Pancreatic cancer is one of the most malignant cancer of all neoplasm.85% cases are not suitable for surgery when at first counsultation to doctors.The effects of chemotherapy and radiotherapy are not satisfactory either.With the advancement of the study of molecular biology,the research of the molecular targeted drugs such as EGFR inhibitor and VEGF inhibitor,become one of the hot spots.This article reviews progress in the research of molecule-targeted therapy.
3.Analysis of hepatitis associated aplastic anemia in 5 children

Objective To explore the clinical feature, treatment and prognosis of hepatitis associated aplastic anemia (HAAA) in children. Method The clinical data of 5 children with HAAA were retrospectively analyzed. Results There were 5 patients (4 males and 1 female) with median age of 10 years (7~13 years ). The detection of hepatitis A, B, C, and E were all negative in these 5 children during their acute hepatitis period. In 2 children, parvovirus B19 antibody and EB virus were negative and the proportion of CD4+T cells and the ratio of CD4+/CD8+ were decreased, and the proportion of CD8+T cells was increased by T-lymphocyte subsets analysis. Three children were treated with anti-thymocyte globulin combined with cyclosporine, 2 of whom achieved complete remission and 1 died of pulmonary fungal infection. One child was treated with cyclosporine only and achieved partial remission. One child lost follow-up after giving up treatment. Conclusion HAAA in children can be caused by viral seronegative hepatitis, and usually has T lymphocyte immune disorders. Immunosuppressive therapy is effective. .

5. Clinical efficacy of Tianmeng Capsule combined with paroxetine in treatment of menopausal women with generalized anxiety disorder

Objective: To study the effect of Tianmeng Capsule combined with paroxetine in the treatment of menopausal women with generalized anxiety disorder (GAD) and its mechanism of the reaction with oxidative stress. Methods: Totally 120 cases of menopausal patients with GAD, without the use of estrogen replacement therapy, were randomly divided into control group and observation group of 60 cases, the control group were treated with paroxetine, the observation group were additionally treated with Tianmeng Capsule, then the clinical effects were compared. Results: The Hamilton anxiety scale (HAMA) scores and Pittsburgh sleep quality index scale (PSQI) scores were significantly decreased in two groups and the indexes in the observation group were lower than control group (P < 0.05). The total effective rate in the observation group was significantly better than that of the control group (P < 0.05 ). There were no obvious adverse drug reactions in the two groups. The serum follicle stimulating hormone (FSH), luteinizing hormone (LH) and estradiol (E2) levels had no significant difference between two groups after treatment. The serum superoxide dismutase (SOD) levels increased, and malondialdehyde (MDA) levels decreased in the observation group (P < 0.05). Conclusion: It is safe and effective to treat GAD patients with Tianmeng Capsule, which may not improve the level of hormone but related to oxidative stress.
6.Histological changes of domestic e-PTFE implanted into subcutenous tissue of rat

Objective To investigate the relative histological change of transplanted domestic expanded polytetrafluoroethylene (e-PTFE), which were treated with different methods, in order to offer the referential data for clinical application. Methods e-PTFE treated with different methods was transplanted into subcutaneous tussue of rat. The samples harvested according to time sequence were examined by using histological and histochemical methods. Dynamic change of the structure between the e-PTFE and it's surrounded tissue was investigated based on the examination. Results Cell and tissue were observed on the inside of all the e-PTFE including the control group and the experiment groups. Heavy cell infiltration on the 3rd day was the most significant in control group, and the quantity of tissue ingrowth was also the most until the 28th day. The next was trimming group. The quantity of both cell infiltration and the tissue ingrowth in high pressure steamed group and forceps squeezed group were less than that in other groups. Conclusion Cell infiltration into domestic e-PTFE is significantly achieved since 72 hrs and fibrovascular ingrowth since the 7days after implanted e-PTFE under subcutaneous tissue of the rat. Different treatment methods of e-PTFE can affect the speed of tissue ingrowth into the e-PTFE, which could be a reference for clinic application of e-PTFE.

