2.Protective effect of alginate polysaccharide JM on brain ischemia injury in rats

Objective To evaluate the effect of alginate polysaccharide JM on rats impaired by brain ischemia,and to elucidate its mechanisms.Methods By using the middle cerebral artery occlusion(MCAO) induced by nylon surgical thread inserted through the internal carotid artery into the anterior cerebral artery in rats,the effects of JM on neurological dysfunction and infarct volumn in rats brain were investigated.Neuroblastoma SH-SY5Y cells were employed to study the inhibitory effects of JM on cytotoxicity induced by hypoxia-hypoglycemia.Results JM at the doses of 12.5 and 25 mg?kg-1,iv,30 min before ischemia impressed neurological dysfunction,characterized by the decreased behavioral obstacle scores of MCAO rats.The infarct volumn declined 24h after ischemia as well.Further investigation by flow cytometry revealed that JM significantly reduces the overloading of intracellular free calcium on and suppresses apoptosis induced by hypoxia-hypoglycemia in human neuroblastoma SH-SY5Y cells.Conclusion JM showed protective effects on brain ischemia,probably related to its inhibitory effect on the overloading of intracellular free calcium ion(\i) and cell apoptosis.
3.Application of Segmental Design Principles in the Small Incision Approach Rigid Internal Fixation for the Treatment of Maxillofacial Fractures

Objective To explore the application effects of segmental design principles in the small incision approach rigid internal fixation for the treatment of maxillofacial fractures.Methods With a retrospective study,from September 2012 to April 2016,172 patients with maxillofacial fractures were selected as the research object in our hospital.All the patients were divided into the observation group of 100 patients and control group of patients according to the different treatment methods.Two groups were treated with mall incision approach rigid internal fixation.The observation group were given the preoperative design based on the segmental design principles.The other operation process were similar with the control group.Results All patients were completed the operation,the fracture were healed by Ⅰ stage.The postoperative 3months of total effective rates of the observation group and the control group were 98.0% and 83.3%.The total effective rate of the observation group was significantly higher than that of the control group (P < 0.05).The postoperative 3months of infection,nerve injury,salivary fistula,screw loosening and complication rate in the observation group was 3.0%,so that was 20.8% in the control group,the observation group was significantly lower than the control group (P < 0.05).The postoperative 3months of occlusal relationship and normal rates of the observation group were 99.0% and 98.0%,respectively.The control group was 93.1%and 90.3%,respectively.There were between the two groups statistically significance difference(P < 0.05).Conclusion The segmental design principles in the small incision approach rigid internal fixation for the treatment of maxillofacial fractures can improve the therapeutic effect,reduce the incidence of postoperative complications,improve the occlusal relationship with normal mouth opening rate.It is an ideal method for the treatment of maxillofacial fracture.

The herpes simplex virus(HSV) Us3 gene encodes a ser/thr protein kinase(PK).As an accessory gene,it plays an important role in the regulation of the apoptosis of infected cells and virus release.Us3-induced alterations in the host cytomorphology are associated with enhanced intercellular virus spread,suggestive of a previously undescribed aspect of alphaherpesvirus spread.
5.The efficacy of hormone replacement therapy for the treatment of rheumatoid arthritis: a Meta-analysis

Objective To explore the efficacy of hormone replacement therapy (HRT) for rheumatoid arthritis (RA).Methods The literature about HRT in the treatment of rheumatoid arthritis were searched.Eleven papers were subjected to a Meta-analysis and a heterogeneity test was conducted to evaluate the efficacy of HRT.Results HRT reduced the level of erythrocyte sedimentation rate (ESR) in RA patients (P=0.016),the SMD was-0.22(-0.40,-0.04); improved bone mineral density (BMD) in RA patients (P=0.022),the WMD was 2.83(0.41,5.26); decreased clinical parameters for disease activity evaluations of RA patients (P=0.048),the SMD was-0.19 (-0.38,0.00); decreased the level of C-reactive protein (CRP) in RA patients,the SMD was-0.08 (-0.37,0.21),but the difference was not statistically significant (P=0.591).Conclusion The findings from this Meta-analysis indicate that HRT can reduce the ESR level,improve clinical indexes and improve BMD level of RA patients.HRT may suppress disease activity and osteoporosis of RA patients,so it may be used as an auxiliary therapy in the treatment of RA.
6.Observation of Pattern Changes of Syndrome in TCM of 258 Cases of Acute Stroke and Therapeutic Effect Evaluation of Small Compound Prescriptions in Stroke Unit with Treatment Based on Syndrome Differentiation

