1.Theoretical hypothesis and clinical practice of Bobath concept: A therapeutical basis for nervous tissues repairing and functional reconstruction

OBJECTIVE: A series of Bobath concept, Bobath theoretical hypothesis and clinical practice were summarized, to proVide a better understanding of Bobath concept involved in nervous tissues repairing and functional reconstruction. METHODS: Articles concerning the Bobath treatment for stroke-related literature were retrieved in Highwire press, MEDLINE, and Google from 2001 to 2009, using the key words of "Bobath concept, stroke physical therapy". Data of each study were extracted and analyzed following retrieval. Inclusion criteria:①Bobath concept and its theoretical hypothesis. ②New development of Bobath concept in clinical practice. Exclusion criteria: Repetitive studies. The results of retrieved literatura were analyzed. RESULTS: The theoretical basis of Bobath concept is based on present-day knowledge of motor control, motor learning, neural and muscle plasticity, and biomechanics. The latest theories and viewpoints are rasourced from International Bobath Instructors Training Association and the British Bobath Tutors Association. They expounded on the Bobath concept from linking participation, activities and underlying impairments, organization of human behavior and motor control, the consequences of injury and dysfunction in the execution of movement, neural and muscle plasticity, motor learning and so. This article also discusses several important questions which meet in clinical practice. CONCLUSION: Bobath therapy, as a traditional method for stroke rehabilitation, is constantly absorbing the research results from current neuroscience, psychology and other disciplines, and blended with other treatments to promote the development of neuro-rehabilitation medicine.
2.A study on the optimal surgical timing for rib internal fixation

Objective To investigate the optimal surgical timing for rib internal fixation.Methods The clinical data of 143 patients with rib fracture were retrospectively analyzed.The patients were divided into early phase fixation group (62 cases) and late phase fixation group (81 cases) according to the time from trauma to internal fixation.The ratio blood lose and body surface area,pulmonary complications and the changes of C reactive protein level before and 5 days after operation were recorded and compared.Results All the patients were healing.The median C reactive protein before operation in late phase fixation group was 45 mg/L,in early phase fixation group was 23 mg/L,there was statistical difference (P < 0.01).The ratio blood lose and body surface area in early phase fixation group was (334.19 ± 37.53) ml/m2,the rate of pulmonary complications was 22.6%(14/61),in late phase fixation group was (438.99 ± 55.24) ml/m2 and 38.3% (31/81),there were statistical differences (P < 0.01 or < 0.05).The median C reactive protein 5 days after operation in early phase fixation group was 189 mg/L,in late phase fixation group was 258 mg/L,there was statistical difference (P < 0.05).Conclusion Early phase (72 hours) rib internal fixation is expected to reduce patient trauma of systemic reactions,reduce the incidence of bleeding and pulmonary complications.

4.Identification of Clostridium perfringens Causing Human Disease by Multiplex PCR

OBJECTIVE To search a method for identifying Clostridium perfringens and genotyping their toxin for gene diagnosis by multiplex PCR.METHODS The mutiplex PCR was developed with three sets of primers(designed) based on the sequences of three C.perfringens toxin genes(CP?,CP? and CPE) published in GenBank to identify C.perfringens and genotype their three toxin genes.RESULTS Three expected(sequences) were (obtained) successfully by multiplex PCR and identified by electrophoresis.CONCLUSIONS The(specific) sequences of C.perfringens could be amplified and their three genes of toxins could be identified by this multiplex PCR(system).Such method should be helpful for developing gene diagnosis well.
5.The clinic application of microwave heliotherapy in the malignant bone tumors of pelvis

Objective To investigate the clinical outcomes of microwave heliotherapy in situ on the primary or metastatic malignant tumors of the pelvis.Methods From February 2000 to April 2009,18 patients with primary or metastatic malignant tumors of the pelvis were treated with microwave heliotherapy in situ,and followed a total or partial tumor resection.There were 11 males and 7 females with an average age of 45 years(range,16-72).Twelve cases were diagnosed as primary malignant tumor and 6 as metastases.Locations of tumors involved:the Ⅰ region 6 cases.the Ⅱ region 10 cases.and the Ⅲ region 2 cases.The exposures of all tumors were via aTtype or ilioinguinal approach.The lesions were heated at 50℃ for 20 min by 2450 MHz microwave,with surrounding soft tissue protected by copper.mesh.The necrotic tumor tissues were total or partial excised after treatment,with preservation of the anatomical continuity of the pelvic ring.Results The duration of surgery was 60-180 min (110 min on average).The blood loss was 400-800ml(480 ml on average).All patients were followed-up for 0.5-7 years(3.5 years on average).Tumor local recurred in 1 case with chondrosarcoma,and was survival in tumor-bearing after 6 months follow up.One case with malignant fibrous histiocytoma died due to brain,pulmonary,and all body metastases.One case with osteosarcoma died due to pulmonary metastases.Five cases with the metastases died due to non-pelvis metastases.Functions of hip joint in 18 patients were as follows:flexion 80°-130°,extension 0°-10°,abduction 25°-35°,and adduction 18°-23°.Conclusion The clinical result demonstrated that the advantages of microwave heliotherapy in situ were quick increase of temperature,sensitive responses,easy control of temperature,and effective inactivation of tumor cells in the malignant bone tumors of pelvis.

