Objective To evaluate the diagnosis, treatment and prognosis of occult breast cancer(OBC). Methods Clinical data of 12 OBC cases from June 1995 to June 2006 were retrospectively analyzed.Results There were 12 female; the mean age was 52.7 years(39～66). Tumor of all cases was in one of breasts, left(10 cases) or right(2 cases), 4 cases in pro- menostasis, 8 cases in post-menopause, and inci-dence of OBC in all kinds of breast cancer was 0.5% (12/2385). All cases were found because of lump in axillary fossa, the biopsy of 10 showed cases were lymph node adenocarcinoma metastatic, the fine needle aspiration of 2 cases were lymph node adenocarcinoma metastatic. Three cases were positive in the determi-nation of estrogen receptor of lymph node adenocareinoma metastatic. Two cases, suspecting breast cancer,could be found fine calcification in the examination of breast X-ray molybdenum target in 10 cases. Buhra-sound in 11 cases and near infrared ray scan in 10 cases did not detect cancer in breast; chest X-ray and Bultrasound in 10 cases and CT and all over the body bone scan in 5 cases did not found any cancer besides breast. Ten cases used modified radical mastectomy for breast cancer, one case radical mastectomy for breast cancer, one palliative operation. Pathological diagnosis of 9 cases after operation found breast primary le-sion, 6 cases were infiltrated duct cancer, 3 eases were intraductal cancer. All cases received radiotherapy and chemotherapy. Three cases which were positive in the determination of estrogen receptor of lymph node adenocarcinoma metastatic received TAM. Followed-up was made from 3 to 10 years, 11 cases were not found recurrence or metastses, and 5 cases had been living over 5 years. Conclusions OBC is one of rare special breast cancers. The powerful infiltrating capability leads to lump in axillary fessa in prophase and dis-rant metastses. The biopsy and fine needle aspiration would be used for diagnosis. Modified radical mastecto-my for breast cancer is the usual therapy method.

Objective To translate the English version of an abbreviated eight-item measure of Penn State Worry Questionnaire (PSWQ-A) into Chinese and to test the reliability and validity of the Chinese version of PSWQ-A in patients after total Hip Replacement.Methods Totally 218 patients after total hip replacement were recruited and were investigated by the Chinese version of PSWQ-A.Results The internal consistency coefficient of the Chinese version of PSWQ-A were 0.885.Factor analysis got two factors,which explain 68.428％ of the total variance.The content validity index was 0.924.Using SAS as an external criterion,The correlation coefficients of the PSWQ-A and SAS scores were 0.891,P＜0.05.Conclusions The Chinese version of PSWQ-A has been proved to be reliable and valid.It can be used as a valid tool for the measurement of self-efficacy in patients after total hip replacement.

Twenty six uncontrolled patients with Graves′ disease were treated with thyroid artery embolism. Twenty two cases were followed for 24 60 months. The long term effect was observed after thyroid artery embolism. The results showed that the long term cure rate is 81.4% (18/22).

Objective: To study the differences of the superficial masseter muscle morphology characteristics between adult high-angle and low-angle facial skeletal types. Methods: The subjects consisted of 37 yellow race adults,17 of them with high-angle facial skeletal type and 20 with low-angle. Ultrasound scanning was used to measure the perimeter, area, width, thickness, and length of masseter muscle. All the items were measured under relaxed, maximal clenching and maximal protrusive mandibular position.Results:①In the various mandibular positions, the masseter muscle size of adults with high-angle facial skeletal type was smaller than that of those with low-angle.②Except for the changes of width from the relaxed to maximal clenching position and from the relaxed to maximal protrusive position(P0.05).③Stature and weight influenced the size of the masseter muscle.Conclusion:The masseter muscle size of adult with high-angle facial skeletal type is smaller than that of those with low-angle.