Objective: To investigate into the etiological factor,distribution of syndrome(patterns),pathogenesis features as well as the curative effects of small compound prescriptions of(Traditional) Chinese Medicine(TCM) in the treatment of acute stroke in stroke unit.Methods:(Cases) corresponding to diagnosis standards were randomly divided into control group(102 cases) and treatment group(156 cases).Conventional treatment methods were given to cases in control group.On the base of control group,small compound prescriptions were added to in treatment group,successively 14-21 days.The relation between types and time and TCM scale(TCMS),the NIH stroke scale(NIHSS) and Barthel index(BI) were observed in the two groups.Results: Stroke was age-related,the older,the more stroke patients.Stroke was closely related to hypertension.Ascending hyperactivity of liver yang was main type in the previous three days,this type(decreased) with the time going on.On the contrary,the type of wind phlegm and blood stasis was secondly main type in the previous three days,it increased with the time going on.Types of ascending hyperactivity of liver yang,wind phlegm and blood stasis as well as phlegm heat and constipation covered 95 per cent of all patients in the previous 21days.In the two groups,the mean scale of TCM and NIH decreased,and BI increased after treatment,this statistical difference could also be seen between the two groups.Conclusion: Excessive types are main types in the acute stage of stroke.Types of ascending hyperactivity of liver yang,wind phlegm and blood stasis as well as phlegm heat and constipation are its main types.Transferring of ascending hyperactivity of liver yang into endogenous wind may be trigger of reaction link of wind,fire,phlegm and blood stasis in TCM theory.Small compound prescriptions of TCM aiming to clearing away liver fire to stop wind,to activate blood for removing blood stasis and phlegm,to removing phlegm and heat from discharge can reduce scale of TCM and NIH,increase BI in the treatment of patients in stroke unit,which provide beneficial proofs for evaluation of effects of TCM.
7.Diagnostic value of procalcitonin,CRP,prealbumin and WBC in patients with severe pneumonia

Objective To evaluate the diagnostic value of procalcitonin(PCT),C-reactive protein(CRP),prealbumin(PA)and white blood cell (WBC)count measurements in patients with severe pneumonia.Methods The serum samples of 34 patients with severe pneumonia,68 non-severe pneumonia patients and 40 healthy volunteers were collected.Serum concentrations of PCT,CRP, PA and WBC count of all samples were determined.Results The levels of PCT,CRP,PA and WBC in patients with severe pneu-monia were (24.07±34.77)ng/mL,(98.75 ±69.63)mg/L,(105.65 ±68.88)mg/L,(12.64±7.62)×109/L,which were signifi-cantly higher than those in non-severe pneumonia patients and healthy group(P <0.05).According to ROC analysis,the sensitivity, specificity and Youden index of PCT were 64.7%,77.9%,0.426.Conclusion The level of serum PCT could be used as good bio-marker for severe pneumonia.Detection of PCT,CRP and WBC together plays an important role in the diagnosis of severe pneumo-nia.
8.Status que of acupuncture for strephenopodia after stroke.

In order to optimize acupuncture protocol for stephenipodia after stroke, related papers of the recent 20 years are retrieved. Interrelated factors which may influence the therapeutic effect of acupuncture are analyzed through the aspects such as acupoints, needling methods and opportunities of acupuncture. It is held that importance should be attached on relation between acupoints and anatomy during acupoint selection. Different acupuncture protocol can be adopted in different phases of stephenipodia according to its time of occurrence. And it is found that the combination of acupuncture and rehabilitation can reinforce the therapeutic effect. Relationship between effect of acupuncture and patient's condition, acupoint selection and prescription, acupuncture manipulations (including directions, angles, reinforcing and reducing, quantity of stimulus) are worth to be further studied. On the base of the above all mentioned studies, optimization can be carried out on selection of acupoints and acupuncture manipulations. And therapeutic effect can be enhanced when the optimized acupuncture protocol is combined with rehabilitation therapies.

