1.Clinical study on the controlled subcutaneous analgesia with different doses of sufentanil for postoperative abdominal surgery patients

ObjectiveTo investigate the effects of controlled subcutaneous analgesia(PCSA) with sufentanil in patients underwent abdominal surgery and the occurrence of adverse reactions. Methods90 patients underwent abdominal surgery were randomly divided intothree groups with 30 cases each. Group Ⅰ :sufentanil 0. 04μg · kg-1 ·h-1 for postoperative analgesia; Group Ⅱ : sufentanil 0. 06μg · kg-1 · h-1 for postoperative analgesia; Group Ⅲ :sufentanil 0.08μg/kg · h for postoperative analgesia;PCSA started at the end of operation with catheter buried in deltoid muscle. Visual analgesia scale(VAS) ,Rasmay scale and side effects were recorded at 4,8,16,24 and 48h after operation. ResultsVAS scores decreased significantly in group Ⅱ and group Ⅲ compared with that in group Ⅰ ( all P < 0.05) ;There were no differences of Ramsay scores between groups ( all P > 0. 05) ;There were three cases of respiratory depression in group Ⅲ. ConclusionPCSA with sufentanil (0.06μg/kg) had exact analgesic effects for postoperative abdominal surgery patients with less adverse reactions.
3.Application of seed cells in tissue-engineered articular cartilage

OBJECTIVE: To expound the research status of the seed cells in tissue-engineered articular cartilage, and explore the resource, in vitro amplification and gene modification, so as to propose the application prospect in future.DATA SOURCES: A computer-based online search of PubMed was undertaken to identify the articles about the seed cells in tissue-engineered articular cartilage published in English between January 1994 and April 2006 with the key words of "cartilage, joint, tissue engineering, stem cell, cell culture, gene modification". Other articles were searched from Tsinghua Tongfang and Vip database according to the journal and article titles.STUDY SELECTION: The data were selected firstly to choose those about the seed cells in tissue-engineered articular cartilage and eliminate those not accorded with the requirements. The full-texts of left articles were searched manually.DATA EXTRACTION: Totally 89 articles were collected, of which 34 were included and 55 were excluded.DATA SYNTHESIS: Cells for the construction of artificial cartilage tissues include autologous or homologous chondrocytes, periost and perichondrium cells, mesenchymal stem cells and embryonic stem cells, all of which could be cultured and amplified by specific culturing system in vitro, they also could be genic modified under certain condition with different strategies and methods to repair articular cartilage defects.CONCLUSION: Seed cells with their particular characteristics play an important role in the repair of articular cartilage defects with tissue engineering technique.
4.Hydroxyapatite surface modification of artificial cornea titanium scaffold:in vivo biocompatibility

BACKGROUND:Artificial cornea titanium scaffold has a higher complication rate in clinical usage; therefore, to seek a kind of artificial corneal scaffold with high biocompatibility is a hot spot.
OBJECTIVE:To explore the biocompatibility of the artificial cornea titanium scaffold with hydroxyapatite surface modification.
METHODS: Twenty-seven New Zealand white rabbits were enroled to make alkali burn models of the right cornea and then randomized into three groups: in experimental group, artificial cornea titanium scaffold with hydroxyapatite surface modification was implanted into the right eye; in control group, artificial cornea titanium scaffold was implanted into the right eye; in blank group, no scaffold was implanted into the right eye. At 2, 4, 16 weeks after implantation, the right corneas were taken out for pathological observation and scanning electron microscope observation.
RESULTS AND CONCLUSION:At 16 weeks after implantation, there was no difference in the number of inflammatory cels and fibroblasts among the three groups. Over time, in the experimental group, the corneal tissues, fibrous tissues and adhered extracelular matrices were increased gradualy. In addition, the experimental group had more adherent corneal tissues, extracelular matrices and better tissue healing than the control and blank groups. These findings indicate that the artificial cornea titanium scaffold with hydroxyapatite surface modification had good biocompatibility that can effectively promote proliferation of corneal cels and corneal vascularization.
5.Quality Standard for Infantile Bronchitis Traditional Chinese Medicine Lotion

