1.Comparison of the different methods to screen metabolic syndrome in the type 2 diabetes population

Objective To evaluate the uniformity of different diagnosis criteria of metabolic syn-drome in patients with type 2 diabetes, especially the criteria that suit for metabolic syndrome in type 2 dia-betes patients.Methods The patients with type 2 diabetes, aged from 20~74 years, who were treated at Department of Endocrinology , the Third Affiliated Hospital , Qiqihar Medical University from January 2008 to November 2009 were enrolled.Four hundred and twenty one patients with integral data were analyzed . Waistline, hip line, height, body mass, blood pressure and so on were surveyed , and according to waistline and hip line the waist-to-hip ratio was calculated .On the basis of body mase/height body mass index ( kg/m2 ) was calculated .The blood sugar was detected with the hexokinase method .The blood cholesterol , tria-cylglycerol , the high density lipoprotein cholesterol were determined with the biochemistry method .Urinates the micro albumin level was determined with chemistry to try the belt technology .Diagnosis standards of WHO (1999), NCEP-ATPⅢ(2001),ACCE(2003),EGIR(1999),the Chinese CDS(2007) suggestion metabolism synthesis drafts , the triacylglycerol-waistline standard of Blackburn P forecast cardiovascular e-vent(2003) were applied to calculate prevalence rate of metabolism synthesis .Results The 421 enrolled testees were involved in the result analysis .⑴Prevalence rates of type 2diabetes and metabolism synthesis in various single component: In the investigation population the differences in body of obesity , hypertension and hypertriacylglycerolemia , the low and high density lipoprotein cholesterol blood sickness between the men and women were insignificant , but the overload (66.1%) in males was remarkably higher than that in females (49.7%).The prevalence rate in abdomen obese in males (74.0%) was remarkably higher than that in females(59.9%).⑵Abnormal distribution of metabolism in patients with type 2 diabetes: Preva-lence rate of one kind of abnormal metabolism was 27.2%.Prevalence rate of two kinds of or above two kinds of above abnormal metabolism was 61.0%.Prevalence rate of three kinds of above abnormal metabo-lism was 26.1%,and the prevalence rate in females was not higher than that of males ( P >0.05 ) .⑶After age and sex lamination ,prevalence rates of metabolism synthesis and it's the correlated disease:The preva-lence rates of the high hypertriacylglycerolemia , hypertension , metabolism synthesis had the tendency of in-creasing along with the age .Prevalence rates of abdominal obesity and the low and high density lipoprotein cholesterol blood sickness had the increased tendency in females , but it was not obvious in males .Except that 35~44 years old of age group prevalence rate of the high hypertriacylglycerolemia in males ( 62.1%) was remarkably higher than that in females ( 29.2%) ( P <0.005 ) , and the 20~35 years old age group prevalence rates of metabolism synthesis in males (41.2%)were remarkably higher than outside the females (20.0%)( P <0.05).In other various age groups ,there was insignificant difference in prevalence rate between males and females .⑷The prevalence rate of WHO diagnosis standard for metabolism synthesis (62.9%) had good concordance with the prevalence rate of the Chinese CDS diagnosis standard for metabo -lism-synthesis ( 66.7%) .The prevalence rate of ACCE diagnosis standard for metabolism synthesis (88.8%) had good concordance with the prevalence rate of the EGIR diagnosis standard for metabolism synthesis (86.6%).The prevalence rate of Blackburn P forecast cardiovascular event (20.9%),which was distanced from the result of the diagnosis standards ( P <0.005 ) .Conclusion ⑴Patients with type 2 di-abetes have high metabolism syndrome proportion rate , and the abnormal metabolism proportion rate in sin-gle group is also higher.⑵WHO (1999) metabolism syndrome diagnosis standard and CDS (2007)sugges-tion standard are suitable for Chinese metabolism -synthesis in type 2 diabetic patients .⑶The metabolism syndrome diagnostic standards of EGIR and ACCE are similar in proportion rate in Chinese patients with type 2 diabetes , which is accorded with the findings of former literatures .Whether that can be used widely deserves further study .⑷The diagnostic criteria of triacylglycerol waistline is not suitable for Chinese type 2diabetic patients.
2.A clinical study of 35 cases of Silver-Russell syndrome

