Objective To explore the efficacy of gastrofiberscope-assisted balloon catheter dilatation of esophageal stricture. Methods A total of 30 children with esophageal stricture underwent gastrofiberscope-assisted balloon dilatation between October 1996 and November 2005 in this hospital. The procedure was conducted using the type JIF-SP gastrofiberscope(Olympus Company,Japan) and a balloon catheter with pressure monitor(Boston Company,USA).The stricture was 0.2~0.6 cm in diameter and 0.6~2.5 cm in length(mean,1.7 cm).Under the direct vision of gastrofiberscopy, the balloon catheter was perorally inserted to the stricture. Then the balloon was insufflated with air while monitoring the pressure. The balloon dilatation lasted for 5 min. Results A total of 113 times of balloon dilation were performed in 29 children, while a conversion to traditional surgery was required in 1 child with serious esophageal stricture due to difficulty to dilatation. In the 29 children, the dilatation was conducted for 2~3 times every month until the strictured esophagus reached 0.6~0.7 cm in diameter after 4~5 times of balloon dilatation.Follow-up surveys for 3 months ~ 5 years in the 29 children showed no dysphagia or recurrence of stricture. Conclusions Gastrofiberscope-assisted balloon catheter dilatation of esophageal stricture is a feasible and safe technique.

Objective To compare curative effects between laparoscopic and open surgery in the treatment of pediatric indirect inguinal hernia. Methods A group of 410 children with indirect inguinal hernia underwent micro-laparoscopic operation(Laparoscopic Group) from April 2001 to January 2006.This group was compared with another group of 408 children with indirect inguinal hernia, treated with open high ligation of the hernial sac(Open Group) from September 2004 to January 2006.In the Laparoscopic Group,a 5-mm trocar was inserted at the umbilicus for the exploration of bilateral internal rings.Another 5-mm trocar was inserted at the site slightly superior to the McBurney's point for introducing a needle holder. At the level of the neck of the hernial sac,the orifice of the vaginal process was circumferentially sutured and ligated. In the Open Group, high ligation of the hernial sac was conducted through a transversal incision at the external ring. Results The operations in the Laparoscopic Group were smoothly completed. The operative time for one side of hernia was significantly shorter in the Laparoscopic Group(33.4?9.6 min) than in the Open Group(38.0?7.6 min)(t=-7.596,P=0.000).In the Laparoscopic Group,no edema of the scrotum, subcutaneous hydrops,or infection occurred. The patients were followed for 1~55 months(mean,23.2 months).Short-term recurrence was observed in 3 children, who required a re-operation of laparoscopic repair. No testicular atrophy, iatrogenic retained testis, intestinal adhesion,or other long-term complications happened. In the Open Group,follow-up observations for 1~18 months(mean,10 months) revealed recurrence in 18 children,8 of whom were then cured with laparoscopic surgery and 10 of whom with open surgery.Iatrogenic retained testis happened in 9 children and a re-operation of open surgery was carried out. Conclusions Two-port laparoscopic high ligation of hernial sac for pediatric indirect inguinal hernia is minimally invasive, safe, and effective, but not suitable for children with cardiac insufficiency or newborn incarcerated hernia.

Objective To study the anti-fatigue function of black currant extracts in mice. Method Kunming mice (SPF) were randomly divided into black currant extracts high, middle, low dose group, black currant natural juice dose group and control group according to their body weights. Mice were administered ig daily for 30 d. The loaded-swimming time ,hepatic glycogen ,blood urea nitrogen were measured. Results Black currant extracts high, middle, low dose groups could increase the time of loading swimming, and the hepatic glycogen content, and decrease blood urea nitrogen in mice. All indices above were more than those in black currant natural juice except the blood urea nitrogen content was lower. Conclusion Black currant extracts could alleviate physical fatigue in mice, and be better than black currant natural juice.