9.Arthroscopy-assisted tibial bone tunnel fixation combined with double-row anchors for tibial eminence fracture

Objective:To evaluate arthroscopy-assisted tibial bone tunnel fixation combined with double-row anchors in the treatment of tibial eminence fracture.Methods:The 23 patients were retrospectively analyzed who had been treated at Department of Orthopedics, Beiing Shunyi District Hospital for tibial eminence fractures by arthroscopy-assisted tibial bone tunnel fixation combined with double-row anchors from October 2015 to December 2019. They were 15 males and 8 females, aged from 12 to 55 years (average, 35.5 years). All the injuries were unilateral (14 right and 9 left sides). According to the modified Meyers-McKeever classification, 2 cases belonged to type Ⅱ, 18 cases to type Ⅲ and 3 cases to type Ⅳ. Range of motion of the knee, Lysholm and International Knee Documentation Committee (IKDC) scores were observed before surgery, 1 month and 12 months after surgery. The anterior tibial slope angle (ATSA) on CT was measured preoperatively and 1 month after surgery for evaluation of fracture reduction.Results:All the 23 patients were followed up for an average of 23 months (from 12 to 52 months). Postoperatively, limited knee movement was observed in 2 patients and non-anatomic reduction in one patient. At 1 month and 12 months after operation, the Lysholm scores (61.4 ± 3.5 and 90.4 ± 4.3) and IKDC scores (69.6 ± 4.2 and 88.5 ± 3.0) were significantly improved compared with the preoperative values (45.4 ± 6.8 and 49.6 ± 3.9, respectively) ( P<0.05). ATSA was significantly restored from preoperative 4.2° ± 5.7° to -11.7° ± 2.9° at 1 month after operation ( P<0.05). Conclusion:In the treatment of tibial eminence fracture, arthroscopy-assisted tibial bone tunnel fixation combined with double-row anchors can achieve anatomical reduction and firm fixation, leading to satisfactory surgical outcomes.
10.Cohort-controlled study on percutaneous laser decompression in treating lumbar disc herniation

BACKGROUND: Percutaneous laser disc decompression(PLDD) is a new technology developed to treat lumbar herniated disk inrecent years; however,only limited literature has been reported about cohort study on its indications.OBJECTIVE: To treat patients with lumbar disc herniations by PLDD and evaluate the effects of PLDD in releasing painandimproving lumbar function after operation.DESIGN: Non-randomized concurrent controlled trial based on thepatients.SETTING: Department of Radiology of the 474 Hospital of Chinese PLA.PTICIPANTS: This study recruited totally 173 patients ( 101 males and 72 females aged from 18 to 75 years) who received PLDD in the Department of Radiology of the 474 Hospital of Chinese PLA from July 1998 to August 2002 and were followed up for over one year.METHODS:The 173 patients were divided into good indication group (Group A, n = 139) and poor indication group(Group B, n =34). Group B was further divided into extrusion group(Group B1 of 8 patients),lumbar canal stenosis group(Group B2 of 15 patients) and other conditions group (Group B3 of 11 patients). VAS scoring criteria and modified Macnab criteria were used to evaluate the curative effects.MAIN OUTCOME MEASURES: Primary results: the degree of pain and lumbar function. Secondary results:the success rate of puncture as well as adverse events and side effects.RESULTS: The success rate of puncture was 96. 3% for performing through L5 - S1 and 100% through L3-4 and L4-5. In Group A, excellent postoperative effects were seen in 63 cases, good in 51 cases, fair in 20 cases and poor in 5 cases, with 82.0% excellent and good rates. The excellent and good rate in Group B was 55.9%. Postoperative back pain could be relieved several days later. Other complications such as disc infection,psoas hematoma,rupture of colon,injuries of nerve roots and bloodvessels did not occur. There was a significant difference between Group A and Group B(x2=10.38, P <0.05).CONCLUSION: (① PLDD is a convenient, safe and reliable procedure in treating lumbar disc herniation because of its high success rate, satisfactory results and fewer complications.② Proper selection of indications helps improve the curative effects of lumbar disc herniation.
