1.Study of visual analogue scale of nasal symptoms between chronic rhinosinusitis patients and doctors.

OBJECTIVE:
To investigate the difference of the symptoms evaluation between CRS patients and doctors.
METHOD:
Seven symptoms including nasal obstruction, watery rhinorrhea, sneezing, rhinocnesmus, itching of eyes, hyposmia and facial pain were evaluated by visual analog scales (VAS) by the patients. The assigned rhinologist finished VAS evaluation of doctor based on the patient's description of their symptoms. Rank test was used to analyze the difference for each VAS scores of symptom between patients and doctor. Spearman test was used to analyze the effect of age in VAS scores and the effect of gender was analyzed by using Mann-Whitney test.
RESULT:
We found that the sum and every single patient's nasal symptoms VAS scores both showed statistical significantly difference (t = 4.51 P < 0.01). VAS scores of nasal blockage showed the significant difference between patients and doctor (r = 359.52 P < 0.01). Age of patients did not show correlation with VAS scores (r = -0.074 P > 0.05) also, gender of patients didn't show statistical significant (P > 0.05).
CONCLUSION
There were differences between patient's and doctor's nasal symptoms VAS scores and the VAS scores of patients were higher than the doctor's. Age and gender of patient were not related to the VAS evaluation.

2.Analysis on autoantibodies spectrum of Uygur patients with autoimmune liver disease in Xinjiang

Objective To analyze the positive rate of autoantibodies in Xinjiang Uygur patients with different types of auto‐immune liver disease and its diagnostic value .Methods Fifty‐one Uygur outpatients and inpatients with of autoimmune liver disease in our hospital from October 2012 to October 2014 were selected ,including 20 cases of autoimmune hepatitis (AIH ,AIH group) and 31 cases of primary biliary cirrhosis (PBC ,PBC group) .The indirect immunofluorescence and Western blotting were used to detect the serum biochemical indexes and autoantobodies ,and the comparative analysis was performed .Results The levels of various bio‐chemical indexes in the two groups were increased ,in which ALT ,γ‐GT ,ALP and IgM levels had statistical difference between the two groups(P<0 .05) .The positive rates of ANA ,SMA ,LKM‐1 antibody ,LC‐1 antibody ,SDLA/LP antibody and AMA in the AIH group were 65 .0% ,40 .0% ,10 .0% ,5 .0% ,10 .0% and 5 .0% respectively ;the positive rates of ANA ,AMA and M2 subtype (AMA‐M2) in the PBC group were 61 .3% ,100 .0% and 96 .8% respectively .Conclusion The autoantibodies spectrum in Xinjiang Uygur patients with autoimmune liver diseases has certain characteristics .The autoantibodies spectrum detection has an important significance for the diagnosis ,classification and differential diagnosis of autoimmune liver diseases .
3.Ca2+/calmodulin-dependent protein kinaseⅡregulates cardiac hypertrophy by mediating autophagy

Objective To elucidate the relationship between Ca2+/calmodulin-dependent protein kinaseⅡ(CaMKⅡ)and autophagy during the development of cardiac hypertrophy. Methods Rat embryonic cardiac cell line H9c2 cells was treated by angiotensin Ⅱ to establish cardiomyocyte hypertrophy model in vitro and using antagonists and gene function gain and loss to analyze AMPK-LC3Ⅱ autophagy signaling pathway. Results The phosphorylation of CaMKⅡ and autophagy related signaling-AMPK and autophagy marker LC3Ⅱ were rapidly increased by angiotensinⅡtreatment at early stage. However ,the above changes were highly blocked by CaMKⅡinhibitor and HDAC4 inhibition. Conclusion CaMKⅡ is the center factor of regulating cardiac hypertrophy ,it mediates autophagy through directly regulating AMPK or indirectly regulating HDAC4 during the development of cardiac hypertrophy.
4.Application of video-assisted thoracoscopic surgery and transsternal thymectomy for treatment of myasthenia gravis in primary hospital

Objective To identify the availability of video-assisted thoracoscopic thymectomy. Methods Retrospectively analyzed 68 patients with myasthenia gravis who underwent thymectomy including 34 cases of video-assisted thoracoscopic thymectomy and 34 cases of transsternal thymectomy,and the patients were followed up for 16 years. Results Patients of the VATS group were given video-assisted thora-coscopic thymectomy. The mean operative time was (90. 1 ± 15. 0) min,mean blood loss was (45. 0 ± 5. 5) mL,mean chest tube drainage time was (2. 5 ± 1. 2) days and mean postoperative hospital stay was (7. 0 ± 1. 2) days in VATS group,and there was no serious complica-tions and surgical death. The mean operative time was (98. 0 ± 12. 5) min,mean blood loss was (118. 5 ± 17. 5) mL,mean chest tube drain-age time was (4. 5 ± 1. 3) days and mean postoperative hospital stay was (11. 0 ± 2. 5) days in transsternal thymectomy group. 3 patients de-veloped MG crisis. There was no significant difference in mid-and long-term effects between the two groups(P>0. 05). ConclusionVideo-as-sisted thoracoscopic thymectomy for MG is safe and feasible with the advantage of less invasion,less surgical trauma,lower rate of complica-tion,and good curative effect compared with transsternal thymectomy.

