1.Research progress on the prevention and treatment of diabetic nephropathy by activated vitamin D3 immu-noregulation

Diabetic nephropathy is caused by manifold factors .Increasing evidences suggest that immunoregulation abnormality caused by inflammatory cell infiltration and proinflammatory factor overexpression in kidney tissue has a key role in the occurrence and development of diabetic nephropathy .As a novel immunomodulator , activated vitamin D 3 could inhibit inflammatory factor and regulate immunity response .The author reviews the effect of activated vitamin D 3 related to diabetic nephropathy , from the pointviews of immu-nocyte, cytokines and rennin-angiotensin system.The roles of macrophage, T lymphocytes, dendritic cell, transforming growth factor-β1, and rennin-angiotensin system in kidney injury and immunal regulation of activated vitamin D 3 are discussed.
2.The Effect of the Parent's Education on the Early Intellectual Development of infants

Objective: To study the effect of parent's education on the early intellectual development of infants. Methods: A total of 3,000 infants in a general hospital were sampled and divided into three groups: 3-month-old, 6-month-old and 9-month-old. All of the infants were tested with DST. Results: The parent's education was shown to have a significant effect on the intellectual development of 6-month-old and 9-month-old infants. The better education the parents had, the brighter the infants were. Conclusions: The parent's education had significant effect on the baby's early intelligence development.
3.The application of larger-bore autologous arteriovenous fistula as dialysis pathways in diabetes patients with chronic renal failure

Objective To observe and discusses the application of larger-bore autologous arteriovenous fistula as dialysis pathways in diabetes patients with chronic renal failure. Methods 86 cases of diabetic patients with chronic renal failure were divided into elderly group and non-elderly group. The patients within each group were assigned randomly into Experimental subgroup and control subgroup. Patients underwent wrist autogenous arteriovenous fistula angioplasty.The surgical method was end to side anastomosis between cephalic vein and radial artery. Vertical incision on the radial artery is about 0.7 cm-0.8 cm in the control subgroup and vertical incision on the radial artery is about 1.0 cm-1.1cm in the experimental subgroup. Results The success rate of the operation in experimental subgroup(90.9%, 95.2%) was significantly higher than that of control group(77.3%, 85.7%). The differences were significant (P<0.05). There was no heart failure in the patients underwent surgery. Conclusions In diabetes patients with chronic renal failure need autogenous arteriovenous fistula angioplasty. Properly increasing the diameter of anastomosis can meet the demand on blood flow of hemodialysis without increase complication of heart failure.

5.Comparison of the efficacy and safety of ziprasidone and risperidone in the treatment of schizophrenia

Objective To investigate the clinical efficacy and safety of ziprasidone and risperidone in the treatment of schizophrenia.Methods According to the digital table,64 patients with schizophrenia were randomly divided into the two groups,32 cases in each group.The control group was given risperidone for treatment,while the observation group was given ziprasidone for treatment.The clinical efficacy was assessed by using the positive and negative symptom scale(PANSS),and adverse events were observed.Results The total effective rates of the control group and the observation group were 84.38% and 87.50%,there was no significant difference between the two groups(x2 =2.66,P > 0.05);The PANSS score and each factor score of the two groups after treatment decreased significantly than before treatment (t =8.11,7.86,7.72,7.90,6.77,6.58,7.52,7.40,all P < 0.05),but there was no significant difference between the two groups (t =2.18,P > 0.05) ;The incidence rate of adverse reaction of the observation group was 18.75%,which was significantly lower than 40.63% of the control group (x2 =8.66,P <0.05).Conclusion Ziprasidone has therapeutic equivalence with risperidone in the treatment of schizophrenia,with less adverse reaction,high security,it has good clinical application value.
6.Study of N-methyl-D-aspartic acid receptor 2B level and calcium/calmodulin-dependent protein kinase Ⅱ activities in the hippocampus of vascular dementia rats and the intervention effects of Memantine

7.Relationship between Hippocampal NMDA Receptor together with Associated Synaptic Proteins and Development of Vascular Dementia in Rats

