Objective To discuss the effect of different mechanical ventilation parameters on intra-abdominal pres-sure. Methods A total of 42 cases with mechanical ventilation suffering from chronic obstructive pulmonary diseases (COPD) and respiratory failure were selected for observing the effects of inspiratory platform time (Tplat), inspiratory time (Ti) and end-expiratory pressure (PEEP) on intra-abdominal pressure. Results In SIMV+PSV ventilation mode, changes of Tplat and Ti had no significant effects on intra-abdominal pressure (P>0.05). There were no significant differences be-tween SIMV+PEEP0 and base line values (P>0.05). Conclusion The alteration of peep has a great influence in intra-ab-dominal pressure, which suggests the importance of selecting proper mechanical ventilation parameters.

BACKGROUND: At present, there are fewer studies on the monitoring of nutritional status of the residents living in the draw-down area of ThreeGorge reservoir.OBJECTIVE: To investigate the nutritional status and dietary pattern of the residents before the formation of the draw-down area of Three-Gorge reservoir, so as to provide basic data for the continuous dynamic surveillance of food and nutrition.DESIGN: A cross-sectional study.SETTING: Department of Nutrition and Food Hygiene, School of Public Health, Chongqing University of Medical Sciences.PARTICIPANTS: The investigation was carried out between February and March 2004 in the draw-down area of Three-Gorge reservoir with denser population, larger area and greater influence, such as Kaixian and Yunyang counties, etc. The first surveillance of food nutritional status of the residents in the draw-down area was performed in 52 urban families and 50 rural families of Kaixian county, one family member was voluntarily selected from each family, whose main duty was to buy vegetables and food. Totally 102 subjects (42 males and 60 females) were investigated in this study. At the same time, the data of physical examination and detection of blood Hb in children from Kaixian county between May 1999 and November 2003 were analyzed.METHODS: The subjects were investigated in their house with questionnaires. Their general condition of family, food consumption and dietary nutrition were surveyed and analyzed. Nutrients intake was assessed by 24-hour recall, combined with food consuming frequency questionnaire, the Engel index, which was an important index for the level of family consumption, was detected in both rural and urban residents. Engel index=cost in food/total family income×100%.MAIN OUTCOME MEASURES: ① Amount of food consumption, intakes of energy and nutrients in residents of the draw-down area; ② Prevalence rate of low body mass and rate of growth retardation in children of the draw-down area.RESULTS: All the 102 residents of the draw-down area entered the analysis of results without deletion. ① Amount of food consumption in residents of the draw-down area: Among the urban residents of the draw-down area, the per capita food consumption was 243 g per day, and the consumptions of vegetable, fruit and fish were lower than the average national level, but that of grease food showed a higher level. Among rural residents of Kaixian county, the per capita food consumption was 268 g per day, and the consumption of foodstuff was obviously decreased, but those of meet and grease food showed an ascending trend. ② Energy intake in residents of the draw-down area: Among urban residents of Kaixian county, the per capita energy intake was 6 909 kJ per day, which was obviously lower than that in the urban residents of Chongqing city in 1998 and the average national urban level. The per capita energy intake in rural residents of Kaixian county was 7 577 kJ per day, which was 48.4% lower than that in the urban residents of Chongqing city in 1998 and 37% lower than the average national rural level. ③ Nutritional status in children of the draw-down area: The incidence rate of low body mass in children younger than 5 years in 1999 was close to the average national level (9.6%), it was obviously decreased from 2000, and then fluctuated between 1.0% and 4.8%. Since 1999, the incidence rate of growth retardation in children younger than 5 years of Kaixian county showed a gradually descending trend, but anemia and other common diseases still kept a higher incidence rate, the detection rate of anemia was 33.5% in 1999, and decreased to 20.8% in 2003.CONCLUSION: The current nutritional status of the Kaixian people living in draw-down area of Three-Gorge reservoir still needs further improvement, and dynamic food and nutrition surveillance in succession is necessary.

