1.Clinical observation on total parenteral nutrition of 45 severe neonates

Objective To evaluate the clinical effects of total parenteral nutrition in the treatment of severe neonates.Methods 45 severe neonates were treated with their essential while injected nutritious liquid.At the same time some concemed targets were tested.Results 2 cases died in 45 neonates,server infections and sudden stop of heartthrob and breath being the causes of death,and the others achieyed satisfactory result.The average body mass increased by 15/30g/d during parenteral nutrition.Conclusion Total parenteral nutrition was clinically significant for increasing the body weight of severe neonates,and it was safe,effective and supportive.
2.Analysis of application of rigid gas permeable lens in aphakic patients

?AIM:To compare the corrected vision and improvement of visual quality after wearing rigid gas permeable corneal lens ( RGPCL) or spectacles in aphakic patients.?METHODS: We selected 29 aphakic patients ( 29 eyes ) caused by different reasons wearing RGPCL and spectacle.The corrected vision, eye condition and visual quality were observed and all patients were followed up for 6mo.? RESULTS: RGPCL was better than spectacle on corrected vision (P<0.05).The patients who wore RGPCL for long had no corneal complications reported. The patients who wore RGPCL had better subjective visual quality than those wore spectacle.?CONCLUSION: RGPCL is a good choice for correcting high myopia and astigmatism for aphakic patients.The patients'compliance is good. Wearing RGPL long has high safety for patients'ocular surface.

4.Comparison on clinical effects of operative versus non-operative treatment for traumatic flail chest combined with sternal fracture

Objective To compare the efficacy of internal fixation with non-operative treatment of traumatic flail chest combined with sternal fracture.Methods A retrospective review was made on 102 patients with traumatic flail chest combined with sternal fracture treated from January 2007 to January 2014.There were 71 patients in operation group and 31 patients in non-operation group.Operation group was allocated to pure titanium rib plating (Group Ⅰ,22 cases),porous titanium screw fixation (Group Ⅱ,20 cases),and nickel-titanium memory alloy sternal rib plating (Group Ⅲ,29 cases).Rib traction was performed in non-operation group.Clinical results and incidence of complications were compared between the two groups.Results At 24 and 72 hours,operation group was superior over non-operation group with regard to HR [(100.4 ± 9.5) times/min vs (105.2 ± 10.1) times/min,(92.1 ± 9.9) times/min vs (102.1 ± 9.2) times/min],MAP [(97.7 ± 14.5) mmHg vs (91.0 ± 13.3) mmHg,(112.5 ± 15.2) mmHg vs (93.1 ± 13.8)mmHg] and CVP [(7.8 ±3.7)cmH2O vs (9.4 ±3.8)cmH2O,(6.2 ±2.9)cmH2O vs (9.1 ±3.2) cmH2 O] (P < 0.05).Duration in ICU [(5.1 ± 0.8) days vs (9.3 ± 1.1) days],ventilation time [(4.5 ±1.0)days vs (8.2 ± 1.4)days],hospitalization time [(14.6 ±3.5)days vs (23.3 ±4.4)days] and incidence of complications were significantly lower in operation group than in non-operation group (P < 0.05).Two months after discharge,tidal volume (VT) [(0.52 ±0.04)L vs (0.40 ±0.03)L],inspiratory capacity (IC)[(1.99 ±0.45)L vs (1.22 ±±0.33)L],forced vital capacity (FVC) [(3.52 ±0.51)L vs (2.44 ±0.42)L],total lung capacity (TLC) [(5.41 ±0.82)L vs (3.72 ±0.56)L] and forced expiratory volume in one second (FEV1) [(2.80 ± 0.43) L vs (1.95 ± 0.50) L] showed significantly better results in operation group than in non-operation group (P < 0.05).Operation time,intraoperative bleeding and daily drainage volume were (38.8 ±9.2)min,(43.5 ±7.6)ml and (10.9 ± 1.1)ml in Group Ⅱ versus (62.5 ± 10.1)min,(100.0 ± 10.5) ml and (26.8 ±3.1)ml in Group Ⅰ and (49.3 ±9.6)min,(61.4 ±8.3)min and (19.8 ±1.6)ml in Group Ⅲ (P < 0.01).Conclusion For traumatic flail chest combined with sternal fracture,the internal fixation should be done as early as possible so as to reduce incidence of complications.Relatively,porous titanium screw fixation has better clinical outcome.

9.Determination of Hydroxycamptothecin Concentration in Rabbit Tissues by HPLC

OBJECTIVE: To establish a HPLC - UV method for determination of hydroxycamptothecin(HCPT) in rabbit tissues (liver, kidney, stomach, lung, spleen, heart, intestine) .METHODS: After homogenization, tissue samples and internal standard, eamptothecine, were precipitated with CH3OH-CH3CN( 1 : 1) and then centrifugalized.20?l of supernatant was injected and measured by HPLC - UV method.The chromatographic column was Lichrospher C18 column: (250mm ? 4.6mm, 5?m), CH3CN - 0.075mol/L NH4AC buffer(pH6.4) (30 : 70, contain 5mol/L tyiethylamine) served as mobile phase with flow rate of 1.0ml/ min .Detection wavelength was 384nm .RESULTS: The retention time of hydroxyeamptothecin was 4.5min.A good linearity was shown in the concentration range of (40- 1 600)ng/ml .The recovery was between 95.24% and 107.58% . The intra day RSD was less than 7.70% and the inter - day RSD was less than 6.69% .CONCLUSION: This method is simple,pratical and accurate.It could be applied to pharmacokinetic study of hydroxyeamptothecin.
10.Effects of homocysteine on post-cerebral ischemic angiogenesis in rats

Objective To explore the role of homocysteine(Hcy)on angiogenesis at peri infarct region after focal cere-bral ischemia in rats, to elucidate inhibitory factors of angiogenesis, and to establish a clinic foundation for clinical brain functional recovery. Methods Spragur-Dawley (SD) male rats (n=36) were randomly divided into three groups with 12 rats in each group including Sham Operation (SO) group, Middle cerebral artery occlusion (MCAO) group and MCAO+Hcy group. The rats in Sham and MCAO groups were intra-peritoneally injected with 5 mL/(kg·d) saline and rats in MCAO+Hcy group were injected with 2%5 mL/(kg·d) Hey solution from the same route. MCAO was introduced by intraluminal filament meth-od after 7 d Hcy intervention. Rat brains were harvested on the 7th day after MCAO. BrdU(50 mg/kg, as a marker of cell pro-liferation)was intraperitoneally injected three days before the rats were killed. High performance liquid chromatography (HPLC)was used to measure serum Hcy concentration in rats. Brain infarction size was observed by TTC staining. Immuno-fluorescence staining was used to detect the number of BrdU+/laminin+cells at the thalamus of infarction side. Results Se-rum Hcy concentration significantly higher in MCAO+Hcy group than in SO and MCAO groups(P<0.05). Brain damage increased and the number of BrdU+/laminin+cells decreased in MCAO+Hcy group compared with those of MCAO group (P<0.05). Conclusion Increased Hcy concentration in rats lead to more severe damage of cerebral infarction as well as to inhibit the angiogenesis at surrounding ischemia area.