Objective:To establish the quality standard for infantile bronchitis traditional Chinese medicine lotion. Methods:TLC was used for the qualitative identification of Ephedra herba, Paeonia lactiflora Pall. , Poria cocos, Fructus aurantii and Epimedium brev-icornu Maxim. . HPLC was used for the content determination of ephedrine hydrochloride, pseudoephedrine hydrochloride and paeoni-florin. The chromatography conditions for ephedrine hydrochloride and pseudoephedrine hydrochloride were as follows: an AElichrom Pdar-Phenyl Polyphenyl ether-bonded phenyl-bonded silica column (250 mm × 4. 6 mm, 5μm) was used, the mobile phase was aceto-nitrile-0. 2% phosphoric acid solution (1 :99), the flow rate was 1. 0ml·min-1, the detection wavelength was 210 nm, the column temperature was 30℃ and the injection volume was 10 μl. The chromatography conditions for paeoniflorin were as follows:an Inertsil ODS-3 C18 column (250 mm × 4. 6 mm, 5 μm) was used, the mobile phase was acetonitrile-0. 1% phosphoric acid solution (15:85), the flow rate was 1. 0 ml·min-1, the detection wavelength was 230 nm, the column temperature was 30℃ and the injection vol-ume was 10 μl. Results:The TLC results of Ephedra herba, Paeonia lactiflora Pall. , Poria cocos, Fructus aurantii and Epimedium brevicornu Maxim. showed clear spots with good resolution. Ephedrine hydrochloride, pseudoephedrine hydrochloride and paeoniflorin had a good linear relationship within the range of 10.11-101.10 μg·ml-1(r=0.9996), 10.08-100.80 μg·ml-1(r=0.9991) and 20.50-102.5 μg·ml-1(r =0.9996), respectively. The average recovery was 98.80% (RSD =1.87%, n =6), 98.77%(RSD=1. 72%, n=6) and 99. 57% (RSD=1. 56%, n=6), respectively. Conclusion: The established quality standard can be used for the quality control of infantile bronchitis traditional Chinese medicine lotion.
6.Clinical value of DR and CT in the secondary pulmonary tuberculosis

Objective To discuss different performance of DR and CT examination on secondary pulmonary tuberculosis,to improve the clinical guidance value of CT in the diagnosis of secondary pulmonary tuberculosis. Methods The image data of 52 cases with secondary pulmonary tuberculosis confirmed by pathology were collected, including chest DR radiography and CT scans.The different image characteristics of two kinds of imaging examination were analyzed and compared.Results In 52 cases of secondary pulmonary tuberculosis,chest X -ray plain film found lesions calcification in 3 cases,2 cases of bronchiectasis,pleural thickening and adhesion in 12 cases,pleural effusion in 1 case,empty cavity in 4 cases,emphysema in 1 case,the lung nodules in 5 cases.Chest CT revealed lesions calcification in 9 cases,16 cases of bronchiectasis,pleural thickening and adhesion in 40 cases,pleural effu-sion in 4 cases,empty cavity in 10 cases,emphysema in 8 cases,the lung nodules in 10 cases,6 cases of pulmonary bullae,pulmonary interstitial change:4 cases of lung reticulocyte shadows,interlobular septal thickening in 4 cases, ground glass sample density in 4 cases.Conclusion DR and CT can be used for inspection of secondary pulmonary tuberculosis,but CT is a better tool,it has important guiding role for clinical diagnosis and treatment.
7.Effect of different tumescent local anesthesia on the proliferation and differentiation of human adipose stem cells culturedin vitro

