3.Analysis of clinical treatment of pancreatic benigntumors: a report of 23 cases

Objective To discuss the diagnosis and treatment of the benign tumors of pancreas,especially the surgical options and the prevention and cure of the postoperative complications.Methods Clinical data of 23 patients with benign lesions of the pancreas were retrospectively analyzed in our hospital from Jan.2004 to Dec.2009.All of the patients underwent surgical operation.Five patients were treated with pancreatic enucleation,8 patients were treated with distal pancreatectomy with splenectomy,1 patient was treated with central pancreatectomy,5 patients were treated with pancreaticoduodenectomy,4 patients were treated with pancreatic cyst- jejunum R-Y anastomosis.Eighteen patients were treated with somatostatin analogue.Results The pathological diagnosis included pancreatic cyst in 8,islet cell tumor in 4,mucous cystadenomas in 6,serous cystadenomas in 3,solid-pseudopapillary tumor in 2.Seven of 10 cases having pancreatic leak occoured secondary abdominal infection.One patient died of multiple organ dysfunction syndrome after pancreaticoduodenectomy.Conclusion There is no specific symptoms and serologic laboratory examinations for pancreatic benign tumors.CT and ERCP is very helpful for diagnosis and treatment.Surgical option is according to the location of the lesion and the judgement of the tumor whether being benign or malignant.Pancreaticoduodenectomy,central pancreatectomy,distal pancreatectomy with or without splenectomy and pancreatic enucleation is the reasonable surgical option.Pancreatic leakage is the main post-operation complication.Ligation the pancreatic duct reliably,treating the pancreatic raw surface appropriately,external drainage of the pancreatic fluid can reduce the rate of pancreatic leakage.

Objective To investigate the effect on apoptosis and extracellular signal regulated kinase (ERK) expression of cholangiocarcinoma RBE cell line induced by paclitaxel and the role of extracellular signal regulated kinase in paclitaxel-induced apoptosis. Objectives:To observe the effect of different formulas of enteral nutrition on the variety of blood glucose and blood lipid in critically ill patients with hyperglycemia. Methods:Ensure, Nutrison and Glucerna were randomly used with equal daily calorie in 45 patients receiving enteral nutrition. Fasting blood glucose, blood routine, serum potassium, serum sodium, serum chlorine, liver and kidney function, prealbumin, tranferrin, urea and creatinine in 24hr urine were monitored in all cases. Objective To determine quercetin content in Polygonum chinense L.from different growing areas.Methods RP-HPLC was used to determine quercetin content.Chromatographic conditions were as follows:chromatographic column of Agilent TC-C18 (4.6 mm?250 mm,5 ?m),mobile phase consisting of methanol-0.6 %phosphoric acid (52:48),with a velocity of 1 mL?min-1,column temperature at (25?1)℃,detection wavelength being 365 nm,elution time of 20min,and an injection volume of 20 ?L.Results Standard curve regression equation of quercetin is:Y=85 355X-48.16(r=0.999 3).A good linearity was showed in the range of 0.004~0.012 mg?mL-1,with average recovery being 100.3 %,and RSD of 1.44 %.The content of quercetin in the samples from different growing areas is in the range of 183.0~848.1 ?g?g-1.Conclusions The method is simple and reliable,and can be used to control the quality of Polygonum chinense L.The existence of some differences of quercetin content between medicinal materials from different growing areas indicates that we should make a quality standard to ensure the safety and effectiveness of Polygonum chinense L.in clinic.
10.Relative Analysis between Drug Sensitivity and Escherichia Coli Induced Enzymes in Childhood Diarrhea

Objective To investigate the distribution of pathogenic bacteria,drug sensitivity,and the relationship between drug sensitivity and Escherichia coli(E.coli) induced enzymes in childhood diarrhea in the last 2 years in Chongqing area,so as to provide important evidence for pediatric clinical therapy.Methods Thirty-one E.coli induced enzymes,extended spectrum ?-laetamases(ESBLs),cephalosporinase(AmpC)detected in different phenotype methods,and drug sensitivity was measured in paper strip method,and the specimens were collected from children′s hospital affiliated to chongqing university of medical sciences from Jan.2005 to Dec.2006 were determined.Among the total,there were 18 enteropathogeic E.coli(EPEC) strains,8 enterotoxigenic E.coli(ETEC) strains and 5 enteroinvasive E.coli(EIEC) strains.In addition,drug resistance tests by paper strip included chloramphenicol(CHL),amikacin(AMK),gentamicin(GEN),norfloxacin(NOF),ciproflocacin(CIP),cefazolin(CEZ),cefoperazone(CPZ),ceftriaxone(CRO),ceftazidime(CAZ),cefotacime(CTX),cefepime(FEP),imipenem(IPM).SPSS 12.0 software was used to analyze the data.Results Three point two percent of the 31 E.coli were drug resistant to IPM,and 35.5%,38.7% to NOF,CIP individually,but more than 60% to AMK,GEN,even more than 67.7% towards cephalosporin(except ceftazidime and cefepime);the gross enzyme-produced rate was 87.1%,rate of single ESBLs,AmpC,and induction of both enzymes simultaneously presented 64.5%,6.5%,16.1% respectively;and there was marked difference in drug resistance when bacteria that produced single AmpC versus bacteria that produced single ESBLs or that produced both ESBLs and AmpC(Pa﹤0.05).Conclusions The relationships among enzyme′s quantity,sort and bacterial resistance are different.These data show E.coli infected by bacterial diarrhea children in Chongqing due to a high rate of induced enzymes,and their drug resistance vary according to the state of induced enzymes.