Objective To explore the value of the square for intracranial puncture orientation applying in the intracranial hematoma microinvasive craniopuncture scavenging technique. Methods Fifty-one patients with hypertensive cerebral hemorrhage were randomly enrolled into the orientation square group( n=27)and the CT orientation routine group (n=24). The puncture point was fixed by certain wayin each group. Both groups received the same intracranial hematoma microinvasive craniopuncture scavenging technique. The punctural precision,the time spended for puncture orientation and the curative effect in two group were observed and compared. To discover novel fluoroquinolone lead compounds as possible anti-infective or/and antitumor chemotherapies, combination principle of pharmacophore-based drug design, a series of novel tricyclic fluoroquinolone title compounds, [1,2,4]triazino[3,4-h][1,8]naphthyridine-8-one-7-carboxylic acid derivatives ( 5a-5p), were designed and synthesized with a fused [1,2,4]-triazine ring unit. Their structures were characterized by spectral data and elemental analysis and the in vitro antibacterial and anti-cell proliferation activities were also evaluated. The results showed that the titled compounds exhibited more significant inhibitory activities against drug-resistant bacteria (Methicillin-resistant Staphylococcus aureus and multi drug-resistant Escherichia coli strains) and three tested cancer cell lines (human hepatoma SMMC-7721, murine leukemia L1210 and human murine leukemia HL60 cells). Interestingly, SAR showed that compounds with electron-donating groups attached to benzene ring had stronger antibacterial activity than antitumor activity, but electron-withdrawing compounds displayed more potential antitumor activity than antibacterial activity, especially antitumor activity of nitro compounds was comparable to comparison doxorubicin. Thus, novel triazine-fused tricyclic fluoroquinolones as potent anti-infective or/and antitumor lead compounds are valuable to pay attention and for further development.

Objective To evaluate the efficacy of acting κ opioid receptor for prevention of high altitude pulmonary edema (HAPE) in rats.Methods Forty male Sprague-Dawley rats,aged 8 weeks,weighing 250-300 g,were randomly divided into 5 groups (n =8 each) using a random number table:control group (group C),hypobaric hypoxia group (group H),normal saline + hypobaric hypoxia group (group NH),U50488H (a selective kappa-opioid receptor agonist) + hypobaric hypoxia group (group UH),and nor-binaltorphimine (norBNI,a selective kappa-opioid receptor antagonist) + U50488H + hypobaric hypoxia group (group NUH).The rats were put into the hyperbaric chamber and exposed to hypobaric hypoxia (atmospheric pressure 355 mmHg,partial pressure of oxygen 74 mmHg) for 2 days to induce HAPE.At 3 days before HAPE,normal saline 0.5 ml,U50488H 1.25 mg/kg,and nor-BNI 2.0 mg/kg were injected intraperitoneally once a day in NH,UH,and NUH groups,respectively,and in addition U50488H 1.25 mg/kg was injected intraperitoneally 10 min later in NUH group.After 2 h exposure to hypobaric hypoxia,mean pulmonary artery pressure (mPAP) was detected,and arterial blood samples were collected for determination of serum malondialdehyde (MDA) and erythropoietin (EPO) levels.The rats were then sacrificed and lungs were removed for microscopic examination and for determination of the levels of nitric oxide (NO),inducible nitric oxide synthase (iNOS),MDA,superoxide dismutase (SOD),endothelin-1 (ET-1),thromboxane B2 (TXB2),and 6-keto-prostaglandin F1α (6-keto-PGF1α) in lung tissues.Lung water content and TXB2/6-keto-PGF1α ratio was calculated.Results Compared with group C,mPAP,lung water content,ET-1,MDA,TXB2 and 6-keto-PGF1α levels,TXB2/6-ketoPGF1α ratio,and serum MDA and EPO levels were significantly increased,and iNOS,NO and SOD levels were decreased in the other four groups (P ＜ 0.05).Compared with group H,mPAP,lung water content,ET-1,MDA,TXB2 and 6-keto-PGF1α levels,TXB2/6-ketoPGF1α ratio and serum MDA and EPO levels were significantly decreased,and iNOS,NO and SOD levels were increased in UH group (P ＜ 0.05),and no significant changes were found in the indexes mentioned above in NH and NUH groups (P ＞ 0.05).The pathological changes of lung tissues were significantly attenuated in group UH as compared with H group.Conclusion Acting κ opioid receptor can produce prevention for HAPE in rats,and inhibition of lipid peroxidation and correction of the imbalance between vasoconstrictive factors and vasodilative factors may be involved in the mechanism.