8.Migration of dendritic cells in orthotopic rat liver transplantation

Objective To investigate migration of dendritic cells(DC)in orthotopic rat liver transplantation models.Methods Allogeneic models(Wistar→SD,experimental group)and syngenic models(Wistar→Wistar,control group)of rat liver transplantation were established.Graft livers and host celiac lymph nodes(n=4)of each group were sampled respectively at day 3,5,and 7 after the transplantation.The acute rejection was graded according to liver histopathological changes.The dy- namic state of DC number within graft and lymph nodes was detected by means of immunohistochemi- cal staining and image analysis.T-cell active proliferative response in lymph nodes was also studied. Results The histological examination revealed that mild to severe rejection occurred on the post-opera- tive days 5 and 7.At day 3 after transplantation,the number of S-100~+ DC in allograft was signifi- cantly increased and reached the peak at day 5,then decreased gradually at day 7.A significant num- ber of S-100~+ cells was detected in the allogeneic host lymph nodes from day 3 after transplantation, and displayed a continuous increasing trend for next several days.Active proliferation of T cells in the lymph nodes was triggered as early as day 3 after transplantation.Conclusion Allogeneic liver trans- plantation induces the accelerated migration of DC within the allograft and host lymphoid nodes.DC delivers a strong and sustained stimulation among T cells in the lymphoid nodes where effector cells are predominantly sensitized and rejected the graft eventually.
9.The value of diagnosis and differential diagnosis of gallbladder wall thickening diseases with DWI

Objective To evaluate the usefulness of diffusion-weighted imaging(DWI) in the diagnosis and differential diagnosis of gallbladder wall thickening diseases.Methods 42 patients with gallbladder wall thickening (16 patients with carcinoma and 26 patients with benign lesion) were included in this study.All patients performed conventional MRI and DWI.The diagnostic performances of three methods (conventional MRI,visual assessment of color fusion image from DWI and T2WI,and ADC measurement) were evaluated by two radiologists.Results The area under the receiver operating characteristic curve were 0.570,0.849,0.901 for conventional MRI,visual assessment and ADC measurement respectively.The accuracy,sensitivity and specificity were 59.5%,62.5%,57.7% for conventional MRI,85.7%,81.2%,88.5% for visual assessment of color fusion image,and 83.3%,80.0%,85.2% for ADC measurement,respectivily.The mean ADC value of gallbladder cacinoma[(1.15±0.35)×10-3mm2/s]was significantly less than that of gallbladder benign lesion [(1.99±0.61)×10-3mm2/s](P<0.01).Conclusion The DWI(visual assessment of color fusion image and ADC measurement)might be a useful tool for diagnosis and differential diagnosis of the gallbladder wall thickening diseases.
10.Comparison of intracranial large artery vascular trunks and hemodynamic change with transcranial Doppler ultrasound and magnetic resonance angiography

BACKGROUND: Although angiography is the gold standard for the diagnosis of cerebrovascular disease, it cannot be used widely because of its traumatotaxis. Transcranial Doppler ultrasound (TCD) and MRA are both non-invasive examinations, which can be used to evaluate intracranial large artery vascular trunks and hemodynamics.OBJECTIVE: To investigate the reliability of diagnosis with TCD on hemodynamic changes of ischemic cerebrovascular disease (ICVD) and the relativity between TCD and MRA.DESIGN: Case analysis based on patients.SETTING: Neurological Department of the First Affiliated Clinical Hos-pital, Harbin Medical University.PARTICIPANTS: From April 2001 to February 2002,totally 45 patients with stroke,selected from outpatients and inpatients of Neurological Department of First Affiliated Clinical Hospital of Harbin Medical University,examined by CT, to be told the fact and accepted, were taken part in the study.METHODS: Patients were examined with TC-2021 TCD instrument produced by German EME Corporation and VISART1.5T superconduct MR instrument produced by Toshiba. Totally 45 patients had TCD and MRI examinations, and the alternation between two examinations was 1 to 15 days. The hemodynamics and morphological changes were observed including middle cerebral artery (MCA), end of internal carotid artery (ICA),anterior cerebral artery (ACA), posterior cerebral artery (PCA), basal artery (BA), and vertebral artery (VA).large artery vascular trunks and hemodynamic results with TCD and MRA;ty, false positive and false negative with TCD with those of MRA.TCD and MRA: Theory data was Tmin=9.91; χ2=107.92, P < 0.005, it was considered that the results with the rate of TCD were related with that of MRA. The detection rate of TCD was 19.06%, and that of MRA was 15.25%. There were significant differences between the detection rate of standard, the sensibility of diagnosing abnormal vessels by TCD was 78.10%, specificity 94.22%, false positive 7.46%, false negative 23.31%,and the coincidence rate 89.36%.CONCLUSION: TCD can assess exactly the functional status of the vessels. It can also reflect early and sensitively the change of hemodynamics of cerebral blood flow. While MRA can display the change of vascular appearance directly so that the combination of TCD and MRA will be improve sensibility and specificity of diagnosis.
