1.Optimization of Extraction Process of Volatile Oil from Wenyang Purgation Granules by Orthogonal Design

Objective To optimize the extraction conditions of volatile oil from Angelica Sinensis and Cassia Bark in Wenyang purgation granules.Methods The volatile oil was extracted from Angelica Sinensis and Cassia Bark by the method of steam distillation light oil device with diethyl ether extraction. The yield of the volatile oil was chosen as the evaluation index. The time of dip in water, the ratio of water to herbal medicine material and the time of distillation were used as the main factors. The optimum extraction conditions were investigated by the L9(34) orthogonal design. Results The optimal conditions for extraction process of volatile oil from Angelica Sinensis and Cassia Bark in Wenyang purgation granules were as follows:the time of dip in water was 3 h;the ratio of water to herbal medicine material was 10∶1;the time of distillation was 6 h. Conclusion The optimized conditions of extraction process are stable and feasible.
2.Preventive effect of bionics-pulsed electromagnetic field on biomechanical property of bone

Objective To study the protective effect of bionics-pulsed electromagnetic field (BEMF) on the biomechanical property of the bones of Wistar rats. Methods Thirty-two 3-month-old female Wistar rats were divided into 4 groups (n=8 in each group). The 8 rats of group A (the control) were treated normally and received injections of saline in the same amount of drugs for the rats of other 3 groups. The rats of group B received injections of desamethasone (1 mg/kg, 2 intramuscular injections/week). The rats of groups C and D received injections of desamethasone in the same procedure as that of group B. In addition, the rats of group C received daily subcutaneous injections of calcitonin in the dosage of 2 IU/kg and those of group D were irradiated with BEMF once daily. The results of the 3 kinds of therapeutic procedures were observed 8 weeks later. Results After the irradiation of BEMF, the biomechanical parameters of the lumbar vertebrae and the femur were far higher in the rats of group D than in those of group B. Conclusion BEMF irradiation is effective to prevent the decrease of biomechanical properties of the bones of the rats.
3.Study on the Quality Standard of Huoxuesan

OBJECTIVE:To establish the quality standard of Huoxuesan. METHODS:Microscopic identification method was adopted for the qualitative identification of Rhizoma pinelliae,Eupolyphaga seu,Phellodendron amurense and Fructus kochiae in the preparation. HPLC was adopted for the contents determination of phellodendrine,polydatin,jatrorrhizine,berberine and emo-din:the column was ZORBAX Eclipse Plus C18 with methanol-0.1% phosphoric acid(gradient elution)at a flow rate of 0.4 ml/min,the detection wavelength was 240 nm,column temperature was 30 ℃,and the injection volume was 5 μl. RESULTS:Micro-scopic identification map of R. pinelliae,E. seu,P. amurense and F. kochiae was clear. The linear range was 0.159-5.073 μg(r=0.997 4)for phellodendrine、0.149-4.761 μg(r=0.999 9)for polydatin、0.239-7.649 μg(r=0.995 5)for jatrorrhizine、0.165-5.268 μg (r=0.997 2)for berberine、0.012-0.382 μg(r=0.999 9)for emodin;RSDs of precision,stability and reproducibility tests were low-er than 3.0%;recoveries were 98.9%-104.1%(RSD=1.9%,n=6),96.1%-101.7%(RSD=2.5%,n=6),99.6%-105.1%(RSD=2.6%,n=6),90.3%-98.2%(RSD=2.9%,n=6)and 92.9%-96.4%(RSD=2.0%,n=6)respectively. CONCLUSIONS:The es-tablished standard can be used for the quality control of Huoxuesan.
4.Research Progress in Clinical Application of Deproteinized Calf Blood

Deproteinized calf blood injection is composed of small protein-free molecules extracted from fresh calf blood. The pre-vious studies showed that it can improve metabolic balance by increasing glucose and oxygen uptake to enhance energy metabolism. In recent years, due to the expanding clinical applications of deproteinized calf blood injection, new observation results and ideas contin-ued to emerge. However, the reports of adverse reactions and excessive applications were also gradually increased. In order to provide reference for the promotion of reasonable clinical application, the paper reviewed the latest development in the studies on the clinical application of deproteinized calf blood injection.
5.Conceptual framework and core data elements of health lifestyle questionnaire

Objective To construct a conceptual framework of a lifestyle questionnaire and define its dimensions,sub-dimensions and core data elements.Methods After review of lifestyle questionnaires of large-scale studies,domestic and international health information standards,health risk factors defined by the World Health Organization and paper-based health records,a conceptual framework was developed by using top-down and bottom-up strategies.Core data elements were chosen according to their importance,utility and experts' advice.Results Our conceptual framework included 9 dimensions (cigarette smoking,alcohol consumption,diet,water drinking,employment,physical exercises,daily living habits,sleep and toxic exposure) and 29 sub-dimensions.For instance,cigarette smoking included status,intensity,duration of smoking and kinds of tobacco.Furthermore,24 core data elements were extracted,and coded values were defined for parts of these data elements.Conclusions Our conceptual framework may serve as a reference for the development of lifestyle questionnaire,and the standardized data elements we extracted could be used in information exchang between institutions.However,standardization of the data elements should be further refined.
7.Influencing factors of puncture site bleeding after trans-radial coronary intervention