Objective To investigate the influence of Memantine on changes of N-methyl-D-aspartic acid receptor 2B (NMDAR-2B) level, the expression of postsynaptic density 95 ( PSD-95) and calcium/calmodulin-dependent protein kinase Ⅱ (CaMK Ⅱ)activities in the hippocampus of vascular dementia(VD)rats and their recognition. Methods The VD rat model was established by permanent,bilateral occlusion of the common carotid arteries. One hundred and forty-four Wistar rats were randomly divided into sham-operated group, VD model group and Memantine-treated group. The water maze test was performed to detect the ability of learning and memory of the rats,the changes of NMDAR-2B level were measured by RT-PCR,the changes in the expression of PSD-95 were detected by immunohistochemical staining,and the changes in the CaMK Ⅱ activities were determined by incorporation of ~(32)P into histone. Results Compared with the sham-operated group, the ability of learning and memory of VD rats was decreased significantly(P<0. 01); the level of NMDAR-2B and the expression of PSD-95 increased significantly at the 4th week after operation(F<0. 05 or 0. 01)and decreased at the 8th to 16th week after operation (P<0. 01). Total CaMK Ⅱ activity was increased significantly at the 4th week after operation(P<0. 01) ,then dropped at the 8th week after operation(P>0. 05) ,and decreased significantly at the 12th,16th week after operation(P<0. 01). As for Memantine-treated group,at the 4th week after operation,there was no statistically significant difference in the parameters mentioned above in contrast with the sham-operated group. At the 8th,and 12th week after operation,the ability of learning and memory,the level of NMDAR-2B and the expression of PSD-95 were significantly increased as compared with VD rats(P<0. 01 or 0. 05) ,but total CaMK Ⅱ activity was similar to that of VD rats. Conclusion With the development of VD,NMDAR-2B level,the expression of PSD-95 and CaMK Ⅱ activities were firstly over-activated,and then decreased greatly. Memantine can improve VD rats' recognition by modulating the expression of NMDAR-2B,but its effect on CaMK Ⅱactivity was limited.
8.Correlation between Ambulatory Arterial Stiffness Index and Renal Abnormalities in Primary Hypertension

Objective To investigate the relationship between ambulatory arterial stiffness index (AASI) derived from blood pressure monitoring and early signs of renal damage in patients with primary hypertension. Methods 74 primary hypertensive outpatients were divided into two groups according to their AASI values: normal AASI group (AASI≤0.51, n=40) and high AASI group (AASI>0.51, n=32). The urinary micro-albumin, glomerular filtration rates (GFR) were measured and compared. The relationship between AASI and micro-albumin, GFR were tested with Pearson correlation and multiple Logistic regression. Results Compared with those in the normal AASI group, the patients in high AASI group showed a higher level of urinary microalbumin (P<0.05) and a reduction in GFR (P<0.01). AASI was positively correlated with urinary microalbumin (r=0.32, P<0.001), and negatively correlated with GFR (r=0.44, P<0.001). After adjusting the potentially confounding variables, the odd ratio (OR) of AASI to renal damage was 2.18 (P=0.008,95%CI:1.76~4.34). Conclusion The increase of AASI is associated with early signs of renal damage in patients with primary hypertension.

10.Biosynthesis of arbutin from crown gall of transgenic Panax quinquefolium

Objective To investigate the biosynthesis of arbutin by suspension culture of crown gall of transgenic Panax quinquefolium. Methods Hydroquinone in methanol(60 mg/mL) was added to the medium of the crown gall of P.quinquefolium after precultured for 20 d,then they were co-cultured for another 60 h.The product was isolated and purified by column chromatography and its structure was identified by physicochemical properties and spectroscopic data.The dynamic curve of biotransformation was investigated by quantative analysis of arbutin with HPLC.Results The product could be obtained from both of the culture and medium,which was isolated and elucidated as 4-hydroxyphenyl-?-D-glucopyranoside(arbutin).After co-cultured for 60 h,the mole conversion ratio of hydroquinone is 88.4%,the product contents in culture and medium are 63.69 mg/flask and 1.86 mg/flask,respectively,and the excretion ratio of arbutin reaches the highest(2.92%).Conclusion It's the first time around the world that the crown gall of transgenic P.quinquefolium is used as a biotransformation system and arbutin which shows varied pharmacological activites have been got successfully.
