rehabilitation in hospital. The observation group received the training playfully. They were assessed with the Home Observation for Measurement of the Environment (HOME) and Bayley Scale of Infant Development (BSID) before and 6 months after treatment. ResultsAfter treatment, The HOME scores in observation group was significantly better than those before treatment and in control group (P<0.01). The Psychomotor Dvelpment Index (PDI) of BISD was improved in both groups (P<0.01) with significant different between them(P<0.05). The Mental Development Index (MDI) of BISD in observation group was significantly better than those before training and in control group (P<0.01). ConclusionPlayful training can improve the development and home environment in children with cerebral palsy.
10.Construction and Application of Liver Injury Risk Prediction Model of Chinese Medicinals based on Indirect Toxicity

ObjectiveTo explore and establish the liver injury risk prediction model of indirect toxicity of Chinese medicinals under the condition of compound formulas, and provide new ideas and methods for the study of evaluation of liver injury of Chinese medicinals based on indirect toxicity. MethodsTaking Buguzhi (Fructus Psoraleae) pre-parations as model drug, the combined Chinese medicinals with Buguzhi (Fructus Psoraleae) of high frequency are screened out, and their components and action targets were obtained through TCMSP, TCMIP and PharmMapper databases. The association strength value and risk value of Chinese medicinals that acted on the nuclear factor κB (NF-κB) pathway were analyzed. For those having greater values than the median association strength value and risk value were regarded as indirect Chinese medicinals of liver injury risk. In this way, a prediction model of liver injury risk of Chinese medicinals was constructed based on immune activation-related indirect liver injury process (taking NF-κB pathway as an example). And verification of the prediction model was performed using Heshouwu (Radix Polygoni Multiflori) preparations. ResultsThe prediction model of liver injury risk based on important immunoactivated pathway (taking NF-κB pathway as an example) found that Yinyanghuo (Herba Epimedii) (association strength value = 0.18, risk value = 0.25) was a Chinese medicinal with potential risk of indirect liver injury within Buguzhi (Fructus Psoraleae) prepartions, which may increase the risk of liver injury by positively regulating Bruton's tyrosine kinase (Btk) and protein kinase C theta (PKCθ) on NF-κB pathway. Further verification of prediction model by Heshouwu (Radix Polygoni Multiflori) preparations showed that Buguzhi (Fructus Psoraleae) (association strength value = 0.25, risk value = 0.33) and Tusizi (Semen Cuscutae) (Semen Cuscutae, association strength value = 0.34, risk value = 0.33) may increase the liver injury risk of Heshouzu. ConclusionThe liver injury risk prediction model of indirect toxicity of Chinese medicinals has been constructed in this study, providing metho-dological reference for the identification of Chinese medicinals of indirect liver injury risk under the condition of compound formulas.