Objective To investigate influencing factors of puncture site bleeding after trans-radial coronary intervention (TRI)in order to provide guidance for prevention of post-operative bleeding complications.Methods A total of 198 patients with TRI hospitalized at the department of interventional cardiology of our hospital from August,2011 to December,2011 were recruited in the study.In the prospective study,they were divided into two groups:bleeding group(n=62)and non-bleeding group(n=136).The general status,medication,position of radial compressor,time of immobilization of the wrist joint,duration of loosing tourniquet between the first time and second time and number of laps,time for depression,duration for total release of compression device and laboratory testing were recorded as data.Cox regression analysis was done to explore factors influencing bleeding.Results The factors for puncture site bleeding after trans-radial coronary intervention included pre-operative medications,location of compression device at the midline of operated forearm,distance between the compression device midpoint and the second wrist crease,and time for total release of compression device,with their RR=2.001,3.521,1.470 and 0.999,respectively.Conclusion Factors contributing to increased risk of local bleeding at puncture site following TRI included pre-operative medications,location of compression device at the midline of operated forearm,distance between the compression device midpoint and the second wrist crease;whereas the time for total release of compression device may be a protective factor.
8.Consistency analysis of the chief inspection conclusions in physical examination reports

Objective To analyze the consistency of individual examination diagnosis with terminologies expressed in the conclusion report of physical examinations by the chief inspection physician. Methods Based on the clinical classifications used in the Guidelines on Prevention and Treatment of Blood Lipid Abnormality for Chinese Adults, and related terminology descriptions of Dyslipidemias that were actually used in four-item blood lipid examinations diagnosis, a lexicon database of Hyperlipidemias was constructed with 39 terms used in the four-item blood lipid examination diagnosis and chief physician conclusions. Totally 11953 electronic chief physician inspection reports from 8 health check-up institutions were included. We investigated the terms of lexicon database using word frequency analysis method, calculated the positive rate in the diagnosis of four single examinations of serum lipid and the positive rate in the chief physician's conclusion. Consistency of chief physician's conclusion with single examination diagnosis was analyzed by Kappa test. Results (1) Among the 39 terms of lexicon database, there are 18 nonstandard terms used in single examination diagnosis, accounted for 46% of all terms; (2) In word frequency analysis, there are only 1% of terms that corresponded to clinical classifications of Hyperlipidemias accurately;(3) The positive rate of Hyperlipidemias in serum lipid four single examinations diagnosis was 47%, the positive rate of physician diagnosis was 35%. The consistency analysis of chief physician conclusions with single examination diagnosis showed Kappa=0.71(P<0.01). Conclusions Although the final conclusion written by the chief physician was not using standard terms strictly, most of synonymous terminology expressed in the physician conclusion is normative, which enabled the construction of lexicon database and text mining. Whether the single examination diagnosis are consistent with physician conclusions can be an evaluation indicator to assess the quality of chief physician conclusions for physical examinations. Kappa>0.75 may be suggested as a favorable value.
9.Analysis of the correlation between the pre-S1 antigen, pre-S2 antigen and DNA of hepatitis B virus in the serum of chronic hepatitis B patients undergoing nucleoside analogue therapy..

OBJECTIVETo investigate the dynamic correlation between pre-S1 antigen, pre-S2 antigen and HBV DNA in the serum of chronic hepatitis B (CHB) patients undergoing nucleoside analogue therapy.
METHODS12 CHB patients with transient virological response after lamivudine treatment, and 20 patients treated with adefovir for 5 years were recruited in this study. Serum samples were collected at four time points when HBV DNA fluctuated sharply during lamivudine treatment, and at 0, 8, 12, 28, 52, 104, 156, 208, 260 weeks following adefovir treatment. HBV DNA was quantified by real-time PCR, pre-S1 and pre-S2 antigens were detected by ELISA.
RESULTSThe titers of pre-S1 and pre-S2 antigens were not correlated with the HBV DNA level in the serum of lamivudine treated patients. Only in one case of the adfovir treated patients, the decrease of pre-S1 and pre-S2 antigens was in parallel with the decrease of HBV DNA. Linear regression analysis indicated that neither pre-S1 antigen nor pre-S2 antigen was correlated with HBV DNA in the serum of lamivudine or adfovir treated patients (P more than 0.05).
CONCLUSIONOur results indicate that the titers of pre-S1 and pre-S2 antigens are not correlated with the serum HBV DNA in CHB patients undergoing nucleoside analogue therapy. Neither pre-S1 nor pre-S2 is a good predictor for the outcome of nucleoside analogue treatment.

10.Study on the treatment of experimental autoimmune encephalomyelitis in rats by thymopentin

Objective To investigate the therapeutic effects of thymopentin(TP-5)for the experimentsl autoimmune encephalomyelitis(EAE)in rats.Metbods The EAE model wss established in wistar rats immunized with fresh guinea pig spinal cord homogenate(GPSCH)and complete Freund's adjuvant (CFA).Wistar rats were divided randomly into five groups:normal control group,EAE group,dexamethaSone(DXM)group,low dose TP-5 treated group,hish dose TP-5 treated group.The levels of IL-12 and IL-10 in serum of wistar rats were detected by sandwich-ELISA on day 7,14 and 21 post immunization.Resuits Morbidity and clinical score of low dose TP-5 treated group and DXM group were significantly lower than those of EAE group and hish dose TP-5 treated group(P<0.01).Morbidity and clinical score of DXM group were significantly lower than those of low dose of TP-5 treated group(P<0.01).The levels of IL-12 of EAE group,DXM group,low dose TP-5 treated group and high dose TP-5 treated group were significantly higher than of normal control group at each time point(P<0.01).The levels of IL-12 of DXM group and low dose TP-5 treated group were significantly lower than that of EAE group(P<0.01),meanwhile the levels of IL-10 of DXM group and low dose TP-5 treated group were significantly higher than that of the rest groups on day 14 and 21 post immunization(P<0.01).Conclusion TP-5 has protective effects on EAE,and its functional mechanism may be associated with down.regulation of IL-12 as well as up-regulation of IL-10,so as to reverse the imbalance of TH1/TH2 responses by bidirectional regulation.