AimTo establish a model of the injury of the vascular endothelial cell(VEC) in rats and observe the function of Chinese herbs of Li Qi Huo Xue(LQHX) will prevent and treat the VEC injury. Methods30 SD rats weredivided into the control, the model and LQHX groups. The model of the VEC injury was established. It tried to demonstrate the effect of LQHX for the coagula tion and fibrinolysis function of the VEC by the C EC count,t-PA, PAI activity, 6-keto-PGF1αcontent and PAgTmax. Results In LQHX group as compared with those of the medol group, The CEC count was redued obviously(P＜0.01). t-PA activity was increased(P＜0.01), so did the percentage of active t-PA(P＜0.01), but PAI activity decreased (P＜0.05), 6-keto-PGF1α content increased (P＜0.01)α the PAgTmax decreasd (P ＜0.01) Conclution LQHX can enhance the anticoagulation and fibrinolysis activities. It is a more effective measure for the VEC protection.

Objective To explore the effect of the combination of video feedback and telephone intervention on postdischarge rehabilitation in patients receiving artificial total hip replacement.Methods A total of 60 patients receiving artificial total hip replacement were divided into intervention group and control group with 30 cases each by random digits table method.The combination of video feedback and telephone intervention was conducted in intervention group,and conventional discharge guidance and telephone follow-up on a regular basis in control group.In intervention group,the patients or their family member were asked to record the video of the exercises and daily living when patients were at home according instructions and researchers assessed the condition of patient's joint and muscle function and provided individual education and instructions to the patients on the basis of the feedback video through telephone.Harris hip function rating scale and Functional Independence Measure (FIM) were investigated at discharge and 1,3,6 months after discharge.Results There were no significant differences in Harris hip function rating scale and FIM score at discharge between 2 groups,P＞0.05.Harris hip function rating scale score were (72.70± 11.65),(83.11±5.12),(94.83±3.08) scores at 1,3,6 months after discharge in intervention group,and (63.20±14.96),(77.00±11.62),(87.50±7.94) scores in control group,there were significant differences,P＜0.05 or ＜0.01.FIM score were (96.30±4.63),(108.76±3.28),(125.79±1.19) scores at 1,3,6 months after discharge in intervention group,and (78.23±5.09),(86.43±6.30),(96.54±5.35) scores in control group,there were significant differences,P＜0.01.Conclusion The combination of video feedback and telephone intervention could help to promote the postdischarge hip function for patients receiving artificial total hip replacement.

Objective To explore the effect of the JAK/STAT signal pathway on the apoptosis-related gene in the synovial tissue of rat rheumatoid arthritis (RA).Methods Fifty rat models of collagen-induced arthritis,whose arthritis index was more than 2,were divided into the model group,the low dose of AG490 group,the medium dose of AG490 group and the high dose of AG490 group.In addition,6 rats were treated intraperitoneal injection.Then,the arthritis index and the change of apoptosis-related genes were compared.Multiple-sample average was analyzed by single-factor x2 test and LSD-t or Tamhane's T 2 test were used for two-two comparison.Results The arthritis index of the model group increased evidently,and the apoptosis inhibitor Bcl-2,Bcl-xl gene and protein expression was up-regulated,which was significantly different when compared with that of the control group(0.931±0.035 vs 0.351±0.024,0.920±0.037 vs 0.271±0.029,0.322±0.047 vs 0.230±0.031 ).The expression of apoptosis promoting factor Bax wasslightly up-regulated.The blockage of JAK/STAT pathway cotld down-regulate the expression levels of the gene and protein of survivins Bcl-2 and Bcl-xl,and up-regulate the gene and protein expressions of Bax.Conclusion In the process of RA development,apoptosis inhibitor Bcl-2,Bcl-xl gene and protein expression is up-regulated.JAK/STAT signal transduction pathway regulates the apoptosis process.