Objective To analyze the clinical features of 35 patients with Silver-Russell syndrome (SRS) in Beijing Children's Hospital in recent 6 years,and to recognize the clinical manifestations and improve the diagnosis of the disease.Methods The diagnostic criteria of SRS were studied retrospectively,the differences among 3 different diagnostic criteria were compared.Results Thirty-five SRS patients were diagnosed by means of 3 criteria,over a period from 2006 to 2012.There were 23 males and 12 females,with age range 0.08 ~ 12.15 yr.77 % of cases met the three criteria,97% met two of the three criteria.There were dwarfism in 33 cases (94.2%),genital abnormalities in 1 case (2.9%),and asymmetry of limbs in 1 case (2.9%).The clinical characteristics with the frequencies accounting for over 80% included short stature (average height-3.61 SDS),leanness (body mass index <-2.11SDS),bone dysmorphism including triangular face,irregular teeth,asymmetry of limbs,clinodactyly of fifth finger,and bone age obviously lagged behind.60% of the cases showed bone age lagging behind more than 1 year.Height lagging behind was more obviously then bone age.Conclusions The diagnosis of SRS is mainly based on Clinical manifestation,laboratory and biochemical examinations are of no avail.There is a high coincidence with the 3 different diagnostic criteria.Clinical features in more than 80% patients with SRS are short stature,craniofacial triangular dysmorphism,irregular teeth,limbs asymmetry and fifth finger clinodactyly,and mild lagging behind of bone age.
3.Effect of dexmedetomidine combined with different-doses of sufentanil on postoperative analgesia in pa-tients undergoing spine surgery

Objective To observe the analgesic effects of dexmedetomidine combined with dif-ferent-doses of sufentanil in patients undergoing spine surgery.Methods Sixty patients(ASA grade Ⅰor Ⅱ degree,age 18-70 years)undergoing spine surgery were randomly assigned into three groups ac-cording to PCA formulation(n =20):3 μg/kg sufentanil group (group S1),1.5 μg/kg dexmedetomi-dine+ 2 μg/kg sufentanil group (group S2 )and 1.5 μg/kg dexmedetomidine + 1 μg/kg sufentanil group (group S3).The same anesthesia method was applied among three groups.Patient-controlled intravenous analgesia pump was applied before 30 minutes prior to the end of surgery.The drugs in each group were diluted to 1 50 ml and infused by a pump at a rate of 3 ml/h with a patient-controlled analgesia (PCA)bolus of 0.5 ml and lock time of 30 minutes.VAS and Ramsay scores at 2 h(T0 ),4 h (T1 ),8 h(T2 ),12 h(T3 ),24 h(T4 )and 48 h(T5 )after surgery were estimated.Postoperative nausea and vomiting,bradyrhythmia and hypersomnia were also recorded.Results Compared with group S1, VAS of groups S2 and S3 was significantly decreased at T1-T5 (P <0.05).There were also no signifi-cant difference in the incidence of postoperative nausea and vomiting,bradyrhythmia and hypersomnia among three groups.Conclusion Dexmedetomidine of 1.5 μg/kg can significantly reduce the dosage of sufentanil on postoperative analgesia in patients undergoing spine surgery,and decrease the rate of postoperative nausea and vomiting without any bradyrhythmia and hypersomnia.
4.Diagnostic value of vascularity index and renal volume with three-dimensional ultrasound in chronic kidney disease

