1.Biomechanical properties of partial carpal ligaments

BACKGROUND: Although hand urgical doctors have drawn the attentions to carpal diseases mainly manifesting as carpalinstability, it is still expected to carry on the researches on biomechanical properties of Chinese carpal ligaments. OBJECTIVE: To understand the biomechanical properties of partial carpal ligaments so as to provide experimental evidence for the development ofclinical hand surgery. DESIGN: A simple specimen study.SETTING:Department of Orthopaedics,Kunming General Hospital of Chengdu Military Area Command of Chinese PLA,and Institute of Clinical Anatomy, First Military Medical University of Chinese PLA.PARTICIPANTS: Sixteen fresh specimens from Chinese adult wrists were obtained from the Departmentof Anatomy, First MilitaryMedical University of Chinese PLA and the experiment was performed in the Laboratory of Biomechanics. INTERVENTIONS:The partial carpalligaments were stretched till fragmentation of ligaments by using the SWD-10materialtesting machine, at the rated velocity of 5 mm per minute so as to determine the biomechanical properties of carpal ligaments. MAIN OUTCOME MEASURES:The tolerated maximum tension and rigidity during fragmentation of radioscaphoid ligament,radioscaphocapitate (RSC) ligament, radiolunate ligament, ulnolunate (UL) ligament, ulnotriquetral(UT) ligament,ulnar bursa,scapholunate interosseous(SLI) ligament,and lunotriquetral interosseous(LTI) ligament. RESULTS: Among the ligaments attached between distal radioulnar bone and carpal bone, the maximum tension and rigidity of UL ligament were the strongest, (219.2 ±55.4) N and (65.5 ± 19.6) N/mm2, of UT ligament and ulnar bursa were the weakest, (54.0 ±25.5) N, (17.8 ± 6.0) N/mm2and (58.7 ± 17.6) N, (13.4 ±4.7) N/mm2 respectively, of SLI ligament was stronger, (286. 1 ±90.8) N, (95.5 ±40.0) N/mm2, and of LTI ligament were more stronger, (375.3 ± 52.6)N, (179.0 ± 39.0 N/mm2.CONCLUSION: Among carpal ligaments, the maximum tension and rigidity of proximal row of intercarpal ligaments were stronger than those of the ligaments attached between distal radioulnar bone and carpal bone. Of proximal row of intercarpal ligaments,the maximum tension and rigidity of LTI ligament were stronger than those of SLI ligament. Of the ligaments attached between distal radioulnar bone and carpal bone,the maximum tension and rigidity of UL ligament were the strongest.
2.The expression and significance of IL-23/IL-17 axis in esophageal tissues of rats with reflux esophagitis

Objective To detect the expressions of interleukin-17(IL-17) and interleukin-23(IL-23) in esophageal tissues of rats with reflux esophagitis(RE) and explore the role of IL-23/IL-17 axis in the pathogenesis of RE.Methods Thirty SD rats were randomly divided into 3 groups,namely,control group,sham-operated group and model group.RE was induced by adopting partial pyloric ligation plus cardiomyotomy.Then rats in each group were fasted for 24 h but had free access to water.They were fed 24 h after operation.Four weeks later,rats were killed and pathological changes of esophagus mucous membrane were observed by HE staining in each group.The expressions of IL-23p19 and IL-17 mRNA in esophageal tissues were examined by real-time fluorescent quantitative reverse transcriptase polymerase chain reaction(RT-FQ-PCR).IL-17 protein content in esophageal tissues was measured by ELISA.Results After four weeks,compared with control group,model group had significant pathological changes of esophagus mucous membrane.Expressions of IL-23p19 and IL-17 mRNA in the esophageal tissues of model group were significantly higher than those of the other two groups;the content of IL-17 protein was significant higher in model group than in the other two groups.There were no significant differences between control group and sham-operated group.Conclusion IL-23/IL-17 axis is an important cellular factor involved in the pathogenesis of RE and may be involved in the chronic inflammation of RE.
3.Treatment of radial neck fracture in children with elastic stable intramedullary nails.