BACKGROUND:Different local anesthetic solutions for tumescent liposuction are necessary and have a certain effect on the biological characteristics of human adipose stem cels.
OBJECTIVE:To explore the effect of different tumescent local anesthesia on the proliferation and differentiation of human adipose stem cels culturedin vitro.
METHODS: Passage 3 human adipose stem cels were intervened by different tumescent local anesthesia (bupivacaine, ropivacaine, lidocaine). Cels cultured in low-glucose DMEM served as controls. Proliferation ability, lactate dehydrogenase activity in the supernatant and percentage of cels under adipogenic differentiation were detected.
RESULTS AND CONCLUSION:After intervention for 1, 2, 3, 4, 5 days, the supernatant lactate dehydrogenase activity in the bupivacaine, ropivacaine, lidocaine groups was significantly higher than that in the control group (P < 0.05), and moreover, the lactate dehydrogenase activity in the bupivacaine group was significantly higher than that in the ropivacaine group and lidocaine group (P < 0.05). The absorbance value of human adipose stem cels was lower in the bupivacaine, ropivacaine, lidocaine groups than the control group (P < 0.05) as wel as lower in the bupivacaine group than the ropivacaine and lidocaine groups (P < 0.05). There was no significant difference in the percentage of cels under adipogenic differentiation between groups (P > 0.05). Overal, these findings indicate that different tumescent local anesthesia (bupivacaine, ropivacaine, lidocaine) can al inhibit the proliferation of human adipose stem cels, but have no certain effects on the adipogenic differentiation.

Objective To determine the serum concentrations of free carnitine in lipid storage myopathy(LSM) and the roles in diagnosis,classification and therapy of LSM. Methods Fasting blood samples of 8 LSM cases identified by muscle biopsies, 9 cases of other neuromuscular diseases and 18 cases of normal controls were enzymatically analyzed for free carnitine levels. Results In 8 LSM cases serum values of free carnitine were 4.4?2.5 mg/L, in 9 cases with other neuromuscular disease, serum levels of free carnitine were 10.2?2.5 mg/L and in 18 normal controls,11.8?2.9 mg/L. 9.Change of calcitonin gene related peptide in spinal motoneuron after upper motoneuron injury

Objective To observe the changes of calcitonin gene related peptide (CGRP) in rat spinal motoneuron after upper motoneuron injury. Methods Thirty six adult male Wistar rats were selected. Spinal cord transection was made at the lower thoracic segment. The lumbar regions of spinal cord were removed immediately, 1, 2, 4, 7 and 14 days after surgery to be divided into dorsal and ventral sections. The content of CGRP in ventral section was detected using radioimmunoassay method.Results The level of CGRP in spinal motoneuron of the lumbar region was decreased significantly after transection of spinal cord, reached the lowest level on the 2nd day after surgery and then maintained at the low level.Conclusions The upper motoneuron injury can result in the decrease of CGRP content in spinal motoneuron.
10.Meta analysis of the efficacy of sequential therapy for eradication of Helicobacter pylori

Objective To systematically evaluate the curative effect of sequential treatment and standard triple therapy in eradicating Helicobacter pylori and the risk of adverse events between the two therapies.Methods The related studies were searched with electronic databases to evaluate the efficacy of sequential therapy and standard triple therapy in eradicating H.pylori.Information was retrieved including the clinical features of patients,quality of research,eradication rate of H.pylori and incidence of adverse events and so on.The pooled relative risk(RR)and 95%confidence intervals(95%CI)were calculated,and the publication bias was assessed with funnel plot. Results Twelve randomized trials were performed,and 3183patients met the internalization criterion.Meta analysis demonstrated a clear superiority of the sequential therapy over 7-day triple therapy with an RRof 1.23(95%CI:1.19-1.27),over 10-day triple therapy with aRRof 1.17(95%CI:1.10-1.24),and over 14-day triple therapy with a RRof 1.25(95%CI:1.06-1.48).The incidence of adverse event was similar with sequential therapy versus that of triple therapy(RR=0.96,95% CI:0.75-1.23).Conclusions Sequential therapy is associated with a higher eradication rate of H.pylori compared with 7-day,10-day and 14-day triple therapy.The adverse event doesn't increase significantly.