Objective To explore the diagnostic value of three-dimensional ultrasound parameters-vascularity index (VI) and the volume of kidney in chronic kidney disease (CKD). Methods A total of 80 CKD patients at different stages and 30 healthy adults were enrolled in the study. VI and the renal volume were calculated with three-dimensional ultrasound, while resistance index (RI), cortical thickness (CT) and the volume of kidney were calculated with two-dimensional ultrasound, and the correlation of those parameters were analyzed statistically. Results VI of CKD2 patients reduced significantly compared with the control group, and RI of CKD3 patients increased significantly, CT of CKD3 patients reduced significantly. The diagnostic sensitivity, specificity and diagnostic value of VI in patients with early and late CKD were all higher than thpse of RI. Compared with the control group, three-dimensional ultrasound of renal volume and two-dimensional ultrasound volume of CKD 1 patients increased significantly, three-dimensional ultrasound volume of CKD2 patients reduced significantly, and two-dimensional ultrasound volume of CKD4 patients reduced significantly. There was significant negative correlation among VI, three-dimensional ultrasound volume, CT, two-dimensional ultrasound volume and pathological change scoring (r=-0.67, -0.52, -0.43, -0.52). Conclusion VI and three-dimensional ultrasound volume can detect renal function damage more earlier and sensitively than RI and two-dimensional ultrasound volume.
5.Rat cranial sutures in vitro: a new experimental model for studying the osteogenic effect to tensile force

Objective To develop an in vitro model for cranial suture of neonatal SD rat. Methods The parietal hones with the sagittal suture were removed from SD rats (19-days old) for organ culture. In the experimental group, tensile force 3.92×10~(-3) N (0.4 g) was applied by helical springs, whereas no tension (0 N) was set in control group. These explants were observed under inverted microscope. At the end of the incubation period for 24 hours, general conditions were observed under inverted microscope and histological conditions were observed after hematoxylin and eosin stain. Results Under inverted micro-scope, sutures had no obvious changes in control group, whereas sutures were enlarged gradually in the experimental group. With histological observation, sutures developed normally in control group, but in experimental group, osteohlasts and capillary vessels proliferated actively in the suture. Conclusions In vitro model of cranial suture can be cultured and grown successively.
6.Predictive value of procalcitonin for outcome of ventilator weaning in patients with acute exacerbation of chronic obstructive pulmonary disease

Objective To investigate the possibility that procalcitonin (PCT) can predict the outcome of ventilator weaning in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD). Methods The AECOPD patients caused by bacteria who received mechanical ventilation in intensive care unit (ICU)of the General Hospital of China National Petroleum Corporation in Jilin from January 2010 to March 2014 were retrospectively selected,and their venous blood PCT levels were monitored daily. When they had passed the spontaneous breathing trial(SBT),their breathing machine and trachea cannula were removed. Afterwards they were closely observed for 48 hours and divided into two groups:success and failure groups according to the outcome of the first weaning,and then the analysis of the PCT difference between the two groups was made. Results In the 52 patients enrolled,there were 38 patients in the success group and 14 patients in the failure group. The baseline information as age,sex,acute physiology and chronic health evaluationⅡ(APACHEⅡ)score between the two groups showed no significant difference(all P>0.05). There was no statistical significant difference in PCT level(μg/L) between success and failure groups when they were admitted(5.33±1.35 and 5.37±1.34,respectively). When they were weaning,the PCT levels of both groups had reduced to some extent(1.37±0.39 and 1.74±0.42,respectively), and the success group was lower than that in the failure group(t=2.971,P=0.005). Conclusion PCT level can be used as a predictive index to guide the time for ventilator weaning in patients with AECOPD.

Objective To observe the effects of bone marrow mesenchymal stem cells (BMSC) transplantation on the expression of nerve growth factor(NGF) and brain derived neurotrophic factor(BDNF) in the rat spinal cord with spina bifida,and to investigate the change in cell apoptosis after BMSC transplantation.Methods Spina bifida aperta was induced with a single intragastric injection of all-trans retinoic acid,then the BMSC was microinjected into spina cord of rat embryos on embryo 16 d(E16),BDNF and NGF were tested by immunofluorescence staining,and TUNEL assay were used for investigating cell apoptosis.Results Transplantation of BMSC enhanced the expression of NGF and BDNF,and reduced cell apoptosis in the defective site of spinal cord.Conclusion The transplantation of BMSC may improve the microenvironment of spinal cord and repair the neurological defects by enhancing the expression of neurotrophic factor and reducing the cell apoptosis.
9.Electron microscopic observation of mitochondrial ultrastructure in melanocytes of perilesional skin from patients with vitiligo