OBJECTIVETo investigate effects of elastic stable intramedullary nails for the treatment of radial neck fracture in children.
METHODSFrom July 2006 to December 2011, 25 children with radical neck fractures, which included 16 males and 9 females aged from 7 to 15 years old (means 10.7), were treated with elastic stable intramedullary nails. According to Judet classification, 6 cases were type II, 17 cases were type III and 2 cases were type IV (including 1 case with type IVa and 1 case with type IVb). The fracture healing, pain, deformity and range of motion of elbow were recorded.
RESULTSAll patients were followed up for 6 to 24 months with an average of 14 months. Twenty-five patients were obtained bone healing. According to Tibone and Stoltz evaluation standard, 18 cases got excellet results, 4 cases in good and 3 cases in moderate.
CONCLUSIONElastic stable intramedullary nails for the treatment of radial neck fracture in children has advantages of simple operation,less trauma and good results.

4.PHARMACOKINETICS OF MELOXICAM IN HEALTHY CHINESE VOLUNTEERS

AIM To assess the pharmacokinetic profile of single doses of meloxicam in healthy Chinese volunteers. METHODS The plasma concentrations of meloxicam after an oral dose of 15 mg to twenty healthy male volunteers were analized by means of a validated HPLC method. The pharmacokinetic parameters were subjected to Shapiro-Wilk test to determine whether these data were fitted to a normal distribution. RESULTS The twenty volunteers can be classified into extensive metabolizers and poor metabolizers according to pharmacokinetic parameters. The main parameters in the two groups obtained were as follows: T1/2 were 21±4 and 38±9 h, AUC0-∞ were 49±10 and 110±8 μg*h*mL-1, respectively. Even the AUC data in extensive metabolizers were 1.7 times as that reported in White volunteers following the same doses of meloxicam. CONCLUSION There were significant individual differences in the pharmacokinetics of meloxicam in Chinese volunteers, which may be due to the genetic polymorphism of CYP2C9.
5.The neuroendocrine hormone changes and clinical significance in the syndrome of brain injury

Objective To study the incidence and clinical significance of abnormal neuroendocrine hormone in patients with the syndrome of brain injury.Methods 67 cases with the syndrome of brain injury were included in the study group,and 95 cases without the post traumatic syndrome after brain injury were included in the control group.The level of FT3,FT4,TSH,growth hormone(GH),andrenocortico hormone(ACTH),cortisol (Cor),prolactin(PRL),testosterone (T),estradiol (E2),follicular stimulating hormone (FSH),luteotropic hormone (LH),and progesterone (P) in peripheral blood were measured by radioimmunoassay.The incidence of the abnormal neuroendocrine hormone after brain injury was statistically analyzed.Patients with abnormal hormone were given hormone replacement therapy and the curative effects were observed.Results The incidence of neuroendocrine hormone abnormalities was 38.8% in patients with the syndrome of brain injury,while it was 10.5% in the control group.There was significant difference between the 2 groups(P<0.05).The symptom remission rate was 88.5% after hormone treatment.Conclusions There was a high incidence of abnormal neuroendocrine hormone secretion in patients with the syndrome of brain injury.The hormone level may be used as an important index to guide clinical therapy.
6.Effect of HP-β-CD on Solubility of 1,8-Oxido-p-menthane in Volatile Oil of Magnoliae Flos and Study on the Extraction Technology of Xinzhi Nasal Drops