Objective To evaluate the effect of 5? reductase inhibitor on vascularity of benign prostatic hyperplasia (BPH). Methods Sixty male adult rats were randomly divided into 4 groups (15 per group): normal control, BPH control group, the group of 5? reductase inhibitor treatment during hyperplasia and the group of treatment after hyperplasia. Immunohistochemistry combined with computer assisted image analysis system were performed to examine the expression of factor Ⅷ related antigen, PCNA and VEGF in the prostatic tissue for all rats. Objective To investigate the mechanism of anti-tumor in lung cancer patients and their effects to immune system undergoing with microwave ablation treatment,radioimmunotherapy of 131Ⅰ tumor cells human mouse chimeric monoclonal antibody injection (131Ⅰ-chTNT) and combined treatment of two anterior method.Methods The 50 cases of lung cancer were divided into three groups randomly,17 cases were in the group of simple microwave ablation treatment,15 cases were in the group of simple radiotherapy immunotherapy,18 cases were in the group of combined microwave ablation treatment with 131Ⅰ-chTNT radioimmunoassay.During the study,the T lymphocyte subsets,the activity of NK cells,the expressing of interleukin (IL)-2,IL-10,IL-12,the changing of the interferon γ(IFN-γ) and tumor necrosis factor α (TNF-α) were detected and compared of before and after treatment on all the patients.Results The CD4+ value,CD4+/CD8+ value and the activity of NK cells in these three groups after treatment were higher than that of before treatment (P < 0.05),there was statistic difference between the combined treatment group and two simple treatment groups (P < 0.01).The expression of IL-2,IL-12,IFN-γ TNF-α in three group patients after treatment were higher than that of before treatment,which had statistical significance (P < 0.05),while there were no significant differences between groups (P > 0.05).Conclusion The method combined microwave ablation treatment with 131Ⅰ-chTNT radioimmunoassay has efficiency to improve the immune function,which could improve the comprehensive therapeutic effect of lung cancer excellently.
8.Effect of filtering surgery on glaucoma

ObjectiveTo investigate the effect of filtering surgery on glaucoma.Methods50 glaucoma cases (60 eyes) were underwent trabeculectomy, including paracentesis in advance, suturing of sclera flap and conjunctiva flap, using mitomycin (MMC) and forming anterior chamber as soon as finished operation. All cases were followed up 1 year.ResultsPostoperative IOP was lower than 21 mmHg in 54 eyes, 6 eyes were <30 mmHg when treated with drugs. After operation, there were only 2 eyes had lower vision, the others had higher vision. Two eyes had conjunctiva filtering, two eyes had choroidal separation, but they recovered after non surgical therapy.ConclusionFiltering surgery can decrease common complications, increase vision of early stage, and make IOP recovered to normal.
9.Retinal Function after Laser in Situ Keratomilensis

ObjectiveTo observe the retinal function and nerve fiber layer (RNFL) thickness after laser in situ keratomileusis (LASIK). MethodsLASIK was performed in 15 cases (30 eyes) with myopia after strict preoperative examination. All examinations such as vision, correction vision, diopter, intraocular pressure, corneal thickness, ocular axis, topography scan and fundus of eye examination were performed before and 1 day, 1 week, 1 month,3 months and 6 months after operation, as well as electroretinography (ERG), visual evoked potential (VEP), optical coherece tomography at same time. ResultsThere was not significant difference in the intraocular pressure, ERG and VEP 1 day after LASIK. The thickness of RNFL decreased 1 week after LASIK (P<0.05) and recovered 1~6 months later. ConclusionLASIK does not disturb the retinal function and RNFL thickness irreversiblely.
10.DNA Staining Efficiencies of Different Fluorescent Dyes in Preparative Agarose Gel Electrophoresis:A Comparative Analysis

OBJECTIVE To set-up safe alternatives to ethidium bromide(EB).METHODS Comparative analysis was performed on the DNA staining efficiencies of 4 fluorescent dyes including SYBR Gold,SYBR Green Ⅰ,(GoldView) and EB in preparative agarose gel electrophoresis.RESULTS Although both SYBR Gold and SYBR Green Ⅰ altered electrophoretic mobility and thus DNA size estimates,they were cost-effective alternatives to EB.SYBR Gold was more sensitive than SYBR Green Ⅰ at detecting short fragments,but 50 bp bands were clearly(visible) using either dye when visualized with a long integration time.CONCLUSIONS SYBR Gold or SYBR Green Ⅰ are sensitive and relatively safe alternatives to EB.