Objective To evaluate the changes to mitochondrial ultrastructure in melanocytes of perilesional skin from patients with vitiligo.Methods Skin specimens were obtained from the perilesional area (0.5-1 cm distal to vitiligo lesions) of 10 patients with progressive vitiligo and 10 patients with stable vitiligo,as well as from the normal skin of 10 healthy volunteers.The morphology of melanocytes was observed by using transmission electron microscopy (TEM).Besides,stereological parameters of mitochondria,such as volume density (Vv),surface density (Sv) and numerical density (Nv),were measured.Results In melanocytes from the healthy controls,there were a large number of melanosomes with the number of melanosomes per melanocyte being 28.57± 3.21,which were mainly at stage Ⅲ and Ⅳ; mitochondria with normal structure and densely packed cristae were regularly arranged; autophagosomes were seen occasionally.Compared with the melanocytes from healthy controls,there was an obvious decrease in the number of melanosomes (especially stage Ⅲ melanosomes) in melanocytes from the perilesional skin of patients,with the number of melanosomes per melanocyte being 22.00 ± 6.16 (P < 0.05) and 17.43 ± 6.24 (P < 0.05) in patients with progressive vitiligo and stable vitiligo,respectively.TEM also showed disorganized or disrupted mitochondria in various shapes and sizes,most of which were swelling with obscure cristae and vacuolization,in melanocytes from the perilesional skin,and no autophagy was observed.The three stereological parameters were significantly different between the three groups of tissue specimens (all P < 0.05),with the Nv,Vv and Sv of mitochondria being (7.194 ± 1.434) μm-3,(4.8 ± 1.2) %,(2.42 ± 0.86) μ m-1 respectively in melanocytes from the healthy controls,(4.055 ± 0.906) μm-3,(7.4 ± 2.1)%,(3.58 ± 1.15)μm-1 respectively from patients with progressive vitiligo,(5.311 ± 0.873) μm-3,(6.5 ± 1.4)%,(2.82 ± 0.94) μm-1 respectively from patients with stable vitiligo.Conclusions Mitochondria are injured in melanocytes from perilesional skin of patients with vitiligo,and the degree of injury is more intense in progressive vitiligo than in stable vitiligo.
10.Construction of cationic anticancer peptide Temporin-1 CEa liposomes and evaluation of anti-breast cancer activity in vitro

Objective To constract and evaluate the cationic anticancer peptide Temporin-1CEa liposomes and evaluate anti-breast cancer activity in vitro.Methods The polyethylene glycol (PEG)-modified liposomes containing Temporin-1CEa, one recently discovered cationic anticancer peptide ( CAP) , were constructed by using reverse-phase evaporation method.The encapsulation efficiency, particle size and Zeta potential of the Temporin-1CEa-containing liposomes (Temporin-1CEa-LIP) were characterized.In addition, that had the furhter evaluated of the stability and specific toxicity against human breast cancer MCF-7 cells in vitro.Results The data suggested that the PEG-modified liposomes served a promising drug delivery system for CAPs, those indicated by the encapsulation efficiency was (55.57 ±1.56)%, the particle size was (105.3 ±1.37) nm and the Zeta potential was ( -16.17 ±0.964) mV.Moreover, the in vitro test also indicated that Temporin-1CEa-LIP exerted good stability in serum, and it could be efficiently uptaken by MCF-7 cells.Most importantly, after 24h exposure, Temporin-1CEa-LIP showed toxicity against MCF-7 cells, as potent as Temporin-1CEa. Conclusion The results demonstrates that the PEG-modified liposome is a good drug-delivery system and Temporin-1CEa-LIP could serve as potential anti-tumor candidate for cancer therapy.