Objective To explore the effects of piperine on cell proliferation and migration in angiotensin Ⅱ (Ang Ⅱ)-treated rat airway smooth muscle cells (ASMCs).Methods The primary ASMCs of rats were cultured by improved tissue-piece digestion inoculation and trypsin digestion.MTT assay was used to detect the effects of Ang Ⅱ and Ang Ⅱ receptor antagonist losartan on cell proliferation activity. After treatment with Ang Ⅱ and piperine, the cell proliferation activity, the cell cycle distribution and the cell migration were detected by MTT, flow cytometry and Transwell assay respectively.ERK1/2 inhibitor PD98059 and losartan were then applied to determine the expression of cyclin D1, MMP-9, p-ERK1/2, ERK1/2, and β-actin proteins by Western blot assay. Results After 24 h culture, Ang Ⅱ treatment promoted the cell proliferative activity in rat ASMCs (P<0.05), and the promotive effect of 10-7 mol/L Ang Ⅱ was the most significant.Additionally, losartan blocked the Ang Ⅱ-induced cell proliferative activity in rat ASMCs (P<0.05).10-7 mol/L Ang Ⅱ treatment resulted in the elevated cell proliferative activity, higher S phase fraction, increased migrated cell number, and enhanced expression of cyclin D1, MMP-9and p-ERK1/2 proteins (P<0.05);these effects were dose-dependently reversed by piperine. Both PD98059 and losartan blocked Ang Ⅱ-induced expression of p-ERK1/2, cyclin D1 and MMP-9 proteins in rat ASMCs.Conclusions Piperine may inhibit Ang Ⅱ-induced cell proliferation and cell migration via ERK1/2 signaling pathway in rat ASMCs.

Aim To investigate the effect of phenyle-thanol glycosides from Cistanche tubulosa(CPhGs) on the proliferation and activation of rrPDGF-BB induced HSC and their target points for resisting hepatic fibro-sis,to elucidate the molecular mechanism in molecular level, and provide basic data for the further develop-ment of new drugs. Methods HSCs were cultivated by CPhGs with different concentrations ( 0 , 3. 91 , 7. 81 , 15. 63 , 31. 25 , 62. 50 , 125. 00 , 250. 00 , and 500 mg ·L-1 ) and IC50 of CPhGs was determined. CPhGs with different concentrations ( 25 , 50 , 75 , 100 mg · L-1 ) were selected, and after the cells were stimulated with rrPDGF-BB, cell proliferation was determined by MTT. ERK1/2 ,α-SMA, c-fos, c-jun and Collagen I mRNA and Erk1/2 ,P-Erk1/2 and CollagenⅠprotein ex-pressions were assayed by RT-PCR and Western blot. Results CPhGs of ( 50 ~100 ) mg · L-1 concentra-tions groups could effectively inhibit rrPDGF-BB-medi-ated proliferation(P<0. 05) and CPhGs of(25~100) mg·L-1 concentrations groups had no significant cyto-toxicity( P >0. 05 ) . CPhGs of ( 25 ~100 ) mg · L-1 concentrations groups could inhibit ERK1/2 ,α-SMA,c-fos, c-jun and CollagenⅠmRNA levels, and also ob-viously inhibited Erk1/2 ,P-Erk1/2 and Collagen Ⅰ pro-tein expression on HSC. Conclusions CPhGs has the protective effect against hepatic fibrosis. The mecha-nism of this process may involve the interference with PDGF/ERK1/2 signaling pathway and inhibiting the activation and proliferation of HSC.

BACKGROUND:Studies have shown that fibrin glue can promote the survival of myoblast grafts, reduce infarct size and induce neovascularization of infarct zone. OBJECTIVE:To understand the condition of revascularization of infarcted heart muscle using endothelial progenitor cells combined with degradable fibrin glue materials. METHODS:A total of 27 Sprague-Dawley rats were randomized into three groups, 9 rats in each group:non-myocardial infarction group, immediate transplantation group and 1-week post-infarction transplantation group. Then, these three groups were sub-grouped into two groups, respectively:endothelial progenitor cells+fibrin glue group (experimental group) and fibrin glue group (control group). At 3 and 8 weeks after transplantation, the rats were sacrificed in each group. The revascularization and function of infracted heart muscle were observed by microscope, immunohistochemistry and echocardiography. RESULTS AND CONCLUSION:Under the microscope, there were some lax connective tissues between the heart and chest in the experimental groups, but no difference existed between the experimental and control groups. The heart structure was normal relatively and difficult to be distinguished between the experimental and control groups histological y and immunological y, and there was no angeioma, vascular malformation and tumor. The number of revascularization of heart muscle showed no difference between experimental and control groups as wel as between different experimental groups. Additional y, there was no significant difference in cardiac function between experimental and control groups. Although there are no positive results of endothelial progenitor cells, we wil modify and improve the strategy and believe that the celldelivery system is of benefit and efficacy.