7.Regulatory effect of interleukin-38 on IκB Kinase complex in patients with active systemic lupus erythematosus

Objective:To observe the expression of interleukin (IL)-38 in patients with active systemic lupus erythematosus (SLE), and to explore the regulatory effect of IL-38 on IκB kinase complex.Methods:The gene expression levels of IL-38 and IκB kinase complex were detected by real-time fluorescence quantitative polymerase chain reaction (PCR). Serum IL-38 levels were detected by enzyme-linked immunosorbent assay (ELISA). The expression of IκB-kinase complex (IκK)α/β and phospho IκKα/β protein weredetermined by Western blotting analysis. Statistical analysis was conducted with or Mann-Whitney rank test.Results:① The expression level of IL-38 mRNA in active SLE patients (0.36±0.09) was significantly lower than that in the normal control group (1.00±0.17) ( Z=-4.07, P<0.01); the expression level of IL-38 protein in active SLE patients (5.86±2.76) significantly reduced as compared with normal control group (18.48±1.35) ( Z=-4.76, P<0.05). ② The expression level of IκKα mRNA (7.45±0.31) and IκKβ mRNA (6.01±1.51) in active SLE patients was significantly higher than that in normal control group (1.16±0.04) and (1.16±0.14) ( Z=-4.67, P<0.05; Z=-4.37, P<0.01), and the expression level of IL-38 mRNA was negatively correlated with IκKα mRNA and IκKβ mRNA in active SLE patients ( r=-0.78, P<0.05; r=-0.83, P<0.05). ③ IκKα/β and phosphorylated IκKα/β protein expression in active SLE patients (2.38±0.03) and (1.90±0.03) increased significantly compared with healthy controls (1.00±0.04) and (1.00±0.08) ( Z=-1.96, P<0.05; Z=-1.99, P<0.05). ④ In vitro experiment, IL-38 caused a significant decrease in the expression level of IκKα mRNA (1.70±0.12) and IκKβ mRNA (2.52±0.10) from active SLE patientscompared with untreated cells (2.56±0.28) and (3.82±0.38) ( Z=-1.96, P<0.05; Z=-1.37, P<0.05). ⑤ In vitro experiment, IL-38 caused a significant decrease in the expression level of IκKα/β (1.54±0.06) and phosphoryl-ated IκKα/β (0.970±0.012) protein expression in active SLE patients compared with untreated cells (2.93±0.08) and (1.572±0.051)( P<0.05). Conclusion:The de-crease of IL-38 level in SLE patients result in the excessive activation of IκB kinase complex IκKα and IκKβ, and therefore triggersystemic lupus erythematosus.
8.Pareto Chart Analysis of Unreasonable Factors of Prescription in Outpatient and Emergency Departments of Our Hospital during 2013-2015

OBJECTIVE:To investigate the current situation of unreasonable prescriptions in outpatient and emergency depart-ments of our hospital,and to provide reference for rational drug use in the clinic. METHODS:The evaluation of outpatient and emergency prescriptions in our hospital during 2013-2015 were summarized and analyzed. Irrational prescriptions were divided into 3 categories,involving 19 items. The main,secondary and general factors of irrational prescriptions were analyzed by Pareto chart. RESULTS:Among 19 types of irrational prescriptions,main factors included unsuitable drug selection,inappropriate usage and dosage,inappropriate drug combination,prescribing not in accordance with the antibiotics management regulations,incomplete di-agnosis. No indications of medication,drug dosage beyond the regulations ofthree urgent seven slowwere treated as the second-ary factors. Other 12 items were general factors. CONCLUSIONS:According to the main and secondary irrational drug use types, clinical pharmacists should carry out effective intervention and pharmaceutical care to improve the level of rational drug use and en-sure the safety of clinical drug use.
9.Cobalt Hexacyanoferrate-modified Graphene Platform Electrodeand Its Electrochemical Sensing toward Hydrogen Peroxide

A large-area graphene platform electrode (GPE) was fabricated by transferring the chemical vapor deposition (CVD) graphene grown on Cu foils to polyethylene terephthalate (PET) substrates with the aid of polymethyl-methacrylate (PMMA).Then cobalt hexacyanoferrate (CoHCF) was electrodeposited on the GPE by cyclic voltammetry to obtain a CoHCF/GPE.The morphology and electrochemical properties of this electrode were studied by scanning electron microscope (SEM), electrochemical impedance spectroscopy (EIS) and the electrochemical measurements.Results showed that the electrode had favorable sensing effect toward H2O2.On the basis of this, a new nonenzyme H2O2 sensor was constructed.Under the optimal experimental conditions, the proposed sensor had a quick response to the addition of H2O2 (about 2 s) with a wide linear rang (5 × 10-3-1.2 mmol/L) and a low detection limit (7.1 nmol/L).This sensor was easy to fabricate and showed excellent stability and anti-interference ability.
10.The microsurgical anatomy of the modified presigmoid transpartial bony labyrinth approach

Objective To investigate the surgical techniques of the modified presigmoid trans-partial bony labyrinth approach and the advantages in exposure of the petroclival region and in treat the lesion of this area.Methods By simulate the modified presigmoid trans-partial bony labyrinth approach in 15 adult cadaveric heads with the aid of an operating microscope and record important structures in the petroclival region.Results The petroclival region,the posterior cavernous sinus,Meckel cave,the vertebral-basilar artery,the anterior inferior cerebellar artery,the superior cerebellar artery,ipsilateral Ⅲ-Ⅹ cranial nerve nere fully exposured and contralateral Ⅵ cranial nerve were fully exposured.The range of presigmoid exposure was (19.41 ± 1.58)mm,the exposurein of inferior temporal was (14.18 ± 1.88) nun,the maximum exposure angle of slope center depression was (60.54 ± 6.93) °,the depth of operation was (55.87 ± 4.34) mm.Conclusion The advantages of the modified presigmoid trans-partial bony labyrinth approach can earn enough exposures of deep part of petroclival region and posterior part of cavernous sinus,improved petroclival exposure,multiple axes of visualization,preservation of hearing and facial nerve function,and early devascularization of the tumor.