OBJECTIVE:To study the effect of HP-β-CD on solubility of 1,8-oxido-p-menthane in volatile oil of magnoliae flos,and optimize the extraction technology of Xinzhi nasal drops. METHODS:Steam distillation method was adopted to extract the volatile oil in magnoliae flos. Using the content of 1,8-oxido-p-menthane in volatile oil of magnoliae flos as index,HPLC was adopted to determine the effects of HP-β-CD with different concentration on solubility of 1,8-oxido-p-menthane in volatile oil of magnoliae flos. Using the comprehensive scores of imperatorin and extract yield as indexes,orthogonal test was used to optimize the amount of ethanol,volume fraction and extraction time in extraction technology of Xinzhi nasal drops. Verification test was car-ried out. RESULTS:50% HP-β-CD aqueous solution can improve the solubility of 1,8-oxido-p-menthane to 7.6 times. The optimal extraction technology of Xinzhi nasal drops was as follow as 10-fold 80% ethanol,extracting twice,1 h every time. In verification test,the average content of imperatorin was 0.078%(RSD=2.01%,n=3),and extract yield was 10.80%(RSD=1.85%,n=3). CONCLUSIONS:HP-β-CD shows good solubilization effect on the 1,8-oxido-p-menthane in volatile oil of magnoliae flos,and the optimized extration technology for Xinzhi nasal drops is feasible.
7.Experimental study of animal chronic myocardial ischemia model by ultrasonic integrated backscatter and doppler tissue imaging

Objective: To establish porcine chronic ischemic myocardium model and to evaluate ultrasonic integrated backscatter(IBS) and Doppler tissue imaging(DTI) in detecting this model. Methods: An Ameroid constrictor was placed around the porcine proximal left circumflex artery. The calibrated average image intensity (%AⅡ), cyclic variation of IBS (CVIB), the subendo-epicardical gradient index (TGI) and the spectrum of left ventricular papillary muscle level short axis view (LVPM-SAM) and apical four chamber view (AP-4CV) were measured at normal state, 2 w, 4 w, 6 w and 8 w after Ameroid placement. Results: Normal %AⅡ, CVIB and TGI were 2.29?0.32,(9.69?2.22)dB and 0.22? 0.08 respectively. After Ameroid placement, the %AII increased gradually, the CVIB decreased gradually, and the decrease was higher in subepicardium than that in subendocardium. 9.Effects of thrombopoietin and thrombopoietinⅡ on human platelet activation

AIM:To investigate effects of thrombopoietin(TPO) and TPOⅡ on human platelet activation in vitro. METHODS:Human platelets were incubated in the phosphate-buffered saline containing rhTPO or TPOⅡ at the concentration of 100 μg/L for five minutes. In order to determine the rate of platelet activation. The CD62P and CD41 expressions on platelets were analysed by flow cytometry using fluorescence labelled monoclonal antibody to CD62P and CD41. RESULTS:The results demonstrated that expression of CD62P on platelets which were incubated with rhTPO or TPOⅡ didn't increase compared with that of contrast group. CONCLUSION:Both rhTPO and TPOⅡdidn't cause the disorder of platelet activation.
10.Study on biological activity of thrombopoietinⅡ in vivo

AIM: To investigate the biological activity of thrombopoietin Ⅱ(TPOⅡ) in vivo , which consists of two new kinds of ligand binding with thrombopoietin receptor. METHODS: Purified ligandⅠof TPOⅡ, artificial compound ligandⅡ of TPOⅡand rhTPO were injected into purebred Babl/c mice respectively in 7 days by intraperitoneal injection once for a day. Then the biological activity of TPOⅡ was analyzed by measuring peripheral platelet counts by the end of the seventh day. RESULTS: On the seventh day, the platelet counts of mice treated by ligandⅠof TPOⅡ were higher than that in the negative control group( P 0.05). On the fourteenth day, the platelet counts increased in two all experimental groups of TPOⅡcompared with negative control group( P 0.05). Moreover the platelet counts of mice in two experimental groups of TPOⅡ and the positive group showed increase with experimental days. CONCLUSION: The purified ligandⅠof TPOⅡ had obvious activity in increasing platelet production, which is not different from the effect of rhTPO.