AlM: To analyze the correlation between tear film changes of allergic conjunctivitis and dry eye, then provide clinical references for better treatment.
METHODS: Fifty patients with allergic conjunctivitis were taken as the observation group, the control group was selected based on 1:1 case control theory, and we chose 50 health volunteers without ocular surface diseases, xerophthalmia and systematic diseases randomly, then fluorescein ( FL) staining, break-up time ( BUT) , Schirner l test ( SⅠt) , tear meniscus high ( TMH) and slit-lamp examinations were performed in the two groups.
RESULTS: ln the observation group, FL, BUT, Slt, TMH of right eyes and left eyes were statistically significant correlated ( P < 0. 05 ). FL and BUT were statistically significant between control group and observation group (P<0. 05). TMH and Slt has positive correlation, while FL and BUT has negative correlation and the differences were statistically significant (P<0. 05). CONCLUSlON: Due to inflammatory mediators participation, allergic conjunctivitis could lead to the stability changes of tear film which cause in dry eye. The stability changes of allergic conjunctivitis correlate to the damage degree of epithelium.

Objective To analyze the clinical data of 20 young and middle-aged patients with bronchial foreign bodies who were hospitalized because of unknown lung shadow. Methods The clinical data of 20 confirmed cases, including history of aspiration, imaging examination results, bronchoscopy findings and therapeutic process were retrospectively analyzed. The outcome of bronchial foreign body granuloma in the airway was observed. Results ⑴100%of the 20 patients with an average age of (46.39±3.26) years were misdiagnosed before bronchoscope examination. (2) The results of chest CT showed that the 20 patients had bronchial stenosis or obstruction with pulmonary consolidation or atelectasis, of which 18 cases were on the right side and 2 on the left side. (3) According to the shape and size of the foreign bodies as well as the length of time of aspiration, the bronchial openings were blocked by foreign bodies in 10 patients, and the foreign bodies were taken out by foreign body forceps. The diagnosis was confirmed in 3-6 (average 4.40±0.92) months in 10 patients. Four patients were obstructed by foreign bodies and granulation tissue, which took 5-12 (average 8.25±2.86) months to diagnose, while in 6 cases, masses were found in tracheal cavity, which took 6-15 (average 10.17±2.85) months to diagnose. The latter 10 (4+6) patients were treated with endotracheal intubation to remove foreign bodies and granulation tissue. (4) After 24-72 hours of treatment, the symptoms of cough, expectoration and blood in the sputum were relieved, and no severe complications occurred in 20 patients. (5) About two weeks after bronchial foreign bodies were removed, the granulation tissue disappeared. Conclusion Foreign body aspiration on the right side is more common in the 20 young and middle-aged patients, and bronchoscope is the best means of diagnosis and treatment.

Objective To investigate the stromal cell derived factor 1 α (SDF-1 α) and chemokine CXC motif receptor 4 (CXCR4) axis using acupoint electroacupuncture (EA) on the brains of rats after focal cerebral ischemia and reperfusion. Methods Ninety-eight Sprague-Dawley (SD) rats were randomly divided into a control group (8 rats),a model group (50 rats),and an EA group (40 rats). The animal model of focal brain ischemia-reperfusion was made with all the rats in the model group and EA group by using the filament occlusion technique. The model and EA groups were subdivided into 5 subgroups according to the sampling time points on the 1 st,3rd,7th,14th or 21st day after ischemia-reperfusion.The EA was administered bilaterally to the rat analog of the Hegu point (LI 4) in the EA group. The model and control groups received no special treatment.Immunohistochemical methods were employed to detect CXCR4-positive cells in the model and EA groups. The expressions of SDF-1α and CXCR4 mRNA were detected with RT-PCR methods in the 3rd,7th and 14th day subgroups. Results With the prolongation of reperfusion,SDF-1α mRNA expression in the model group had a single peak-like increase in the ischemic area of the cerebral cortex. It had increased significantly by the 3rd day, reached its peak value at the 7th day and then decreased gradually.SDF-1α mRNA expression in the EA group behaved similarly, but SDF-1 mRNA expression was significantly higher in the EA group than in the other two.In the model and EA groups CXCR4 mRNA relative values were higher at the 7th and 14th day than at day 3,and the expression at day 14 was significantly greater than at day 7.CXCR4 mRNA values in the EA group were significantly higher than in the model group at each time point. The expression of CXCR4-positive cells began to increase on the 1 st day in the model group, reached its peak value at the 7th day, then decreased by day 14,but it was still strongly expressed highly at the 21st day.Compared with the model group, the expression in the EA group showed the same pattern, but the number of CXCR4-positive cells in EA group was significantly higher. Conclusion Point EA can activate the SDF-1α and CXCR4 axis to promote angiogenesis after focal cerebral ischemia and reperfusion,at least in rats.
